    La Clinique de pédiatrie transculturelle de l’Hôpital Maisonneuve-Rosemont a la particularité d’être implantée dans un service de pédiatrie générale. Dans ce contexte, c’est l’enfant qui est le porteur de symptômes pour la famille et cette clinique permet de le prendre en charge, dans sa globalité corporelle, affective et culturelle. Les concepts de filiation et d’affiliation étant parmi ceux les plus utilisés par l’équipe de la clinique lors des interventions, les auteurs tentent de les approfondir et de les illustrer par deux cas cliniques. Leur analyse soulève une question importante à laquelle ils tentent de répondre : la résilience est-elle possible en contexte migratoire chez les enfants souffrant de problèmes de filiation et d’affiliation ?The Paediatric Transcultural Clinic of the Maisonneuve-Rosemont Hospital is unique in the fact that it is part of a General Paediatric Unit. Therefore, the child’s symptoms are often the result of a larger problem affecting the whole family. This clinic aims to provide a complete care by addressing physical, emotional and cultural issues. The clinic’s professionals among the most often use the concepts of filiation and affiliation and the authors attempt to explain and illustrate them with two clinical cases. Their analysis raises an important question: how resilient can children be in an immigration context when dealing with issues of filiation and affiliation?La Clínica de Pediatría Transcultural del Hospital Maisonneuve-Rosemont tiene la particularidad de encontrarse en un servicio de pediatría general. En este contexto, el niño es el portador de los síntomas de la familia y esta clínica permite hacerse cargo de él en su totalidad corporal, afectiva y cultural. Por medio de dos casos clínicos, los autores intentan profundizar e ilustrar los conceptos de filiación y afiliación, de los más utilizados por el equipo de la clínica en el momento de las intervenciones.     High-Gain Observer–Based Sliding Mode Control of Multimotor Drive Systems

    Multimotor drive systems have been widely used in many modern industries. It is a nonlinear, multi-input, multi-output (MIMO) and strong-coupling complicated system, including the effect of friction, elastic, and backlash. The control law for this drive system much depends on the determining of the tension. However, it is hard to obtain this tension in practice by using a load cell or a pressure meter due to the accuracy of sensors or external disturbance. In order to solve this problem, a high-gain observer is proposed to estimate the state variables in this drive system, such as speeds and tension. An emerging proposed technique in the control law is the use of high-gain observers together with adaptive sliding mode control scheme to obtain a separation principle for the stabilization of whole system. The theory analysis and simulation results point out the good effectiveness of the proposed output feedback for the drive system

    Based on the sustainability assessment indicators conducted previously, the authors studied and built the sustainable indicators for the factors involved in the assessment with 6 themes and 38 indicators as well as sustainable levels (5 levels) in order to assess the sustainable conditions in coastal zone at the  pilot site in Phu Cat district. The authors also proposed to apply the method of analysis hierarchy process (AHP) to establish a set of weights for each subject and indicator. After calculating the assessment results for each indicator through the method of fuzzy comprehensive evaluation (FCE) by the membership functions, the overall evaluation results for the entire Phu Cat district were done. Through the implementation of these methods the evaluation results showed that the sustainability of Phu Cat district is at average level.Dựa vào các bộ chỉ thị đánh giá tính bền vững đã công bố, các tác giả nghiên cứu, thiết lập bộ chỉ thị với 6 nhóm chủ đề và 38 chỉ thị và các bậc bền vững (5 bậc) cho các yếu tố phục vụ công tác đánh giá tính bền vững đới bờ, thí điểm tại huyện Phù Cát, tỉnh Bình Định. Nghiên cứu này đã áp dụng phương pháp phân tích tiến trình cấp bậc (AHP) để thiết lập tập hợp trọng số cho từng chủ đề và từng chỉ thị. Sau khi tính toán kết quả đánh giá với phương pháp đánh giá toàn diện mờ (FCE) cho từng chỉ thị theo hàm thành viên, ta có được kết quả đánh giá tổng thể cho toàn bộ huyện Phù Cát. Từ kết quả tính toán và đánh giá các tiêu chí cho thấy mức độ bền vững của huyện Phù Cát ở mức trung bình

    Efficient architectures and implementation of arithmetic functions approximation based stochastic computing

    Stochastic computing (SC) has emerged as a potential alternative to binary computing for a number of low-power embedded systems, DSP, neural networks and communications applications. In this paper, a new method, associated architectures and implementations of complex arithmetic functions, such as exponential, sigmoid and hyperbolic tangent functions are presented. Our approach is based on a combination of piecewise linear (PWL) approximation as well as a polynomial interpolation based (Lagrange interpolation) methods. The proposed method aims at reducing the number of binary to stochastic converters. This is the most power sensitive module in an SC system. The hardware implementation for each complex arithmetic function is then derived using the 65nm CMOS technology node. In terms of accuracy, the proposed approach outperforms other well-known methods by 2 times on average. The power consumption of the implementations based on our method is decreased on average by 40 % comparing to other previous solutions. Additionally, the hardware complexity of our proposed method is also improved (40 % on average) while the critical path of the proposed method is slightly increased by 2.5% on average when comparing to other methods

    Lovastatin for adult patients with dengue: protocol for a randomised controlled trial.

    BACKGROUND: Dengue is the most important vector-borne viral infection of man, with approximately 2 billion people living in areas at risk. Infection results in a range of manifestations from asymptomatic infection through to life-threatening shock and haemorrhage. One of the hallmarks of severe dengue is vascular endothelial disruption. There is currently no specific therapy and clinical management is limited to supportive care. Statins are a class of drug initially developed for lipid lowering. There has been considerable recent interest in their effects beyond lipid lowering. These include anti-inflammatory effects at the endothelium. In addition, it is possible that lovastatin may have an anti-viral effect against dengue. Observational data suggest that the use of statins may improve outcomes for such conditions as sepsis and pneumonia. This paper describes the protocol for a randomised controlled trial investigating a short course of lovastatin therapy in adult patients with dengue. METHODS/DESIGN: A randomised, double-blind, placebo-controlled trial will investigate the effects of lovastatin therapy in the treatment of dengue. The trial will be conducted in two phases with an escalation of dose between phases if an interim safety review is satisfactory. This is an exploratory study focusing on safety and there are no data on which to base a sample size calculation. A target sample size of 300 patients in the second phase, enrolled over two dengue seasons, was chosen based on clinical judgement and feasibility considerations. In a previous randomised trial in dengue, about 10% and 30% of patients experienced at least one serious adverse event or adverse event, respectively. With 300 patients, we will have 80% power to detect an increase of 12% (from 10% to 22%) or 16% (from 30% to 46%) in the frequency of adverse events. Furthermore, this sample size ensures some power to explore the efficacy of statins. DISCUSSION: The development of a dengue therapeutic that can attenuate disease would be an enormous advance in global health. The favourable effects of statins on the endothelium, their good safety profile and their low cost make lovastatin an attractive therapeutic candidate.
