    Role of yogic practices in individuals with hypertension and low-Peak Expiratory Flow Rate (PEFR) of Ahmedabad city

    589-594Hypertension is one of the most important risk factors for various heart related diseases in India, especially in South-Asian region. Nowadays because of very fast life style, breathing pattern and its duration is changed considerably. Breathing duration becomes very short. The main aim of the present study was to assess the therapeutic role of yoga on various cardiovascular parameters, peak expiratory flow rate (PEFR) through pulmonary function test and peripheral capillary oxygen saturation (SpO2), amount of oxygen in the blood in Ahmedabad population. Total 50 individuals with hypertension, low-PEFR and low-SpO2 were selected for the present study. All participants were subjected to yoga therapy (pranayama, yoga postures and meditation) for various time intervals of 0, 3, 6, 9, and 12 months. Heart rate (HR), systolic pressure (SP), diastolic pressure (DP), pulse pressure (PP), mean arterial pressure (MAP), rate pressure product (RPP), double product (DoP), PEFR and SpO2 were measured from all individuals at different intervals. At 0 month, all individuals had very high heart rate (HR), systolic pressure (SP), diastolic pressure (DP), pulse pressure (PP), mean arterial pressure (MAP), rate pressure product (RPP), double product (DoP), but PEFR and SpO2 levels were very low. At the end of 12 month of yoga intervention, significant decrease in all cardiovascular parameters whereas significant elevation of PEFR and SpO2 levels were observed. In conclusion, a comprehensive yoga therapy programme has immense potential to augment the beneficial effects of standard medical management of hypertension, lungs function and total oxygen concentration. Hence it can be used as an effective complementary therapy for heart related diseases

    Plantar loading during jumping while wearing a rigid carbon graphite footplate

    Fifth metatarsal stress fractures are common in sports and often result in delayed and non-union. The purpose of this study was to examine the effect of a rigid carbon graphite footplate (CGF) on plantar loading during take-off and landing from a jump. Nineteen recreational male athletes with no history of lower extremity injury in the past 6 months and no foot or ankle surgery in the past 3 years participated in this study. Subjects completed 7 jumping tasks while wearing a standard running shoe and then the shoe plus the CGF while plantar loading data was recorded. A series of paired t-tests were used to examine differences between the two footwear conditions independently for both takeoff and landing (α=0.05). The contact area in the medial midfoot (p<.001) and forefoot (p=.010) statistically decreased when wearing the CGFP. The force-time integral was significantly greater when wearing the CGFP in the middle (p<.001) and lateral forefoot (p=.019). Maximum force was significantly greater beneath the middle (p<.001) and lateral forefoot (p<.001) when wearing the CGFP, while it was decreased beneath the medial midfoot (p<.001). During landing, the contact area beneath the medial (p=.017) and lateral midfoot (p=.004) were significantly decreased when wearing the CGFP. The force-time integral was significantly decrease beneath the medial midfoot (p<.001) when wearing the CGFP. The maximum force was significantly greater beneath the medial (p=.047) and middle forefoot (p=.001) when the subject was wearing the CGFP. The maximum force beneath the medial midfoot (p<.001) was significantly reduced when wearing the carbon graphite footplate. The results of the study indicate that the CGF is ineffective at reducing plantar loading during jumping and landing. © 2013 Elsevier B.V

    Plantar Loading During Cutting While Wearing a Rigid Carbon Fiber Insert.

    Context : Stress fractures are one of the most common injuries in sports, accounting for approximately 10% of all overuse injuries. Treatment of fifth metatarsal stress fractures involves both surgical and nonsurgical interventions. Fifth metatarsal stress fractures are difficult to treat because of the risks of delayed union, nonunion, and recurrent injuries. Most of these injuries occur during agility tasks, such as those performed in soccer, basketball, and lacrosse. Objective : To examine the effect of a rigid carbon graphite footplate on plantar loading during 2 agility tasks. Design :  Crossover study. Setting : Laboratory. Patients or Other Participants : A total of 19 recreational male athletes with no history of lower extremity injury in the past 6 months and no previous metatarsal stress fractures were tested. Main Outcome Measure(s) :  Seven 45° side-cut and crossover-cut tasks were completed in a shoe with or without a full-length rigid carbon plate. Testing order between the shoe conditions and the 2 cutting tasks was randomized. Plantar-loading data were recorded using instrumented insoles. Peak pressure, maximum force, force-time integral, and contact area beneath the total foot, the medial and lateral midfoot, and the medial, middle, and lateral forefoot were analyzed. A series of paired t tests was used to examine differences between the footwear conditions (carbon graphite footplate, shod) for both cutting tasks independently (α = .05). Results : During the side-cut task, the footplate increased total foot and lateral midfoot peak pressures while decreasing contact area and lateral midfoot force-time integral. During the crossover-cut task, the footplate increased total foot and lateral midfoot peak pressure and lateral forefoot force-time integral while decreasing total and lateral forefoot contact area. Conclusions : Although a rigid carbon graphite footplate altered some aspects of the plantar- pressure profile during cutting in uninjured participants, it was ineffective in reducing plantar loading beneath the fifth metatarsal

