Az emlőrák patológiai diagnosztikája, feldolgozása és kórszövettani leletezése. Szakmai útmutatás a III. Emlőrák Konszenzus Konferencia alapján
There have been relevant changes in the diagnosis and treatment of breast cancer to implement the updating of the 2010 recommendations made during the 2nd national consensus conference on the disease. Following a wide interdisciplinary consultation, the present recommendations have been finalized after their public discussion at the 3rd Hungarian Consensus Conference on Breast Cancer. The recommendations cover non-operative and intraoperative diagnostics, the work-up of operative specimens, the determination of prognostic and predictive markers and the content of the cytology and histology reports. Furthermore, it touches some special issues such as the current status of multigene molecular markers, the role of pathologists in clinical trials and prerequisites for their involvement, some relevant points about the future
A molekuláris onkogenezis mechanizmusai gyakori daganatokban = Mechanisms of molecular oncogenesis in common malignancies
Munkacsoportunk két évtizede megkezdett kutatási programját folytatta, amelynek fő célkitűzése új molekuláris rákgenetikai ismeretek szerzése a rák iránti fokozott genetikai fogékonyság molekuláris tényezőinek megismerésére. Vizsgálataink a Magyarországon gyakori daganatos megbetegedésekre (emlőrák, vastagbélrák, ivarszervi daganatok), elsősorban ezek familiáris formáira irányultak. Folytattuk, illetve megkezdtük a daganatszindrómák hajlamosító génjeinek elemzését örökletes emlő- és petefészekrákokban (BRCA1/2, CHEK2), familiáris adenomatozus polyposisban (APC), és herediter, nem a polyposis talaján kialakuló vastagbélrák szindróma (HNPCC) "mutátor gén"-jeiben (MSH2, MLH1). Az emlőrákos családokon, valamint kohorszokon nyert kutatási eredmények adatainak nemzetközi szintű összesítése, molekuláris epidemiológiai kiértékelése révén az örökletes daganatok kialakulására hajlamosító mutációt hordozók rákkockázatáról, a prevenciós tényezőkről és a betegség genetika-klinikai-pathológiai összefüggéseiről szereztünk további új ismereteket. Nemzetközi együttműködésben hozzájárultunk a csírasejtes hererákra hajlamosító első genetikai variánsok azonosításához. Daganatos megbetegedésre hajlamosító gének (BRCA1, STK11) esetében elemeztük az alternatív splicing szerepét a betegség kialakulásában. Újabban megkezdett génexpressziós profilvizsgálatok révén bepillantást nyertünk a molekuláris genetikai útvonalak és a metabolikus útvonalak kölcsönhatásába. | We have extended our molecular cancer genetic studies that were initiated in Hungary two decades ago and were aimed at providing new knowledge on molecular cancer genetics, with a focus on genetic susceptibility to cancer. The studies were conducted on common malignancies in Hungary (breast, colorectal and genitourinary cancers). Extended analysis of the predisposing genes of cancer syndromes was initiated or continued for hereditary breast- and/or ovarian cancer (BRCA1, BRCA2, CHEK2), for familial adenomatous polyposis (APC), for Peutz-Jeghers syndrome (STK11), and for hereditary non-polyposis colorectal carcinoma, HNPCC ("mutator genes" such as MSH2, MLH1). Forwarding the result and data of our molecular genetic analysis on breast cancer families and cancer cohorts for international data integration and molecular epidemiological analysis resulted in generation of new knowledge on cancer risk for the carriers of deleterious germ-line mutations, and on genetic-clinical and pathological correlations in development of breast cancer. Participating in international studies we have contributed to identification of new genetic variants predisposing to germ cell testicular cancer. The role of alternative splicing in development of cancer was investigated in cancer susceptibility genes BRCA1 and STK11. By recent introduction of gene-expression profiling we have gained preliminary insight into the interaction of metabolic and molecular pathways

Absztrakt
Bevezetés: A European Society of Breast Cancer Specialists
létrehozta az emlőrákellátást végző onkológiai központok és szakorvosok európai
minőségbiztosítási feltételrendszerét, hogy egységes minimumstandardok
bevezetésével csökkentse az emlőrák mortalitását a kontinensen.
Célkitűzés: Jelen vizsgálat célja, hogy az Országos
Onkológiai Intézet Emlő- és Lágyrészsebészeti Osztályán operált, illetve az
intézetben multidiszciplinárisan kezelt emlődaganatok komplex onkológiai
ellátását a European Society of Breast Cancer Specialists minőségbiztosítási
kívánalmai szerint elemezze és a nagy esetszámú, reprezentatív minta révén a
magyar emlőrákos populációról részletes klinikopatológiai adatokat szolgáltasson
az onkológiai ellátórendszer számára. Módszer: 2011. június 1.
és 2012. május 31. között multidiszciplinárisan kezelt emlődaganatok részletes
klinikopatológiai retrospektív adatfeldolgozását végezték a European Society of
Breast Cancer Specialists egységes nemzetközi kritériumrendszere szerint.
Eredmények: A vizsgált időszakban 906 betegnél történt
malignus, illetve jóindulatú emlődaganat miatt műtét. Az EUSOMA kötelező
minőségbiztosítási minimumelvárásainak az Országos Onkológiai Intézet eredményei
mindenben megfeleltek. Következtetések: Az Országos Onkológiai
Intézet emlődiagnosztikával és multidiszciplináris onkológiai kezelésével
foglalkozó emlőrák-ellátási szervezeti egysége a European Society of Breast
Cancer Specialists által meghatározott minimumesetszámot, személyi és tárgyi

