Using parent metaphors for learning about the neonatal care experience: an interpretive perspective

It is available at: https://doi.org/10.1177/1367493519875853.This study focuses on how metaphors are used by parents who have had a premature baby to describe their neonatal care experience and how these can contribute to empathic learning of health professionals. In health, metaphors are commonly used to communicate and explain difficult topics. When patients tell their story, metaphor can be a means of expression from which we can learn about their experience of illness or hospitalisation. Limited research exits on how metaphor can improve our understanding of parent’s emotional experience in neonatal care and subsequently inform education in this field. Employing narrative inquiry within an interpretive, constructivist paradigm, 20 narrative interviews with 23 parents of premature babies were analysed using a process of metaphor identification. Findings revealed common metaphors used to describe experience. Metaphor clusters used by parents in order of frequency were journeying, altered reality, darkness, breaking, connections, fighting, salvation and being on the edge. Parents widely used compelling and emotive metaphors to describe and express both difficult and challenging times as well as progression forward. Metaphors serve as a powerful way for health professionals to learn about the emotional experiences of parents and potentially enhance their empathic understanding.Peer reviewe

Effects of a Mindfulness-Based Intervention on Distress, Weight Gain, and Glucose Control for Pregnant Low-Income Women: A Quasi-Experimental Trial Using the ORBIT Model.
BackgroundStress can lead to excessive weight gain. Mindfulness-based stress reduction that incorporates mindful eating shows promise for reducing stress, overeating, and improving glucose control. No interventions have tested mindfulness training with a focus on healthy eating and weight gain during pregnancy, a period of common excessive weight gain. Here, we test the effectiveness of such an intervention, the Mindful Moms Training (MMT), on perceived stress, eating behaviors, and gestational weight gain in a high-risk sample of low income women with overweight/obesity.MethodWe conducted a quasi-experimental study assigning 115 pregnant women to MMT for 8 weeks and comparing them to 105 sociodemographically and weight equivalent pregnant women receiving treatment as usual. Our main outcomes included weight gain (primary outcome), perceived stress, and depression.ResultsWomen in MMT showed significant reductions in perceived stress (β = - 0.16) and depressive symptoms (β = - 0.21) compared to the treatment as usual (TAU) control group. Consistent with national norms, the majority of women (68%) gained excessive weight according to Institute of Medicine weight-gain categories, regardless of group. Slightly more women in the MMT group gained below the recommendation. Among secondary outcomes, women in MMT reported increased physical activity (β = 0.26) and had lower glucose post-oral glucose tolerance test (β = - 0.23), being 66% less likely to have impaired glucose tolerance, compared to the TAU group.ConclusionA short-term intervention led to significant improvements in stress, and showed promise for preventing glucose intolerance. However, the majority of women gained excessive weight. A longer more intensive intervention may be needed for this high-risk population. Clinical Trials.gov #NCT01307683
The right posterior paravermis and the control of language interference
Auditory and written language in humans' comprehension necessitates attention to the message of interest and suppression of interference from distracting sources. Investigating the brain areas associated with the control of interference is challenging because it is inevitable that activation of the brain regions that control interference co-occurs with activation related to interference per se. To isolate the mechanisms that control verbal interference, we used a combination of structural and functional imaging techniques in Italian and German participants who spoke English as a second language. First, we searched structural MRI images of Italian participants for brain regions in which brain structure correlated with the ability to suppress interference from the unattended dominant language (Italian) while processing heard sentences in their weaker language (English). This revealed an area in the posterior paravermis of the right cerebellum in which gray matter density was higher in individuals who were better at controlling verbal interference. Second, we found functional activation in the same region when our German participants made semantic decisions on written English words in the presence of interference from unrelated words in their dominant language (German). This combination of structural and functional imaging therefore highlights the contribution of the right posterior paravermis to the control of verbal interference. We suggest that the importance of this region for language processing has previously been missed because most fMRI studies limit the field of view to increase sensitivity, with the lower part of the cerebellum being the region most likely to be excluded
Puzzle-based versus traditional lecture: comparing the effects of pedagogy on academic performance in an undergraduate human anatomy and physiology II lab
BACKGROUND: A traditional lecture-based pedagogy conveys information and content while lacking sufficient development of critical thinking skills and problem solving. A puzzle-based pedagogy creates a broader contextual framework, and fosters critical thinking as well as logical reasoning skills that can then be used to improve a student’s performance on content specific assessments. This paper describes a pedagogical comparison of traditional lecture-based teaching and puzzle-based teaching in a Human Anatomy and Physiology II Lab. METHODS: Using a single subject/cross-over design half of the students from seven sections of the course were taught using one type of pedagogy for the first half of the semester, and then taught with a different pedagogy for the second half of the semester. The other half of the students were taught the same material but with the order of the pedagogies reversed. Students’ performance on quizzes and exams specific to the course, and in-class assignments specific to this study were assessed for: learning outcomes (the ability to form the correct conclusion or recall specific information), and authentic academic performance as described by (Am J Educ 104:280–312, 1996). RESULTS: Our findings suggest a significant improvement in students’ performance on standard course specific assessments using a puzzle-based pedagogy versus a traditional lecture-based teaching style. Quiz and test scores for students improved by 2.1 and 0.4 % respectively in the puzzle-based pedagogy, versus the traditional lecture-based teaching. Additionally, the assessments of authentic academic performance may only effectively measure a broader conceptual understanding in a limited set of contexts, and not in the context of a Human Anatomy and Physiology II Lab. CONCLUSION: In conclusion, a puzzle-based pedagogy, when compared to traditional lecture-based teaching, can effectively enhance the performance of students on standard course specific assessments, even when the assessments only test a limited conceptual understanding of the material
