    Well-Being in Alpine Space: How Subjective Determinants Affect Urban and Rural Areas. A Case Study Analysis in South Tyrol, Italy

    The paper analyzes urban-rural difference on the individual psychological well-being of residents living in the Autonomous Province of Alto Adige, region on the border between Italy and Austria. Data comes from a cross-sectional survey undertaken in 2010 on a statistical representative sample, based on the PGWBI, an instrument specifically used to measure individual subjective well-being. The study examines the influence of socio-demographic factors, as well as cultural determinants, on the PGWBI. Urban inhabitants were found to perceive higher level of psychological well-being compared to rural ones, while the determinants affecting individual subjective had a greater impact on the rural one

    Direct-acting antivirals and hepatocellular carcinoma in chronic hepatitis C: A few lights and many shadows

    With the introduction of direct-acting antiviral agents (DAA), the rate of sustained virological response (SVR) in the treatment of hepatitis C virus (HCV) has radically improved to over 95%. Robust scientific evidence supports a beneficial role of SVR after interferon therapy in the progression of cirrhosis, resulting in a decreased incidence of hepatocellular carcinoma (HCC). However, a debate on the impact of DAAs on the development of HCC is ongoing. This review aimed to analyse the scientific literature regarding the risk of HCC in terms of its recurrence and occurrence after the use of DAAs to eradicate HCV infection. Among 11 studies examining HCC occurrence, the de novo incidence rate ranged from 0 to 7.4% (maximum follow-up: 18 mo). Among 18 studies regarding HCC recurrence, the rate ranged from 0 to 54.4% (maximum "not well-defined" followup: 32 mo). This review highlights the major difficulties in interpreting data and reconciling the results of the included studies. These difficulties include heterogeneous cohorts, potential misclassifications of HCC prior to DAA therapy, the absence of an adequate control group, short follow-up times and different kinds of follow-up. Moreover, no clinical feature-based scoring system accounts for the molecular characteristics and pathobiology of the tumours. Nonetheless, this review does not suggest that there is a higher rate of de novo HCC occurrence or recurrence after DAA therapy in patients with previous HCV infection. \ua9 2018 The Author(s).     Irritable bowel syndrome and endometriosis are two diseases affecting a significant part of the female population, either together or individually, with remarkable consequences in the quality of life. Several studies suggest an epidemiological association between them.Their association may not be just an epidemiological phenomenon, but the manifestation of a pathophysiological correlation, which probably generates a mutual promotion phenomenon. In particular, both clinical entities share the presence of a chronic low-grade inflammatory state at the basis of the disease persistence.Recognizing this association is highly significant due to their prevalence and the common clinical manifestation occurring with a chronic abdominal pain. A further multi disciplinary approach is suggested in these patients' management in order to achieve an adequate diagnostic work up and a targeted therapy.This paper analyses some common pathophysiological mechanisms, such as activation of mast cell line, neuronal inflammation, dysbiosis and impaired intestinal permeability. The aim was to investigate their presence in both IBS and endometriosis, and to show the complexity of their relationship in the generation and maintenance of chronic inflammation

    The aim of this chapter is to analyse sustainable food consumption in Mediterranean countries. This topic has increasingly affected the international debate over the time, because it is directly related to economic, social, environmental and institutional issues. In particular, in recent decades, the scientific literature and international institutions have highlighted the need to focus not only on consumption trends but also on consumers’ choices, by considering the evolution of lifestyles values and socio-cultural factors that affect individuals’ consumption and purchasing behaviour. Clearly, in countries characterized by a different degree of development, the concept of sustainable food consumption assumes different meanings. Notably, in Mediterranean countries, the problem of food and nutrition security coexists with an increasing diffusion of obesity and overweight issues, although with differing relevance depending on the context specificities. Differences among various socio-economic systems represent a further element of complexity, whose consideration cannot be neglected in an attempt to define public policies aimed at promoting the diffusion and development of (really) sustainable models of production, trade and consumption

    Motivations and Actions to Prevent Food Waste among Young Italian Consumers

    Food waste is a relevant global problem due to its consequences on food security, economy, and environmental sustainability. This study focuses, in detail, on finding the main motivation for food waste among the young and the principal actions to prevent it. The paper focuses on Italian reality, since Western countries are partly accountable for wasting large amounts of food. What is more, the focus has been shifted specifically on to the youths and young adults, as they are the portion of the population that are most inclined to waste food. Data were collected using a questionnaire survey performed on a sample of n = 904 Italian consumers. In line with previous research, the results of this study confirm that avoidable food waste comes from three main behavioural antecedents: over preparation, excessive purchase, and inappropriate conservation. The research shows that food waste cannot be reduced by just one-way from the consumers; rather, it goes both ways, between consumers and retailers. Therefore, only holding the consumers accountable and expecting them to solve it will not solve the problem of food waste; marketing and retailers should also consider ethics when it comes to food distribution

    Background and Objectives: Acute lymphoblastic leukaemia (ALL) carrying t(9;22) or t(4;11) genetic abnormalities represents a very high risk subtype of disease (VHR-ALL). Hematopoietic stem cell transplantation (HSCT) still remains the only curative option also in the Imatinib era. In the last years low molecular level of minimal residual disease (MRD) before HSCT was reported as one of the best favourable indexes for survival in ALL. Here we observed that even these patients can show a favourable outcome, if submitted to HSCT with very low MRD. Methods: We considered 18 consecutive VHR-ALL patients eligible to HSCT. 16 of them were transplanted upon first remission, as soon as possible, employing myelo-ablative conditioning regimens. Molecular MRD has been evaluated before and after HSCT.Results: Immediately before HSCT MRD revealed: complete molecular remission (MRDneg) for 5 patients and a level <1x10-3 for 7 patients; 100 days after HSCT we had: MRDneg for 7 patients and a decrease for all the others after HSCT. After tapering of immunosuppressive drugs, 13 patients reached the MRDneg in a median time of 8 months (range 3-16); Based on intention to treat analysis: 14/18 patients are alive and disease free at the time of analysis, overall survival and event free survival is of 78% and 66% respectively, with an average follow-up of 45 months (range 6-84) since HSCT. Conclusion: Early transplantation with low MRD level seems to be correlated with a favourable outcome also in VHR-AL
