Sprendimų anatomija: praktinių įgūdžių lavinimas
Įvairius sprendimus priimame ir jų padarinių patiriame kasdien – tiek asmeniniame gyvenime, tiek darbe. Kai kuriems sprendimams priimti pakanka kasdienių žinių ir asmeninės patirties. Jų padariniai dažniausiai nėra ilgalaikiai ar ypač reikšmingi, todėl galime leisti sau suklysti ir nepatirdami didelių nuostolių, ką nors nuspręsti tiesiog spėliodami. Deja, kai kurie sprendimai gali būti labai reikšmingi, o jų padariniai gali brangiai kainuoti ir paveikti ne tik mus asmeniškai, bet ir gerokai daugiau žmonių ar net organizacijų. Tokios situacijos dažnai būna baigtinės, nesikartojančios, o jų padarinių nebegalima pakeisti, todėl kartais priimant sprendimus tenka naudotis jau sukurtais ir praktikos patikrintais standartiniais būdais. Tad sprendinių anatomijos išmanymas ne tik apsaugo nuo nenumatytų padarinių, bet ir sudaro galimybę elgtis racionaliai ir gauti didžiausią įmanomą naudą. Tiesa, toks efektas galimas tik tam tikromis aplinkybėmis, kurios apibrėžiamos vienareikšmiškai: kol sprendžiame problemą, informacija ir duomenys nesikeičia. Tada geriausias sprendinys – vienintelis teisingas ir racionalus pasirinkimas. Kitais atvejais, kai sprendimui priimti reikalinga informacija stochastiškai kinta, sprendinys taip pat gali būti geriausias, tačiau lieka tik tam tikra tikimybė, kad jis bus priimtas. Itin sudėtingomis aplinkybėmis, kai duomenų neturime, jų negalime surinkti arba jie nuolat keičiasi ir nebūtinai kartojasi, geriausio sprendinio rasti negalima, nes jo nėra. Tada sprendėjas ar jų komanda gali ieškoti ne geriausio, bet priimtino varianto. Kyla naujų iššūkių, reikia apsispręsti, kas yra priimtinas sprendinys, koks priimtinumo lygis tenkina, kas tas subjektas, į kurį atsižvelgiant vertinamas priimtinumo lygis
Electronic Health System Evaluation Model
The aim of dissertation “Electronic Health System Evaluation Model” is better understand the determinants of an effectiveness and ineffectiveness e-health system, to identify users expectations and problematic aspects in the development and implementation of an e-health system. On this basis was developed an e-health system evaluation model, which reflects the objective and subjective needs of all users in different stages of e-health system implementation, evaluation criteria and measures to achieve this goal were established, and the Lithuanian e-health system was evaluated using the developed model. In information systems research, there is a tendency to base the evaluation of the e-health system mainly on economic and technical criteria, which are rationally objective, but do not respond sufficiently to the specific needs of users. The dissertation pays special attention to social and organizational evaluation criteria, which create a more comprehensive picture of e-health system cognition. The developed e-health system evaluation model includes technical, economic, social and organizational criteria, which are focused on the goal of the e-health system, can be measured regardless of the actual moment of e-health system implementation.Disertacijoje „Elektroninės sveikatos sistemos vertinimo modelis“ siekiama geriau suprasti veiksmingos ir neveiksmingos e.sveikatos sistemos lemiančius veiksnius, nustatyti naudotojų lūkesčius ir probleminius aspektus, kuriant ir diegiant e.sveikatos sistemą. Tuo pagrindu suformuotas e.sveikatos sistemos vertinimo modelis, kuris atspindi objektyvius ir subjektyvius visų naudotojų poreikius skirtingose e.sveikatos sistemos diegimo etapuose, nustatyti vertinimo kriterijai ir priemonės šiam tikslui pasiekti, bei parengtu modeliu įvertinta Lietuvos e.sveikatos sistema. Informacinių sistemų tyrimuose vyrauja tendencijos e.sveikatos sistemos vertinimą daugiausiai grįsti ekonominiais ir techniniais kriterijais, kurie yra racionaliai objektyvūs, tačiau nepakankamai kryptingai atliepia naudotojų specifinius poreikius. Disertacijoje ypatingas dėmesys skiriamas socialiniams ir organizaciniams vertinimo kriterijams, kurie sukuria visapusiškesnį e.sveikatos sistemos pažinimo vaizdą. Parengtas e.sveikatos sistemos vertinimo modelis apima techninius, ekonominius, socialinius ir organizacinius kriterijus, kurie orientuoti į e.sveikatos sistemos tikslą, gali būti matuojami nepriklausomai nuo faktinio e.sveikatos sistemosįgyvendinimo momento
Organizational capital theory expression of electronic health
Implementation of e-health is a very difficult and complex process
in terms of health policy that requires both organizational ability to meet market
requirements and well-managed internal communication, which is carried out
through organizational capital and high organizational culture and philosophy.
This process requires a change in the organizational processes of health care institutions,
ensuring the management and use of health information in order to
improve the functioning of health care institutions.
