    Tendon Interposition and Ligament Reconstruction with ECRL Tendon in the Late Stages of Kienböck’s Disease: A Cadaver Study

    Background. The optimal surgical treatment for Kienböck’s disease with stages IIIB and IV remains controversial. A cadaver study was carried out to evaluate the use of coiled extensor carpi radialis longus tendon for tendon interposition and a strip obtained from the same tendon for ligament reconstruction in the late stages of Kienböck’s disease. Methods. Coiled extensor carpi radialis longus tendon was used to fill the cavity of the excised lunate, and a strip obtained from this tendon was sutured onto itself after passing through the scaphoid and the triquetrum acting as a ligament to preserve proximal row integrity. Biomechanical tests were carried out in order to evaluate this new ligamentous reconstruction. Results. It was biomechanically confirmed that the procedure was effective against axial compression and distributed the upcoming mechanical stress to the distal row. Conclusion. Extensor carpi radialis longus tendon has not been used for tendon interposition and ligament reconstruction in the treatment of this disease before. In view of the biomechanical data, the procedure seems to be effective for the stabilization of scaphoid and carpal bones

    Bu çalışmada önce özel bir metrik ile elde edilen belirli bir manifold üzerinde sonsuz küçük hareketler veya Killing Vektör Alanlarının belirlenmesi problemi incelenmiştir. Bu amaçla T. Otsuki tarafından tanımlanan ve Ot-metriği adı verilen metrikten özel bir şartla elde edilen Riemann Manifoldu üzerinde Killing Vektör Alanlarının denklemleri metrik katsayılar ve Christoffel Sembolleri kullanılarak bulunmuş sonrada çeşitli metodlar ile bu diferansiyel denklemler çözülmüştür. Böylece söz konusu manifold üzerinde Killing Vektör Alanlarının belirlenmesi probleminin çözümü bir teoremle ifade edilmiştir. Daha sonra ise aynı manifold üzerinde Harmonik, Konform Killing ve Homotetik Killing Vektör Alanlarının belirlenmesi problemi ele alınmış ve denklemleri oluşturulmuştur. Söz konusu denklemler, çözümü incelenen Killing Diferansiyel Denklemleri ile benzerlik gösterdiğinden bu denklemlerin çözümü başka bir araştırmaya konu olabilecektir. In this study, first, the problem of determining infinitesimal motions or Killing Vector Fields of a specific manifold which were obtained by a special metric was considered. For this purpose, equations of Killing Vector Fields on Riemann Manifold, which was defined by T. Otsuki and obtained by a special condition from a metric named Ot-metric, were obtained by using metric coefficients and Christoffel Symbols. Then, those differential equations were solved with different methods. Thus, the solution of the problem of determining Killing Vector Fields on that manifold was expressed by a theorem. Afterwards, the problem of determining Harmonic, Conform Killing and Homothetic Killing Vector Fields were investigated on the same manifold and their equations were formed. Those equations have similarities with the Killing Differential Equations investigated in this study, and their solutions may be subject to future studies

The Publisher's final version can be found by following the DOI link.Polydatin or piceid, is the 3-O-glucoside of resveratrol and is found abundantly in grapes, peanuts, wine, beer, and cacao products. Although anti-cancer-activity of polydatin was reported before, and potential antiproliferative mechanisms of polydatin have been proposed, its direct effects on DNA and inhibitory potential against topoisomerase enzymes have remained unknown. In this study we aimed to reveal the link between polydatin’s effects on DNA and DNA-topoisomerases and its antiproliferative promise. For this purpose, we evaluated the effects of polydatin on DNA and DNA topoisomerase using in vitro and in silico techniques. Polydatin was found to protect DNA against Fenton reaction-induced damage while not showing any hydrolytic nuclease effect. Further, polydatin inhibited topoisomerase II but not topoisomerase I. According to molecular docking studies, polydatin preferably showed minor groove binding to DNA where the stilbene moiety was important for binding to the DNA-topoisomerase II complex. As a result, topoisomerase II inhibition might be another anti-cancer mechanism of polydatin

    Pediatric dysphagia overview: best practice recommendation study by multidisciplinary experts

    Background Currently, there is no comprehensive and multidisciplinary recommendation study covering all aspects of pediatric dysphagia (PD). This study aimed to generate PD management recommendations with methods that can be used in clinical practice to fill this gap in our country and in the world, from the perspective of experienced multidisciplinary experts. Methods This recommendation paper was generated by a multidisciplinary team, using the seven-step process and a three-round modified Delphi survey via e-mail. First, ten open-ended questions were created, and then detailed recommendations including management, diagnosis, treatment, and follow-up were created with the answers from these questions. Each recommendation item was voted on by the experts as overall consensus (strong recommendation), approaching consensus (weak recommendation) and divergent consensus (not recommended). Results In the 1st Delphi round, a questionnaire of 414 items was prepared based on the experts' responses to ten open-ended questions. In the 2nd Delphi round, 59.2% of these items were accepted as pre-recommendation. In the 3rd Delphi round, 62.6% of 246 items were accepted for inclusion in the proposals. The final version recommendations consisted of 154 items. Conclusions This study includes comprehensive and detailed answers for every problem that could be posed in clinical practice for the management of PD, and recommendations are for all pediatric patients with both oropharyngeal and esophageal dysphagia
