Retention of Low Income Children in Three Dental Studies Investigating Early Childhood Caries
Background: To our knowledge no dental studies have looked closely at subject retention, which is crucial to better understand oral health disparities. In this paper, we report retention rates and review and attempt to assess which retention strategies utilized in 3 dental research studies investigating ECC were effective for retaining WIC-enrolled children. The purpose of this paper is to discuss challenges that were encountered when working with these populations, describe characteristics of those not retained, and summarize some recommendations for future dental studies working at WIC sites. Methods: Three dental studies were conducted at WIC clinics in Iowa. Retention strategies focused on maintenance of contact over time, persistence in rescheduling appointments, utilization of incentives, high recruitment, and frequent communication with parents and program staff. Results: Retention rates in the studies ranged from 60 to 75 percent at the final research interventions. Studies were challenged by frequent moves of subjects, missed appointments, disconnected phones, busy schedules of parents, transportation problems, loss of child custody, family illness, and lack of interest. Those not retained in the studies were more likely to be younger, single, and less educated, with a lower household income and a non-Caucasian child. Lower retention was also associated with the presence of carious lesions. Conclusions: Despite many challenges, studies had good retention rates and benefited from the retention strategies. Future dental studies at WIC clinics may also benefit from arranging transportation, obtaining a free, 800 callback number, and offering after-hours appointments for working parents

The elemental mechanism of transcriptional pausing.
Transcriptional pausing underlies regulation of cellular RNA biogenesis. A consensus pause sequence that acts on RNA polymerases (RNAPs) from bacteria to mammals halts RNAP in an elemental paused state from which longer-lived pauses can arise. Although the structural foundations of pauses prolonged by backtracking or nascent RNA hairpins are recognized, the fundamental mechanism of the elemental pause is less well-defined. Here we report a mechanistic dissection that establishes the elemental pause signal (i) is multipartite; (ii) causes a modest conformational shift that puts Îł-proteobacterial RNAP in an off-pathway state in which template base loading but not RNA translocation is inhibited; and (iii) allows RNAP to enter pretranslocated and one-base-pair backtracked states easily even though the half-translocated state observed in paused cryo-EM structures rate-limits pause escape. Our findings provide a mechanistic basis for the elemental pause and a framework to understand how pausing is modulated by sequence, cellular conditions, and regulators
Data indexing techniques for the EUVE all-sky survey
This poster describes techniques developed for manipulating large full-sky data sets for the Extreme Ultraviolet Explorer project. The authors have adapted the quatrilateralized cubic sphere indexing algorithm to allow us to efficiently store and process several types of large data sets, such as full-sky maps of photon counts, exposure time, and count rates. A variation of this scheme is used to index sparser data such as individual photon events and viewing times for selected areas of the sky, which are eventually used to create EUVE source catalogs
Harmonising nursing terminologies using a conceptual framework
The International Classification for Nursing Practice (ICNPÂź) and the Clinical Care Classification (CCC) System are standardised nursing terminologies that identify discrete elements of nursing practice, including nursing diagnoses, interventions, and outcomes. While CCC uses a conceptual framework or model with 21 Care Components to classify these elements, ICNP, built on a formal Web Ontology Language (OWL) description logic foundation, uses a logical hierarchical framework that is useful for computing and maintenance of ICNP. Since the logical framework of ICNP may not always align with the needs of nursing practice, an informal framework may be a more useful organisational tool to represent nursing content. The purpose of this study was to classify ICNP nursing diagnoses using the 21 Care Components of the CCC as a conceptual framework to facilitate usability and inter-operability of nursing diagnoses in electronic health records. Findings resulted in all 521 ICNP diagnoses being assigned to one of the 21 CCC Care Components. Further research is needed to validate the resulting product of this study with practitioners and develop recommendations for improvement of both terminologie
An evaluation of ocean color model estimates of marine primary productivity in coastal and pelagic regions across the globe
Nearly half of the earth\u27s photosynthetically fixed carbon derives from the oceans. To determine global and region specific rates, we rely on models that estimate marine net primary productivity (NPP) thus it is essential that these models are evaluated to determine their accuracy. Here we assessed the skill of 21 ocean color models by comparing their estimates of depth-integrated NPP to 1156 in situ C-14 measurements encompassing ten marine regions including the Sargasso Sea, pelagic North Atlantic, coastal Northeast Atlantic, Black Sea, Mediterranean Sea, Arabian Sea, subtropical North Pacific, Ross Sea, West Antarctic Peninsula, and the Antarctic Polar Frontal Zone. Average model skill, as determined by root-mean square difference calculations, was lowest in the Black and Mediterranean Seas, highest in the pelagic North Atlantic and the Antarctic Polar Frontal Zone, and intermediate in the other six regions. The maximum fraction of model skill that may be attributable to uncertainties in both the input variables and in situ NPP measurements was nearly 72%. On average, the simplest depth/wavelength integrated models performed no worse than the more complex depth/wavelength resolved models. Ocean color models were not highly challenged in extreme conditions of surface chlorophyll-a and sea surface temperature, nor in high-nitrate low-chlorophyll waters. Water column depth was the primary influence on ocean color model performance such that average skill was significantly higher at depths greater than 250 m, suggesting that ocean color models are more challenged in Case-2 waters (coastal) than in Case-1 (pelagic) waters. Given that in situ chlorophyll-a data was used as input data, algorithm improvement is required to eliminate the poor performance of ocean color NPP models in Case-2 waters that are close to coastlines. Finally, ocean color chlorophyll-a algorithms are challenged by optically complex Case-2 waters, thus using satellite-derived chlorophyll-a to estimate NPP in coastal areas would likely further reduce the skill of ocean color models

