    Diese randomisierte Cross-Over-Pilotstudie untersucht die Machbarkeit und Wirkung von Short-term Fasting (STF) während einer Chemotherapie bei Patientinnen mit gynäkologischen Krebserkrankungen. Methode: Eingeschlossen wurden 34 Patientinnen mit adjuvant/primär systemisch behandelten, nicht-palliativen Mamma- oder Ovarialkarzinomen und 4-6 geplanten Chemotherapiezyklen. Die Teilnehmerinnen (Durchschnittsalter 51 Jahre, Spannweite 28-69 Jahre) wurden in 2 Gruppen randomisiert, STF in der ersten Hälfte der Chemotherapiezyklen, gefolgt von normokalorischer Ernährung (Gruppe A; n=18) oder vice versa (Gruppe B; n=16). Das Fasten begann 36h vor und endete 24h nach der Chemotherapie (60h Fastenzeit). Die Quality-of-Life (QoL) wurde mittels FACT-G und des Fatiguefragebogens (FACIT-F) bewertet. Erhebungszeitpunkte waren direkt vor und 8 Tage nach jeder Chemotherapie. Eine statistisch und klinisch signifikante positive Wirkung der gefasteten Zyklen (c1-c3) im Vergleich zu den folgenden Zyklen (c4-c6) auf die QoL/Fatigue zeigte sich in Gruppe A. Entsprechend zeigten Patientinnen in Gruppe B eine geringere Beeinträchtigung der QoL in mit STF unterstützten Hälfte der Chemotherapien (c4-c6). Bei STF war die chemotherapieinduzierte Reduktion der QoL geringer als die MID (definitionsgemäß MID(FACT-G) ≤5), während die MID für normokalorische Zyklen bei >5 lag. Der Durchschnitt der MID von FACIT-F betrug 10,1 (c1-c3) und 24,4 (c4-c6) für Gruppe A und 13,0 (c1-c3) und 10,8 (c4-c6) für Gruppe B. Es gab keine schwerwiegenden Nebenwirkungen des Fastens. Fazit: STF während der Chemotherapie ist gut verträglich und scheint QoL und Fatigue während der Chemotherapie zu verbessern. Größere Studien sind notwendig, um die vielversprechende Wirkung von STF als Ergänzung zur Chemotherapie weiter zu belegen.This pilot study investigated the feasibility and effects of STF during chemotherapy in patients with gynecological cancer. Methods: In an individually-randomized cross-over trial 34 patients with breast and ovarian cancer with 4-6 planned chemotherapy cycles were included. 34 patients (median age 51y(28-69y)) were randomized to STF the first half of chemotherapies followed by normocaloric diet (group A; n=18) or vice versa (group B; n=16). Fasting started 36h before and ended 24h after chemotherapy (60h-fasting period). QoL was assessed by the FACIT-measurement system including the general (FACT-G) and the fatigue questionnaire (FACIT-F). Outcomes were assessed baseline, before and 8 days after each chemotherapy. A statistically and clinically significant beneficial effect of the fasted cycles (c1-c3) versus the following cycles (c4-c6) on QoL/fatigue was found in group A. Respectively, patients in group B showed less impairment of QoL in the fasted second half of chemotherapies. With STF the chemotherapy-induced reduction of QoL was less than the Minimally Important Difference (<MID(FACT-G)=5) whereas for non-fasted periods the differences were greater than MID. The average of mean differences of FACIT-F was 10.1(c1-c3) and 24.4(c4-c6) for group A and 13.0(c1-c3) and 10.8(c4-c6) for group B. There were no serious adverse effects of fasting. Conclusion: STF during chemotherapy is well tolerated and appears to improve QoL and fatigue during chemotherapy. Larger studies are warranted to prove the promising effect of STF as an adjunct to chemotherapy

    La reducción de accidentes eléctricos por contacto indirecto fue bastante elevada, pasando de 18.048 siniestros en 2.008 a 7.487 en el 2.014, es decir, se produjeron un 58,52% menos , aunque hay que tener en cuenta que también se redujo el número de persones trabajando. La probabilidad que tener un accidente eléctrico por contacto indirecto fue menor en 2014 que en el 2008, exactamente del 0,76 ( 0,74-0,77). Sin embargo pasó el contrario con los accidentes eléctricos por contacto directo, que fue de 1,22 (1,20 – 1,24) , que significa que el riesgo por persona de sufrir una electrocución fue más elevado para los trabajadores en el año 2.014 que en el 2.008Peer ReviewedPostprint (published version

    Medical Safety and Device Reliability of Active Transcutaneous Middle Ear and Bone Conducting Implants: A Long-Term Multi-Centre Observational Study

    Active bone-conducting hearing devices (aBCHD; e.g., MEDEL Bonebridge® (BB)) and active middle ear implants (aMEI; e.g., MEDEL Vibrant Soundbridge® (VSB)) use radio frequency transmission to send information from an external microphone and sound processor to an internally implanted transducer. These devices potentially have an advantage over devices with percutaneous links because the skin is closed over the implantable components, which should reduce the risk of skin problems and infection. On the other hand, surgical procedures are more complex, with a greater risk of damage due to surgery. The objectives of this research were to quantify the reliability and long-term survival of MEDEL VSB and BB devices, determine the adverse and serious adverse device-related complications, and consider associated causes. A multi-center observational retrospective and prospective study was conducted at eleven auditory implant centers in the United Kingdom. Data was collected using a surgical questionnaire and audiological reports. Data were obtained from patient notes or from prospective cases that had a minimum follow-up of one year post-implant. Consecutive patient records were reviewed. Datasets from 109 BB and 163 VSB were reviewed. Of these, 205 were retrospective case note reviews, and 67 were prospective cases. The mean follow-up was 4 and 6 years, respectively, for BB and VSB. Kaplan–Meier Survival analyses indicated that the BB survival was 97% and 93.3% at 1 and 5 years, respectively, and the VSB was 92.1% and 87% at the same time points. This is a large cohort study for the field and has indicated that BB and VSB are safe interventions. Care should be taken to monitor magnet strength in the first few months. For the majority of device-related effects, there was no apparent association with etiology. However, an interesting pattern emerged for individuals who exhibited an inflammatory response, e.g., adhesions or device extrusion, and those with a history of chronic suppurative otitis media. This should be considered in future work and is not surprising given that many VSB recipients have a complicated hearing history, often associated with otitis media

    Graph Regionalization with Clustering and Partitioning: an Application for Daily Commuting Flows in Albania

    The paper presents an original application of the recently proposed spatial data mining method named GraphRECAP on daily commuting flows using 2011 Albanian census data. Its aim is to identify several clusters of Albanian municipalities/communes; propose a classification of the Albanian territory based on daily commuting flows among municipalities/communes. Starting from 373 local units, we first applied a spatial clustering technique without imposing any constraining strategy. Based on the input variables, we obtained 16 clusters. In the second step of our analysis, we impose a set of constraining parameters to identify intermediate areas between the local level (municipality/commune) and the national one. We have defined 12 derived regions (same number as the actual Albanian prefectures but with different geographies). These derived regions are quite different from the traditional ones in terms of both geographical dimensions and boundarie
