The Parkinson's progression markers initiative (PPMI) - establishing a PD biomarker cohort.
ObjectiveThe Parkinson's Progression Markers Initiative (PPMI) is an observational, international study designed to establish biomarker-defined cohorts and identify clinical, imaging, genetic, and biospecimen Parkinson's disease (PD) progression markers to accelerate disease-modifying therapeutic trials.MethodsA total of 423 untreated PD, 196 Healthy Control (HC) and 64 SWEDD (scans without evidence of dopaminergic deficit) subjects were enrolled at 24 sites. To enroll PD subjects as early as possible following diagnosis, subjects were eligible with only asymmetric bradykinesia or tremor plus a dopamine transporter (DAT) binding deficit on SPECT imaging. Acquisition of data was standardized as detailed at www.ppmi-info.org.ResultsApproximately 9% of enrolled subjects had a single PD sign at baseline. DAT imaging excluded 16% of potential PD subjects with SWEDD. The total MDS-UPDRS for PD was 32.4 compared to 4.6 for HC and 28.2 for SWEDD. On average, PD subjects demonstrated 45% and 68% reduction in mean striatal and contralateral putamen Specific Binding Ratios (SBR), respectively. Cerebrospinal fluid (CSF) was acquired from >97% of all subjects. CSF (PD/HC/SWEDD pg/mL) α-synuclein (1845/2204/2141) was reduced in PD vs HC or SWEDD (P < 0.03). Similarly, t-tau (45/53) and p-tau (16/18) were reduced in PD versus HC (P < 0.01).InterpretationPPMI has detailed the biomarker signature for an early PD cohort defined by clinical features and imaging biomarkers. This strategy provides the framework to establish biomarker cohorts and to define longitudinal progression biomarkers to support future PD treatment trials
İzmir İlindeki Çiftçilerin Kırsal Nüfusun Yaşlanma Eğilimi ve Tarımsal Faaliyetlerin Devamlılığına İlişkin Görüşleri
Amaç: Türkiye’de kırsal nüfusun yaş ortalaması artmaktadır. Kırsal nüfusun büyük bir çoğunluğu geçimini tarım sektöründen sağlamakta ve yaşlanma eğilimi sektörün karşılaşabileceği muhtemel sorunlardan biri olarak karşımıza çıkmaktadır. Bu nedenle bu çalışmada çiftçilerin kırsal alandaki yaşlanma eğilimi, yaşlanmanın tarımsal faaliyetlere etkisi ve kendilerinden sonra tarımsal üretimin devamlılığına ilişkin görüşlerinin belirlenmesi amaçlanmıştır. Tasarım/Metodoloji/Yaklaşım: Çalışmanın ana materyalini İzmir iline bağlı Ödemiş ve Bayındır ilçelerinden seçilmiş mahallelerdeki 118 çiftçi ile yüz yüze yapılan anket verileri oluşturmaktadır. Araştırmada gençlerin dışında kalan çiftçiler ile görüşülmesi hedeflenmiş ve 41 yaş ve üzeri çiftçilerle görüşülmüştür.Bulgular: Görüşülen çiftçilerin yaklaşık %75’i tarımla uğraşan kesimin yaş ortalamasının arttığını ve bunun esas nedeninin genç nüfusun kırsal alandan göç etmesi olduğunu belirtmiştir. Tarımla uğraşan kesimin yaşlanma eğiliminde olması çiftçilerin tarımsal faaliyetlere katılımını azaltmaktadır. Çiftçilerin yaklaşık %56’sı tarımsal faaliyetlere devam etmek istememekte ve yaklaşık %58’i kendilerinden sonra çocuklarının ve/veya yakınlarının tarımsal faaliyete devam etmeyeceğini ifade etmiştir.Özgün/Değer: Şu ana kadar yapılan çalışmalarda genellikle genç çiftçiler ele alınmış ve gençlerin tarımda kalmalarının sağlanması amaçlanmıştır. Bununla birlikte bu çalışmada gençlerin dışında kalan çiftçiler ile görüşülmesi ve kırsal alandaki yaşlanma eğiliminin tarımsal faaliyetlerin devamlılığı açısından incelenmesi hedeflenmiştir
Multiple modality biomarker prediction of cognitive impairment in prospectively followed de novo Parkinson disease
