Duszpasterstwo par kohabitujących – propozycje dla obecnej sytuacji w Polsce
Premarital cohabitation in Poland, although still not as widespread and common as in many western countries, is becoming an attractive form of being in an intimate relationship for a growing number of young adults. This state of things – in spite of the recent, more welcoming attitude towards cohabiting couples introduced by Francis – is not fully recognised by the Polish Episcopate. The Church in Poland had to wait over two years to receive a general guideline for the application of Amoris Laetitia. Also, until now the Polish bishops have not been able to propose any systematic marriage preparation program (MP) or working document – which would offer principles on how to deal pastorally with cohabiting couples. In this article I specifically concentrate on the newest empirical research regarding the quality of cohabiting unions (prior to and after they transit to marriages) as well as on the new orientation towards those living in ‘irregular’ situations introduced by Francis. Based on these two perspectives, I make several suggestions on how to deal with the Polish cohabiting couples in the context of marriage preparation in a pastorally sensitive and at the same time effective and realistic manner.Kohabitacja przedmałżeńska w Polsce, mimo iż nadal nie jest tak powszechnie praktykowana i akceptowana jak w wielu krajach zachodnich, staje się bardziej atrakcyjną formą bycia w związku (intymnym) dla coraz większej liczby młodych Polaków. Ten stan rzeczy – pomimo wyraźnych zmian w podejściu do par kohabitujących zaproponowanych przez papieża Franciszka w encyklice Amoris Laetitia – nie jest w pełni zrozumiany przez Kościół katolicki w Polsce. Przygotowanie 11-stronicowego dokumentu zawierającego ogólne wytyczne do stosowania encykliki zajęły Episkopatowi Polski ponad dwa lata. Jak dotąd nie zaproponowano również propozycji programu formacyjnego (na poziomie diecezji czy metropolii), który oferowałby konkretną pomoc w przygotowaniu par kohabitujących pragnących zawrzeć sakrament małżeństwa. W tym artykule autorka koncentruje się na najnowszych badaniach empirycznych dotyczących jakości związków kohabitacyjnych (przed i po zawarciu małżeństwa), a także na nowym kierunku w postępowaniu z parami, które żyją w tzw. nieregularnych sytuacjach, wyznaczonym przez papieża Franciszka. Opierając się na tych dwóch perspektywach, przedstawia kilka sugestii, jak podchodzić do par kohabitujących w kontekście przygotowania małżeńskiego w sposób pastoralnie przyjazny, a jednocześnie skuteczny i realistyczny
Rola wybranych składników odżywczych we wspomaganiu leczenia farmakologicznego choroby Alzheimera
Choroba Alzheimera (AD, Alzheimer’s disease) jest najczęstszą, postępującą chorobą neurodegeneracyjną, charakteryzującą się utratą pamięci i utratą zdolności do samodzielnego życia u osób w podeszłym wieku. W mechanizmie choroby podstawową rolę odgrywa niedobór acetylocholiny oraz upośledzenie przekaźnictwa cholinergicznego. Na podstawie badań przedklinicznych, w których wykazano, że niektóre składniki odżywcze, takie jak nukleotydurydyny, cholina i wielonienasycone kwasy tłuszczowe omega 3, działając synergistycznie, zwiększają stężenie fosfolipidów w błonach komórkowych synaps, prowadzą do tworzenia nowych synaps i poprawy ich funkcji, opracowano i poddano badaniom klinicznym wieloskładnikowy preparat Souvenaid® (Nutricia N.V., Zoetermer, Netherlands). W przeprowadzonych badaniach klinicznych, którym poddano 438 chorych z wczesną postacią AD, dowiedziono, że Souvenaid® jest bezpieczny, dobrze tolerowany przez pacjentów i powoduje statystycznie znamienną poprawę pamięci odroczonej i natychmiastowej. Konieczne są jednak dalsze badania kliniczne w celu potwierdzenia skuteczności tej koncepcji terapeutycznej
Żywienie kliniczne w uczelniach medycznych w Polsce. Dlaczego żywienie kliniczne nie jest nauczane na studiach medycznych?
Niedożywienie jest chorobą. Figuruje w Międzynarodowej Klasyfikacji Chorób i Przyczyn Zgonów (ICD 10, E40–E46) i jak każda choroba powinno być rozpoznawane i leczone zgodnie z obowiązującymi zasadami. Jest to tym ważniejsze, że w momencie przyjęcia do szpitala występuje ono u 35–55% chorych na choroby nienowotworowe i u 40–80% chorych na nowotwory. Związane z chorobą niedożywienie jest bezpośrednią przyczyną zwiększonej chorobowości, śmiertelności, przedłużonego pobytu w szpitalu i zwiększonych kosztów leczenia. Tymczasem mimo że w Polsce od 2012 roku ocena stanu odżywienia jest obowiązkowa, niedożywienie jest rzadko rozpoznawane i jeszcze rzadziej leczone. Główną przyczyną niedożywienia związanego z chorobą będącego najczęstszym jatrogennym powikłaniem hospitalizacji jest brak wiedzy lekarzy na temat znaczenia stanu odżywienia w profilaktyce i leczeniu chorób spowodowany nieuwzględnieniem leczenia żywieniowego w kształceniu przeddyplomowym lekarzy
Disease-related malnutrition (undernutrition and obesity) in patients hospitalized in the Department of Cardiology
