    Hérnia Diafragmática Congénita. Casuística de 10 Anos

    Os autores fizeram a revisão dos casos de hérnia diafragmática congénita admitidos na Unidade de Cuidados Intensivos Neonatais do Hospital de Dona Estefânia de Janeiro de 1984 a Dezembro de 1993 (10 anos). Neste período foram internados 53 recém-nascidos (RN) com hérnia diafragmática congénita tipo Bochdalek. A pesquisa incidiu sobre os índices de gravidade clínicos, radiológico, ventilatório, de oxigenação e mortalidade. Em 54,7% dos RN houve asfixia neonatal. A dificuldade respiratória teve início antes das 6 horas de vida em 43 dos RN (81.1%), mas o início dos sintomas foi imediato em 31. A hérnia localizava-se à esquerda em 77.4% dos casos. Em 5 casos não foi possível a correcção cirúrgia do defeito diafragmático. A mortalidade global foi 47.2%. Dos 48 RN operados faleceram 20 (41.7%). Em todas as crianças que faleceram, excepto numa, houve início imediato dos sintomas. A mortalidade dos RN com índice radiológico >6 foi de 81.5%. O índice ventilatório >1000 e o índice de oxigenação >40 tiveram valor preditivo de morte (mortalidade de 100% em ambos os casos). Nesta casuística, os achados que melhor se correlacionaram com o prognóstico foram: início imediato dos sintomas, I.R. >6, I.V.> 1000 e I.O.> 40

    Transformer-based language models for semantic search and mobile applications retrieval

    Search engines are being extensively used by Mobile App Stores, where millions of users world-wide use them every day. However, some stores still resort to simple lexical-based search engines, despite the recent advances in Machine Learning, Information Retrieval, and Natural Language Processing, which allow for richer semantic strategies. This work proposes an approach for semantic search of mobile applications that relies on transformer-based language models, fine-tuned with the existing textual information about known mobile applications. Our approach relies solely on the application name and on the unstructured textual information contained in its description. A dataset of about 500 thousand mobile apps was extended in the scope of this work with a test set, and all the available textual data was used to fine-tune our neural language models. We have evaluated our models using a public dataset that includes information about 43 thousand applications, and 56 manually annotated non- exact queries. The results show that our model surpasses the performance of all the other retrieval strategies reported in the literature.     Em um estudo retrospectivo, indivíduos com a forma mucosa da leishmaniose foram pareados (sexo, idade e ocupação) a outros pacientes com a forma cutânea em atividade, não demonstrando-se diferença significante da freqüência nos grupos raciais. Do mesmo modo, o teste intradérmico de Montenegro não se mostrou diferente nos diversos grupos raciais, entre os 96 pares de pacientes estudados. Uma maior enduração foi observada em pacientes com o acometimento mucoso quando comparado aos cutâneos. _______________________________________________________________________________________ ABSTRACTIn a retrospective study patients with mucosal leishmaniasis were paired (as regards age sex and occupation) with patients with active cutaneous disease.     The functional and numerical responses, reproductive characteristics, and viability of Campoletis flavicincta (Hymenoptera: Ichneumonidae) as well as the mortality after parasitism of the host Spodoptera frugiperda (Lepidoptera: Noctuidae) were analyzed in the laboratory. Campoletis flavicincta pairs were maintained until female death with 10, 20, 30, 40, or 50 caterpillars day -1 of the host S. frugiperda. A type III functional response curve was fitted to the average number of caterpillars supplied per day during the female wasp lifespan, as the explanatory variable. The handling time was 0.5940 0.0875h, and the instantaneous search 0.0047 0.0020 h-1. The functional response for each of the first five days of the host was a type III. The longevity at the five host densities and the parasitism rate showed a significant linear decrease with the host density. The offspring production showed an increasing quadratic variation with increased host density. The production of females by C. flavicincta, the offspring sex ratio, the viability of the parasitoid pupae and the percentage of mortality of S. frugiperda caterpillars were not affected by host density. The function al and numerical responses of C. flavicincta indicate that this parasitoid could be a candidate for biological control of S. frugiperda
