A 10-YEARS LONGITUDINAL STUDY OF ADOLESCENT DIABETICS WITH INITIAL NEPHROPATHY

    Get PDF
The data from a 10-years prospective study among 85 boys and 64 girls, mean age 14,1 ±3,8 years with mean diabetes duration 8,45 ±3,7 years are analyzed. Using a complex prospective screening we found the frequency of microalbuminuria (MAU) and estimated some parameters of progression of the renal damage. The patients were divided into 3 groups: no MAU, with MAU and with clinical signs of nephropathy. We traced the dynamics of glomerular fdtration, albumin excretion and arterial pressure. The effect of ACEIs was according to the level of proteinuria. In diabetics with macroalbuminuria a short-lasting effect run out to the end of the 2-nd year. In MAU-positive ptients ACEIs led to delay of nephropathy for 10-years in 86,48%, progression in 8,1% and regression to normoalbuminuria in 5,4%. In 15% of all the diabetics we found a significant raise of proteinuria. In 21,73% of patients with "borderline" albuminuria there was a progression after the 5-th year with a cumulative index 4,58% or 0,91%

Critical currents in vicinal YBa2_2Cu3_3O7δ_{7-\delta} films

Most measurements of critical current densities in YBa2_2Cu3_3O7δ_{7-\delta} thin films to date have been performed on films where the \textit{c}-axis is grown normal to the film surface. With such films, the analysis of the dependence of jcj_c on the magnetic field angle is complex. The effects of extrinsic contributions to the angular field dependence of jcj_c, such as the measurement geometry and disposition of pinning centres, are convoluted with those intrinsically due to the anisotropy of the material. As a consequence of this, it is difficult to distinguish between proposed FLL structure models on the basis of angular critical current density measurements on \textit{c}-axis films. Films grown on mis-cut (vicinal) substrates have a reduced measurement symmetry and thus provide a greater insight into the critical current anisotropy. In this paper previous descriptions of the magnetic field angle dependence of jcj_c in YBa2_2Cu3_3O7δ_{7-\delta} are reviewed. Measurements on YBa2_2Cu3_3O7δ_{7-\delta} thin films grown on a range of vicinal substrates are presented and the results interpreted in terms of the structure and dimensionality of the FLL in YBa2_2Cu3_3O7δ_{7-\delta}. There is strong evidence for a transition in the structure of the flux line lattice depending on magnetic field magnitude, orientation and temperature. As a consequence, a simple scaling law can not, by itself, describe the observed critical current anisotropy in YBa2_2Cu3_3O7δ_{7-\delta}. The experimentally obtained jc(θ)j_c(\theta) behaviour of YBCO is successfully described in terms of a kinked vortex structure for fields applied near parallel to the \textit{a-b} planes.Comment: 10 pages, 12 figures, Submitted to PR

Interactions between branched DNAs and peptide inhibitors of DNA repair

The RecG helicase of Escherichia coli unwinds both Holliday junction (HJ) and replication fork DNA substrates. Our lab previously identified and characterized peptides (WRWYCR and KWWCRW) that block the activity of RecG on these substrates. We determined that the peptides bind HJ DNA and prevent the binding of RecG. Herein, we present further evidence that the peptides are competitive inhibitors of RecG binding to its substrates. We have generated structural models of interactions between WRWYCR and a junction substrate. Using the fluorescent probe 2-aminopurine, we show that inhibitors interact with highest affinity with HJs (Kd = 14 nM) and ∼4- to 9-fold more weakly with replication fork substrates. The fluorescence assay results agree with the structural model, and predict the molecular basis for interactions between HJ-trapping peptides and branched DNA molecules. Specifically, aromatic amino acids in the peptides stack with bases at the center of the DNA substrates. These interactions are stabilized by hydrogen bonds to the DNA and by intrapeptide interactions. These peptides inhibit several proteins involved in DNA repair in addition to RecG, have been useful as tools to dissect recombination, and possess antibiotic activity. Greater understanding of the peptides’ mechanism of action will further increase their utility