Scientists emphasize that health care institutions, with a high organizational
capital can effectively promote the development of e-health, by consistently improving
the quality of healthcare services and increasing the confidence and sense
of security by enhancing the individual‘s ability to become more involved in the
health care system. Nevertheless, in Lithuania, the progress of health care institutions
in the field of e-health varies significantly, therefore, different tendencies of ehealth
care indicators tend to be related to organizational capital. Organizational
capital covering the various dimensions of social context analysis, and organizational
capital theory is a paradigm that attempts to explain the e-health inequalities
at the level of organizations.
This paper has evaluated the organizational capital theory expression of the
e-health level, to identify and analyze organizational capital assessment indices in
the field of e-health and modeled organizational capital assessment scheme.Elektroninės sveikatos (toliau – e. sveikata) diegimas yra labai sudėtingas ir
kompleksinis procesas sveikatos politikos įgyvendinimo prasme, reikalaujantis
tiek organizacinių gebėjimų atitikti rinkos reikalavimus, tiek gerai valdomos vidinės
komunikacijos, kuri vykdoma per organizacinį kapitalą (toliau – OK), tiek
aukštos organizacinės kultūros ir filosofijos. Šis procesas reikalauja sveikatos priežiūros
įstaigų (toliau – SPĮ) organizacinių procesų pakeitimo, užtikrinant sveikatos
informacijos valdymą ir naudojimą, siekiant pagerinti SPĮ veiklą.
Mokslininkai pabrėžia, kad SPĮ, turinčioms didelį organizacinį kapitalą, gali
efektyviau skatinti e. sveikatos plėtrą, nuosekliai gerinant sveikatos priežiūros paslaugų
kokybę bei didinant pasitikėjimo ir saugumo jausmą, gerinant individo galimybes
labiau įsitraukti į sveikatos priežiūros sistemą. Nepaisant to, Lietuvoje yra
labai skirtingi e. sveikatos pasiekimai SPĮ, todėl skirtingus e. sveikatos priežiūros
rodiklių rezultatus linkę sieti su organizaciniu kapitalu. Organizacinio kapitalo
dimensijos taikomos įvairioms socialinėmis aplinkybėmis analizuoti, todėl organizacinio
kapitalo teorija tampa paradigma, kuria bandoma paaiškinti e. sveikatos
nelygybę organizacijų lygmenyje.

Knowledge richness for electronic health development: the trap of excessive knowledge for usability of electronic prescription
Objectives: The present research has a significant value for the theory of and research in health informatics and clinical practice. The aim of the article was to investigate how important is knowledge richness andlinkage as knowledge quality for theinformation technology system success decomposing existing electronic Prescription systems in the context of comparison regarding the building process. Methods:The four most actively used Lithuanian electronic Prescription modules have been selected for the research. To achieve the purpose, decomposition of electronic Prescription processes have been carried out to assess relations between knowledge richness and richness adopted in the electronic Prescription module and knowledge usability.The method of heuristic assessment and cognitive walkthroughs was used. The heuristic method is used for the evaluation of an information system by a specialist in the field or by qualified members of the information system development team. Results:A completely developed electronic Prescription system functioning in the entire state has been implemented only in several European countries and Finland is one of the best examples. Despite the fact that several information systems have been developed in Lithuania that prescribes electronic Prescription with 40 million Euros spent on information technology systems, still, 15% of all prescriptions are written on paper in Lithuania. Conclusions:A dynamic selection rule could be apply to address the issue of electronic Prescription use.Such analysiscan help the healthcare administrators and professionals to evaluate the potential of enabling information technology and the opportunity it created to rethink or reengineer the e.Prescription process and the associated activities based on the enabling information technology capability. Aim of the article is to highlight and assess the case manager‘s role in improving performance of health system, the benefits of patients, doctors and nurse, and the influence on quality of services. Analysiss existing organization of primary health care problems and an overview of the Lithuanian health system development framework for 2011-2020, approved by Lithuanian Parliament Seimas, in which problems in the PSP is proposed for case management. In practice, this is still not widespread, and the health institutions care is a novelty. The aim was to try and check usefulnessthe, so case manager project was launched in the Centro Out-patient clinic Performed analysis of the role of case manager will disclose whether it is good, useful and effective

Užsienio šalių praktikoje tai dar nėra plačiai paplitę, o Lietuvos asmens sveikatos priežiūros įstaigose tai naujas dalykas. Siekiant išbandyti ir patikrinti atvejo vadybos naudingumą, bandomasis atvejo vadybos principais paremtas projektas pradėtas įgyvendinti Vilniaus VšĮ Centro poliklinikoje. Tikėtina, kad atliekama atvejo vadybininko vaidmens analizė leis atskleisti, ar šis modelis naudingas. Straipsnio tikslas – apžvelgti egzistuojančius atvejo vadybos modelius ir pristatyti VšĮ Centro poliklinikos atvejo vadybos patirties studiją, įvertinant, kiek šis modelis yra tinkamas ir efektyvus, siekiant gerinti sveikatos sistemos veiklos rezultatyvumą, jo naudą pacientams, gydytojo ir slaugytojo komandai bei įtaką asmens sveikatos priežiūros paslaugų kokybei
Analysis of the Content of Electronic Registration Services at the User’s Views
To measure benefits of e-services is difficult, because of the variety of information
and communications technology and the rapid transformation. In the health care, as in
the context, contributes more confusion, because is it specific area, which includes all groups
of people. Therefore, the provision of public health services is more and more consumer oriented.
