    Comparison of effects of diet versus exercise weight loss regimens on LDL and HDL particle size in obese adults

    <p>Abstract</p> <p>Background</p> <p>Obesity is associated with an atherogenic lipid profile characterized by a predominance of small LDL and HDL particles. Weight loss, by dietary restriction or exercise, increases LDL particle size. Whether these interventions can augment HDL size <it>in conjunction </it>with LDL size remains unknown.</p> <p>Objective</p> <p>This study compared the effects of alternate day fasting (ADF), calorie restriction (CR), and endurance exercise on LDL and HDL particle size in overweight and obese subjects.</p> <p>Methods</p> <p>In a 12-week parallel-arm trial, adult subjects (n = 60) were randomized to 1 of 4 groups: 1) ADF (75% energy restriction for 24-h alternated with ad libitum feeding for 24-h), 2) CR (25% energy restriction every day), 3) exercise (moderate intensity training 3 x/week), or 4) control.</p> <p>Results</p> <p>Body weight was reduced (<it>P </it>< 0.001) by ADF, CR, and exercise (5.2 Ā± 1.1%, 5.0 Ā± 1.4%, 5.1 Ā± 0.9%, respectively). Plasma LDL cholesterol decreased (<it>P </it>< 0.05) with ADF (10 Ā± 4%) and CR (8 Ā± 4%), whereas HDL cholesterol increased (<it>P </it>< 0.05) with exercise (16 Ā± 5%). Integrated LDL particle size was augmented (<it>P </it>= 0.01) by ADF and CR. The proportion of small LDL particles decreased (<it>P </it>= 0.04) with ADF only, and the proportion of large HDL particles increased (<it>P </it>= 0.03) with exercise only.</p> <p>Conclusion</p> <p>These results indicate that dietary restriction increases LDL particle size, while endurance training augments HDL particle size, with minimal weight loss. None of these interventions concomitantly increased both LDL and HDL particle size, however.</p

    The effectiveness of mobile app usage in facilitating weight loss: An observational study

    Aim: With increasing rates of global obesity and associated health issues, there is an everā€increasing need for weight management solutions to be more accessible. Mobile applications offer accessible support systems and have the potential to offer a viable and effective weight management solution as an alternative to traditional healthcare models. Objective: To evaluate the effectiveness of the SIMPLE mobile application for timeā€restricted eating in achieving weight loss (WL). Methods: User data were analyzed between January 2021 and January 2023. Inā€app activity was calculated as the proportion of active days over 12, 26 and 52 weeks. A day is considered active if it contains at least one inā€app action (e.g., logging weight, food, fasting, or physical activity). Users were categorized into four inā€app activity levels: inactive (inā€app activity <33%), medium activity (33%ā€“66%), high activity (66%ā€“99%), and maximal activity (100%). Weight change among inā€app activity groups was assessed at 12, 26, and 52 weeks. Results: Out of 53,482 users, a positive association was found between the use of the SIMPLE app and WL. Active app users lost more weight than their less active counterparts. Active users had a median WL of 4.20%, 5.04%, and 3.86% at 12, 26, and 52 weeks, respectively. A larger percentage of active usersā€”up to 50.26%ā€”achieved clinically significant WL (ā‰„5%) when compared to inactive users. A doseā€response relationship between WL and app usage was found after adjusting for gender, age, and initial Body Mass Index; a 10% increase in app activity correlated with increased WL by 0.43, 0.66 and 0.69 kg at 12, 26, and 52 weeks, respectively. Conclusions: The study demonstrates that the SIMPLE app enables effective WL directly associated with the level of app engagement. Mobile health applications offer an accessible and effective weight management solution and should be considered when supporting adults to lose weight

    Dietary and physical activity adaptations to alternate day modified fasting: implications for optimal weight loss

