Cardiovascular Risk Factors and Dehydroepiandrosterone Sulfate Among Latinos in the Boston Puerto Rican Health Study.
Low blood dehydroepiandrosterone sulfate (DHEAS) levels have strong positive associations with stroke and coronary heart disease. However, it is unclear whether DHEAS is independently associated with cardiovascular risk factors. Therefore, we examined the association between cardiovascular risk factors and DHEAS concentration among a high-risk population of Latinos (Puerto Ricans aged 45 to 75 years at baseline) in a cross-sectional analysis of the Boston Puerto Rican Health Study. Of eligible participants, 72% completed baseline interviews and provided blood samples. Complete data were available for 1355 participants. Associations between cardiovascular risk factors (age, sex, total cholesterol, high-density lipid cholesterol, triglycerides, and glucose) and log-transformed DHEAS (μg/dL) were assessed. In robust multivariable regression analyses, DHEAS was significantly inversely associated with age (β = -12.4; 95% CI: -15.2, -9.7; per 5 years), being female (vs. male) (β = -46; 95% CI: -55.3, -36.6), and plasma triglyceride concentration (β = -0.2; 95% CI: -0.3, -0.1; per 10 mg/dL) and was positively associated with total cholesterol and plasma glucose levels (β = 1.8; 95% CI: 0.6, 3 and β = 0.2; 95% CI: 0.04, 0.3, respectively, per 10 mg/dL) after adjustment for smoking, alcohol, and physical activity and for postmenopausal hormone use in women. Estimates were unchanged after adjustment for measures of chronic disease and inflammation. Women exhibited a stronger age-related decline in DHEAS and a positive association with glucose in contrast to findings among men (P interaction < 0.05). In conclusion, in this large study of Latinos with a heavy cardiovascular risk factor burden, we observed significant associations between cardiovascular disease (CVD) risk factors and DHEAS, with variations by sex. These findings improve our understanding of the role DHEAS may play in CVD etiology
Violence Prevention: Reaching Adolescents with the Message
To identify an effective medium for communicating with adolescents in a large-scale, cost-effective violence prevention program.
A set of youth violence prevention programs was established at The Stamford Hospital, a level II trauma center. The traveling version of the program was presented to middle school students in four parts: 1) a rap music video created by our violence prevention staff, 2) a facilitated discussion about dealing with anger, 3) a video of a trauma resuscitation in our emergency department, and 4) a commercial video of a teenage boy paralyzed after a gunshot wound. A written questionnaire with a five-point rating scale (1 to 5) was used to survey the audience 1 month after the program. The survey assessed the respondents\u27 recall of each part of the program and the perceptions of the value of each part in identifying the problem of violence and reducing violent behavior.
Results: Of 99 respondents, the highest ratings for retention, problem identification, and impact were given to the commercial video (combined average category ranking of 11.394) and the rap music video (11.182). The trauma resuscitation video and the discussion of anger were ranked as being less effective (10.253 and 9.383, respectively). The audience seemed to comprehend the main point of the program and ranked the program, as a whole, higher than any of the parts when measured by success at problem identification and impact.
