    Psychophysiological support of orthopedic treatment in patients with dento-alveolar form of dentition and occlusion

    The purpose: to improve the treatment of patients with orthopedic dento-alveolar deformation of dentition and occlusion by using a range of methods of psychophysiological support of patient care. Material and methods. The study included 86 patients, men and women aged from 18 to 49 years with dento-alveolar deformation of dentition and occlusion. The study used clinical, anthropometric, radiological, photometric and static methods. At each stage of treatment, to investigate the characteristics of the functional status of patients methods of SAN, IPC «adaptive», self-esteem of patients by developed questionnaire were used. Results. A direct, reliable, moderate correlation has been established between the estimates that have given quality of care to patients with a regularity of the complex of psychophysiological methods proposed. Conclusion. A positive impact of the proposed complex of psychophysiological methods to support the functional characteristics of the patients and their assessment of the treatment, and the effectiveness of orthopedic treatment have been determined

    ИНВАЗИВНАЯ СТРЕПТОКОККОВАЯ ИНФЕКЦИЯ (ГРУППЫ А) МЯГКИХ ТКАНЕЙ В ХИРУРГИЧЕСКОМ СТАЦИОНАРЕ Г. МОСКВЫ

    Aim. To study features and risk factors  of soft tissue's invasive streptococcal infection in surgical unit's patients: a) emm-types of the most often recovered streptococcus; b) human immune response for streptococcal antigens (streptolysin-O and the group A carbohydrate antigen); c) main patient's conditions that complicate the course of soft tissue's streptococcal infection. Patients and methods. 101 surgical unit's patients with soft tissue's infection, which group A streptococcal (GAS) nature, were examined. Emm-types were determined by comparison of emm gene sequenced part with data of CDC Streptococcus Laboratory. ASO and specific anti-group-carbohydrate IgM were measured with photometry and ELISA accordingly.  Patient’s personal data were analyzed by standard statistical programs. Results. Among 34 emm-types of all streptococcal isolates emm28 and emm64 were registered only in invasive infection.  Emm-types 1, 49, 60, 66, 77, 84, 88 were most often. Among 23 sera from invasive GAS-infection's patients, 15 (65%) were ASO-negative, 6 (26%) were ASO-low-positive, and 2 (8%) were positive, but negative for the anti-group-A-carbohydrate IgM. Conclusion. Lower levels of the humoral immune response as well as risk factors such as trauma, alcoholism, diabetes, chronic venous insufficiency contributed to development of severe forms of GAS-infection. The main streptococcal emm-types for soft tissue's invasive GAS infection in one of Moscow surgical unit's patients in 2008-2011 were registered. Цель. Изучить особенности и факторы риска развития инвазивной стрептококковой инфекции мягких тканей у пациентов хирургического стационара, определив: а)emm-типы наиболее часто высеваемых стрептококков; б)уровень иммунного ответа макроорганизма  на антигены стрептококка (стрептолизин-О, полисахарид); в)состояния, осложняющие течение стрептококковой инфекции мягких тканей. Пациенты и методы. В исследование включали пациентов хирургического стационара (101чел.), у которых выделяли стрептококки группы А. Emm-типы стрептококков устанавливали сравнением секвенированной части гена emm с данными Международного банка данных emm-типов CDC. В сыворотках определяли антитела (IgM) к полисахариду (методом ИФА) и к стрептолизину-О (спектрофотометрически). Персональные данные о больных обработаны с помощью стандартных статистических программ. Результаты. Среди идентифицированных 34-х emm-типов стрептококков 28-ой и 64-ый встречались только при инвазивной инфекции.  Кроме того, наиболее часто определяли emm-типы 1, 49, 60, 66, 77, 84, 88. Из 23 сывороток больных инвазивной формой в 15-ти (65%) отмечали низкий уровень гуморального иммунного ответа на стрептолизин-О, в 6-ти (26%)  наблюдали незначительное превышение нормального уровня, в двух случаях (8%) показатели были высокие, но при низком уровне гуморального иммунного ответа  на полисахарид.  Заключение. К факторам риска отнесены травмы, алкоголизм, диабет, хроническая венозная недостаточность нижних конечностей и низкий уровень гуморального иммунного ответа на антигены стрептококка. Определен набор emm-типов стрептококка, вызывающих  развитие инвазивных форм стрептококковой (группы А) инфекции мягких тканей у пациентов хирургического стационара г.Москвы (2008-2011годы).

    In the course of daily duty in clinic, the indeces of functional status, functional reserve of organism and mnestic (operator’s) performance ability of intern doctors with different success level activity in extreme conditions were determined twice (in the beginning and at the end of the duty).     Objectives: to determine the rate of modern thematic module "Organizational Behavior" (Fundamentals of Management) from "Healthcare Management" course) for students of Department of Raising Skills and Professional Development for Saratov State Medical University specialists. Material and methods. 61 students — medical administrators and specialists — were anonymously surveyed after their vocational training with the help of a specially designed questionnaire. The received data were processed mathematically and statistically with "Statistica-10" program. Results. The modern issues of the studied course is evaluated differently by groups of medical administrators and specialists. Medical administrators' rate has been higher than specialists'. Both groups' rate is "higher than average". Conclusion. Doctors clearly realize the necessity of professional development in management and have a clear idea of the areas where they lack professional skills. The conducted monitoring of students' opinion implies forehanded reaction to doctors' demands, increasing effectiveness of their training and level of their satisfaction as well as marketability of the higher educational institution providing the demanded educational services