Abstract
Introduction: The European Society of Breast Cancer Specialists
has created quality indicators for breast units to establish minimum standards
and to ensure specialist multimodality care with the conscious aim of improving
outcomes and decreasing breast cancer mortality. Aim: The aim
of this study was to analyse the breast cancer care in the National Institute of
Oncology according to the European Society of Breast Cancer Specialists
requirements and in a large number of cases in order to present representative
clinico-pathological data on the incidence of breast cancer in Hungary.
Method: According to the European Society of Breast Cancer
Specialists uniformed criteria clinico-pathological data of multimodality
treated breast cancer cases were retrospectively analysed between June 1, 2011
and May 31, 2012. Results: During the period of interest 906
patients underwent breast surgery for malignant or benign lesions. According to
the European Society of Breast Cancer Specialists quality indicators the breast
cancer care of the National Institute of Oncology is eligible.
Conclusions: The diagnostic modalities and multimodality
care of breast cancer of the National Institute of Oncology breast unit meets
the critical mass and minimum standards of the European Society of Breast Cancer

Emlőrák miatti szervmegtartó műtétek reszekciós széleinek vizsgálata. Multicentrikus retrospektív elemzés = Evaluation of resection margins following breast conserving surgery for breast cancer. A multicentric retrospective study
Absztrakt:
Bevezetés: Szervmegtartó műtéttel eltávolított emlőrákoknál
fontos a sebészi szélek állapotának vizsgálata, dokumentálása. Ha az eltávolítás
nem az épben történik, általában kiegészítő kezelés (ismételt sebészet,
tumorágy-besugárzás) javasolt. Célkitűzés: Szervmegtartó
műtéttel eltávolított női emlőrákok patológiai leleteinek sebészi szélekre
vonatkozó információtartalmának elemzése, a nem ép szélek gyakoriságának
felmérése és nem ép szélek esetén a további kezelések gyakoriságának becslése.
Módszer: Nyolc magyar patológiai részlegből származó,
mellrák miatt végzett szervmegtartó műtéthez tartozó patológiai leletek
elemzése, nem ép szél esetén további lokális kezelésre vonatkozó adatok
gyűjtésével. Eredmények: Kizárások után 386 leletet elemeztünk.
Az ép szél korábbi definíciója (<5 mm) alapján 200, az újabb értelmezés (0
mm) alapján 32 lelet dokumentált nem ép széleket. Lobularis carcinoma esetén
gyakoribb volt a szélek pozitivitása, mint nem lobularis rákok esetén. Az épben
eltávolított tumorok műtéti anyaga nagyobb tömegű volt. Reoperáció 43/180,
illetve 12/22 esetben történt a régi és az új definíció szerinti nem ép szélek
esetén. A nem reoperált betegek közül csak 75/137 kapott biztosan külön
tumorágy-besugárzást; sok esetben a ’boost’ kezelésről nem állt rendelkezésre
információ. Reziduális tumor a reoperált betegek közül csak 15/43-nál
igazolódott, 9-nél közülük úgy, hogy a festett szélben a primer műtéti anyagban
nem írtak le daganatot. Következtetések: A patológiai leletek
sebészi szélre vonatkozó információtartalma nem teljes körű. Az új biztonságos
szél definíciója csökkentette a nem ép szél előfordulási gyakoriságát, de
alkalmazásakor ép szélek mellett is maradhat reziduális daganat az emlőben,
amint azt a reoperációs minták igazolják. Nem épben történő tumoreltávolításkor
az esetek egy részében nem történik sem további sebészi kezelés, sem külön

