Introduction. The distribution and severity of dental caries among preschool children vary according to the socio-economic and ethnic differences within and between countries. Understanding socio-economic influences on child oral health could inform early interventions to reduce the oral health burden throughout the life-cycle. The aim of this study is to examine the socio-economic and ethnic influences on oral health among preschoolers in Kegalle, Sri Lanka. Methods. The study involved 784 children aged between 48-72 months recruited from 84 pre-schools in the Kegalle district in Sri Lanka. Cross-sectional data were collected by means of an oral examination of the children and a self-administered questionnaire to their parents/caregivers. The Early Childhood Oral Health Impact Scale (ECOHIS) was used to assess Oral Health related Quality of Life (OHQoL). Univariate and multivariate models of Poisson regression were used to investigate the associations between the variables. Results: Compared to children whose fathers had tertiary education, those whose fathers did not study beyond grade 5, had more caries measured in terms of decayed, missing and filled surfaces (dmfs) (IRR = 2.30; 95% CI: 1.30, 4.06; p < 0.01) and experienced poor OHQoL at child (IRR = 2.52; 95% CI: 1.20, 5.31; p < 0.05) and family (IRR = 1.59; 95% CI: 1.11, 2.27; p < 0.05) levels. However, lower educational attainment among mothers was associated with better OHQoL among children. Compared to the Sinhalese ethnic group, Tamils had more gingival bleeding (bleeding surfaces) (IRR = 3.04; 95% CI: 1.92, 4.81; p < 0.001) and poor OHQoL at child level (IRR = 2.07; 95% CI: 1.19, 3.60; p < 0.01), whereas Muslims had poor OHQoL at family level (IRR = 1.42; 95% CI: 1.10, 1.84; p < 0.01). Children of low-income families had more gum bleeding (IRR = 1.00; 95% CI: 0.99, 1.00; p < 0.05) compared to children of high-income families. Conclusions: Socio-economic and ethnic differences in oral health outcomes exist among this population of preschoolers. Interventions targeting children of fathers with low educational levels and ethnic minority groups are required to reduce inequalities in oral health in Sri Lanka and other similar countries

Reactivity studies of metal halide ions, niobium cluster ions and thermochemistry of selected cationic cobalt (aromatic heterocycles) using Fourier transform ion cyclotron resonance mass spectrometry

The gas-phase reactions of halogen substituted first row transition metal ions with small and cyclic alkanes, photochemistry of Co\sp{+} substituted with aromatic heterocycles (pyrrole, furan, thiophene and selenophene) and the unique reactivity of Nb\sp{+} as well as Nb\rm\sb {n}\sp{+}\ (n=2-13) cluster ions towards CH\sb3I have been studied using Fourier transform mass spectrometry. Several gas phase techniques have been used to gather information on reaction mechanisms and thermochemistry of each of these systems. Reactions of CoI\sp{+} with alkanes revealed that CoI\sp{+} inserts exclusively into the C-H bonds of small linear and cyclic alkanes. A generalized mechanism involving C-H insertion followed by β\beta-hydrogen transfer is proposed as preceeding steps for dehydrogenation processes. Unlike bare metal ions, the resultant CoI(alkene)\sp{+} complex does not isomerize to a hydrido π\pi-allyl complex ion. Literature cited bond energy data have been used to explain the absence of HI loss. The unique 2,3-dehydrogenation of n-butane with CoI\sp{+} was further examined for several other metal halide ions (MX\sp{+}) where M = Fe, Cr, V and X = Cl, Br, I. Nb\sp{+} has shown unique reactivity towards CH\sb3I. This unique reactivity of Nb\sp{+} is explained on the basis of multiple bond formation between Nb\sp{+} and C. In a related study of the reactivity of Nb\rm\sb{n}\sp{+}\ (n = 2-13), saturation limits have been used to probe the structures of some of the niobium cluster ions. Photodissociation methods have been employed to measure the bond energies of CoL\sp{+} and CoL\sb2\sp{+} systems (L = pyrrole, furan, thiophene and selenophene). These bond energy measurements are further corroborated by ligand displacement reactions and competitive collision-induced dissociation experiments. The CO loss product ion, obtained from the reaction of Co\sp{+} with furan, is explained by using respective literature cited thermochemical data

Program evaluation of the inner south community health oral health program for priority populations

Background. This study evaluated an Oral Health Program for Priority Populations (OHPPP) in the Inner South of Melbourne, Victoria. Methods. Social Ecological Theory and the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) framework informed the study. It involved administering satisfaction questionnaires to 29 clients and conducting eight in-depth interviews and four focus group discussions with either clients, health care workers or partner agencies. Results. Most (92%) clients surveyed felt more positive about their health as a result of the OHPPP. Three themes emerged from the qualitative analyses: 1) good oral health is central to improving general health; 2) the OHPPP is valuable; and 3) there are difficulties in implementing the OHPPP. Conclusions. Clients and service providers view the OHPPP as accessible and rewarding. Our findings point to the need for policies that recognise the greater treatment needs of disadvantaged populations and that streamline the provision of their dental care

A child with mosaicism for deletion (14)(q11.2q13)

In this case report we describe a child with a de novo deletion in the (q11.2q13) region of chromosome 14. The child presented with dysmorphic features - anophthalmia, microcephaly, and growth retardation. Cytogenetic studies showed mosaicism. The karyotype was 46,XX,del(14)(q11.2;q13) [16] /46,XX [9]. We compared the features observed in this child with that of others with the same deletion reported in scientific literature and found that this is the first report of a child mosaic for this deletion. It is also the first time it has been reported in association with anophthalmia
