    Allergy to polyethilenglicole of anti-sars cov2 vaccine recipient: A case report of young adult recipient and the management of future exposure to sars-cov2

    The main contraindication to the anti-SARS CoV2 vaccine is an anaphylactic reaction to a vaccine component. The need to vaccinate allergic people who are at higher risk can be of public health interest and this report shows a case of an allergic reaction to PEG of a HCW who had received the first dose of anti-SARS CoV2 vaccine. For 5 h after the administration of the vaccine, she had the appearance of erythematous spots on the face and neck, and a feeling of a slurred mouth and hoarseness. In order to treat the event, she was administered 8 mg intravenous dexamethasone, 1 vial intravenous chlorphenamine maleate, 250 mL intravenous 0.9% NaCl, and conventional oxygen therapy (2 L/min) with complete resolution of the suspected adverse drug reaction. According to the contraindication to the cutaneous test for this patient, BAT was used for further investigations. The patient who suffered the adverse reaction to the COVID-19 vaccine and other five allergic patients who did not report any adverse reaction after the vaccination were tested. There was a significant activation of the vaccine-reactive patient’s basophils with 14.79 CD203chigh% at the concentration of 0.2 mg/mL, while other patients were negative. People who have a confirmed reaction to a vaccine component should undergo further investigation to discover other possible cross-reactions and select the right vaccine to immunize them

    Embolization for acute nonvariceal bleeding of upper and lower gastrointestinal tract: a systematic review

    Background Acute non-variceal gastrointestinal bleedings (GIBs) are pathological conditions associated with significant morbidity and mortality. Embolization without angiographic evidence of contrast media extravasation is proposed as an effective procedure in patients with clinical and/or laboratory signs of bleeding. The purpose of this systematic review is to define common clinical practice and clinical and technical outcomes of blind and preventive embolization for upper and lower gastrointestinal bleeding.Main bodyThrough the PubMed, Embase and Google Scholar database, an extensive search was performed in the fields of empiric and preventive embolization for the treatment of upper and lower gastrointestinal bleedings (UGIB and LGIB). Inclusion criteria were: articles in English for which it has been possible to access the entire content; adults patients treated with empiric or blind transcatheter arterial embolization (TAE) for UGIB and/or LGIB. Only studies that analysed clinical and technical success rate of blind and empiric TAE for UGIB and/or LGIB were considered for our research. Exclusion criteria were: recurrent articles from the same authors, articles written in other languages, those in which the entire content could not be accessed and that articles were not consistent to the purposes of our research. We collected pooled data on 1019 patients from 32 separate articles selected according to the inclusion and exclusion criteria. 22 studies focused on UGIB (total 773 patients), one articles focused on LGIB (total 6 patients) and 9 studies enrolled patients with both UGIB and LGIB (total 240 patients). Technical success rate varied from 62% to 100%, with a mean value of 97.7%; clinical success rate varied from 51% to 100% with a mean value of 80%. The total number of complications was 57 events out of 1019 procedures analysed.ConclusionTAE is an effective procedure in the treatment of UGIB patients in which angiography does not demonstrate direct sign of ongoing bleeding. The attitude in the treatment of LGIBs must be more prudent in relation to poor vascular anastomoses and the high risk of intestinal ischemia. Blind and preventive procedures cumulatively present a relatively low risk of complications, compared to a relatively high technical and clinical success

    The bastard grunt Pomadasys incisus (Bowdich, 1825) (Teleostei: Haemulidae) in Cyprus (eastern Mediterranean Sea) - a late arrival or just a neglected species?

