Variation in enamel mechanical properties throughout the crown in catarrhine primates.
Enamel mechanical properties vary across molar crowns, but the relationship among mechanical properties, tooth function, and phylogeny are not well understood. Fifteen primate lower molars representing fourteen taxa (catarrhine, n = 13; platyrrhine, n = 1) were sectioned in the lingual-buccal plane through the mesial cusps. Gradients of enamel mechanical properties, specifically hardness and elastic modulus, were quantified using nanoindentation from inner (near the enamel-dentine junction), through middle, to outer enamel (near the outer enamel surface) at five positions (buccal lateral, buccal cuspal, occlusal middle, lingual cuspal, lingual lateral). Cuspal positions had higher mechanical property values than lateral positions. Middle enamel had higher mean hardness and elastic modulus values than inner and outer locations in all five crown positions. Functionally, the thicker-enameled buccal cusps of lower molars did not show evidence of increased resistance to failure; instead, lingual cusps-which show higher rates of fracture-had higher average mechanical property values, with no significant differences observed between sides. Preliminary phylogenetic results suggest there is relatively little phylogenetic signal in gradients of mechanical properties through the enamel or across the crown. There appears to be common mechanical property patterns across molar crowns in Catarrhini and potentially among primates more broadly. These results may allow more precise interpretations of dental biomechanics and processes resulting in mechanical failure of enamel in primates, such as wear and fracture. Doctors' learning habits: CME activities among Norwegian physicians over the last decade
<p>Abstract</p> <p>Background</p> <p>Coping with the increasing body of medical knowledge is a main challenge to all doctors. The aim of this study was to investigate self reported reading and learning habits among Norwegian doctors and their subjective ability to keep professionally updated.</p> <p>Methods</p> <p>A cross sectional survey among a randomised sample of Norwegian doctors was undertaken in 2004 (n = 1005, response rate 71%). A similar study with many identical questions was done in 1993 (n = 1041, response rate 71%) and a comparison of the results was made.</p> <p>Results</p> <p>Attending courses/congresses and reading medical literature were reported to be the most important sources of professional information in 2004, just like in 1993. Less time was spent on courses/congresses in 2004 than in 1993, and more time was spent on medical reading. The internet was regarded as useful for their professional life for three out of five, mostly among the younger and least among GPs.</p> <p>Two out of three doctors felt that they could obtain sufficient information for keeping updated in 2004, the same proportion as in 1993. A correlation was found between subjective coping with the information and a high level of continuing medical education (CME)-activities. The information copers had a higher level of job satisfaction than non-copers.</p> <p>Conclusion</p> <p>Over the last decade Norwegian doctors spend less time on attending courses/congresses and more time on medical reading, while the level of self perceived coping with information has been unchanged. The changing pattern of professional updating may reflect a more general individualistic trend in society. The consistent finding of a correlation between reading and attending courses, subjective coping and job satisfaction gives good reasons for recommending a high level of CME-activities among doctors.</p
Network inference - with confidence - from multivariate time series
Networks - collections of interacting elements or nodes - abound in the
natural and manmade worlds. For many networks, complex spatiotemporal dynamics
stem from patterns of physical interactions unknown to us. To infer these
interactions, it is common to include edges between those nodes whose time
series exhibit sufficient functional connectivity, typically defined as a
measure of coupling exceeding a pre-determined threshold. However, when
uncertainty exists in the original network measurements, uncertainty in the
inferred network is likely, and hence a statistical propagation-of-error is
needed. In this manuscript, we describe a principled and systematic procedure
for the inference of functional connectivity networks from multivariate time
series data. Our procedure yields as output both the inferred network and a
quantification of uncertainty of the most fundamental interest: uncertainty in
the number of edges. To illustrate this approach, we apply our procedure to
simulated data and electrocorticogram data recorded from a human subject during
an epileptic seizure. We demonstrate that the procedure is accurate and robust
in both the determination of edges and the reporting of uncertainty associated

Evaluation of Important Treatment Parameters in Supraphysiological Thermal Therapy of Human Liver Cancer HepG2 Cells
This study was aimed at simulating the effect of various treatment parameters like heating rate (HR), peak temperature (PT) and hold/total treatment time on the viability of human liver cancer HepG2 cells subjected to different thermal therapy conditions. The problem was approached by investigating the injury kinetics obtained using experimentally measured viability of the cells, heated to temperatures of 50–70°C for 0–9 min at HRs of 100, 200, 300 and 525°C min(−1). An empirical expression obtained between the activation energy (E) and HR was extended to obtain the E values over a broad range of HRs from 5 to 600°C min(−1) that mimic the actual conditions encountered in a typical thermal therapy protocol. Further, the effect of the HR (5–600°C min(−1)) and PT (50–85°C) on the cell survival was studied over a range of hold times. A significant drop in survival from 90% to 0% with the simultaneous increase in HR and PT was observed as the hold time increased from 0 to 5 min. For complete cell death, the hold time increased with the increase in the HR for a given PT, while the total time showed presence of minima for 60, 65 and 70°C at HRs of 50, 100 and 200°C min(−1), respectively