    Scanning thermal microscopy with heat conductive nanowire probes

    Scanning thermal microscopy (SThM), which enables measurement of thermal transport and temperature distribution in devices and materials with nanoscale resolution is rapidly becoming a key approach in resolving heat dissipation problems in modern processors and assisting development of new thermoelectric materials. In SThM, the self-heating thermal sensor contacts the sample allowing studying of the temperature distribution and heat transport in nanoscaled materials and devices. The main factors that limit the resolution and sensitivities of SThM measurements are the low efficiency of thermal coupling and the lateral dimensions of the probed area of the surface studied. The thermal conductivity of the sample plays a key role in the sensitivity of SThM measurements. During the SThM measurements of the areas with higher thermal conductivity the heat flux via SThM probe is increased compared to the areas with lower thermal conductivity. For optimal SThM measurements of interfaces between low and high thermal conductivity materials, well defined nanoscale probes with high thermal conductivity at the probe apex are required to achieve a higher quality of the probe-sample thermal contact while preserving the lateral resolution of the system. In this paper, we consider a SThM approach that can help address these complex problems by using high thermal conductivity nanowires (NW) attached to a tip apex. We propose analytical models of such NW-SThM probes and analyse the influence of the contact resistance between the SThM probe and the sample studied. The latter becomes particularly important when both tip and sample surface have high thermal conductivities. These models were complemented by finite element analysis simulations and experimental tests using prototype probe where a multiwall carbon nanotube (MWCNT) is exploited as an excellent example of a high thermal conductivity NW. These results elucidate critical relationships between the performance of the SThM probe on one hand and thermal conductivity, geometry of the probe and its components on the other. As such, they provide a pathway for optimizing current SThM for nanothermal studies of high thermal conductivity materials. Comparison between experimental and modeling results allows us to provide direct estimates of the contact thermal resistances for various interfaces such as MWCNT-Al (5×10−9±1×10−9 K m2 W−1), Si3N4–Al (6×10−8±2.5×10−8 K m2 W−1) and Si3N4−graphene (~10−8 K m2 W−1). It was also demonstrated that the contact between the MWCNT probe and Al is relatively perfect, with a minimal contact resistance. In contrast, the thermal resistance between a standard Si3N4 SThM probe and Al is an order of magnitude higher than reported in the literature, suggesting that the contact between these materials may have a multi-asperity nature that can significantly degrade the contact resistance

    The reactions of thallium salt Tl[7-SMe2-7,8-C2B9H10] with [(cod)RhCl]2 and [Cp*RuCl]4 are accompanied by room-temperature polyhedral rearrangement, giving rhoda- and ruthenacarboranes 1-cod-8-SMe2-1,2,8-RhC2B9H10 (2) and 1-Cp*-8-SMe2-1,2,8-RuC2B9H10 (3), respectively. According to DFT calculations, the rearrangement could be attributed to the triangular face rotation mechanism. The reaction of iridium derivative 1-cod-8-SMe2-1,2,8-IrC2B9H10 (1) with anhydrous hydrohalic acids HX (X = Cl, Br, I) results in the dimeric halide iridacarboranes [1,1-X2-8-SMe2-1,2,8-IrC2B9H10]2 [4a–c; X = Cl (a), Br (b), I (c)]. Bromide 4b reacts with Tl[Tl(η-7,8-C2B9H11)] and TlCp giving iridacarboranes 8′-SMe2-1,1′-Ir(2,3-C2B9H11)(2′,8′-C2B9H10) (5) and 1-Cp-8-SMe-1,2,8-IrC2B9H10 (6). The formation of the latter compound is accompanied by demethylation of the SMe2 substituent. The structures of 2, 3, 5 and 6 were determined by single-crystal X-ray diffraction. Iridacarboranes 4b and [1,1-Br2-4-SMe2-1,2,3-IrC2B9H10]2 (7) catalyze the dimerization of diphenylacetylene, giving 1,2,3-triphenylnaphthalene in 20–24 % yields. Compounds 4b and 7 also effectively catalyze the reductive amination reaction between aldehydes (or ketones) and primary (or secondary) amines in the presence of carbon monoxide, giving the corresponding secondary and tertiary amines in high yields (60–85 %). © 2017 WILEY-VCH Verlag GmbH & Co.     VACTERL association is a set of certain abnormalities in one newborn which include defects of the esophagus, rectum, heart, kidneys, vertebral column and extremities. Research papers devoted to the study of this complex disease are still rare. The majority of researchers insist that the syndrome was composed at least of three components (abnormalities of vertebra, tracheoesophageal fistula and anal atresia) though others believe that the constituents of VACTERL are constant. Treatment of the syndrome is usually restricted by surgeries which are needed to restore the esophagus, anus, eliminate abnormalities of the cardiovascular and excretory system and extremities. The scientific research is devoted to the study of VACTERL, correct interpretation of its components, determination of knowledge weaknesses and description of modern therapeutic approaches intended for the correction of surgical abnormalities included into the syndrome.Ассоциация VACTERL представляет собой набор определенных аномалий развития, сосредоточенных у одного новорожденного, которые включают пороки развития пищевода, прямой кишки, сердца, почек, позвоночного столба и конечностей. Научные работы, посвященные изучению этого сложного заболевания, по-прежнему являются редкими. Большинство исследователей настаивают на том, чтобы в составе синдрома присутствовали по крайней мере три компонента (аномалия позвонков, трахеопищеводная фистула и анальная атрезия), хотя другие стоят на постоянстве составляющих VACTERL-ассоциации.
