    Traumatic fistula between the brachiocephalic trunk and the brachiocephalic vein due to gunshot wound

    We describe the case of a 49-year old male patient who suffered a gunshot wound in the chest which transfixed the medial mediastinum. He was hemodynamically stable, but had tachycardia and tachypnea. He was submitted to integrated work-up with chest radiogram, transthoracic echocardiography, computerized chest tomography, and arteriography of the aortic arch. A traumatic fistula was evidenced between the brachiocephalic trunk and the brachiocephalic vein. Surgical repair was performed using extracorporeal circulation and deep hypothermia with total circulatory arrest. The patient progressed well and was discharged on the fifth day postoperatively.Descrevemos caso de paciente do sexo masculino, 49 anos, que sofreu ferimento por arma de fogo no tórax, transfixando o mediastino médio. Apresentava-se estável hemodinamicamente, contudo taquicárdico e taquipnéico. Ele foi submetido à propedêutica armada com radiograma de tórax, ecocardiografia transtorácica, tomografia computadorizada de tórax e arteriografia do arco aórtico. Evidenciou-se fístula traumática do tronco braquiocefálico com a veia braquiocefálica. Realizou-se correção cirúrgica com a utilização de circulação extracorpórea e hipotermia profunda com parada circulatória total. O paciente evolui bem com alta no quinto pós-operatório.e21e2

    Analysis of the hydrodynamic profile in different roller pumps models used in cardiopulmonary bypass

    OBJECTIVE: Among the equipments used in cardiopulmonary bypass the roller pumps have great importance with various models available from several manufacturers. The calibration is an important factor in hemolysis rates and its potential is different in each. Researchers do not always approach details on the pump bed profiles assuming that the standardized calibration settings ensure equal and comparable values for all models of roller pumps. We have mainly two methods for calibration of pumps which also interferes on the hemolytic potential. In both of them, the characteristics of fluid impulsion defined by the pump bed design are not considered. The aim of this study is to compare the hydrodynamic profile of three models of roller pumps available in the Brazilian market. METHODS: The rollers occlusion was performed by measures of drop and dynamic calibration. Two different silicone diameter tubes were used (3/8 x 1/16 and 1/2 x 3/32 inches). RESULTS: The profiles showed differences in their variances, P<0.01 for drop rate measures and P<0.0001 for dynamic calibration. Different changes in pressure were found between the pumps analyzed (P<0.002). CONCLUSION: The measures of occlusion are dependent on the design of the pump bed and comparisons involving roller pumps should be performed with caution. Blood tests should be performed to verify the influence of changes in hemolysis pressure.OBJETIVO: Dentre os equipamentos utilizados em circulação extracorpórea, as bombas de rolete têm grande importância, com diversos modelos disponíveis de vários fabricantes. A calibração é um fator importante nas taxas de hemólise e o seu potencial difere em cada uma delas. Pesquisadores nem sempre abordam detalhes sobre os perfis do leito rígido, supondo que as formas padronizadas de calibração garantem valores iguais e comparáveis para todos os modelos de bombas de rolete. Dispomos principalmente de dois métodos para a calibração das bombas, o que também interfere com o potencial hemolítico. Nenhum dos métodos considera as características de impulsão do fluído, definido pela forma construtiva do leito rígido. O objetivo é avaliar o perfil hidrodinâmico de três modelos de bombas de roletes disponíveis no mercado brasileiro. MÉTODOS: A oclusão dos roletes foi feita por medidas de velocidade de queda e calibração dinâmica. Foram utilizados dois diferentes diâmetros de tubos de silicone (3/8 x 1/16 e 1/2 x 3/32 polegadas). RESULTADOS: Os perfis apresentaram diferenças em suas variâncias. P<0,01 para medidas de velocidade de queda e P<0,0001 para medidas de calibração dinâmica. Foram encontradas diferenças nas variações de pressão entre as bombas analisadas (P<0,002). CONCLUSÃO: As medidas de oclusão são dependentes da forma do leito rígido e comparações envolvendo bombas de rolete devem ser feitas com cautela. Testes com sangue deveriam ser realizados para verificar a influência das variações de pressão na hemólise.18819