Abstract:
Introduction: Breast conserving surgery for breast cancer
requires the analysis of surgical margins. If the tumor is not removed
completely, additional treatments (reoperation, boost irradiation) are generally
recommended. Aim: To analyze the information content of
histopathology reports on surgical margins in consecutive cases of breast
conservation for invasive female breast cancer, to evaluate the frequency of
incompletely removed tumors and to estimate the rate of further treatments after
incomplete removal. Method: Analysis of margin related data of
consecutive histopathology reports from 8 Hungarian pathology units with
locoregional treatment related data in case of unsafe margins.
Results: 386 reports were analyzed after exclusions. 200
and 32 cases were identified as having unsafe margins according to the previous
(<5 mm) and the new (0 mm) definition of unclear margin, respectively. Unsafe
margins were more common with lobular carcinomas. Specimens with clear margins
weighed more. Reoperations for unsafe margins were performed in 43/180 and 12/22
cases according to the previous and the new definitions, respectively. Only
75/137 patients without reoperation received boost irradiation of the tumor bed;
information on boost radiotherapy was often missing. Residual cancer was
identified in 15/43 reoperated patients, of whom 9 had >0 mm margin distance.
Conclusions: Some pathology reports lack information on
surgical margins. Unsafe margin rates decreased with the new definition.
Residual cancer may be left behind in case of clear margins with no ink on
tumor. Neither reoperation, nor boost radiotherapy is given to some patients

Sentinel lymph node biopsy following previous axillary surgery in recurrent breast cancer.
Ipsilateral breast recurrence or second primary breast cancer can develop in patients who have undergone breast conserving surgery (BCS) and axillary surgery. The purpose of this study was to examine the feasibility of a reoperative sentinel lymph node biopsy (SLNB) as a repeated axillary staging procedure.From August 2014 through January 2017 patients with locally recurrent breast cancer or with BRCA mutation requiring risk reduction mastectomy as a second surgical procedure, underwent repeat SLNB in three Hungarian Breast Units with a radiocolloid (and blue dye) technique.Hundred and sixty repeat SLNBs were analysed, 80 after previous SLNB and 80 after previous total or partial axillary lymph node dissection (ALND). SLN identification was successful in 106 patients (66%); 77/80 (77.5%) and 44/80 (55%) in the SLNB and ALND groups, respectively. (p < 0.003). Extra-axillary lymph drainage was more frequent in the ALND group (19/44, 43,2% versus 7/62, 11,3%; p < 0.001). Lymphatic drainage to the contralateral axilla was observed in 14 patients (11 in the ALND group, p = 0.025), isolated parasternal drainage was detected in 4 patients (p = 0.31). Only 9/106 patients with successful repeat SLNB (8,8%, all with 1 SLN removed) had SLN metastases CONCLUSIONS: Repeat SLNB is feasible in patients with ipsilateral breast tumor recurrence or new ipsilateral primary tumor after previous BCS and axillary staging. Repeat SLNB should replace routine ALND as the standard axillary restaging procedure in recurrent disease with a clinically negative axilla. Preoperative lymphoscintigraphy is important to explore extra-axillary lymphatic drainage in this restaging setting

Abstract
This text is based on the recommendations accepted by the 4th Hungarian Consensus Conference on Breast Cancer, modified on the basis of the international consultation and conference within the frames of the Central-Eastern European Academy of Oncology. The recommendations cover non-operative, intraoperative and postoperative diagnostics, determination of prognostic and predictive markers and the content of cytology and histology reports. Furthermore, they address some specific issues such as the current status of multigene molecular markers, the role of pathologists in clinical trials and prerequisites for their involvement, and some remarks about the future.The 1st Central-Eastern European Professional Consensus Statements on Breast Cancer were initiated, organized and granted by the Central-Eastern European Academy of Oncology (CEEAO), the National Institute of Oncology, Hungary and the Bács-Kiskun County Teaching Hospital. Az emlőrák korszerű patológiai diagnosztikája = Modern Pathologic Diagnostics of Breast Cancer
Az emlőrák diagnózisa morfológiai alapú. A daganat mérete, a nyirokcsomóstátus és a hisztológiai grade olyan patológai paraméterek, amelyek a klinikai gyakorlatban jól használhatók a terápia tervezésében. Az ösztrogénreceptor-,
a progeszteronreceptor- és a HER2-státust is rutinszerűen meghatározzák ma már a patológiai laboratóriumokban, hogy a betegek ellátásához szükséges prognosztikai és prediktív faktorokról további információt nyújtsanak. Az újabb molekuláris technikákat, így a génexpressziós profil meghatározását kiterjedten használják az emlőrák tanulmányozására, és ezeknek a tudományos erőfeszítéseknek köszönhetően már a klinikai gyakorlatban is elérhetővé váltak molekuláris prognosztikus tesztek. A szerzők áttekintést nyújtanak az invazív emlőrák tradicionális patológiai diagnosztikájáról és molekuláris klasszifikációjáról, elsősorban azok prognosztikai jelentőségére összpontosítva.
The diagnosis of breast cancer is morphologically based. Pathologic parameters, such as tumor size, lymph node status, and histological grade are well accepted to guide treatment decisions in clinical practice. Estrogen receptor,
progesterone receptor and HER2 status are also routinely assessed in today’s pathology laboratories to provide further information on predictive and prognostic factors affecting patients’ care. Newer molecular techniques, including gene-expression profiling have been widely used to study breast cancer and several molecular prognostic tests already available for clinical use stemmed from these scientific efforts.