Breast cancer and communication: Monocentric experience of a self-assessment questionnaire
Background: The communication of the diagnosis of breast cancer induces to the patient a strong psychological trauma. Radiologists are at the forefront of communicating, either for a biopsy or the probable diagnosis of malignancy. This is a complex task, which requires the knowledge and application of correct âcommunicative modelsâ, among which the SPIKES protocol rep-resents a fundamental reference. Design and methods: 110 patients, with a history of breast cancer, filled out a questionnaire consisting of six questions: five aimed at defining communication compliance with the SPIKES protocol, the sixth, consisting of six feelings, aimed at the knowledge of the next emotional state. Results: Regarding compliance with various âstrategic pointsâ of the SPIKES protocol, questionnaires show that 70% of patients reported no omissions, while the remaining 30% reported omissions relatively to perception (56%), emotions (23%), setting (13%), knowledge (6%) and invitation (2%). The results showed the existence of a correlation between the final emotional state and the correct application of the SPIKES protocol; in fact, patients who reacted with a positive final emotional state-reported greater adherence to the strategic points of the SPIKES protocol. Conclusions: In healthcare, knowing the communicative compliance of a team in giving âbad newsâ is fundamental, especially in breast cancer. The SPIKES protocol is recognized by the Literature as a fundamental reference able to affect âpositivelyâ the emotional state of patients. The proposed questionnaire is a valid tool to identify the weak points of communication and related criticalities, to improve clinical practice

Combined heat and power's potential to meet New York City's sustainability goals
Combined Heat and Power (CHP) has been proven as a mature technology that can benefit both building owners and utility operators. As the economic and environmental benefits of CHP in urban centers gain recognition, regulations and policies have evolved to encourage their deployment. However, the question remains whether these policies are sufficient in helping to achieve the larger
sustainability goals, such as the New York City-specific goal of incorporating 800 MW of distributed generation. In this paper, the current regulatory and policy environment for CHP is discussed. Then, an engineering analysis estimating the potential for CHP in NYC at the individual building and microgrid
scale, considered a city block, is performed. This analysis indicates that over 800 MW of individual building CHP systems would qualify for the current incentives but many systems would need to undergo more cumbersome air permitting processes reducing the viable capacity to 360 MW. In addition microgrid CHP
systems with multiple owners could contribute to meeting the goal even after considering air permits; however, these systems may incorporate many residential customers. The regulatory framework for microgrids with multiple owners and especially residential customers is particularly uncertain therefore
additional policies would be needed to facilitate their development
Correlated electron-hole plasma in organometal perovskites
Organic-inorganic perovskites are a class of solution-processed semiconductors holding promise for the realization of low-cost efficient solar cells and on-chip lasers. Despite the recent attention they have attracted, fundamental aspects of the photophysics underlying device operation still remain elusive. Here we use photoluminescence and transmission spectroscopy to show that photoexcitations give rise to a conducting plasma of unbound but Coulomb-correlated electron-hole pairs at all excitations of interest for light-energy conversion and stimulated optical amplification. The conductive nature of the photoexcited plasma has crucial consequences for perovskite-based devices: in solar cells, it ensures efficient charge separation and ambipolar transport while, concerning lasing, it provides a low threshold for light amplification and justifies a favourable outlook for the demonstration of an electrically driven laser. We find a significant trap density, whose cross-section for carrier capture is however low, yielding a minor impact on device performance