    Role of echocardiography in the ventricular assessment of the transplanted heart versus heart rejection

    BACKGROUND: Heart transplantation is an alternative for individuals with end-stage heart disease. However, episodes of heart rejection (HR) are frequent and increase morbidity and mortality, requiring the use of an accurate non-invasive exam for their diagnosis, since endomyocardial biopsy (EMB) is not a complication-free procedure. OBJECTIVE: To compare the parameters obtained by use of Doppler echocardiography in a group of transplanted patients with HR (TX1) and another group of transplanted patients without rejection (TX0), having as reference a control group (CG) and observing the behavior of the left ventricular systo-diastolic function expressed as the myocardial performance index (MPI) METHODS: Transthoracic echocardiographies were performed from January 2006 to January 2008 to prospectively assess 47 patients divided into three groups: CG (36.2%); TX0 (38.3%); and TX1 (25.5%). The MPI was compared between the groups, and data were analyzed by use of Fisher exact test and nonparametric Kruskal-Wallis test, both with significance level of 5%. RESULTS: The groups did not differ regarding age, weight, height, and body surface. When compared to GC, TX0 and TX1 showed a change in the left ventricular systo-diastolic function, expressed as an increase in MPI, which was greater in TX1 [0.38 (0.29 - 0.44); 0.47 (0.43 - 0.56); 0.58 (0.52 - 0.74), respectively; p < 0.001]. CONCLUSION: Echocardiography was a very accurate test to detect changes in the systo-diastolic function of the transplanted heart; however, it did not prove to be reliable to replace BEM in the safe diagnosis of HR.FUNDAMENTO: O Transplante Cardíaco (TC) é uma alternativa para os indivíduos com doença cardíaca terminal. Na evolução pós-transplante, a ocorrência de episódios de Rejeição Cardíaca (RC) é evento frequente que aumenta a morbimortalidade, sendo necessário o emprego de exame não invasivo com boa acurácia para seu diagnóstico, pois a Biópsia Endomiocárdica (BEM) não é um procedimento isento de complicações. OBJETIVO: Comparar parâmetros obtidos com o princípio Doppler, entre os pacientes transplantados com RC (TX1) e os pacientes transplantados sem rejeição (TX0); utilizando como referência o Grupo Controle (GC) e observando o comportamento da função sistodiastólica ventricular esquerda expressa por meio do Índice de Performance Miocárdica (IPM). MÉTODOS: Foram realizados ecocardiogramas transtorácicos no período de janeiro de 2006 a janeiro de 2008, para a avaliação prospectiva de 47 pacientes, subdivididos em GC (36,2%), TX0 (38,3%) e TX1 (25,5%), comparando-se o IPM entre eles. Para a análise dos dados foram realizados os testes exato de Fisher e o não paramétrico de Kruskal-Wallis, ambos com nível de significância de 5%. RESULTADOS: Os grupos não diferiram em relação a idade, peso, altura e superfície corpórea. Quando comparado ao GC, TX0 e TX1 apresentaram alteração da função sistodiastólica ventricular esquerda, expressa como aumento do IPM, que foi mais intenso no TX1 [0,38 (0,29 - 0,44) X 0,47 (0,43 - 0,56) X 0,58 (0,52 - 0,74) p < 0,001]. CONCLUSÃO: O ecocardiograma mostrou-se como exame de boa acurácia na detecção das alterações da função sistodiastólica do coração transplantado; entretanto, não foi confiável como método substituto da BEM para o diagnóstico seguro de RC.1031103

    Brazilian Perfusionists And Arterial Roller Pump Adjustment: Comparison Between Static And Dynamic Calibration Method.

