Perfil soro-epidemiológico da hepatite b nas localidades de cachoeira de Teotônio e vila amazonas, Porto Velho/RO - Brasil

Experimental.O vírus da hepatite B (VHB) tem distribuição mundial e estima-se em mais de
dois bilhões o número de pessoas com evidência sorológica de infecção passada ou
presente, e destes, 350 milhões são portadores crônicos deste vírus. Em duas
localidades ribeirinhas do município de Porto Velho, Rondônia, foi realizado um
estudo observacional de corte transversal para avaliar o perfil soroepidemiológico do
VHB e possíveis fatores de risco de transmissão. Foram analisadas 660 amostras
através de exames sorológicos por ELISA para o VHB (Anti-HBc Total, Anti-HBs e
HBsAg). Os resultados mostraram que 50,7% das amostras apresentaram
positividade para algum tipo de marcador sorológico. O HBsAg esteve presente em
12 (1,8%) amostras, sendo 11 (91,6%) do sexo masculino; O Anti-HBc Total
presente em 209 (32,1%) amostras e distribuído em todas as faixas etárias, com
exceção da faixa de 11-15 anos de idade; Anti-HBs presente em 239 (36,6%) das
amostras, com as maiores prevalências nas faixas etárias a partir dos 21 anos de
idade. O estudo permitiu observar que as localidades analisadas possuem baixa
endemicidade para portadores crônicos. Entretanto, observou-se uma alta
prevalência para infecção. A porcentagem de indivíduos susceptíveis foi de 49,2%
(n=325), sendo maiores entre indivíduos entre 6-15 anos de idade. A análise das
informações obtidas entre os participantes do estudo demonstrou que alguns fatores
de transmissão, como extrações dentárias, cirurgias e compartilhamento de
materiais de higiene pessoal estão possivelmente associados à infecção pelo VHB

Análise espacial e epidemiológica da malária no município de Candeias do Jamari no estado de Rondônia

requisito para obtenção do título de Mestre.A malária é uma das principais doenças infecto-parasitárias do mundo. No
Brasil 99,7% dos casos notificados ocorrem na Amazônia Legal, cuja área possui
característica geográfica e ecológica propensa à transmissão da doença. Para
entender estes padrões, ferramentas de análise espacial vêm sendo ultimamente
utilizadas, para auxiliar no monitoramento e controle dessa doença. O presente estudo
analisou a distribuição da incidência da malária no município de Candeias do Jamari,
localizada no Estado de Rondônia, utilizando as ferramentas do SIG (Sistemas de
Informações Geográficas), para dar suporte no controle e vigilância da doença na
região. Os dados sobre os casos de malária, população e localidades foram obtidos
no Sistema de Vigilância Epidemiológica da malária (SIVEP-Malária). O banco de
dados foi gerado no software MS Excel®. As informações cartográficas do município,
foram obtidas através homepages de domínio público, disponibilizados no Instituto
Brasileiro de Geografia e Estatística (IBGE) e Agência Nacional das Águas (ANA).
Com o uso de equipamento portátil (GPS) foi possível determinar as coordenadas
geográficas das localidades do município. As localidades georreferenciadas foram
exportados para software GPS TrackMaker® para construção de mapas digitalizados
do município de Candeias do Jamari. Para a análise espacial foram utilizados os
dados da Incidência Parasitária anual do período de 2008 a 2013 de cada localidade.
