Features of transport of potassium ions in ischemia-reperfusion syndrome of lower extremities, abdominal trauma and hypovolemic shock

80 pubescent white male Wistar rats weighing 190-220 g were used in the experiment. Rats have been on a standard vivarium diet. Animals were divided into 1 control (CG) and 3 experimental groups (EG) (8 animals per group). Haemostatic tourniquets were applied for 120 minutes proximal to the lower paws of animals of the first experimental group under the thiopental-sodium anesthesia (40 mg kg-1). Applying of tourniquets caused the development of ischemic-reperfusion syndrome. Blunt abdominal trauma was simulated in the second experimental group by applying two strikes to the abdomen; Hypovolemic shock was modeled in a group by cutting the femoral vessels and bloodletting from 20 to 22% of the circulating blood volume. Injuries of the third experimental group were combination of injuries of the first two groups. Animals of the control group were inducted into the anesthesia without any injury. The functional status of the kidneys of experimental animals was determined by the method of water loading at 1st, 3rd and 7th day. The concentration of potassium ions was determined in urine and blood serum and the level of its proximal and distal transport, as well as excretion, were calculated. Animals of experimental groups were withdrawn from the experiment by the method of total blood-flow from the heart under thiopental sodium anesthesia. Thus, simulated injuries are accompanied by a significant increasing potassium level in serum in all experimental groups. Moreover, in EG-1 there was a tendency of gradual decrease of indicator on 7 day of observation, in contrast to the indicators in EG-2 and EG-3 where increased on 7 day. In all EG the maximum increase of the indicator was observed on the 1 day of the post-traumatic period. The simulation of various causes of mechanical trauma leads to a significant increase of the excretion of potassium ions in the urine, with the exception of injuries of the abdominal trauma with massive blood loss and applied haemostatic tourniquets on both lower limbs, when on the 7 day the indicator is approaching the level of control group. This can be explained by a significant decrease of diuresis in the experimental group

A comparative study of the effectiveness endoscopic and direct decompression of the median nerve in the carpal tunnel syndrome was done. Surgical treatment was performed in 54 patients. 28 patients included in the group of endoscopic surgery, 26 - in the open surgery group. To evaluate results of study the clinical features, the data of dynamometry, the study of discriminatory sensitivity of compressed nerve, electroneuromyography and an ultrasound scan of the median nerve in the carpal canal were used. Treatment results were evaluated 7 and 30 days after surgery. Based on the analysis of research highlighted the advantages of endoscopic and open techniques of decompression of the median nerve in patients with carpal tunnel syndrome.РезюмеАвторами исследования внедрена в клиническую практику эндоскопическая методика декомпрессии срединного нерва в области карпального канала. Проведено сравнительное исследование эффективности эндоскопической и прямой декомпрессии срединного нерву при карпальном туннельном синдроме. Хирургическое вмешательство выполнено 54 пациентам. 28 пациентов вошли в группу эндоскопической хирургии, 26 – в группу открытой хирургии.  Для оценки результатов использовали особенности клинической картину, данные динамометрических проб, исследование дискриминационной чувствительности в дерматоме компримированного нерва, электронейромиографию и ультразвуковое сканирование срединного нерва над и в карпальном канале. Результаты лечения оценивались на 7 и 30 сутки после проведения хирургического вмешательства. На основе анализа результатов исследования разработаны практические рекомендации и выделены преимущества эндоскопической и открытой методики декомпрессии срединного нерву у пациентов с карпальным туннельным синдромом

Opposite, bidirectional shifts in excitation and inhibition in specific types of dorsal horn interneurons are associated with spasticity and pain post-SCI

Spasticity, a common complication after spinal cord injury (SCI), is frequently accompanied by chronic pain. The physiological origin of this pain (critical to its treatment) remains unknown, although spastic motor dysfunction has been related to the hyperexcitability of motoneurons and to changes in spinal sensory processing. Here we show that the pain mechanism involves changes in sensory circuits of the dorsal horn (DH) where nociceptive inputs integrate for pain processing. Spasticity is associated with the DH hyperexcitability resulting from an increase in excitation and disinhibition occurring in two respective types of sensory interneurons. In the tonic-firing inhibitory lamina II interneurons, glutamatergic drive was reduced while glycinergic inhibition was potentiated. In contrast, excitatory drive was boosted to the adapting-firing excitatory lamina II interneurons while GABAergic and glycinergic inhibition were reduced. Thus, increased activity of excitatory DH interneurons coupled with the reduced excitability of inhibitory DH interneurons post-SCI could provide a neurophysiological mechanism of central sensitization and chronic pain associated with spasticity

ПОДАТКОВІ АЛЬТЕРНАТИВИ ЩОДО РЕАЛІЗАЦІЇ ФІСКАЛЬНОГО ПОТЕНЦІАЛУ ІНТЕРНЕТ-ДІЯЛЬНОСТІ В УКРАЇНІ

In an article carried out examination of theoretical bases of internet – activities and defined the specifics of its taxation. The study found that the Internet is one of the most perspective activities, in the work revealed that Internet activity can become a significant source of filling the budget. In a study it is made by analysis of changes to market Internet activity and its fiscal capacity. The authors justify that the role of taxes in the national economic security repeatedly increases, the need to study the process of increasing the level of security of the tax as one of the main components. Based on the study of foreign experience, the authors recommend a post on Web sites of the subjects of Internet activity a distinctive sign (so-called «tags trust»). The authors found major flaws in the taxation of Internet activity and determined the main directions of improving its taxation as one of the elements ensuring tax security of the State. Рассмотрены теоретические основания интернет–деятельности и специфики ее налогообложения. Установлено, что интернет-деятельность является одной из наиболее перспективных видов деятельности и может стать существенным источником наполнения бюджета. Проанализировано динамику состояния рынка интернет – деятельности и ее фискальный потенциал. Обосновано, что роль налогов в обеспечении национальной экономической безопасности многократно возрастает, определяя необходимость исследования процесса повышения уровня налоговой безопасности как одной из основных ее составляющих. Выявлены основные недостатки в налогообложении интернет – деятельности и определены основные направления совершенствования ее налогообложения как одного из элементов обеспечения налоговой безопасности государства. Розглянуто теоретичні основи інернет-діяльності та специфіки її оподаткування. Встановлено, що інтернет-діяльність є однією із найперспективніших видів діяльності та може стати значним джерелом наповнення бюджету. Проаналізовано динаміку стану ринку Інтернет – діяльності та її фіскальний потенціал. Обґрунтовано, що роль податків у забезпеченні національної економічної безпеки багаторазово зростає, визначаючи необхідність дослідження процесу підвищення рівня податкової безпеки як однієї із основних її складових. Виявлено основні недоліки в оподаткуванні інтернет–діяльності та визначено основні напрями вдосконалення її оподаткування як одного із елементів забезпечення податкової безпеки держави.

Instruments of tax simulating investment in energysaving in Ukraine

Стаття присвячена дослідженню проблем енергозбереження в Україні та вдосконаленню інструментів податкового стимулювання залучення інвестицій у цей процес. Визначені можливі джерела інвестиційних ресурсів для забезпечення процесу енергозбереження в Україні та розглянуті подальші перспективи їх використання. Проаналізовано ряд існуючих податкових пільг щодо запровадження енергозберігаючих технологій та обґрунтована необхідність подальшого вдосконалення шляхом їх спрямування на залучення коштів населення у здійснення енергозберігаючих заходів.