    Roller pumps play an important role in extracorporeal circulation. However, occlusion of the rollers should be adequately performed and this can be adjusted mainly by two methods: static and dynamic. To investigate how the Brazilian perfusionists adjust arterial roller pumps in their services and evaluate the application of a Device to Assist Calibration (DAC) that facilitates roller adjustment by the dynamic calibration method. We installed a roller pump with accessories to perform adjustment by drop rate (static calibration) and dynamic calibration methods during the XXVIII Brazilian Congress of Extracorporeal Circulation. Perfusionists were asked to adjust the roller pump according to the procedure they usually do in their service. After each adjustment pressure was measured by dynamic calibration method with DAC. The research was approved by the Research Ethics Committee of UNICAMP, Nº 1144/2010. There were 56 perfusionists in this study. Pressure average of 56 measurements of dynamic calibration was 434 ± 214 mmHg; 76% of measurements were within the recommended range for the use of the dynamic calibration method (between 150 and 500 mmHg). Brazilian perfusionists tend to adjust roller pumps with less occlusive settings. The amplitudes of the dynamic calibration pressure tend to be smaller for more experienced perfusionists because their skills increase with time. The device can be used by the perfusionist to adjust roller pumps with greater accuracy and mainly repeatability in few minutes.26205-1

    Echocardiographic assessment of cardiac resynchronization therapy: two-year follow-up period

    BACKGROUND: The cardiac resynchronization therapy (CRT) is an effective option for patients with advanced heart failure (HF). Clinical, electrocardiographic and echocardiographic criteria have been studied in an attempt to find the patients that will benefit from the CRT, considering that the echocardiogram is the method that is used both in the selection and in the assessment of such therapy. OBJECTIVE: The objective of this work is to analyze the use of echocardiogram to assess the CRT, in a ten-day follow-up period and after two years of evolution. METHODS: The assessment considered 20 patients subjected to CRT, for a period of two years, 80% of which were male. The Minnesota Living with Heart Failure Questionnaire (MLWHF) was filled out. Patients underwent a six-minute walking test. Then, the two-dimensional echo-Doppler-cardiogram was performed. The initial assessment was repeated ten days after and two year after the implantation of the biventricular pacemaker. RESULTS: In two years, 5 patients (25%) died; 4 had cardiomyopathy caused by the Chagas's disease. There was no statistically significant change in the ejection fraction between the pre-operation period and the following ten days, but there was a significant change between the pre-operation period and two years after that, and the ten-day period and two years after that. In the ten-day follow-up period, there was the worsening of the intraventricular dyssynchrony, as evaluated by the tissue Doppler method, and the living with heart failure score was higher in the group of deaths. CONCLUSION: Out of the echocardiographic parameters assessed, only the intraventricular dyssynchrony assessment through the tissue Doppler method, after the procedure, was capable of predict the CRT efficiency with respect to the death rate.FUNDAMENTO: A terapia de ressincronização cardíaca (TRC) é uma opção efetiva para os pacientes com insuficiência cardíaca (IC) avançada. Critérios clínicos, eletrocardiográficos e ecocardiográficos têm sido estudados na tentativa de selecionar os pacientes que serão beneficiados com a TRC, sendo o ecocardiograma um método utilizado tanto na seleção quanto na avaliação desta terapêutica. OBJETIVO: O objetivo deste trabalho é analisar a utilização do ecocardiograma na avaliação da TRC, no seguimento de dez dias e após dois anos de evolução. MÉTODOS: Foram avaliados 20 pacientes submetidos à TRC, por um período de dois anos, sendo 80% do sexo masculino. Foi aplicado o Questionário de Qualidade de Vida de Minnesota, o teste de caminhada de seis minutos e realizado o ecodopplercardiograma bidimensional. Dez dias após o implante do marca-passo biventricular, bem como dois anos depois, foi repetida a avaliação inicial. RESULTADOS: Em dois anos, 5 pacientes (25%) foram a óbito; 4 apresentavam cardiomiopatia de etiologia chagásica. Não houve alteração estatisticamente significante da fração de ejeção entre o período pré-operatório e os dez dias seguintes, mas sim uma alteração significante nos períodos de pré-operatório e dois anos e de dez dias e dois anos. No seguimento de dez dias, houve piora da dissincronia intraventricular avaliada pelo Doppler tecidual, assim como a pontuação no escore de qualidade de vida foi maior no grupo óbito. CONCLUSÃO: Dos parâmetros ecocardiográficos avaliados, somente a avaliação da dissincronia intraventricular pelo Doppler tecidual após o procedimento foi capaz de predizer a eficácia da TRC em relação à mortalidade.FUNDAMENTO: La terapia de resincronización cardiaca (TRC) es una opción efectiva para los pacientes con insuficiencia cardiaca (IC) avanzada. Se vienen estudiando criterios clínicos, electrocardiográficos y ecocardiográficos en el intento de seleccionar a los pacientes que serán beneficiados con la TRC, siendo el ecocardiograma un método utilizado tanto en la selección como en la evaluación de este tratamiento. OBJETIVO: El objetivo de este trabajo es analizar la utilización del ecocardiograma en la evaluación de la TRC, en el seguimiento de diez días y luego de dos años de evolución. MÉTODOS: Fueron evaluados 20 pacientes sometidos a la TRC, por un período de dos años, siendo el 80% de sexo masculino. Se aplicó el Cuestionario de Calidad de Vida de Minnesota, el test de caminata de 6 minutos y se realizó el ecodopplercardiograma bidimensional. Diez días tras el implante del marcapasos biventricular, así como dos años después, se repitió la evaluación inicial. RESULTADOS: En dos años, 5 pacientes (25%) obitaron; 4 presentaban miocardiopatía de etiología chagásica. No hubo alteración estadísticamente significativa de la fracción de eyección entre el período preoperatorio y los diez días siguientes, pero sí una alteración significativa en los períodos de preoperatorio, dos años y diez días y dos años. En el seguimiento de diez días, hubo un empeoramiento de la disincronía intraventricular evaluada por doppler tisular, así como la puntuación en el escore de calidad de vida fue mayor en el grupo óbito. CONCLUSIÓN: De los parámetros ecocardiográficos evaluados, solamente la evaluación de la disincronía intraventricular por doppler tisular tras el procedimiento fue capaz de predecir la eficacia de la TRC en relación a la mortalidad.11912