A análise espacial foi realizada por meio do programa Terra View® versão 4.2.0,
utilizando o método de estimativa de densidade de Kernel, que gera uma superfície
de densidade que permite a identificação de áreas quentes. Neste estudo foram
identificadas e georreferenciadas as 84 localidades do município de Candeias do
Jamari cadastradas no SIVEP-Malária. Destas, as rurais responderam por 68,1% dos
casos de malária em 2008 e de 54,9% ao longo de 2013. No perímetro urbano do
município, no entanto, foi observado um aumento gradativo na proporção de casos de
malária no mesmo período, atingindo 45,1% em 2013. A análise espacial mostrou que
os bairros Satélite e Santa Izabel apresentaram maior intensidade para incidência da
malária nos períodos de 2012 e 2013. O presente estudo demonstrou que está
ocorrendo mudanças no padrão da distribuição da malária no município de Candeias
do Jamari, da área rural para o perímetro urbano. A técnica de Kernel se mostrou uma
ferramenta eficiente para direcionar as tomadas de decisão no controle e vigilância da
malária na região
Mosquiteiros impregnados com inseticida em Rondônia, Brasil: avaliação do impacto na incidência da malária
O uso de mosquiteiros impregnados com inseticida de longa duração (MILD), quando obedecidas as orientações da Organização Mundial da Saúde, é medida de controle de vetores da malária que pode apresentar excelentes resultados. Em 2012 foram instalados aproximadamente 150.000 MILDs em nove municípios do estado de Rondônia. Concomitantemente não houve estudo de avaliação de impacto na redução da incidência. O presente estudo analisou dados secundários da incidência, na expectativa de avaliar o impacto dos MILDs na incidência parasitária anual (IPA). Os resultados estatísticos mostram que, no período de um ano após a instalação dos MILDs, não houve diferença estatisticamente significativa na variação da IPA em relação a outros municípios que não receberam os MILDs. A adoção de medidas de controle vetorial deve ser acompanhada de estudos epidemiológicos e de avaliação de uso e eficácia para oferecer subsídios mais robustos que justifiquem a adoção desta medida de controle da malária na Região Amazônica.Mosquito nets treated with long-lasting insecticide (LLINs), when used in compliance with guidelines of the World Health Organization, may be effective for malaria vector control. In 2012, approximately 150,000 LLINs were installed in nine municipalities in the state of Rondônia. However, no studies have assessed their impact on the reduction of malaria incidence. This study analyzed secondary data of malaria incidence, in order to assess the impact of LLINs on the annual parasite incidence (API). The results showed no statistically significant differences in API one year after LLIN installation when compared to municipalities without LLINs. The adoption of measures for malaria vector control should be associated with epidemiological studies and evaluations of their use and efficiency, with the aim of offering convincing advantages that justify their implementation and limit malaria infection in the Amazon Region
Endemias e epidemias na Amazônia: malária e doenças emergentes em áreas ribeirinhas do Rio Madeira. Um caso de escola
The authors present a short review on the evolution of malaria incidence in the Brazilian Amazon, with particular reference to the Rondônia State and the Municipality of Porto Velho, where dramatic epidemics of malaria and other tropical diseases have been registered in the past. Next, they analyze the present endemic malaria situation in the Madeira River Valley where two important Hydroelectric Power Plants will be constructed in the localities of Santo Antônio and Jirau. Longitudinal surveys performed in the last four years in this area allow to demonstrate a high prevalence of symptomatic and asymptomatic malaria carriers in the area, associated to the presence of high densities of the malaria vector Anopheles darlingi all over the year. These elements are correlated to the expected arrival of a large number of human migrants originated from non endemic areas of Rondônia and other Brazilian States, attracted by the possibility of jobs in the Hydroelectric Power Plants and by secondary opportunities in commerce, leisure, education and domestic activities. These associations create favorable conditions for malaria outbreaks and other tropical diseases that must be avoided by the establishment of additional control measures, in particular in the sanitation domainApós fazer uma análise da