    Assessing the water, sanitation and hygiene needs of people living with HIV and AIDS in Papua New Guinea

    People living with HIV and AIDS (PLHIV) are susceptible to opportunistic infections, and access to safe water, adequate sanitation and hygiene (WASH) are crucial to safeguarding the health of PLHIV and their families. This study was designed to gain an understanding of how WASH affects the lives of PLHIV in Papua New Guinea (PNG). The research was conducted in partnership with WaterAid and Anglicare PNG. Anglicare staff administered surveys to 93 respondents across urban, peri-urban and rural PNG. The research revealed that PLHIV in PNG have increased needs for WASH, and that these needs are not being adequately met. Stigma and discrimination were found to be barriers to access to WASH for respondents and their families. In addition, this study identified priorities of where service providers can direct their future activities and best integrate WASH into programming for PLHIV

    Hoffa’s fat pad thickness: a measurement method with sagittal MRI sequences

    Background: Hoffa’s fat pad is a structure located within the fibrous joint capsule of the knee joint, but outside the synovial cavity. It plays an important biomechanical and metabolic role in knee joint, reducing the impact of forces generated by loading and producing cytokines. Changes in its size can induce modifications in the knee homeostasis. However, a great variability exists regarding its measurements. This work aims to evaluate the reliability of a measurement method of Hoffa’s fat pad dimensions through MRI. Methods: 3T sagittal IW 2D TSE fat-suppressed MRI sequences, taken from the OAI (Osteoarthritis initiative) database, of 191 male and female patients, aged between 40 and 80 years, were analysed; a manual measurement of the thickness of Hoffa’s fat pad of each subject was then performed by two different readers. The interobserver reliability and intraobserver reliability of the measurements were described by coefficient of variation (CV), Pearson correlation and Bland–Altman plots. Results: All statistical analyses have shown that not significant intra- or interobservers differences were evident (intraobserver CV % for the first observer was 2.17% for the right knee and 2.24% for the left knee, while for the second observer 2.31% for the right knee and 2.24% for the left knee; linear correlation was for the first observer r = 0.96 for the right knee and r = 0.96 for the left knee, while for the second observer r = 0.97 for the right knee and r = 0.96 for the left knee; in addition, the interobserver CV % was 1.25% for the right knee and 1.21% for the left knee and a high interobserver linear correlation was found: r = 0.97 for the right knee and r = 0.96 for the left knee). All results suggest that this manual measurement method of Hoffa’s fat pad thickness can be performed with satisfactory intra- and interobserver reliability. Conclusions: Hoffa’s fat pad thickness can be measured, using sagittal MRI images, with this manual method that represents, for his high reliability, an effective means for the study of this anatomical structure

    Pet ownership and cognitive decline in older people

    Animals can have a positive influence on human health. However, it is not yet known whether pet ownership can prevent cognitive decline. Therefore, we aimed to investigate cross-sectional and prospective associations between pet ownership and cognitive function in a large, representative sample of older adults. Data were from the English Longitudinal Study of Ageing (ELSA) using data collected in wave 5 and six years later in wave 8. Pet ownership was categorized as no pet, dog, cat or other pet. Cognitive function was assessed using tests of verbal fluency (assessed by asking how many different animals the participants could name in 60 seconds) and memory (sum of immediate and delayed verbal recall). Multiple linear regression, adjusted for potential confounders, was used to test the associations between pet ownership and cognitive function. A total of 8291 people (mean age: 66.72 years) were included. In cross-sectional analyses, dog owners had better verbal fluency than individuals with no pet, but there was no significant difference between cat or other pet owners and those with no pet. In prospective analyses, dog owners had a significantly larger decline in recall than those with no pet, whilst cat owners had a significantly smaller decline in verbal fluency. These results provide some evidence to suggest that pet ownership may have positive effects on cognition in later life. However, benefits of pet ownership were not unilaterally observed across different types of pet and measures of cognitive function suggesting that further research is required