    Study of therapeutic role of yoga (Hathyoga) on lipid profile in dyslipidemic individuals of Ahmedabad city

    333-338Abnormal lipid profile is a common problem among hypertensive as well as working individuals. The rate of mortality is increasing day-by-day due to cardiovascular problems which occurs due to dyslipidemia. The aim of the present study was to assess the therapeutic role of yoga on lipid profile in Ahmedabad population. Total 50 normal healthy control and 50 individuals with dyslipidemia aged >20 years were enrolled for the present study. They were divided into two groups. Individuals in Group-1 were normal healthy, whereas individuals in Group-2 were dyslipidemics. All participants were subjected to yoga practices (Hathyogic practices - Pranayama, yoga postures and meditation) for the various intervals of 0, 3, 6, 9, and 12 months. Serum lipid profile was estimated for all individuals at different intervals. Before beginning the yoga intervention i.e., at 0 month the levels of Total Cholesterol, Total Triglycerides, LDL-cholesterol and VLDL-cholesterol were significantly high, whereas HDL-cholesterol levels were significantly low. After completion of 12 months yoga intervention a significant reduction was observed in Total Cholesterol, Total Triglycerides, LDL-cholesterol and VLDL-cholesterol as well as a significant elevation of HDL-cholesterol was observed. Yoga (Hathyoga) can be a new added adjuvant and cost effective therapy for the patients with abnormal lipid profile

    Green fluorescent protein-based expression screening of membrane proteins in Escherichia coli.

    The production of recombinant membrane proteins for structural and functional studies remains technically challenging due to low levels of expression and the inherent instability of many membrane proteins once solubilized in detergents. A protocol is described that combines ligation independent cloning of membrane proteins as GFP fusions with expression in Escherichia coli detected by GFP fluorescence. This enables the construction and expression screening of multiple membrane protein/variants to identify candidates suitable for further investment of time and effort. The GFP reporter is used in a primary screen of expression by visualizing GFP fluorescence following SDS polyacrylamide gel electrophoresis (SDS-PAGE). Membrane proteins that show both a high expression level with minimum degradation as indicated by the absence of free GFP, are selected for a secondary screen. These constructs are scaled and a total membrane fraction prepared and solubilized in four different detergents. Following ultracentrifugation to remove detergent-insoluble material, lysates are analyzed by fluorescence detection size exclusion chromatography (FSEC). Monitoring the size exclusion profile by GFP fluorescence provides information about the mono-dispersity and integrity of the membrane proteins in different detergents. Protein: detergent combinations that elute with a symmetrical peak with little or no free GFP and minimum aggregation are candidates for subsequent purification. Using the above methodology, the heterologous expression in E. coli of SED (shape, elongation, division, and sporulation) proteins from 47 different species of bacteria was analyzed. These proteins typically have ten transmembrane domains and are essential for cell division. The results show that the production of the SEDs orthologues in E. coli was highly variable with respect to the expression levels and integrity of the GFP fusion proteins. The experiment identified a subset for further investigation

    Effect of cold start on engine performance and emissions from diesel engines using IMO-Compliant distillate fuels

    © 2019 Elsevier Ltd Emissions from ships at berth are small compared to the total ship emissions; however, they are one of the main contributors to pollutants in the air of densely-populated areas, consequently heavily affecting public health. This is due to auxiliary marine engines being used to generate electric power and steam for heating and providing services. The present study has been conducted on an engine representative of a marine auxiliary, which was a heavy duty, six-cylinder, turbocharged and after-cooled engine with a high pressure common rail injection system. Engine performance and emission characterisations during cold start are the focus of this paper, since cold start is significantly influential. Three tested fuels were used, including the reference diesel and two IMO (International Maritime Organization) compliant spiked fuels. The research engine was operated at a constant speed and 25% load condition after 12 h cooled soak. Results show that during cold start, significant heat generated from combustion is used to heat the engine block, coolant and lubricant. During the first minute, compared to the second minute, emissions of particle number (PN), carbon monoxide (CO), particulate matter (PM), and nitrogen oxides (NOx) were approximately 10, 4, 2 and 1.5 times higher, respectively. The engine control unit (ECU) plays a vital role in reducing engine emissions by changing the engine injection strategy based on the engine coolant temperature. IMO-compliant fuels, which were higher viscosity fuels associated with high sulphur content, resulted in an engine emission increase during cold start. It should be taken into account that auxiliary marine diesel engines, working at partial load conditions during cold start, contribute considerably to emissions in coastal areas. It demonstrates a need to implement practical measures, such as engine pre-heating, to obtain both environmental and public health advantages in coastal areas

    “Cycles upon cycles, stories upon stories” : contemporary audio media and podcast horror’s new frights

