    Dynamic muscle quality of the plantar flexors is impaired in claudicant patients with peripheral arterial disease and associated with poorer walking endurance

    Objective Peripheral arterial disease and intermittent claudication (PAD-IC) negatively affects physical activity and function. There is evidence for plantarflexor muscle dysfunction and weakness; however, the extent to which this dysfunction can be attributed to reduced muscle size or quality, or both, is not yet known. This study investigated whether in vivo plantarflexor muscle quality during static and dynamic contractions is altered by PAD-IC and whether such changes are associated with impaired walking endurance according to initial and absolute claudication distances. Methods The study recruited 22 participants, consisting of 10 healthy controls and 12 claudicant patients with occlusion of the superficial femoral artery (seven unilateral and five bilateral). Muscle quality of the combined gastrocnemius muscles during static contractions was calculated by normalizing the estimated maximal potential muscle force to the physiological cross-sectional area of the lateral and medial gastrocnemius. Muscle quality during dynamic contractions of the combined plantarflexor muscles was calculated as the ratio of peak voluntary concentric plantarflexor power and the summed volume of lateral and medial gastrocnemius. Results Dynamic muscle quality was 24% lower in the claudicating-limb and asymptomatic-limb groups compared with controls (P = .017 and P = .023). The differences were most apparent at the highest contraction velocity (180°/s). Dynamic muscle quality was associated with reduced walking endurance (R = 0.689, P = .006 and R = 0.550, P = .042 for initial and absolute claudication distance, respectively). The claudicating-limb group demonstrated a trend toward reduced static muscle quality compared with controls (22%, P = .084). The relative contribution of the soleus muscle to plantarflexion maximum voluntary contraction was significantly higher in the claudicating-limb and asymptomatic-limb groups than in controls (P = .012 and P = .018). Conclusions The muscle strength of the plantarflexors in those with PAD-IC appears to be impaired at high contraction velocities. This may be explained by some reduction in gastrocnemii muscle quality and a greater reliance on the prominently type I-fibered soleus muscle. The reduced dynamic capability of the plantarflexor muscles was associated with disease severity and walking ability; therefore, efforts to improve plantarflexor power through dynamic exercise intervention are vital to maintain functional performance

Fortunately, graduate programs in each field are providing women opportunities that were not apparent a few decades ago. We examine Business and Physical Education for transgressions of discrimination and provide evidence from personal interviews which indicate progress has been made for women in these fields. We provide several cross-disciplinary themes that transcend gender barriers to promote an equitable workforce in addition to field specific recommendations

    Frequency of cannabis and illicit opioid use among people who use drugs and report chronic pain: A longitudinal analysis.

    BACKGROUND:Ecological research suggests that increased access to cannabis may facilitate reductions in opioid use and harms, and medical cannabis patients describe the substitution of opioids with cannabis for pain management. However, there is a lack of research using individual-level data to explore this question. We aimed to investigate the longitudinal association between frequency of cannabis use and illicit opioid use among people who use drugs (PWUD) experiencing chronic pain. METHODS AND FINDINGS:This study included data from people in 2 prospective cohorts of PWUD in Vancouver, Canada, who reported major or persistent pain from June 1, 2014, to December 1, 2017 (n = 1,152). We used descriptive statistics to examine reasons for cannabis use and a multivariable generalized linear mixed-effects model to estimate the relationship between daily (once or more per day) cannabis use and daily illicit opioid use. There were 424 (36.8%) women in the study, and the median age at baseline was 49.3 years (IQR 42.3-54.9). In total, 455 (40%) reported daily illicit opioid use, and 410 (36%) reported daily cannabis use during at least one 6-month follow-up period. The most commonly reported therapeutic reasons for cannabis use were pain (36%), sleep (35%), stress (31%), and nausea (30%). After adjusting for demographic characteristics, substance use, and health-related factors, daily cannabis use was associated with significantly lower odds of daily illicit opioid use (adjusted odds ratio 0.50, 95% CI 0.34-0.74, p < 0.001). Limitations of the study included self-reported measures of substance use and chronic pain, and a lack of data for cannabis preparations, dosages, and modes of administration. CONCLUSIONS:We observed an independent negative association between frequent cannabis use and frequent illicit opioid use among PWUD with chronic pain. These findings provide longitudinal observational evidence that cannabis may serve as an adjunct to or substitute for illicit opioid use among PWUD with chronic pain