evolução da malária na Amazônia brasileira, detalhando em particular a situação em Rondônia e no município de Porto Velho, onde ocorreram episódios dramáticos de epidemias de malária no passado, os autores apresentam o quadro atual da prevalência de malária nas áreas do Vale do Rio Madeira, que sofrerão impactos com a construção das hidrelétricas de Santo Antônio e Jirau, e alertam sobre a situação particular da malária em áreas ribeirinhas. Nessas áreas, observam-se alta incidência de malária vivax e falciparum, a presença de grande número de portadores assintomáticos de parasitas e altas densidades do vetor Anopheles darlingi o ano todo. Esses elementos, associados à provável chegada de migrantes oriundos de áreas não-endêmicas de Rondônia e de outros Estados do país, atraídos pela possibilidade de trabalho nessas hidrelétricas e oportunidades de comércio, lazer, educação e atividades domésticas, criam condições favoráveis à ocorrência de epidemias de malária e de outras doenças tropicais se não forem realizadas intervenções adequadas de controle, em particular no domínio do saneamento
Mansonella ozzardi in the municipality of Tefé, Amazonas, Brazil, 60 years after the first report: An epidemiologic study
The prevalence of mansonelliasis was studied in the municipality of Tefé, state of Amazonas, Brazil. The prevalence (thick blood smear method) was 13.6% (147/1,078), higher in the Solimões River region (16.3%) than in the Tefé River region (6.3%). In the sampled communities in the Solimões River region, a higher density of cases was observed, as indicated by a kernel analysis (odds ratio 0.34; 95% confidence interval: 0.20-0.57). Males had a higher prevalence (χ2 = 31.292, p < 0.001) than women. Mansonella ozzardi prevalence was higher in retirees and farmers (28.9% and 27%, respectively). Prevalence also significantly increased with age (χ2 = -128.17, p < 0.001), with the highest numbers occurring in persons older than 67 years
Correlation between presence of Leishmania RNA virus 1 and clinical characteristics of nasal mucosal leishmaniosis
AbstractIntroductionMucosal leishmaniosis (ML) is a severe clinical form of leishmaniosis. Complex factors related to the parasite and the host are attributed to the development of mucosal lesions. Leishmania RNA virus 1 (LRV1) can disrupt immune response, and may be the main determinant of severity of the disease; it should be investigated.ObjectiveTo study the existence of clinical differences between patients with ML with endosymbiosis by LRV1 and. those without it.MethodsA cross-sectional cohort study with clinical evaluation, polymerase chain reaction (PCR) detection of Leishmania, species classification, and search of LRV1 was performed. Only patients with confirmed diagnosis of ML by positive PCR and with nasal mucosa injuries were included in this analysis.ResultsOut of 37 patients, 30 (81.1%) were diagnosed with Leishmania braziliensis, five (13.5%) with Leishmania guyanensis, and two (5.4%) with mixed infection of L. braziliensis and L. guyanensis. LVR1 virus was present in 26 (70.3%) of the cases.ConclusionCorrelation between clinical phenotype and presence of LRV1 was not observed, although the frequency of the virus is two-fold higher in mucosal lesions than that found in the literature on skin lesions in the same geographical area
An Unbiased CO Survey Toward the Northern Region of the Small Magellanic Cloud with the Atacama Compact Array. II. CO Cloud Catalog
The nature of molecular clouds and their statistical behavior in sub-solar
metallicity environments are not fully explored yet. We analyzed an unbiased
CO( = 2-1) survey data at a spatial resolution of 2 pc in the northern
region of the Small Magellanic Cloud (SMC) with the Atacama Compact Array to
characterize the CO cloud properties. A cloud decomposition analysis identified
426 spatially/velocity-independent CO clouds and their substructures. Based on
the cross-matching with known infrared catalogs by Spitzer and Herschel, more
than 90% CO clouds show spatial correlations with point sources. We
investigated the basic properties of the CO clouds and found that the
radius-velocity linewidth (-) relation follows the Milky Way
(MW) like power-low exponent, but the intercept is 1.5 times lower than
that in the MW. The mass functions () of the CO luminosity and virial
mass are characterized by an exponent of 1.7, which is consistent with

Asymptomatic infection with Plasmodium falciparum and Plasmodium vivax in the Brazilian Amazon Basin: to treat or not to treat?