    During the last ten years the ever-fertile horror and Gothic genres have birthed a new type of fright-fiction: podcast horror. Podcast horror is a narrative horror form based in audio media and the properties of sound. Despite association with oral ghost tales, radio drama, and movie and TV soundscapes, podcast horror remains academically overlooked. Podcasts offer fertile ground for the revitalization and evolution of such extant audio-horror traditions, yet they offer innovation too. Characterized by their pre-recorded nature, individualized listening times and formats, often “amateur” or non-corporate production, and isolation from an ongoing media stream more typical of radio or TV, podcasts potentialize the instigation of newer audio-horror methods and traits. Podcast horror shows vary greatly in form and content, from almost campfire-style oral tales, comprising listener-produced and performed content (Drabblecast; Tales to Terrify; NoSleep); to audio dramas reminiscent of radio’s Golden Era (Tales from Beyond the Pale; 19 Nocturne Boulevard); to dramas delivered in radio-broadcast style (Welcome to Night Vale; Ice Box Theatre); to, most recently, dramas, which are themselves acknowledging and exploratory of the podcast form (TANIS; The Black Tapes Podcast; Lime Town). Yet within this broad spectrum, sympathies and conventions arise which often not only explore and expand notions of Gothic sound, but which challenge broader existing horror and Gothic genre norms. This article thus demonstrates the extent to which podcast horror uses its audio form, technology and mediation to disrupt and evolve Gothic/horror fiction, not through a cumulative chronological formulation of podcast horror but through a maintained and alternately synthesized panorama of forms. Herein new aspects of generic narration, audience, narrative and aesthetic emerge. Exploring a broad spectrum of American and British horror podcasts, this article shows horror podcasting to utilize podcasting’s novel means of horror and Gothic distribution/consumption to create fresh, unique and potent horror forms.     The association of circulating amylin with β-amyloid in familial Alzheimer's disease.

    INTRODUCTION: This study assessed the hypothesis that circulating human amylin (amyloid-forming) cross-seeds with amyloid beta (Aβ) in early Alzheimer's disease (AD). METHODS: Evidence of amylin-AD pathology interaction was tested in brains of 31 familial AD mutation carriers and 20 cognitively unaffected individuals, in cerebrospinal fluid (CSF) (98 diseased and 117 control samples) and in genetic databases. For functional testing, we genetically manipulated amylin secretion in APP/PS1 and non-APP/PS1 rats. RESULTS: Amylin-Aβ cross-seeding was identified in AD brains. High CSF amylin levels were associated with decreased CSF Aβ42 concentrations. AD risk and amylin gene are not correlated. Suppressed amylin secretion protected APP/PS1 rats against AD-associated effects. In contrast, hypersecretion or intravenous injection of human amylin in APP/PS1 rats exacerbated AD-like pathology through disruption of CSF-brain Aβ exchange and amylin-Aβ cross-seeding. DISCUSSION: These findings strengthened the hypothesis of circulating amylin-AD interaction and suggest that modulation of blood amylin levels may alter Aβ-related pathology/symptoms

    A systematic review of strategies to recruit and retain primary care doctors

    Background There is a workforce crisis in primary care. Previous research has looked at the reasons underlying recruitment and retention problems, but little research has looked at what works to improve recruitment and retention. The aim of this systematic review is to evaluate interventions and strategies used to recruit and retain primary care doctors internationally. Methods A systematic review was undertaken. MEDLINE, EMBASE, CENTRAL and grey literature were searched from inception to January 2015.Articles assessing interventions aimed at recruiting or retaining doctors in high income countries, applicable to primary care doctors were included. No restrictions on language or year of publication. The first author screened all titles and abstracts and a second author screened 20%. Data extraction was carried out by one author and checked by a second. Meta-analysis was not possible due to heterogeneity. Results 51 studies assessing 42 interventions were retrieved. Interventions were categorised into thirteen groups: financial incentives (n=11), recruiting rural students (n=6), international recruitment (n=4), rural or primary care focused undergraduate placements (n=3), rural or underserved postgraduate training (n=3), well-being or peer support initiatives (n=3), marketing (n=2), mixed interventions (n=5), support for professional development or research (n=5), retainer schemes (n=4), re-entry schemes (n=1), specialised recruiters or case managers (n=2) and delayed partnerships (n=2). Studies were of low methodological quality with no RCTs and only 15 studies with a comparison group. Weak evidence supported the use of postgraduate placements in underserved areas, undergraduate rural placements and recruiting students to medical school from rural areas. There was mixed evidence about financial incentives. A marketing campaign was associated with lower recruitment. Conclusions This is the first systematic review of interventions to improve recruitment and retention of primary care doctors. Although the evidence base for recruiting and care doctors is weak and more high quality research is needed, this review found evidence to support undergraduate and postgraduate placements in underserved areas, and selective recruitment of medical students. Other initiatives covered may have potential to improve recruitment and retention of primary care practitioners, but their effectiveness has not been established