    <p>Abstract</p> <p>Background</p> <p>Alternate day modified fasting (ADMF) is an effective strategy for weight loss in obese adults.</p> <p>Objective</p> <p>The objective of this study was to examine the dietary and physical activity adaptations that occur during short-term ADMF, and to determine how these modulations affect rate of weight loss.</p> <p>Methods</p> <p>Sixteen obese subjects (12 women/4 men) completed a 10-week trial consisting of 3 phases: 1) 2-week control phase, 2) 4-week ADMF controlled feeding phase, and 3) 4-week ADMF self-selected feeding phase.</p> <p>Results</p> <p>Body weight decreased (<it>P </it>< 0.001) by 5.6 Ā± 1.0 kg post-treatment. Energy intake on the fast day was 26 Ā± 3% of baseline needs (501 Ā± 28 kcal/d). No hyperphagic response occurred on the feed day (95 Ā± 6% of baseline needs consumed, 1801 Ā± 226 kcal/d). Daily energy restriction (37 Ā± 7%) was correlated to rate of weight loss (<it>r </it>= 0.42, <it>P </it>= 0.01). Dietary fat intake decreased (36% to 33% of kcal, <it>P </it>< 0.05) with dietary counseling, and was related to rate of weight loss (<it>r </it>= 0.38, <it>P </it>= 0.03). Hunger on the fast day decreased (<it>P </it>< 0.05) by week 2, and remained low. Habitual physical activity was maintained throughout the study (fast day: 6416 Ā± 851 steps/d; feed day: 6569 Ā± 910 steps/d).</p> <p>Conclusion</p> <p>These findings indicate that obese subjects quickly adapt to ADMF, and that changes in energy/macronutrient intake, hunger, and maintenance of physical activity play a role in influencing rate of weight loss by ADMF.</p

    Impulsive phase flare energy transport by large-scale Alfven waves and the electron acceleration problem

    The impulsive phase of a solar flare marks the epoch of rapid conversion of energy stored in the pre-flare coronal magnetic field. Hard X-ray observations imply that a substantial fraction of flare energy released during the impulsive phase is converted to the kinetic energy of mildly relativistic electrons (10-100 keV). The liberation of the magnetic free energy can occur as the coronal magnetic field reconfigures and relaxes following reconnection. We investigate a scenario in which products of the reconfiguration - large-scale Alfven wave pulses - transport the energy and magnetic-field changes rapidly through the corona to the lower atmosphere. This offers two possibilities for electron acceleration. Firstly, in a coronal plasma with beta < m_e/m_p, the waves propagate as inertial Alfven waves. In the presence of strong spatial gradients, these generate field-aligned electric fields that can accelerate electrons to energies on the order of 10 keV and above, including by repeated interactions between electrons and wavefronts. Secondly, when they reflect and mode-convert in the chromosphere, a cascade to high wavenumbers may develop. This will also accelerate electrons by turbulence, in a medium with a locally high electron number density. This concept, which bridges MHD-based and particle-based views of a flare, provides an interpretation of the recently-observed rapid variations of the line-of-sight component of the photospheric magnetic field across the flare impulsive phase, and offers solutions to some perplexing flare problems, such as the flare "number problem" of finding and resupplying sufficient electrons to explain the impulsive-phase hard X-ray emission.Comment: 31 pages, 6 figure

    Governing a shared hidden resource: A review of governance mechanisms for transboundary groundwater security

    Ā© 2017 Elsevier B.V. Globally, groundwater is by far the largest store of liquid freshwater, making it a key component of a secure water supply. However, over the past few decades the amount of usable groundwater available around the world has rapidly decreased. This depletion is caused primarily by mismanagement (e.g., overpumping, contamination, and under-regulation), but also by reduced natural recharge due to climate change and urbanization. Management of groundwater resources is particularly challenging for the nearly 600 aquifers that are transboundary, meaning that they extend across international political borders. To understand how governance mechanisms can reduce water insecurity in transboundary groundwater contexts, we review key literature from what we view as the most relevant fields: groundwater management, water security, international water law and international water governance. We then formulate a set of recommendations for improved groundwater governance that can address the specific physical nature of groundwater systems, enhance water security, and apply to transboundary groundwater settings. We argue that groundwater governance in transboundary contexts requires processes that (1) enhance context-specific and flexible international mechanisms; (2) address the perpetual need for groundwater data and information; (3) prioritize the precautionary principle and pollution prevention, in particular; (4) where appropriate, integrate governance of surface and subsurface water and land; and (5) expand institutional capacity, especially of binational or multinational actors

    Societal Adaptation to Decadal Climate Variability in the United States

    The search for evidence of decadal climatic variability (DCV) has a very long history. In the past decade, a research community has coalesced around a series of roughly biennial workshops that have emphasized description of past DCV events; their causes and their teleconnections responsible for droughts, floods, and warm and cold spells around the world; and recently, the predictability of DCV events. Researchers studying climate change put great emphasis on prospective impacts, but the DCV community has yet to do so. To begin rectifying this deficiency, a short but ambitious workshop was convened in Waikoloa, near Kona, Hawaii, from 26-28 April 2007. This workshop, sponsored by the Center for Research on the Changing Earth System (CRCES), NOAA, the U.S. Geological Survey, and the U.S. Army Corps of Engineers, brought together climatologists and sectoral specialists representing agriculture, water resources, economics, the insurance industry, and developing country interests

    Treatment of Full-Thickness Acetabular Chondral Flaps during Hip Arthroscopy: Bone Marrow Aspirate Concentrate versus Microfracture