    Our objective is to review the pathophysiology and treatment of the nephrotic syndrome. This article includes definitions and causes of nephrotic syndrome, and an assessment of the pathophysiology of the nephrotic edema as well as of the causes and treatment of refractory edema. An algorithm for the treatment nephrotic edema is proposed taking into account the complexity of refractory edema. We also address measures aimed at reducing proteinuria, such as the use of angiotensin-converting enzyme inhibitors, hypoprotein diet, and the roles of non-steroidal anti-inflammatory drugs in this context. We also review the complications of nephrotic syndrome such as hyperlipidemia and the hypercoagulability with regards to their pathophysiology and current treatment.Foram revisados aspectos da fisiopatologia e do tratamento sindrômico do estado nefrótico. São citados o conceito e as causas de síndrome nefrótica. Foram revisadas a fisiopatologia do edema, bem como as causas e o tratamento do edema refratário. É apresentado um fluxograma para o manejo clínico do edema nefrótico, considerando a complexidade do edema refratário. Foram abordadas medidas para redução de proteinúria, como o uso de inibidores da enzima de conversão da angiotensina, a dieta hipoproteica e o papel dos antiinflamatórios não esteróides neste contexto. Foram revistas as complicações da síndrome nefrótica, como dislipidemia e hipercoagulabilidade, no que concerne à sua fisiopatologia e tratamento atual

    Mortality attributable to COVID-19 in nursing home residents: a retrospective study

    Aim: Coronavirus-19 disease (COVID-19) is a widespread condition in nursing home (NH). It is not known whether COVID-19 is associated with a higher risk of death than residents without COVID-19. Therefore, the aim of this study was to assess whether COVID-19 is associated with a higher mortality rate in NH residents, considering frailty status assessed with the Multidimensional Prognostic Index (MPI). Methods: In this retrospective study, made in 31 NHs in Venice, Italy, the presence of COVID-19 was ascertained with a nasopharyngeal swab. Frailty was evaluated using the MPI, modified according to the tools commonly used in our NHs. A Cox’s regression analysis was used reporting the results as hazard ratios (HRs) with 95% confidence intervals (CIs), using COVID-19 as exposure and mortality as outcome and stratified by MPI tertiles. Similar analyses were run using MPI tertiles as exposure. Results: Overall, 3946 NH residents (median age = 87 years, females: 73.9%) were eligible, with 1136 COVID-19 +. During a median follow-up of 275 days, higher values of MPI, indicating frailer people, were associated with an increased risk of mortality. The incidence of mortality in COVID-19 + was more than doubled than COVID-19- either in MPI-1, MPI-2 and MPI-3 groups. The presence of COVID-19 increased the risk of death (HR = 1.85; 95% CI 1.59–2.15), also in the propensity score model using MPI as confounder (HR = 2.48; 95% CI 2.10–2.93). Conclusion: In this retrospective study of NH residents, COVID-19 was associated with a higher risk of all-cause mortality than those not affected by COVID-19 also considering the different grades of frailty. © 2021, The Author(s)

    Long-term intensive endurance exercise training is associated to reduced markers of cellular senescence in the colon mucosa of older adults

    Regular endurance exercise training is an effective intervention for the maintenance of metabolic health and the prevention of many age-associated chronic diseases. Several metabolic and inflammatory factors are involved in the health-promoting effects of exercise training, but regulatory mechanisms remain poorly understood. Cellular senescence—a state of irreversible growth arrest—is considered a basic mechanism of aging. Senescent cells accumulate over time and promote a variety of age-related pathologies from neurodegenerative disorders to cancer. Whether long-term intensive exercise training affect the accumulation of age-associated cellular senescence is still unclear. Here, we show that the classical senescence markers p16 and IL-6 were markedly higher in the colon mucosa of middle-aged and older overweight adults than in young sedentary individuals, but this upregulation was significantly blunted in age-matched endurance runners. Interestingly, we observe a linear correlation between the level of p16 and the triglycerides to HDL ratio, a marker of colon adenoma risk and cardiometabolic dysfunction. Our data suggest that chronic high-volume high-intensity endurance exercise can play a role in preventing the accumulation of senescent cells in cancer-prone tissues like colon mucosa with age. Future studies are warranted to elucidate if other tissues are also affected, and what are the molecular and cellular mechanisms that mediate the senopreventative effects of different forms of exercise training

    Field emission properties of carbon nanotube arrays grown in porous anodic alumina

