    Bridging the gap? The role of transition advisers in the move from compulsory education

    The quality and impartiality of careers advice has been in question for as long as it has been provided, with numerous studies (see Morris, 2004 for a summary of NfER research in this area) pointing to the efficacy of impartial guidance, but also pointing to gaps in provision for particular groups. It is possible to distinguish three areas of educational policy - although this is not exhaustive - which have had a clear effect on young people's experience of advice and guidance over recent years. The first area is in provision of careers advice itself. The second area is the 14-19 learning environment. Linked to this shift in the 14-19 environment is the third area, the impact of market-driven reforms starting with the Education Reform Act in 1988.</p

    Evaluation of the CHUMS Child Bereavement Group : A Pilot Study Examining Statistical and Clinical Change

    This is an Accepted Manuscript of an article published by Taylor & Francis in Death Studies on 7 February 2015, available online at: http://dx.doi.org/10.1080/07481187.2014.913085.This article describes the largest evaluation of a UK child bereavement service to date. Change was assessed using conventional statistical tests as well as clinical significance methodology. Consistent with the fact that the intervention was offered on a universal, preventative basis, bereaved young people experienced a statistically significant, small to medium-sized decrease in symptoms over time. This change was equivalent across child age and gender. Type of bereavement had a slight impact on change when rated by parents.     Unemployment and attitudes to work: asking the ‘right’ question

    Attitudes research has repeatedly demonstrated that the vast majority of unemployed people want a job, and that their work commitment is generally at least as strong as employed people’s. But until now it has not asked if they are more likely than employed people to prefer unemployment to an unattractive job. While this oversight reflects a noted widespread reluctance to respond directly to right-wing authors’ assertions, this article argues that it is partly attributable to existing studies using survey questions inappropriate for researching unemployment. Responses to the British Cohort Study / National Child Development Study agree / disagree statement ‘having almost any job is better than being unemployed’ were analysed. Being ‘unemployed and seeking work’ associated strongly with disagreeing with the statement across all recent datasets in both studies, even when a number of relevant variables were controlled for

    Global systematic review and ecological analysis of HIV in people who inject drugs:National population sizes and factors associated with HIV prevalence

    Background People who inject drugs (PWID) are at elevated risk of HIV infection. Data on population sizes of PWID living with HIV are needed to inform the implementation of prevention, treatment and care programs. We estimated national population sizes of people who recently (past 12 months) injected drugs living with HIV and evaluated ecological associations with HIV prevalence in PWID. Methods We used national data on the prevalence of injecting drug use and of HIV among PWID, derived from systematic reviews, to estimate national population sizes of PWID living with HIV. Uncertainty was estimated using Monte Carlo simulation with 100,000 draws. We extracted data on sample characteristics from studies of HIV prevalence among PWID, and identified national indicators that have been observed or hypothesised to be associated with HIV prevalence in PWID. We used linear regression to evaluate associations between these variables and HIV prevalence in PWID. Results Four countries comprised 55% of the estimated global population of PWID living with HIV: Russia (572,500; 95% uncertainty interval (UI) 235,500–1,036,500); Brazil (462,000; 95% UI 283,500–674,500); China (316,500; 95% UI 171,500–493,500), and the United States (195,500; 95% UI 80,000–343,000). Greater anti-HCV prevalence and national income inequality were associated with greater HIV prevalence in PWID. Conclusion The countries with the largest populations of PWID living with HIV will need to dramatically scale up prevention, treatment and care interventions to prevent further increases in population size. The association between anti-HCV prevalence and HIV prevalence among PWID corroborates findings that settings with increasing HCV should implement effective interventions to prevent HIV outbreaks. The association between income inequality and HIV among PWID reinforces the need to implement structural interventions alongside targeted individual-level strategies

    The interlayer cohesive energy of graphite from thermal desorption of polyaromatic hydrocarbons