Effective communication with adolescents is possible through many avenues. Children of the video age respond well to visual material. A violence prevention program should incorporate effective multimedia presentations. A variety of methods hi combination proves to be most effective
Case Notes
For decades, optical time-domain searches have been tuned to find ordinary supernovae, which rise and fall in brightness over a period of weeks. Recently, supernova searches have improved their cadences and a handful of fast-evolving luminous transients have been identified(1-5). These have peak luminosities comparable to type Ia supernovae, but rise to maximum in less than ten days and fade from view in less than one month. Here we present the most extreme example of this class of object thus far: KSN 2015K, with a rise time of only 2.2 days and a time above half-maximum of only 6.8 days. We show that, unlike type Ia supernovae, the light curve of KSN 2015K was not powered by the decay of radioactive elements. We further argue that it is unlikely that it was powered by continuing energy deposition from a central remnant (a magnetar or black hole). Using numerical radiation hydrodynamical models, we show that the light curve of KSN 2015K is well fitted by a model where the supernova runs into external material presumably expelled in a pre-supernova mass-loss episode. The rapid rise of KSN 2015K therefore probes the venting of photons when a hypersonic shock wave breaks out of a dense extended medium.NASA

Detailed Structure of a CDW in a Quenched Random Field
Using high resolution x-ray scattering, we have measured the structure of the
Q_1 CDW in Ta-doped NbSe_3. Detailed line shape analysis of the data
demonstrates that two length scales are required to describe the phase-phase
correlation function. Phase fluctuations with wavelengths less than a new
length scale are suppressed and this is identified with the amplitude
coherence length. We find that xi_a* = 34.4 \pm 10.3 angstroms. Implications
for the physical mechanisms responsible for pinning are discussed.Comment: revtex 3.0, 3 postscript uuencoded figure
Effects of Placental Transfusion on Neonatal and 18 Month Outcomes in Preterm Infants: A Randomized Controlled Trial
Objective: To assess the effect of delayed cord clamping (DCC) vs immediate cord clamping (ICC) on intraventricular hemorrhage (IVH), late onset sepsis (LOS), and 18-month motor outcomes in preterm infants.
Study Design: Women (n = 208) in labor with singleton fetuses (\u3c32 weeks gestation) were randomized to either DCC (30-45 seconds) or ICC (\u3c10 seconds). The primary outcomes were IVH, LOS, and motor outcomes at 18-22 months corrected age. Intention-to-treat was used for primary analyses.
Results: Cord clamping time was 32 ± 16 (DCC) vs 6.6 ± 6 (ICC) seconds. Infants in the DCC and ICC groups weighed 1203 ± 352 and 1136 ± 350 g and mean gestational age was 28.3 ± 2 and 28.4 ± 2 weeks, respectively. There were no differences in rates of IVH or LOS between groups. At 18-22 months, DCC was protective against motor scores below 85 on the Bayley Scales of Infant Development, Third Edition (OR 0.32, 95% CI 0.10-0.90, P = .03). There were more women with preeclampsia in the ICC group (37% vs 22%, P = .02) and more women in the DCC group with premature rupture of membranes/preterm labor (54% vs 75%, P = .002). Preeclampsia halved the risk of IVH (OR 0.50, 95% CI 0.2-1.0) and premature rupture of membranes/preterm labor doubled the risk of IVH (OR 2.0, 95% CI 1.2-4.3). Conclusions: Although DCC did not alter the incidence of IVH or LOS in preterm infants, it improved motor function at 18-22 months corrected age
A Bright Submillimeter Source in the Bullet Cluster (1E0657--56) Field Detected with BLAST
We present the 250, 350, and 500 micron detection of bright submillimeter
emission in the direction of the Bullet Cluster measured by the Balloon-borne
Large Aperture Submillimeter Telescope (BLAST). The 500 micron centroid is
coincident with an AzTEC 1.1 mm point-source detection at a position close to
the peak lensing magnification produced by the cluster. However, the 250 micron
and 350 micron centroids are elongated and shifted toward the south with a
differential shift between bands that cannot be explained by pointing
uncertainties. We therefore conclude that the BLAST detection is likely
contaminated by emission from foreground galaxies associated with the Bullet
Cluster. The submillimeter redshift estimate based on 250-1100 micron
photometry at the position of the AzTEC source is z_phot = 2.9 (+0.6 -0.3),
consistent with the infrared color redshift estimation of the most likely IRAC
counterpart. These flux densities indicate an apparent far-infrared luminosity
of L_FIR = 2E13 Lsun. When the amplification due to the gravitational lensing
of the cluster is removed, the intrinsic far-infrared luminosity of the source
is found to be L_FIR <= 10^12 Lsun, consistent with typical luminous infrared
galaxies.Comment: Accepted for publication in the Astrophysical Journal. Maps are
