    Application of the COM-B model to barriers and facilitators to chlamydia testing in general practice for young people and primary care practitioners: a systematic review

    Background Chlamydia is a major public health concern, with high economic and social costs. In 2016, there were over 200,000 chlamydia diagnoses made in England. The highest prevalence rates are found among young people. Although annual testing for sexually active young people is recommended, many do not receive testing. General practice is one ideal setting for testing, yet attempts to increase testing in this setting have been disappointing. The Capability, Opportunity, and Motivation Model of Behaviour (COM-B model) may help improve understanding of the underpinnings of chlamydia testing. The aim of this systematic review was to (1) identify barriers and facilitators to chlamydia testing for young people and primary care practitioners in general practice and (2) map facilitators and barriers onto the COM-B model. Methods Qualitative, quantitative, and mixed methods studies published after 2000 were included. Seven databases were searched to identify peer-reviewed publications which examined barriers and facilitators to chlamydia testing in general practice. The quality of included studies was assessed using the Critical Appraisal Skills Programme. Data (i.e., participant quotations, theme descriptions, and survey results) regarding study design and key findings were extracted. The data was first analysed using thematic analysis, following this, the resultant factors were mapped onto the COM-B model components. All findings are reported in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Results Four hundred eleven papers were identified; 39 met the inclusion criteria. Barriers and facilitators were identified at the patient (e.g., knowledge), provider (e.g., time constraints), and service level (e.g., practice nurses). Factors were categorised into the subcomponents of the model: physical capability (e.g., practice nurse involvement), psychological capability (e.g.: lack of knowledge), reflective motivation (e.g., beliefs regarding perceived risk), automatic motivation (e.g., embarrassment and shame), physical opportunity (e.g., time constraints), social opportunity (e.g., stigma). Conclusions This systematic review provides a synthesis of the literature which acknowledges factors across multiple levels and components. The COM-B model provided the framework for understanding the complexity of chlamydia testing behaviour. While we cannot at this juncture state which component represents the most salient influence on chlamydia testing, across all three levels, multiple barriers and facilitators were identified relating psychological capability and physical and social opportunity. Implementation should focus on (1) normalisation, (2) communication, (3) infection-specific information, and (4) mode of testing. In order to increase chlamydia testing in general practice, a multifaceted theory- and evidence-based approach is needed

    Superconducting Diamond on Silicon Nitride for Device Applications

    Chemical vapour deposition (CVD) grown nanocrystalline diamond is an attractive material for the fabrication of devices. For some device architectures, optimisation of its growth on silicon nitride is essential. Here, the effects of three pre-growth surface treatments, often employed as cleaning methods of silicon nitride, were investigated. Such treatments provide control over the surface charge of the substrate through modification of the surface functionality, allowing for the optimisation of electrostatic diamond seeding densities. Zeta potential measurements and X-ray photoelectron spectroscopy (XPS) were used to analyse the silicon nitride surface following each treatment. Exposing silicon nitride to an oxygen plasma offered optimal surface conditions for the electrostatic self-assembly of a hydrogen-terminated diamond nanoparticle monolayer. The subsequent growth of boron-doped nanocrystalline diamond thin films on modified silicon nitride substrates under CVD conditions produced coalesced films for oxygen plasma and solvent treatments, whilst pin-holing of the diamond film was observed following RCA-1 treatment. The sharpest superconducting transition was observed for diamond grown on oxygen plasma treated silicon nitride, demonstrating it to be of the least structural disorder. Modifications to the substrate surface optimise the seeding and growth processes for the fabrication of diamond on silicon nitride devices

    The flickering radio jet from the quiescent black hole X-ray binary A0620-00

    Weakly accreting black hole X-ray binaries launch compact radio jets that persist even in the quiescent spectral state, at X-ray luminosities <1e-5 of the Eddington luminosity. However, radio continuum emission has been detected from only a few of these quiescent systems, and little is known about their radio variability. Jet variability can lead to misclassification of accreting compact objects in quiescence, and affects the detectability of black hole X-ray binaries in next-generation radio surveys. Here we present the results of a radio monitoring campaign of A0620-00, one of the best-studied and least-luminous known quiescent black hole X-ray binaries. We observed A0620-00 at 9.8 GHz using the Karl G. Jansky Very Large Array on 31 epochs from 2017 to 2020, detecting the source ~75% of the time. We see significant variability over all timescales sampled, and the observed flux densities follow a lognormal distribution with a mean of 12.5 uJy and standard deviation of 0.22 dex. In no epoch was A0620-00 as bright as in 2005 (51 +/- 7 uJy), implying either that this original detection was obtained during an unusually bright flare, or that the system is fading in the radio over time. We present tentative evidence that the quiescent radio emission from A0620-00 is less variable than that of V404 Cyg, the only other black hole binary with comparable data. Given that V404 Cyg has a jet radio luminosity ~20 times higher than A0620-00, this comparison could suggest that less luminous jets are less variable in quiescence.Comment: 9 pages, 7 figures, accepted for publication in MNRA