    We have studied the interaction of polyaromatic hydrocarbons (PAHs) with the basal plane of graphite using thermal desorption spectroscopy. Desorption kinetics of benzene, naphthalene, coronene and ovalene at sub-monolayer coverages yield activation energies of 0.50 eV, 0.85 eV, 1.40 eV and 2.1 eV, respectively. Benzene and naphthalene follow simple first order desorption kinetics while coronene and ovalene exhibit fractional order kinetics owing to the stability of 2-D adsorbate islands up to the desorption temperature. Pre-exponential frequency factors are found to be in the range 101410^{14}-1021s110^{21} s^{-1} as obtained from both Falconer--Madix (isothermal desorption) analysis and Antoine's fit to vapour pressure data. The resulting binding energy per carbon atom of the PAH is 52±52\pm5 meV and can be identified with the interlayer cohesive energy of graphite. The resulting cleavage energy of graphite is 61±561\pm5~meV/atom which is considerably larger than previously reported experimental values.Comment: 8 pages, 4 figures, 2 table

    Electron acceleration by a transient intense-laser-plasma electrode

    Rapid strides in the technology of laser plasma-based acceleration of charged particles leading to high brightness, tunable, monochromatic energetic beams of electrons and ions has been driven by potential multidisciplinary applications in cancer therapy, isotope preparation, radiography and thermonuclear fusion. Hitherto laser plasma acceleration schemes were confined to large-scale facilities generating a few tens of terawatt to petawatt laser pulses at repetition rates of 10 Hz or less. However, the need to make viable systems using high-repetition-rate femtosecond lasers has impelled recent research into novel targetry [1,2]. Of contemporary importance is the generation of supra thermal electrons, beyond those predicted by the scaling relation, reflected in both theoretical and computational work [3,4]. In this work we present evidence of generation of relativistic electrons (temperature >200 keV, maximum energies >1 MeV) at intensities that are two orders of magnitude lower than the relativistic intensity threshold. The novel targets [6] are 15 micron sized crystals suspended as aerosols in a gas interacting with a kHz, few-mJ femtosecond laser focussed to intensities of 10 PW/cm2. A pre-pulse with 1-5% of the intensity of the main pulse, arriving 4 ns early, is critical for hot electron generation. In addition to this unprecedented energy enhancement, we also characterize the dependence of X-ray spectra on the background gas of the aerosol. Intriguingly, easier the gas is to ionise, greater is the number of hot electrons observed, while the electron temperature remains the same. 2-D Radiation hydrodynamics and Particle-in-cell (PIC) simulations explain both the experimentally observed electron emission and the role of the low-density plasma in yield enhancement. We observe a two-temperature electron spectrum with about 50 and 240 keV temperatures consistent with the measurements made in the experiments. The simulations show that the following features contribute to the high-energy electron emission. The pre-pulse generates a hemispherical plasma-profile that enhances the coupling of the laser light. Overdense plasma is generated about the hemispherical cavity on the particle due to the main pulse interaction. The gradient in the plasma density in and around the cavity serves as a reservoir of low energy electrons to be injected into the particle potential and enables the hot electron generation observed in the experiments. Higher energy electron emission is dominantly from the edges of the hemispherical cavitation. The increase in total X-ray yield observed in the experiments scales with the number of electrons generated in the low density neighborhood surrounding the particle. In a simple-man picture, the laser interacts with the particle and ejects electrons from the particle. The particle acquires a strong positive potential that can only be brought down by ion expansion that occurs over 10's of picoseconds. The particle with strong positive potential acts as an 'accelerating electrode' for the electrons ionized in the low-density gas neighborhood. These results assume importance in the context of applications such as fast fuel ignition [6] or in medical applications of laser plasmas [7] where high irradiance of energetic electrons is of consequence. 1.     Composition of the pericellular matrix modulates the deformation behaviour of chondrocytes in articular cartilage under static loading

    The aim was to assess the role of the composition changes in the pericellular matrix (PCM) for the chondrocyte deformation. For that, a three-dimensional finite element model with depth-dependent collagen density, fluid fraction, fixed charge density and collagen architecture, including parallel planes representing the split-lines, was created to model the extracellular matrix (ECM). The PCM was constructed similarly as the ECM, but the collagen fibrils were oriented parallel to the chondrocyte surfaces. The chondrocytes were modelled as poroelastic with swelling properties. Deformation behaviour of the cells was studied under 15% static compression. Due to the depth-dependent structure and composition of cartilage, axial cell strains were highly depth-dependent. An increase in the collagen content and fluid fraction in the PCMs increased the lateral cell strains, while an increase in the fixed charge density induced an inverse behaviour. Axial cell strains were only slightly affected by the changes in PCM composition. We conclude that the PCM composition plays a significant role in the deformation behaviour of chondrocytes, possibly modulating cartilage development, adaptation and degeneration. The development of cartilage repair materials could benefit from this information