    Perturbed cholesterol and vesicular trafficking associated with dengue blocking in Wolbachia-infected Aedes aegypti cells

    Wolbachia are intracellular maternally inherited bacteria that can spread through insect populations and block virus transmission by mosquitoes, providing an important approach to dengue control. To better understand the mechanisms of virus inhibition, we here perform proteomic quantification of the effects of Wolbachia in Aedes aegypti mosquito cells and midgut. Perturbations are observed in vesicular trafficking, lipid metabolism and in the endoplasmic reticulum that could impact viral entry and replication. Wolbachia-infected cells display a differential cholesterol profile, including elevated levels of esterified cholesterol, that is consistent with perturbed intracellular cholesterol trafficking. Cyclodextrins have been shown to reverse lipid accumulation defects in cells with disrupted cholesterol homeostasis. Treatment of Wolbachia-infected Ae. aegypti cells with 2-hydroxypropyl-β-cyclodextrin restores dengue replication in Wolbachia-carrying cells, suggesting dengue is inhibited in Wolbachia-infected cells by localised cholesterol accumulation. These results demonstrate parallels between the cellular Wolbachia viral inhibition phenotype and lipid storage genetic disorders

Research over the past twenty years has revealed variability in clinical presentations, ranging from mild to more severe phenotypes. Genotype and phenotype relationships have emerged. The increasing availability of genetic panels for PCD continue to redefine these genotype-phenotype relationships and reveal milder forms of disease that had previously gone unrecognized

    Trajectory sampling and finite-size effects in first-principles stopping power calculations

    Real-time time-dependent density functional theory (TDDFT) is presently the most accurate available method for computing electronic stopping powers from first principles. However, obtaining application-relevant results often involves either costly averages over multiple calculations or ad hoc selection of a representative ion trajectory. We consider a broadly applicable, quantitative metric for evaluating and optimizing trajectories in this context. This methodology enables rigorous analysis of the failure modes of various common trajectory choices in crystalline materials. Although randomly selecting trajectories is common practice in stopping power calculations in solids, we show that nearly 30% of random trajectories in an FCC aluminium crystal will not representatively sample the material over the time and length scales feasibly simulated with TDDFT, and unrepresentative choices incur errors of up to 60%. We also show that finite-size effects depend on ion trajectory via "ouroboros" effects beyond the prevailing plasmon-based interpretation, and we propose a cost-reducing scheme to obtain converged results even when expensive core-electron contributions preclude large supercells. This work helps to mitigate poorly controlled approximations in first-principles stopping power calculations, allowing 1-2 order of magnitude cost reductions for obtaining representatively averaged and converged results

    Preferential Myosin Heavy Chain Isoform B Expression May Contribute to the Faster Velocity of Contraction in Veins versus Arteries

    Smooth muscle myosin heavy chains occur in 2 isoforms, SMA (slow) and SMB (fast). We hypothesized that the SMB isoform is predominant in the faster-contracting rat vena cava compared to thoracic aorta. We compared the time to half maximal contraction in response to a maximal concentration of endothelin-1 (ET-1; 100 nM), potassium chloride (KCl; 100 mM) and norepinephrine (NE; 10 µM). The time to half maximal contraction was shorter in the vena cava compared to aorta (aorta: ET-1 = 235.8 ± 13.8 s, KCl = 140.0 ± 33.3 s, NE = 19.8 ± 2.7 s; vena cava: ET-1 = 121.8 ± 15.6 s, KCl = 49.5 ± 6.7 s, NE = 9.0 ± 3.3 s). Reverse-transcription polymerase chain reaction supported the greater expression of SMB in the vena cava compared to aorta. SMB was expressed to a greater extent than SMA in the vessel wall of the vena cava. Western analysis determined that expression of SMB, relative to total smooth muscle myosin heavy chains, was 12.5 ± 4.9-fold higher in the vena cava compared to aorta, while SMA was 4.9 ± 1.2-fold higher in the aorta than vena cava. Thus, the SMB isoform is the predominant form expressed in rat veins, providing one possible mechanism for the faster response of veins to vasoconstrictors