Institutionalizing Provider-Initiated HIV Testing and Counselling for Children: An Observational Case Study from Zambia
Background: Provider-initiated testing and counselling (PITC) is a priority strategy for increasing access for HIV-exposed children to prevention measures, and infected children to treatment and care interventions. This article examines efforts to scale-up paediatric PITC at a second-level hospital located in Zambia’s Southern Province, and serving a catchment area of 1.2 million people. Methods and Principal Findings: Our retrospective case study examined best practices and enabling factors for rapid institutionalization of PITC in Livingstone General Hospital. Methods included clinical observations, key informant interviews with programme management, and a desk review of hospital management information systems (HMIS) uptake data following the introduction of PITC. After PITC roll-out, the hospital experienced considerably higher testing uptake. In a 36-month period following PITC institutionalization, of total inpatient children eligible for PITC (n = 5074), 98.5 % of children were counselled, and 98.2 % were tested. Of children tested (n = 4983), 15.5 % were determined HIVinfected; 77.6 % of these results were determined by DNA polymerase chain reaction (PCR) testing in children under the age of 18 months. Of children identified as HIV-infected in the hospital’s inpatient and outpatient departments (n = 1342), 99.3 % were enrolled in HIV care, including initiation on co-trimoxazole prophylaxis. Effects of elevated seawater pCO2 on gene expression patterns in the gills of the green crab, Carcinus maenas
Background: The green crab Carcinus maenas is known for its high acclimation potential to varying environmental
abiotic conditions. A high ability for ion and acid-base regulation is mainly based on an efficient regulation
apparatus located in gill epithelia. However, at present it is neither known which ion transport proteins play a key
role in the acid-base compensation response nor how gill epithelia respond to elevated seawater pCO2 as
predicted for the future. In order to promote our understanding of the responses of green crab acid-base
regulatory epithelia to high pCO2, Baltic Sea green crabs were exposed to a pCO2 of 400 Pa. Gills were screened
for differentially expressed gene transcripts using a 4,462-feature microarray and quantitative real-time PCR.
Results: Crabs responded mainly through fine scale adjustment of gene expression to elevated pCO2. However, 2%
of all investigated transcripts were significantly regulated 1.3 to 2.2-fold upon one-week exposure to CO2 stress.
Most of the genes known to code for proteins involved in osmo- and acid-base regulation, as well as cellular stress
response, were were not impacted by elevated pCO2. However, after one week of exposure, significant changes
were detected in a calcium-activated chloride channel, a hyperpolarization activated nucleotide-gated potassium
channel, a tetraspanin, and an integrin. Furthermore, a putative syntaxin-binding protein, a protein of the
transmembrane 9 superfamily, and a Cl-/HCO3
- exchanger of the SLC 4 family were differentially regulated. These
genes were also affected in a previously published hypoosmotic acclimation response study.
Conclusions: The moderate, but specific response of C. maenas gill gene expression indicates that (1) seawater
acidification does not act as a strong stressor on the cellular level in gill epithelia; (2) the response to hypercapnia
is to some degree comparable to a hypoosmotic acclimation response; (3) the specialization of each of the
posterior gill arches might go beyond what has been demonstrated up to date; and (4) a re-configuration of gill
epithelia might occur in response to hypercapnia
Nonvirally Modified Autologous Primary Hepatocytes Correct Diabetes and Prevent Target Organ Injury in a Large Preclinical Model
BACKGROUND: Current gene- and cell-based therapies have significant limitations which impede widespread clinical application. Taking diabetes mellitus as a paradigm, we have sought to overcome these limitations by ex vivo electrotransfer of a nonviral insulin expression vector into primary hepatocytes followed by immediate autologous reimplantation in a preclinical model of diabetes. METHODS AND RESULTS: In a single 3-hour procedure, hepatocytes were isolated from a surgically resected liver wedge, electroporated with an insulin expression plasmid ex vivo and reimplanted intraparenchymally under ultrasonic guidance into the liver in each of 10 streptozotocin-induced diabetic Yorkshire pigs. The vector was comprised of a bifunctional, glucose-responsive promoter linked to human insulin cDNA. Ambient glucose concentrations appropriately altered human insulin mRNA expression and C-peptide secretion within minutes in vitro and in vivo. Treated swine showed correction of hyperglycemia, glucose intolerance, dyslipidemia and other metabolic abnormalities for > or = 47 weeks. Metabolic correction correlated significantly with the number of hepatocytes implanted. Importantly, we observed no hypoglycemia even under fasting conditions. Direct intrahepatic implantation of hepatocytes did not alter biochemical indices of liver function or induce abnormal hepatic lobular architecture. About 70% of implanted hepatocytes functionally engrafted, appeared histologically normal, retained vector DNA and expressed human insulin for > or = 47 weeks. Based on structural tissue analyses and transcriptome data, we showed that early correction of diabetes attenuated and even prevented pathological changes in the eye, kidney, liver and aorta. CONCLUSIONS: We demonstrate that autologous hepatocytes can be efficiently, simply and safely modified by electroporation of a nonviral vector to express, process and secrete insulin durably. This strategy, which achieved significant and sustained therapeutic efficacy in a large preclinical model without adverse effects, warrants consideration for clinical development especially as it could have broader future applications for the treatment of other acquired and inherited diseases for which systemic reconstitution of a specific protein deficiency is critical