In this study, we determined whether the treatment of asymptomatic parasites carriers (APCs), which are frequently found in the riverside localities of the Brazilian Amazon that are highly endemic for malaria, would decrease the local malaria incidence by decreasing the overall pool of parasites available to infect mosquitoes. In one village, the treatment of the 19 Plasmodium falciparum-infected APCs identified among the 270 residents led to a clear reduction (Z = -2.39, p = 0.017) in the incidence of clinical cases, suggesting that treatment of APCs is useful for controlling falciparum malaria. For vivax malaria, 120 APCs were identified among the 716 residents living in five villages. Comparing the monthly incidence of vivax malaria in two villages where the APCs were treated with the incidence in two villages where APCs were not treated yielded contradictory results and no clear differences in the incidence were observed (Z = -0.09, p = 0.933). Interestingly, a follow-up study showed that the frequency of clinical relapse in both the treated and untreated APCs was similar to the frequency seen in patients treated for primary clinical infections, thus indicating that vivax clinical immunity in the population is not species specific but only strain specific
Subtype-specific gout susceptibility loci and enrichment of selection pressure on ABCG2 and ALDH2 identified by subtype genome-wide meta-analyses of clinically defined gout patients
Objectives
Genome-wide meta-analyses of clinically defined gout were performed to identify subtype-specific susceptibility loci. Evaluation using selection pressure analysis with these loci was also conducted to investigate genetic risks characteristic of the Japanese population over the last 2000–3000 years.
Methods
Two genome-wide association studies (GWASs) of 3053 clinically defined gout cases and 4554 controls from Japanese males were performed using the Japonica Array and Illumina Array platforms. About 7.2 million single-nucleotide polymorphisms were meta-analysed after imputation. Patients were then divided into four clinical subtypes (the renal underexcretion type, renal overload type, combined type and normal type), and meta-analyses were conducted in the same manner. Selection pressure analyses using singleton density score were also performed on each subtype.
Results
In addition to the eight loci we reported previously, two novel loci, PIBF1 and ACSM2B, were identified at a genome-wide significance level (p<5.0×10–8) from a GWAS meta-analysis of all gout patients, and other two novel intergenic loci, CD2-PTGFRN and SLC28A3-NTRK2, from normal type gout patients. Subtype-dependent patterns of Manhattan plots were observed with subtype GWASs of gout patients, indicating that these subtype-specific loci suggest differences in pathophysiology along patients’ gout subtypes. Selection pressure analysis revealed significant enrichment of selection pressure on ABCG2 in addition to ALDH2 loci for all subtypes except for normal type gout.
Conclusions
Our findings on subtype GWAS meta-analyses and selection pressure analysis of gout will assist elucidation of the subtype-dependent molecular targets and evolutionary involvement among genotype, phenotype and subtype-specific tailor-made medicine/prevention of gout and hyperuricaemia
Genome-wide association study revealed novel loci which aggravate asymptomatic hyperuricaemia into gout
Objective
The first ever genome-wide association study (GWAS) of clinically defined gout cases and asymptomatic hyperuricaemia (AHUA) controls was performed to identify novel gout loci that aggravate AHUA into gout.
Methods
We carried out a GWAS of 945 clinically defined gout cases and 1003 AHUA controls followed by 2 replication studies. In total, 2860 gout cases and 3149 AHUA controls (all Japanese men) were analysed. We also compared the ORs for each locus in the present GWAS (gout vs AHUA) with those in the previous GWAS (gout vs normouricaemia).
Results
This new approach enabled us to identify two novel gout loci (rs7927466 of CNTN5 and rs9952962 of MIR302F) and one suggestive locus (rs12980365 of ZNF724) at the genome-wide significance level (p<5.0×10– 8). The present study also identified the loci of ABCG2, ALDH2 and SLC2A9. One of them, rs671 of ALDH2, was identified as a gout locus by GWAS for the first time. Comparing ORs for each locus in the present versus the previous GWAS revealed three ‘gout vs AHUA GWAS’-specific loci (CNTN5, MIR302F and ZNF724) to be clearly associated with mechanisms of gout development which distinctly differ from the known gout risk loci that basically elevate serum uric acid level.
Conclusions
This meta-analysis is the first to reveal the loci associated with crystal-induced inflammation, the last step in gout development that aggravates AHUA into gout. Our findings should help to elucidate the molecular mechanisms of gout development and assist the prevention of gout attacks in high-risk AHUA individuals