    Hemodynamic Disorders Related To Beating Heart Surgery Using Cardiac Stabilizers: Experimental Study.

    To study in swine the hemodynamic changes secondary to the use of stabilizers for off-pump coronary artery bypass graft surgeries by means of both a suction device Octopus and a compression device (Speroni). Ten swine underwent median sternotomy. Monitoring of ECG, continuous cardiac output, mean arterial pressure, mean pulmonary artery pressure, mean right and left atrial pressures, and right and left ventricular diastolic pressure were performed. Stroke volume and systemic vascular resistance were calculated. Both stabilizers were studied placed on three vessels: anterior interventricular branch, posterior interventricular branch, and marginal branch of the circumflex branch. Each animal was randomly designed to application regarding the type of stabilizer and the target artery. The measurements were carried out 5 minutes before and after the stabilizer application. In the anterior interventricular branch changes have occurred only with the compression device, thus reducing cardiac output, stroke volume, and mean arterial pressure, but increasing the systemic vascular resistance. In the posterior interventricular branch changes have occurred with the compression device (Speroni), reducing cardiac output and stroke volume, but increasing the heart rate. With the suction device (octopus) there was an increase of both heart rate and systemic vascular resistance, but a decrease in stroke volume. In the marginal branch of the circumflex branch there was a decrease of cardiac output, stroke volume, and mean arterial pressure with both stabilizers. Also, there was a decrease in the mean pulmonary artery pressure and an increase in the mean right atrial pressure with the compression device (Speroni). Both stabilizers have caused hemodynamic changes. The compression device (Speroni) is more associated with changes than the suction device (Octopus).22407-1

    Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure &lt;= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.