    Pomadasys incisus is a thermophilous coastal subtropical fish species belonging to the family Haemulidae. Originally described from Gambia, this species is widely distributed in the Eastern Atlantic from Galicia to South Africa. It has also been recorded in the Mediterranean Sea since 1840, presumably expanding its distribution in the next decades, although the species could have been already present in the basin, but simply overlooked until the mid XIX century. In this study, we first record P. incisus from Cyprus (eastern Mediterranean Sea), based on two opportunistic observations obtained through a citizen-science project and review the distribution of this species in the Mediterranean Sea. The present sighting raises the question on whether this species is a late arrival in the country or its presence has just been neglected until now. Based on present data, the most likely hypothesis is the latter one, with P. incisus occurring in low densities and being overlooked due to the absence of field studies. Whatever is true, some intrinsic or extrinsic factors may have played a role in limiting its spread or wide establishment in the above-mentioned country.</jats:p

    D-cycloserine-augmented one-session treatment of specific phobias in children and adolescents.

    BACKGROUND: D-Cycloserine has potential to enhance exposure therapy outcomes. The current study presents a preliminary randomized, placebo-controlled double-blind pilot trial of DCS-augmented one-session treatment (OST) for youth (7-14 years) with specific phobia. A secondary aim of this pilot study was to explore the effects of youth age and within-session fear reduction as potential moderators of DCS outcomes in order to generate hypotheses for a larger trial. It was hypothesized that DCS would be associated with greater improvements than placebo, that children (7-10 years) would have greater benefits than adolescents (11-14 years), and that DCS effects would be stronger for participants with the greater within-session fear reduction during the OST. METHODS: Thirty-five children and adolescents were randomized to either OST combined with DCS (n = 17), or OST combined with placebo (PBO; n = 18) and assessed at 1 week, 1 month, and 3 month following treatment. RESULTS: There were no significant pre- to post-treatment or follow-up benefits of DCS relative to placebo. Secondary analyses of age indicated that relative to PBO, DCS was associated with greater improvements for children (but not adolescents) on measures of severity at 1-month follow-up. Children in the DCS condition also showed significantly greater improvement to 1 month on global functioning relative to other groups. Conversely, adolescents had significant post-treatment benefits in the PBO condition on symptom severity measures relative to DCS, and adolescents in the DCS condition had significantly poorer functioning at 3 months relative to all other groups. Finally, there was a trend for within-session fear reduction to be associated with moderating effects of DCS, whereby greater reduction in fear was associated with greater functioning at one-month follow-up for children who received DCS, relative to PBO. LIMITATIONS: The study sample was small and therefore conclusions are tentative and require replication. CONCLUSIONS: Age and within-session fear reduction may be important moderators of DCS-augmented one-session exposure therapy, which requires testing in a fully powered randomized controlled trial

    Filling gaps via citizen science: Phyllorhiza punctata von Lendenfeld, 1884 (Cnidaria: Scyphozoa: Mastigiidae) in Cyprus (eastern Mediterranean Sea)

    The Mediterranean Sea is one of the most heavily impacted areas by biological invasions worldwide, with over 650 non-indigenous species (NIS) reported as established in 2019. Information about the distribution of NIS in the basin is often fragmented or non-existent due to factors such as lack of finance, expertise, and appropriate surveillance systems. Citizen science might represent a reliable tool in monitoring the spread of NIS, being able to overcome the challenges of scientific monitoring. In the context of the citizen science project “Is it alien to you? Share it!!!”, we hereby first report the presence of the Australian spotted jellyfish Phyllorhiza punctata von Lendenfeld, 1884 in the marine waters of Cyprus and review its known distribution in the Mediterranean Sea. Although this taxon does not represent a direct threat to human health or tourism as it bears no painful sting, it already caused important ecological and economic damages in other regions of the world, and thus its further spread in Cypriot waters requires appropriate monitoring

    Acute Colonic Diverticulitis: CT Findings, Classifications, and a Proposal of a Structured Reporting Template