To assess the neurobiological substrate of initial cognitive decline in Parkinson's disease (PD) to inform patient management, clinical trial design, and development of treatments.We longitudinally assessed, up to 3 years, 423 newly diagnosed patients with idiopathic PD, untreated at baseline, from 33 international movement disorder centers. Study outcomes were four determinations of cognitive impairment or decline, and biomarker predictors were baseline dopamine transporter (DAT) single photon emission computed tomography (SPECT) scan, structural magnetic resonance imaging (MRI; volume and thickness), diffusion tensor imaging (mean diffusivity and fractional anisotropy), cerebrospinal fluid (CSF; amyloid beta [Aβ], tau and alpha synuclein), and 11 single nucleotide polymorphisms (SNPs) previously associated with PD cognition. Additionally, longitudinal structural MRI and DAT scan data were included. Univariate analyses were run initially, with false discovery rate = 0.2, to select biomarker variables for inclusion in multivariable longitudinal mixed-effect models.By year 3, cognitive impairment was diagnosed in 15-38% participants depending on the criteria applied. Biomarkers, some longitudinal, predicting cognitive impairment in multivariable models were: (1) dopamine deficiency (decreased caudate and putamen DAT availability); (2) diffuse, cortical decreased brain volume or thickness (frontal, temporal, parietal, and occipital lobe regions); (3) co-morbid Alzheimer's disease Aβ amyloid pathology (lower CSF Aβ 1-42); and (4) genes (COMT val/val and BDNF val/val genotypes).Cognitive impairment in PD increases in frequency 50-200% in the first several years of disease, and is independently predicted by biomarker changes related to nigrostriatal or cortical dopaminergic deficits, global atrophy due to possible widespread effects of neurodegenerative disease, co-morbid Alzheimer's disease plaque pathology, and genetic factors

Clinical and Dopamine Transporter Imaging Characteristics of Leucine Rich Repeat Kinase 2 (LRRK2) and Glucosylceramidase Beta (GBA) Parkinson's Disease Participants in the Parkinson's Progression Markers Initiative: A Cross-Sectional Study.
BackgroundThere are limited data on the phenotypic and dopamine transporter (DAT) imaging characterization of the Parkinson's disease (PD) patients with leucine rich kinase 2 (LRRK2) and glucosylceramidase beta (GBA) mutations.ObjectiveThe objective of this study was to examine baseline clinical and DAT imaging characteristics in GBA and LRRK2 mutation carriers with early PD compared with sporadic PD.MethodsThe Parkinson's Progression Markers Initiative is an ongoing observational longitudinal study that enrolled participants with sporadic PD, LRRK2 and GBA PD carriers from 33 sites worldwide. All participants are assessed annually with a battery of motor and nonmotor scales, 123-I Ioflupane DAT imaging, and biologic variables.ResultsWe assessed 158 LRRK2 (89% G2019S), 80 GBA (89 %N370S), and 361 sporadic PD participants with the mean (standard deviation) disease duration of 2.9 (1.9), 3.1 (2.0), and 2.6 (0.6) years, respectively. When compared with sporadic PD, the GBA PD patients had no difference in any motor, cognitive, or autonomic features. The LRRK2 PD patients had less motor disability and lower rapid eye movement behavior disorder questionnaire scores, but no meaningful difference in cognitive or autonomic features. Both genetic cohorts had a higher score on the impulse control disorders scale when compared with sporadic PD, but no difference in other psychiatric features. Both genetic PD cohorts had less loss of dopamine transporter on DAT imaging when compared with sporadic PD.ConclusionsWe confirm previous reports of milder phenotype associated with LRRK2-PD. A previously reported more aggressive phenotype in GBA-PD is not evident early in the disease in N370s carriers. This observation identifies a window for potential disease-modifying interventions. Longitudinal data will be essential to define the slope of progression for both genetic cohorts.Trial registrationClinicalTrials.gov (NCT01141023). © 2020 The Authors.