No less important is their knowledge about such services and the will and the ability to
use them. This article presents research of electronic regostration service stakeholders in 2016.
Patients and employees participated in the quantitative research, which aimed to reveal their
approach to electronic registration service content and its various aspects. Electronic registration
service application users must be a continuous process, during which need to experiment
with content and analyze the results. This process depends directly on the users, their needs
and on the specifics of electronic services. The will to use electronic services caused by many
factors such as usage patterns and user behavior.Pamatuoti teikiamą elektroninių paslaugų naudą yra sunku dėl informacijos
ir ryšių technologijų taikymo įvairovės bei greitos transformacijos. Sveikatos priežiūros
sferoje atsiranda dar daugiau sunkumų dėl šios srities specifiškumo ir plataus suinteresuotųjų
rato. Todėl teikiant viešąsias elektronines sveikatos paslaugas, vis labiau orientuojamasi
į vartotoją. Ne mažiau svarbu ir vartotojų žinios apie tokias paslaugas, noras ir gebėjimas
jomis naudotis. Straipsnyje pristatomas 2016 m. atliktas elektroninės prisirašymo paslaugos
suinteresuotųjų šalių tyrimas. Pacientai ir sveikatos priežiūros įstaigos darbuotojai dalyvavo
kiekybiniame tyrime, kuriuo siekta atskleisti jų požiūrį į elektroninę prisirašymo paslaugą
ir įvairius jos aspektus. Elektroninės prisirašymo paslaugos taikymas vartotojams turi būti
nuolatinis procesas, kurio metu reikia eksperimentuoti su paslauga ir analizuoti rezultatus.
Šis procesas tiesiogiai priklauso nuo elektroninės paslaugos specifikos ir vartotojų poreikių
bei noro ja naudotis. Norą naudotis elektroninėmis paslaugomis nulemia daugelis veiksnių,
tokių kaip naudojimo įpročiai ir vartotojo elgsena.
Tyrime naudojama aprašomosios statistikos ir neparametrinės statistikos analizė, kurios
pagrindą sudaro tyrime dalyvaujančių kintamųjų tarpusavio ryšys. Keliamos hipotezės, remiantis
demografinių ir kitų aspektų priklausomybe. Statistinė duomenų analizė atskleidė,
kad elektroninės prisirašymo paslaugos elementai yra priklausomi nuo tam tikrų demografinių
aspektų, tokių kaip amžius arba išsilavinimas. Nors elektroninėmis paslaugomis labiau
linkęs pasinaudoti jaunimas (49,1 proc.), negalima eliminuoti nė vieno iš segmentų, nes kiekvienas jų yra potencialus pacientas. Kuriant elektroninę prisirašymo paslaugos vizualizaciją,
reikėtų vadovautis universaliu dizainu, kai kuriama vartotojo aplinka nereikalauja jokio
specialaus pasirengimo ir ja gali naudotis visi: vaikai, suaugusieji, vyrai, moterys, senyvo
amžiaus žmonės, neįgalieji, įvairių tautybių ir kitų skirtumų turintys žmonės
The Impact of Citizen Participation on Public Sentiments during Crises: Comparative Study of Estonia, Latvia, and Lithuania
Citizen participation is identified as one of the essential tools of modern public governance in developing sustainable cities and communities. This is especially important during crises. This analysis revisits the debate regarding citizen participation and its impact on two public sentiments: the sense of security of the citizen and the citizen’s attitude toward the future during a crisis. The aim of this analysis was to discover how the variables of citizen participation in support, decision-making, and physical assistance to state institutions during crises affect these two public sentiments within geographically proximate countries. A cross-country omnibus survey was conducted on a random sample of 2875 citizens in the three Baltic countries: 959 in Estonia, 931 in Latvia, and 985 in Lithuania, respectively. Poisson regression procedures and linear regression analysis models were used for data analysis. Citizen participation was examined as a complex phenomenon manifested in the domains of citizen-led and government-led participation. It was found that the correlation between citizen participation, sense of security, and attitude toward the future varies from country to country despite the same nature of the crisis and despite the geographical proximity of the countries in question. The correlation analysis provided the only exception to trust in government (element of citizen-led participation), which was directly associated with the citizen’s sense of security. In all other cases, the correlations found were country-specific and not engagement-specific. The findings suggest that the phenomenon of citizen participation during times of crisis has not yet been fully explored. Furthermore, despite prevailing paradigms about the positive impact of citizen participation in a non-crisis situation, its impact is not necessarily direct or positive
