Pancreatic fibrosis assessment and association with pancreatic cancer: comparison with the extracellular volume fraction

Clinical Radiology 79, e1356 (2024); https://doi.org/10.1016/j.crad.2024.08.007.AIMS: To compare the iodine washout rate (IWR) from multiphasic contrast-enhanced computed tomography (CT) with the extracellular volume fraction (fECV) for assessing pancreatic fibrosis and its association with pancreatic cancer. MATERIALS AND METHODS: The study included 51 individuals (33 men; median age: 69 years; 21 with pancreatic cancer, 30 with other diseases) who underwent multiphasic contrast-enhanced CT and histological evaluation for fibrotic changes in pancreas. The histological pancreatic fibrosis fraction (HPFF) was assessed on Azan-stained sections. Pancreatic parenchymal enhancement values were measured to calculate IWR and fECV. Statistical methods, such as Spearman's rho and Mann–Whitney U-test, were used. Linear regression models using IWR and fECV were constructed to predict HPFF, with the performance expressed as root mean squared error (RMSE) and Akaike information criterion (AIC). RESULTS: HPFF correlated with all CT parameters at the estimated transection line, strongest for IWRPPP−EP (r=−0.69, P<0.01). HPFF and fECV values were higher in the pancreatic cancer group than in controls (30% vs. 12.5%, P<0.01; 40.3% vs. 33.0%, P<0.01), whereas IWR values were lower (IWRPPP-EP: 43.3% vs. 55.0%, P<0.01; IWRPVP-EP: 25.0% vs. 33.5%, P<0.01). Linear regression models combining IWRPPP−EP + fECV and IWRPVP−EP + fECV were superior for predicting HPFF, with lower RMSE (9.23–9.35) and AIC (379.38–380.72) values than models with IWR or fECV alone. CONCLUSION: IWRPPP−EP, IWRPVP−EP, and fECV were reliable biomarkers for noninvasively assessing pancreatic fibrosis and were associated with pancreatic cancer risk. Linear regression combining these variables showed enhanced predictive accuracy for pancreatic fibrosis
A hybrid discrete bubble-lattice Boltzmann–discrete element model for gas-charged sediments
This paper presents a hybrid discrete bubble-lattice Boltzmann–discrete element modelling framework for simulating gas-charged sediments, especially in the seabed. A discrete bubble model proposed in chemical engineering is adapted in the coupled discrete element/lattice Boltzmann method to model the migration of gas bubbles in saturated sediments involving interactions between gas bubbles and fluid/solid phases. Surface tension is introduced into the discrete bubble model in this work, so that it can handle the complex gas–fluid–solid interface. The lattice Boltzmann and discrete element methods are, respectively, employed to simulate fluid flows and mechanical behaviours of sediments. A velocity interpolation-based immerse boundary method is utilised to resolve the coupling between the fluid flow and the solid/gas phase. The proposed technique is preliminarily validated using simulations of bubble migration in fluids, which is followed by high-resolution investigations of the transport of a gas bubble in seabed sediments. It is demonstrated that this hybrid method can reproduce, to a certain degree, the characters of bubbles moving in seabed sediment tests
Regeneration of Graft Livers and Limited Contribution of Extrahepatic Cells After Partial Liver Transplantation in Humans
Background Liver regeneration is still not fully understood. Partial liver transplantation (LT) can provide the opportunity to investigate the mechanisms of liver regeneration, including the contribution of extrahepatic cells to liver regeneration. Methods Of 61 patients transplanted with partial liver graft between August 1997 and October 2006, 56 patients were studied, including 49 adults and 7 children. Sequential computed tomography volumetric analysis was performed for volume measurement, while proliferating cell nuclear antigen (PCNA) labeling index was investigated for liver cell proliferation in nonprotocol liver biopsy specimens. In addition, 15 male recipients who had female liver grafts were investigated in order to detect Y chromosomes as extrahepatic cells in nonprotocol liver biopsy specimens. Results Graft volume per standard liver volume was markedly increased after adult-to-adult living-donor (LD) LT. In pediatric transplants, there was no volume increase over time. PCNA labeling index was vigorous in adult-to-adult LDLT in the early period after LDLT. No Y chromosome was evident in hepatocytes from female-donor male-recipient grafts during or after liver regeneration. However, in the cases of failing grafts of this type, many Y-chromosome-positive cells were observed in the graft liver. The character of those cells was CD34(−), CK9(−), hepatocyte-specific antigen(−), and CD68(+/−). Conclusion In adult-to-adult LDLT, vigorous liver regeneration occurs in the graft liver, demonstrated by not only volumetric but cell kinetic analysis. Involvement of extrahepatic cells in normal liver regeneration seems limited
Imaging-guided chest biopsies: techniques and clinical results
Background
This article aims to comprehensively describe indications, contraindications, technical aspects, diagnostic accuracy and complications of percutaneous lung biopsy.
Methods
Imaging-guided biopsy currently represents one of the predominant methods for obtaining tissue specimens in patients with lung nodules; in many cases treatment protocols are based on histological information; thus, biopsy is frequently performed, when technically feasible, or in case other techniques (such as bronchoscopy with lavage) are inconclusive.
Results
Although a coaxial system is suitable in any case, two categories of needles can be used: fine-needle aspiration biopsy (FNAB) and core-needle biopsy (CNB), with the latter demonstrated to have a slightly higher overall sensitivity, specificity and accuracy.
Conclusion
Percutaneous lung biopsy is a safe procedure even though a few complications are possible: pneumothorax, pulmonary haemorrhage and haemoptysis are common complications, while air embolism and seeding are rare, but potentially fatal complications

Emergence of terpene cyclization in Artemisia annua
The emergence of terpene cyclization was critical to the evolutionary expansion of chemical diversity yet remains unexplored. Here we report the first discovery of an epistatic network of residues that controls the onset of terpene cyclization in Artemisia annua. We begin with amorpha-4,11-diene synthase (ADS) and (E)-b-farnesene synthase (BFS), a pair of terpene synthases that produce cyclic or linear terpenes, respectively. A library of B27,000 enzymes is generated by breeding combinations of natural amino-acid substitutions from the cyclic into the linear producer. We discover one dominant mutation is sufficient to activate cyclization, and together with two additional residues comprise a network of strongly epistatic interactions that activate, suppress or reactivate cyclization. Remarkably, this epistatic network of equivalent residues also controls cyclization in a BFS homologue from Citrus junos.