    Background: The optimal treatment strategy for patients with full-thickness chondral flaps undergoing hip arthroscopy is controversial. Purpose: To compare functional outcomes of patients who underwent bone marrow aspirate concentrate (BMAC) application with those of patients who underwent microfracture. Study Design: Cohort study; Level of evidence, 3. Methods: This was a retrospective case series of prospectively collected data on patients who underwent arthroscopic acetabular labral repair by 1 surgeon between June 2014 and April 2020. The inclusion criteria for this study were age ā‰„18 years, preoperative radiographs of the pelvis, arthroscopic acetabular labral repair, exposed subchondral bone with overlying chondral flap seen at the time of hip arthroscopy, microfracture or BMAC to address this lesion, and completed patient-reported outcome measures (PROMs) (International Hip Outcome Tool-33 [iHOT-33], Hip Outcome Score-Activities of Daily Living [HOS-ADL], Hip Outcome Score-Sports Subscale [HOS-Sport], modified Harris Hip Score [mHHS], and visual analog scale [VAS] for pain) at enrollment and 12-month follow-up. Clinical outcomes were assessed using PROM scores. Results: A total of 81 hips with full-thickness chondral flaps were included in this study: 50 treated with BMAC and 31 treated with microfracture. There were no significant differences between groups in age, sex, body mass index, tear size, radiographic osteoarthritis, or radiographic femoroacetabular impingement. In the BMAC cohort, all PROM scores improved significantly from preoperatively to follow-up: 41.7 to 75.6 for iHOT-33, 67.6 to 91.0 for HOS-ADL, 41.5 to 72.3 for HOS-Sport, 59.4 to 87.2 for mHHS, and 6.2 to 2.2 for VAS pain (P \u3c .001 for all). In the microfracture cohort, the score improvements were 48.0 to 65.1 for iHOT-33 (P = .001), 80.5 to 83.3 for HOS-ADL (P = .275), 59.2 to 62.4 for HOS-Sport (P = .568), 70.4 to 78.3 for mHHS (P = .028), and 4.9 to 3.6 for VAS pain (P = .036). Regarding clinically meaningful outcomes, 77.6% of the BMAC group and 50.0% of the microfracture group met the minimal clinically important difference for iHOT-33 at the 12-month follow-up (P = .013). Conclusion: Patients with full-thickness chondral flaps at the time of hip arthroscopy experienced greater improvements in functional outcome scores at the 12-month follow-up when treated with BMAC as opposed to microfracture

    Gastrointestinal nematodes of dairy goats, anthelmintic resistance and practices of parasite control in Northern Italy

    Background: Gastrointestinal nematodes (GINs) is one of the main constraints to ruminant production worldwide. Anthelmintic resistance (AR) has been reported in goats throughout Europe, yet little is known about the AR status in Italy. The aims of the study were: i) determine the frequency of AR in GINs in goat flocks in Northern Italy, Italy, ii) survey goat farmers on the current practices of parasite control, iii) update the species composition of the gastrointestinal helminthofauna. Thirty three flocks were enrolled and 1288 individual fecal samples were collected. Based on the egg per gram (EPG), 15 flocks were selected to evaluate the presence of AR in GINs with the Fecal Egg Count Reduction Test (FECRT). A questionnaire surveyed 110 dairy goat farmers to acquire information about farm management and drenching practices against GINs. Further, the gastrointestinal tracts of 42 goats were analyzed. Results: The FECRs indicated that five of the 15 flocks had problems of AR, which was identified in all two of the anthelmintic classes tested. Resistance and suspected resistance was found in 40% of the flocks selected for AR testing that were treated with benzimidazoles while 20% of the flocks treated with eprinomectin had resistant GINs. Teladorsagia/Trichostrongylus L3 were isolated from the post-treatment coprocultures of all flocks with resistance but not from the flock with suspected oxfendazole resistance. Treatments against helminths were performed once annually in 73.63% of the flocks, but 20.00% of farmers declared not regularly treating their goats every year. Annual treatments usually occurred in autumn or winter at dose rate for sheep. Te. circumcincta, H. contortus, Tr. colubriformis, Skrjabinema caprae and Oesophagostomum venulosum were the most abundant and prevalent species of the gastrointestinal tract. Conclusions: Strategies to prevent the development of AR should be widely adopted in Northern Italy. Further, farmers and practitioners should be educated about the importance of using the correct dose rates in goats. In addition, the presence of highly pathogenic GINs coupled with high worm burden in all sector of gastrointestinal tract and the prevalence values further suggest that improved diagnosis and active surveillance of GIN infection is needed