Detecting the B-mode Polarisation of the CMB with Clover

We describe the objectives, design and predicted performance of Clover, which is a ground-based experiment to measure the faint ``B-mode'' polarisation pattern in the cosmic microwave background (CMB). To achieve this goal, clover will make polarimetric observations of approximately 1000 deg^2 of the sky in spectral bands centred on 97, 150 and 225 GHz. The observations will be made with a two-mirror compact range antenna fed by profiled corrugated horns. The telescope beam sizes for each band are 7.5, 5.5 and 5.5 arcmin, respectively. The polarisation of the sky will be measured with a rotating half-wave plate and stationary analyser, which will be an orthomode transducer. The sky coverage combined with the angular resolution will allow us to measure the angular power spectra between 20 < l < 1000. Each frequency band will employ 192 single polarisation, photon noise limited TES bolometers cooled to 100 mK. The background-limited sensitivity of these detector arrays will allow us to constrain the tensor-to-scalar ratio to 0.026 at 3sigma, assuming any polarised foreground signals can be subtracted with minimal degradation to the 150 GHz sensitivity. Systematic errors will be mitigated by modulating the polarisation of the sky signals with the rotating half-wave plate, fast azimuth scans and periodic telescope rotations about its boresight. The three spectral bands will be divided into two separate but nearly identical instruments - one for 97 GHz and another for 150 and 225 GHz. The two instruments will be sited on identical three-axis mounts in the Atacama Desert in Chile near Pampa la Bola. Observations are expected to begin in late 2009.Comment: 5 pages, 3 figures. Beyond in-phase and anti-phase coordination in a model of joint action

In 1985, Haken, Kelso and Bunz proposed a system of coupled nonlinear oscillators as a model of rhythmic movement patterns in human bimanual coordination. Since then, the Haken–Kelso–Bunz (HKB) model has become a modelling paradigm applied extensively in all areas of movement science, including interpersonal motor coordination. However, all previous studies have followed a line of analysis based on slowly varying amplitudes and rotating wave approximations. These approximations lead to a reduced system, consisting of a single differential equation representing the evolution of the relative phase of the two coupled oscillators: the HKB model of the relative phase. Here we take a different approach and systematically investigate the behaviour of the HKB model in the full four-dimensional state space and for general coupling strengths. We perform detailed numerical bifurcation analyses and reveal that the HKB model supports previously unreported dynamical regimes as well as bistability between a variety of coordination patterns. Furthermore, we identify the stability boundaries of distinct coordination regimes in the model and discuss the applicability of our findings to interpersonal coordination and other joint action tasks

Real-world experience of nintedanib for progressive fibrosing interstitial lung disease in the UK

Background Nintedanib slows progression of lung function decline in patients with progressive fibrosing (PF) interstitial lung disease (ILD) and was recommended for this indication within the United Kingdom (UK) National Health Service in Scotland in June 2021 and in England, Wales and Northern Ireland in November 2021. To date, there has been no national evaluation of the use of nintedanib for PF-ILD in a real-world setting. Methods 26 UK centres were invited to take part in a national service evaluation between 17 November 2021 and 30 September 2022. Summary data regarding underlying diagnosis, pulmonary function tests, diagnostic criteria, radiological appearance, concurrent immunosuppressive therapy and drug tolerability were collected via electronic survey. Results 24 UK prescribing centres responded to the service evaluation invitation. Between 17 November 2021 and 30 September 2022, 1120 patients received a multidisciplinary team recommendation to commence nintedanib for PF-ILD. The most common underlying diagnoses were hypersensitivity pneumonitis (298 out of 1120, 26.6%), connective tissue disease associated ILD (197 out of 1120, 17.6%), rheumatoid arthritis associated ILD (180 out of 1120, 16.0%), idiopathic nonspecific interstitial pneumonia (125 out of 1120, 11.1%) and unclassifiable ILD (100 out of 1120, 8.9%). Of these, 54.4% (609 out of 1120) were receiving concomitant corticosteroids, 355 (31.7%) out of 1120 were receiving concomitant mycophenolate mofetil and 340 (30.3%) out of 1120 were receiving another immunosuppressive/modulatory therapy. Radiological progression of ILD combined with worsening respiratory symptoms was the most common reason for the diagnosis of PF-ILD.Conclusion We have demonstrated the use of nintedanib for the treatment of PF-ILD across a broad range of underlying conditions. Nintedanib is frequently co-prescribed alongside immunosuppressive and immunomodulatory therapy. The use of nintedanib for the treatment of PF-ILD has demonstrated acceptable tolerability in a real-world setting
