    Istanbul, the capital of Byzantine and Ottoman Empires for over a millennium, had a distinct and historical role in the world. Today, with increased immigrations and poorly developed settlements, the distinct cultural heritage of Istanbul has deteriorated. Furthermore, the infrastructure is inadequate. Most importantly, the earthquake fault lines in Marmara Sea are expected to "break" within the next 30 years, causing chaos to the city of Istanbul and its inhabitants. In order to plan for such a devastating event, the impact of an earthquake and tsunami were evaluated to determine the best urban planning to preserve Istanbul's complex historical sites, while addressing physical (structural) and infrastructural problems

    WOS: 000492028600001PubMed: 31640548Background Acute myocarditis (AM) can be defined as an inflammatory disease of the myocardium and characterized by large heterogeneity of clinical presentation. Myocarditis is becoming increasingly recognized as a contributor to unexplained mortality, and is thought to be a major cause of sudden cardiac death in the first two decades of life. the present study aimed to search the assessment of repolarization dispersion measured from the 12-lead surface electrocardiogram (including Tp-e interval, Tp-e/QT and Tp-e/QTc ratios) in AM patients. Methods Totally 56 patients (mean age was 22 +/- 3.7 years and 67% of the patients were male) with AM and 56 control subjects (23 +/- 4.7 years and 64% of the patients were male) were enrolled. Tp-e intervals, Tp-e/QT and Tp-e/corrected QT (QTc) ratios were calculated from 12-lead electrocardiogram. Results Heart rate, QT and QTc values were similar between groups. QRS interval was lower in AM group compared to the control group (p < 0.001). Tp-e, Tp-e/QT and Tp-e/ QTc were significantly higher in AM group (p < 0.001, p < 0.001, p = 0.03 respectively) and they were significantly correlated with high troponin and high sensitive C reactive protein levels. in hospital follow-up time was 6 +/- 2 days. Four patients have non sustained ventricular tachyarrhythmias and 1 patient dead because of cardiac arrest. Conclusions Our study demonstrated that Tp-e intervals, Tp-e/QT and Tp-e/QTc ratios were higher in patients with AM than control subjects. the increased frequency of ventricular arrhythmias can be clarified by increased indexes of ventricular repolarization parameters in patients with AM

    INTRODUCTION: Giant cervical and mediastinal goiter may lead to acute respiratory failure caused by laryngotracheal compression and airway obstruction. Here, we present a case admitted to the emergency service with a giant goiter along with respiratory failure and poor general health status, which required urgent surgical intervention. PRESENTATION OF CASE: A 71-year-old female admitted to the emergency room with shortness of breath and poor general health status resulting from a giant cervical swelling progressively increased during the last 7 years and constituted severe respiratory failure which has become severe in the last one month. A giant nodular goiter of the left thyroid lobe extending retrosternally, causing tracheal compression, limiting the neck movements was detected with clinical examination and bedside ultrasound. Emergency thyroidectomy was planned. Fiberoptic-assisted awake nasal intubation was performed in the operating room. Emergency total thyroidectomy was performed for the life-threatening respiratory failure. Postoperative period was uneventful. She was transferred from intensive care unit to the ward on postoperative day 3 and was discharged from the hospital on the postoperative 7th day. Benign multinodular hyperplasia was reported on the histopathological report. Patient was included in routine follow-up. DISCUSSION: In the present case tracheal destruction due to compression of the giant goiter was found in agreement with previous reports. Emergency thyroidectomy was performed after awake intubation since it is a common surgical option for the treatment of giant goiter causing severe airway obstruction. CONCLUSION: Respiratory failure due to giant nodular goiter is a life-threatening situation and should be treated immediately by performing awake endotracheal intubation following emergency total thyroidectomy

    In the implementation of natural orifice transluminal endoscopic surgery ( NOTES) there is a risk of infection arising from the orifices of the body and the endoscope used. This study investigates the need for and efficiency of gastric lavage in reducing the risk of peritoneal infection in transgastric intraperitoneal surgery. In this study, Wistar-Albino rats were divided into 4 groups of 8. Gastric lavage was performed with three, but it was not performed on group 4. Blood samples were taken on the first and 14th days postoperatively, and a leukocyte count was made and C-reactive protein was measured. On the 14th postoperative day, the rats were sacrificed; aerobic and anaerobic culture specimens were taken from the peritoneal cavity, and biopsy samples were taken. Pathological examination of biopsy lavage in those groups, all for less inflammatory reaction and foreign body giant cell formation mikroabses were observed. In the all groups Leukocyte count and CRP measurements according to measurement values are rising. Culture examination of samples of seven different rats, while producing, anaerobic culture was not isolated in. As a result of gastric lavage should be done to ensure that surgery more safely performed of transgastric peritoneoscopy.