Using fire scars and growth release in subfossil Scots pine to reconstruct prehistoric fires
Fire scars indicating low- to moderate-intensity fires on peat deposits have been sampled from subfossil Scots pine (Pinus sylvestris L.) at sites in England, Wales and south-west Ireland. Analysis of ring-width responses to one fire event in 2800 BC illustrates its rejuvenating effect on Pinus sylvestris woodland, supporting a growing body of modern and palaeoecological data that illustrates the regenerative role played by fire in persistence of Pinus sylvestris woodland. Both the scale and timing of these fires suggest that infrequent low- to moderate-intensity fires are sufficient to stimulate Pinus sylvestris growth. This effect is shown by average increases in ring-width following the fire of between 0.62 and 1.16 mm in non-scarred trees and between 0.92 and 2.74 mm in fire-scarred individuals. Growth release in non-scarred trees may prove to be a more reliable method of detecting fire than using the relatively rare fire scars alone. Radii at time of scarring varied between 1.85 and 11.2 cm, much smaller than is predicted to survive from modern studies. © 2000 Elsevier Science B.V
Recording and Reconstruction of Wood Macrofossils in Three-Dimensions
Building on an upsurge of interest in European wetland archaeology, which has led to methodological advances in field techniques, a new palaeoecological technique is outlined to enable reconstruction of wood macrofossils in their originalin situpositions within a peat deposit. Such three-dimensional reconstruction provides data on the temporal sequence and succession of mire woodland phases; these can be related to the fossil pollen record and to evidence of past human impact. Chronology building using tree ring-width series, is facilitated, and may assist in dating site records. The technique has wider application at sites where subfossiliferous peat is underlain by sand substrates, aiding palaeoenvironmental reconstruction and interpretation of the cultural landscape. © 1995 Academic Press Limited
Geometrically-frustrated interactions drive structural complexity in amorphous cal- cium carbonate
Amorphous calcium carbonate is an important precursor for biomineralization in marine organisms. Key outstanding problems include understanding the structure of amorphous calcium carbonate and rationalizing its metastability as an amorphous phase. Here we report high-quality atomistic models of amorphous calcium carbonate generated using state-of-the-art interatomic potentials to help guide fits to X-ray total scattering data. Exploiting a recently developed inversion approach, we extract from these models the effective Ca⋯Ca interaction potential governing the structure. This potential contains minima at two competing distances, corresponding to the two different ways that carbonate ions bridge Ca2+-ion pairs. We reveal an unexpected mapping to the Lennard-Jones–Gauss model normally studied in the context of computational soft matter. The empirical model parameters for amorphous calcium carbonate take values known to promote structural complexity. We thus show that both the complex structure and its resilience to crystallization are actually encoded in the geometrically frustrated effective interactions between Ca2+ ions
Variations of training load, monotony, and strain and dose-response relationships with maximal aerobic speed, maximal oxygen uptake, and isokinetic strength in professional soccer players
This study aimed to identify variations in weekly training load, training monotony, and training strain across a 10-week period (during both, pre- and in-season phases); and to analyze the dose-response relationships between training markers and maximal aerobic speed (MAS), maximal oxygen uptake, and isokinetic strength. Twenty-seven professional soccer players (24.9±3.5 years old) were monitored across the 10-week period using global positioning system units. Players were also tested for maximal aerobic speed, maximal oxygen uptake, and isokinetic strength before and after 10 weeks of training. Large positive correlations were found between sum of training load and extension peak torque in the right lower limb (r = 0.57, 90%CI[0.15;0.82]) and the ratio agonist/antagonist in the right lower limb (r = 0.51, [0.06;0.78]). It was observed that loading measures fluctuated across the period of the study and that the load was meaningfully associated with changes in the fitness status of players. However, those magnitudes of correlations were small-to-large, suggesting that variations in fitness level cannot be exclusively explained by the accumulated load and loading profile
Regulation of the water status in three co-occurring phreatophytes at the southern fringe of the Taklamakan Desert
Aims We investigated the regulation of the water status in three predominant perennial C3 phreatophytes (Alhagi sparsifolia, Populus euphratica, Tamarix ramosissima) at typical sites of their occurrence at the Southern fringe of the hyperarid Taklamakan Desert (north-west China). Methods In the foreland of the river oasis of Qira (Cele), we determined meteorological variables, plant biomass production, plant water potentials (psi(l)) and the water flux through the plants. We calculated the hydraulic conductance on the flow path from the soil to the leaves (k(SL)) and tested the effects of k(SL) psi(L) and the leaf-to-air difference in the partial pressure of water vapour (Delta w) on stomatal regulation using regression analyses. Important Findings Despite high values of plant water potential at the point of turgor loss, all plants sustained psi(L) at levels that were high enough to Maintain transpiration throughout the growing season. In A. sparsitolia, stomatal resistance (r(s); related to leaf area or leaf mass) was most closely correlated with k(SL);. whereas in P. euphratica, similar to 70%, of the variation in r(s) was explained by Delta w. In T. ramosissima, leaf area-related r(s) was significantly correlated with psi(L) and k(SL). The regulation mechanisms are in accordance with the growth patterns and the Occurrence of the species in relation to their distance to the ground water