    AbstractCarbon nanotubes (CNTs), with their excellent electronic properties and extremely high aspect ratio, represent an ideal material for building electron sources based on field emission. Fowler‐Nordheim equation describes quite successfully the field emission phenomenon, especially for single or isolated tips. However, some complications arise when populations of CNTs are considered, where collective effects and large variability in the emitters features influence the measured I–V characteristics. In this work, the emission properties of multi‐walled CNTs grown within ordered anodic alumina templates are investigated. These CNT matrices produce current densities up to some tens of mA/cm2, and the field enhancement factor for collective emission sources can be estimated. Such material can be modelled as an ordered and uniform array of emitters and a simulation of the electrostatic field on the emission tips can be done in order to evaluate the field enhancement factor and its dependence on various geometries. This allows comparing predictions from simulation and experimental measurements, in a direct way. (© 2009 WILEY‐VCH Verlag GmbH & Co. KGaA, Weinheim

    Doença renal ateroembólica: uma causa de insuficiência renal aguda pouco explorada

    O ateroembolismo é uma doença multisistêmica que afeta vários órgãos, entre os quais o rim, através da liberação de êmbolos de colesterol de uma placa aterosclerótica erosada, ocasionando obstrução vascular em diversos tecidos. A doença renal ateroembólica (DRAE), histologicamente representada por cristais de colesterol nas arteríolas do rim acompanhados de um infiltrado inflamatório perivascular, é causa de insuficiência renal aguda muitas vezes grave e prolongada, que ocorre semanas ou mesmo meses após o episódio embólico. A DRAE apresesenta prognóstico ruim com elevada mortalidade. Apresentamos neste relato o caso de um paciente com DRAE que se manifestou clinicamente dois meses após a realização de um cateterismo cardíaco seguido de uma angioplastia coronária. A prevalência, manifestações clínicas, histologia renal, tratamento e o prognóstico da DRAE são discutidos.Atheroembolism is a multisytemic disease which affects many organs, including the kidneys, by the release of cholesterol emboli to tissues from an erosed atherosclerotic plaque, causing vascular obstruction in many tissues. The atheroembolic renal disease (AERD) is histologically represented by cholesterol crystals in renal arterioles with an inflammatory infiltrate around the vessels, and causes acute renal failure that may be severe and prolonged, weeks or even months after the embolic episode. The AERD carries a bad prognosis, with a high mortality. We herein report a case of a patient presenting AERD which was manifested two months after he was submitted to a cardiac catheterism and coronary angioplasty. The prevalence, clinical findings, renal histology, treatment and prognosis of AERD are discussed

    Microglial activity in people at ultra high risk of psychosis and in schizophrenia; an [11C]PBR28 PET brain imaging study

    Objective: The purpose of this study was to determine whether microglial activity, measured using translocator-protein positron emission tomography (PET) imaging, is increased in unmedicated persons presenting with subclinical symptoms indicating that they are at ultra high risk of psychosis and to determine whether microglial activity is elevated in schizophrenia after controlling for a translocator-specific genetic polymorphism. Method: The authors used the second-generation radioligand [11C]PBR28 and PET to image microglial activity in the brains of participants at ultra high risk for psychosis. Participants were recruited from early intervention centers. The authors also imaged a cohort of patients with schizophrenia and matched healthy subjects for comparison. In total, 56 individuals completed the study. At screening, participants were genotyped to account for the rs6971 polymorphism in the gene encoding the 18Kd translocator protein. The main outcome measure was total gray matter [11C]PBR28 binding ratio, representing microglial activity. Results: [11C]PBR28 binding ratio in gray matter was elevated in ultra-high-risk participants compared with matched comparison subjects (Cohen’s d >1.2) and was positively correlated with symptom severity (r=0.730). Patients with schizophrenia also demonstrated elevated microglial activity relative to matched comparison subjects (Cohen’s d >1.7). Conclusions: Microglial activity is elevated in patients with schizophrenia and in persons with subclinical symptoms who are at ultra high risk of psychosis and is related to at-risk symptom severity. These findings suggest that neuroinflammation is linked to the risk of psychosis and related disorders, as well as the expression of subclinical symptoms