    Acute colonic diverticulitis (ACD) is the most common complication of diverticular disease and represents an abdominal emergency. It includes a variety of conditions, extending from localized diverticular inflammation to fecal peritonitis, hence the importance of an accurate diagnosis. Contrast-enhanced computed tomography (CE-CT) plays a pivotal role in the diagnosis due to its high sensitivity, specificity, accuracy, and interobserver agreement. In fact, CE-CT allows alternative diagnoses to be excluded, the inflamed diverticulum to be localized, and complications to be identified. Imaging findings have been reviewed, dividing them into bowel and extra-intestinal wall findings. Moreover, CE-CT allows staging of the disease; the most used classifications of ACD severity are Hinchey’s modified and WSES classifications. Differential diagnoses include colon carcinoma, epiploic appendagitis, ischemic colitis, appendicitis, infectious enterocolitis, and inflammatory bowel disease. We propose a structured reporting template to standardize the terminology and improve communication between specialists involved in patient care

    The prevalence and experience of Australian naturopaths and Western herbalists working within community pharmacies

    <p>Abstract</p> <p>Background</p> <p>Naturopaths and Western herbal medicine (WHM) practitioners were surveyed to identify their extent, experience and roles within the community pharmacy setting and to explore their attitudes to integration of complementary medicine (CM) practitioners within the pharmacy setting.</p> <p>Method</p> <p>Practising naturopaths and WHM practitioners were invited to participate in an anonymous, self-administered, on-line survey. Participants were recruited using the mailing lists and websites of CM manufacturers and professional associations.</p> <p>Results</p> <p>479 practitioners participated. 24% of respondents (n = 111) reported they had worked in community pharmacy, three-quarters for less than 5 years. Whilst in this role 74% conducted specialist CMs sales, 62% short customer consultations, 52% long consultations in a private room and 51% staff education. This was generally described as a positive learning experience and many appreciated the opportunity to utilise their specialist knowledge in the service of both customers and pharmacy staff. 14% (n = 15) did not enjoy the experience of working in pharmacy at all and suggested pharmacist attitude largely influenced whether the experience was positive or not. Few practitioners were satisfied with the remuneration received. 44% of the total sample provided comment on the issue of integration into pharmacy, with the main concern being the perceived incommensurate paradigms of practice between pharmacy and naturopathy. Of the total sample, 38% reported that they would consider working as a practitioner in retail pharmacy in future.</p> <p>Conclusions</p> <p>The level of integration of CM into pharmacy is extending beyond the mere stocking of supplements. Naturopaths and Western Herbalists are becoming utilised in pharmacies</p

    According to reports, the following 16 species have extended their distribution to other Mediterranean areas or have made a new appearance in other regions. The first category includes the following organisms: The rare and common Indo-Pacific seaweed Codium arabicum (Lebanese coasts), the acari Thalassarachna affinis (Marmara Sea), and the non-indigenous nudibranch Flabellina rubrolineata, which has also been found in many other areas of the Aegean Sea. In addition, the rare sea slug Thecacera pennigera (Piccolo of Taranto), the fangtooth moray Enchelycore anatina (National Marine Park of Zakynthos, Ionian Sea), the carangid Seriola fasciata (Gulf of Antalya), Lagocephalus sceleratus (SE. Ionian Sea), the reticulated leatherjacket Stephanolepis diaspros (Slovenia, N. Adriatic Sea), the marbled stingray, Dasyatis marmorata (NE Levantine), the starry smooth-hound Mustelus asterias (Iskenderun Bay, NE Mediterranean), the cephalopod Ommastrephes bartramii (Ionian Sea) have also been reported. The Atlantic crab Dyspanopeus sayi has expanded to many Italian areas and the blue crab Callinectes sapidus to a lake in N. Greece and in the S. Adriatic Sea. Finally, Farfantepenaeus aztecus has been found in the Ionian Sea, thus showing its wide expansion in the Mediterranean. The larval stages of Faccionella oxyrhyncha have been found, after many years, in the Aegean Sea and the first report of an existence on intersexual acari Litarachna duboscqi in Split(Adriatic Sea) was reported