    Diffuse gas in galaxies sheds new light on the origin of Type Ia supernovae

    Date of Acceptance: 05/05/2014We measure the strength of He II λ4686 nebular emission in passively evolving ('retired') galaxies, aiming to constrain their populations of hot accreting white dwarfs (WDs) in the context of the single-degenerate (SD) scenario of Type Ia supernovae (SNe Ia). In the SD scenario, as a WD burns hydrogen-rich material accreted from a companion star, it becomes a powerful source of ionizing ultraviolet emission. If significant populations of such sources exist in galaxies, strong emission in the recombination lines of He II should be expected from the interstellar medium. To explore this conjecture, we select from the Sloan Digital Sky Survey ~11 500 emission-line galaxies with stellar ages >1 Gyr showing no signs of active galactic nuclei activity and co-add their spectra in bins of stellar population age. For the first time, we detect He II λ4686 nebular emission in retired galaxies and find it to be significantly weaker than that expected in the SD scenario, especially in the youngest age bin (1-4 Gyr) where the SN Ia rate is the highest. Instead, the strength of the observed He II λ4686 nebular emission is consistent with post-asymptotic giant branch stars being the sole ionizing source in all age bins. These results limit populations of accretingWDs with photospheric temperatures (Teff) in the range ~(1.5-6) × 105 K to the level at which they can account for no more than ~5-10 per cent of the observed SN Ia rate. Conversely, should all WD progenitors of SN Ia go through the phase of steady nuclear burning with Teff ~ (1.5-6) × 105 K, they do not increase their mass by more than ~0.03M⊙ in this regime.Peer reviewedFinal Accepted Versio

    Negative symptoms and impaired social functioning predict later psychosis in Latino youth at clinical high risk in the North American prodromal longitudinal studies consortium

    AIM: Examining ethnically related variables in evaluating those at risk for psychosis is critical. This study investigated sociodemographic and clinical characteristics of Latino versus non-Latino clinical high-risk (CHR) subjects and healthy control (HC) subjects in the first North American Prodrome Longitudinal Study. METHODS: Fifty-six Latino CHR subjects were compared to 25 Latino HC and 423 non-Latino CHR subjects across clinical and demographic variables. Thirty-nine of the 56 CHR subjects completed at least one subsequent clinical evaluation over the 2.5-year period with 39% developing a psychotic illness. Characteristics of Latino CHR subjects who later converted to psychosis (‘converters’) were compared to those who did not (‘non-converters’). RESULTS: Latino CHR subjects were younger than non-Latino CHR subjects and had less education than Latino HC subjects and non-Latino CHR counterparts. Latino CHR converters had higher scores than Latino non-converters on the Structured Interview for Prodromal Syndromes total negative symptoms that were accounted for by decreased expression of emotion and personal hygiene/social attentiveness subsections. Latino CHR converters scored lower on the global functioning:social scale, indicating worse social functioning than Latino non-converters. CONCLUSION: Based on this sample, Latino CHR subjects may seek treatment earlier and have less education than non-Latino CHR subjects. Deficits in social functioning and impaired personal hygiene/social attentiveness among Latino CHR subjects predicted later psychosis and may represent important areas for future study. Larger sample sizes are needed to more thoroughly investigate the observed ethnic differences and risk factors for psychosis in Latino youth

    Functional development in clinical high risk youth: Prediction of schizophrenia versus other psychotic disorders

    This study evaluates premorbid social and academic functioning in clinical high-risk individuals as predictors of transition to schizophrenia versus another psychotic disorder. Participants were 54 individuals enrolled in phase one of the North American Prodrome Longitudinal Study who over two and a half years of follow-up met criteria for schizophrenia/schizophreniform disorder (n=28) or another psychotic disorder (n=26). Social and academic functioning in childhood, early adolescence, and late adolescence was assessed at baseline using the Cannon-Spoor Premorbid Adjustment Scale. Social maladjustment in late adolescence predicted significantly higher odds of transition to schizophrenia versus another psychotic disorder independent of childhood and early adolescent adjustment (OR = 4.02) and conveyed unique risk over academic maladjustment (O R= 5.64). Premorbid academic maladjustment was not associated with psychotic disorder diagnosis. Results support diagnostic specificity of premorbid social dysfunction to schizophrenia in clinical high-risk youth and underscore an important role for social maladjustment in the developmental pathology of schizophrenia and its prediction.     Relation between cannabis use and subcortical volumes in people at clinical high risk of psychosis

    Among people at genetic risk of schizophrenia, those who use cannabis show smaller thalamic and hippocampal volumes. We evaluated this relationship in people at clinical high risk (CHR) of psychosis. The Alcohol and Drug Use Scale was used to identify 132 CHR cannabis users, the majority of whom were non-dependent cannabis users, 387 CHR non-users, and 204 healthy control non-users, and all participants completed magnetic resonance imaging scans. Volumes of the thalamus, hippocampus and amygdala were extracted with FreeSurfer, and compared across groups. Comparing all CHR participants with healthy control participants revealed no significant differences in volumes of any ROI. However, when comparing CHR users to CHR non-users, a significant ROI × Cannabis group effect emerged: CHR users showed significantly smaller amygdala compared to CHR non-users. However, when limiting analysis to CHR subjects who reported using alcohol at a ‘use without impairment’ severity level, the amygdala effect was non-significant; rather, smaller hippocampal volumes were seen in CHR cannabis users compared to non-users. Controlling statistically for effects of alcohol and tobacco use rendered all results non-significant. These results highlight the importance of controlling for residual confounding effects of other substance use when examining the relationship between cannabis use and neural structure