    Usage of low dead space syringes and association with hepatitis C prevalence amongst people who inject drugs in the UK

    IntroductionSyringes with attached needles (low dead space syringes [LDSS]) retain far less blood following injection than syringes with detachable needles (high dead space syringes [HDSS]). People who inject drugs (PWID) who share needles/syringes may be less likely to acquire Hepatitis C virus (HCV) infection using LDSS, compared with HDSS, but data are limited.MethodsUtilising drug behaviour and HCV antibody testing data from the UK 2014/2015 Unlinked Anonymous Monitoring Survey of PWID, we calculated the percentage of syringes used in the past month that were LDSS. We investigated which injecting characteristics and demographic factors were associated with 100% LDSS (against 0-99%) usage, and whether 100% LDSS use was associated with antibody HCV-status, after adjusting for confounders.ResultOf 2,174 participants, 55% always used LDSS, 27% always used HDSS, and 17% used both LDSS and HDSS. PWID that had injected into their groin during the past month were unlikely to use LDSS, adjusted odds ratio (aOR) 0.14 (95% confidence interval 0.11-0.17), compared to those not using the groin. Those injecting crack were less likely to use LDSS than those not, aOR 0.79 (0.63-0.98). Polydrug use was negatively associated with LDSS use, aOR 0.88 (0.79-0.98) per additional drug. LDSS use was associated with lower prevalent HCV among all PWID (aOR 0.77, [0.64-0.93]), which was stronger among recent initiates (aOR 0.53 [0.30-0.94]) than among experienced PWID (aOR 0.81 [0.66-0.99]).DiscussionPeople who inject into their groin were less likely to use LDSS. Exclusive LDSS use was associated with lower prevalence of HCV amongst PWID that started injecting recently, suggesting LDSS use is protective against HCV

    Associations of modern initial antiretroviral drug regimens with all-cause mortality in adults with HIV in Europe and North America: a cohort study

    Background: Over the past decade, antiretroviral therapy (ART) regimens that include integrase strand inhibitors (INSTIs) have become the most commonly used for people with HIV starting ART. Although trials and observational studies have compared virological failure on INSTI-based with other regimens, few data are available on mortality in people with HIV treated with INSTIs in routine care. Therefore, we compared all-cause mortality between different INSTI-based and non-INSTI-based regimens in adults with HIV starting ART from 2013 to 2018. Methods: This cohort study used data on people with HIV in Europe and North America from the Antiretroviral Therapy Cohort Collaboration (ART-CC) and UK Collaborative HIV Cohort (UK CHIC). We studied the most common third antiretroviral drugs (additional to nucleoside reverse transcriptase inhibitor) used from 2013 to 2018: rilpivirine, darunavir, raltegravir, elvitegravir, dolutegravir, efavirenz, and others. Adjusted hazard ratios (aHRs; adjusted for clinical and demographic characteristics, comorbid conditions, and other drugs in the regimen) for mortality were estimated using Cox models stratified by ART start year and cohort, with multiple imputation of missing data. Findings: 62 500 ART-naive people with HIV starting ART (12 422 [19·9%] women; median age 38 [IQR 30–48]) were included in the study. 1243 (2·0%) died during 188 952 person-years of follow-up (median 3·0 years [IQR 1·6–4·4]). There was little evidence that mortality rates differed between regimens with dolutegravir, elvitegravir, rilpivirine, darunavir, or efavirenz as the third drug. However, mortality was higher for raltegravir compared with dolutegravir (aHR 1·49, 95% CI 1·15–1·94), elvitegravir (1·86, 1·43–2·42), rilpivirine (1·99, 1·49–2·66), darunavir (1·62, 1·33–1·98), and efavirenz (2·12, 1·60–2·81) regimens. Results were similar for analyses making different assumptions about missing data and consistent across the time periods 2013–15 and 2016–18. Rates of virological suppression were higher for dolutegravir than other third drugs. Interpretation: This large study of patients starting ART since the introduction of INSTIs found little evidence that mortality rates differed between most first-line ART regimens; however, raltegravir-based regimens were associated with higher mortality. Although unmeasured confounding cannot be excluded as an explanation for our findings, virological benefits of first-line INSTIs-based ART might not translate to differences in mortality.