Background: The nutritional status of patients with cardiovascular diseases significantly affects prognosis and disease course. Our study, which assessed the nutritional status of patients admitted to the Cardiology Department, revealed a substantial proportion of patients at risk of malnutrition, which increased with age. Malnutrition, including undernutrition and obesity, affects 40–60% of adult patients hospitalized in Europe and is associated with increased mortality, morbidity, length of hospital stay, and costs. Although malnutrition is a disease listed in the International Classification of Diseases and should be recognized and treated as such, it is rarely identified and even less often treated, which can have tragic consequences. In this analysis, we examined the nutritional status of adults hospitalized for cardiovascular diseases.
Methods: Nutritional status was assessed using the Subjective Global Assessment (SGA), Nutritional Risk Screening 2002 (NRS 2002), Body Mass Index (BMI), and Waist-to-Hip circumference ratio (WHR). Additionally, serum concentrations of albumin, cholesterol, hemoglobin, and C-reactive protein were measured. This retrospective study included 95 consecutive patients.
Results: Using NRS 2002 and SGA, the estimated risk of undernutrition was reported in 60% and 81% of patients, respectively. The prevalence of malnutrition (undernutrition, overweight, and obesity) increased with age, regardless of the scale used.
Conclusions: Hospitalized patients with cardiovascular diseases should be screened for the risk of malnutrition using NRS 2002 or SGA. Although our data is based on a small number of patients, it should encourage clinicians to pay more attention to nutritional status to prevent malnutrition in at-risk patients
Indirect calorimetry, indications, interpretation, and clinical application
The aim of this review is to inform clinicians about the new possibilities in clinical nutrition (enteral or parenteral) based on indirect calorimetry. This method allows for the provision of an appropriate number of calories to undernourished hospitalized patients for nutritional repletion or maintenance. Providing too few or too many calories may result in adverse clinical consequences and worsen the course of the disease, especially in critically ill patients. Thus, optimizing nutrition support according to the individual and specific needs of patients is an urgent task. Indirect calorimetry (IC) is the only practical, clinical method to measure resting energy expenditure (REE). However, from a clinical perspective, knowledge of only one part of total energy expenditure (TEE) is insufficient to plan feeding that covers the actual energy needs of an individual patient. Currently, there is no method better than IC for accurately determining the resting energy needs of hospitalized patients. Although many predictive equations for REE have been developed, none of them can account for all the factors associated with changes in metabolism in critically ill patients. The most widely used equation for this purpose, the Harris and Benedict equation, was issued over 100 years ago and is inaccurate in about 40% of patients. To date, indirect calorimetry remains the gold standard for assessing REE. The W bosons are reconstructed in the decays W→ℓν , where ℓ denotes muon or electron, while the b and c quarks are reconstructed as jets. Multidifferential study of identified charged hadron distributions in -tagged jets in proton-proton collisions at 13 TeV
Jet fragmentation functions are measured for the first time in proton-proton
collisions for charged pions, kaons, and protons within jets recoiling against
a boson. The charged-hadron distributions are studied longitudinally and
transversely to the jet direction for jets with transverse momentum 20 GeV and in the pseudorapidity range . The
data sample was collected with the LHCb experiment at a center-of-mass energy
of 13 TeV, corresponding to an integrated luminosity of 1.64 fb. Triple
differential distributions as a function of the hadron longitudinal momentum
fraction, hadron transverse momentum, and jet transverse momentum are also
measured for the first time. This helps constrain transverse-momentum-dependent
fragmentation functions. Differences in the shapes and magnitudes of the
measured distributions for the different hadron species provide insights into

Study of the decay
The decay is studied
in proton-proton collisions at a center-of-mass energy of TeV
using data corresponding to an integrated luminosity of 5
collected by the LHCb experiment. In the system, the
state observed at the BaBar and Belle experiments is
resolved into two narrower states, and ,
whose masses and widths are measured to be where the first uncertainties are statistical and the second
systematic. The results are consistent with a previous LHCb measurement using a
prompt sample. Evidence of a new
state is found with a local significance of , whose mass and width
are measured to be and , respectively. In addition, evidence of a new decay mode
is found with a significance of
. The relative branching fraction of with respect to the
decay is measured to be , where the first
uncertainty is statistical, the second systematic and the third originates from

sample of proton-proton collision data corresponding to 3.0 fb of
integrated luminosity recorded by the LHCb experiment during 2011 and 2012. The
tau lepton is identified in the decay mode

statistical and the second is systematic. The correlation between these
measurements is . Results are consistent with the current average
of these quantities and are at a combined 1.9 standard deviations from the
