    Isolasi Dan Identifikasi Fraksi Aktif Antelmintik Dari Rimpang Zingiber Purpureum Roxb

    Telah dilakukan isolasi, identifikasi dan uji daya antelmintik terhadap fraksi aktif antelmintik dari rim- pang Zingiber purpureum Roxb, balk secara in vitro dengan cara rendaman maupun in viva dengan cara Penurun¬an Jumlah Telur dan Uji Terkontrol. Digunakan cacing Ascaridia galli Schrank sebagai parasit dan ayam Denis pedaging AS 101 sebagai binatang laboratorium. Penelitian ins dilakukan dengan tujuan mengetahu1 golongan senyawa aktif antelmintik dan daya antelmintik bersangkutan, sebagai dasar pengembangan antelmintika bahan slam nabati. Dan hash penapisan fitokimia rimpang, ditemukan antara lain minyak atsiri, kurkuminoid dan flavonoid. Dari hasil uji daya antelmintik in vitro perasan rimpang, ekstrak PE dan Me0H, minyak atsiri dan residu menunjukkan bahwa minyak atsiri mempunyai daya mematikan yang kuat terhadap Ascaridfa galli yang digunakan seba¬gai racing percobaan. Potensi relatif terhadap piperasin sitrat dari pe¬rasan rimpang, ekstrak PE,ekstrak Me0H, mi nyak atsiri dan residu berturut-turut adalah 6,40% 0,53, 176,84% 38,24,75, 93% 19,20, 201,14% 51,23 dan 84,487 7,61. Minyak atsiri yang diperoleh dengan cara destilasi ekstrak PE, merupakan minyak jernih kekuningan. Warna, bau dan beberapa tetapan alami minyak hasil pemisahan sesuai dengan hasil penelitian sebelumnya (Cassey,et a?, 1972; Lawrence, et al, 1970). Pemisahan komponen minyak atsiri dengan cara KGC, menggunakan kolom kapi1er FFAP 85m x 0,5mm„menghasilkan lebih kurang 42 komponen, diantaranya sabinen 21.6%, terpinen-4-01 37,7%, seskuifelandren 6,7%, trans-1(3,4- dimetoksifenil)but-1-en 7,59% dan trans-1(3,4-dimetok-sifenil)but-1,3-dien 15,1%. Analisis komponen minyak dengan cara KGC pada mi¬nyak hasil pemisahan ditemukan be berapa komponen yang belum ditemukan peneliti sebelumnya (Cassey,et a7,1972 dan Lawrence, et a7,1970), diantaranya zingiberen, beta¬bisabolen, seskui-felandren dan 3, 4 -dimetoksi-benzal¬dehida. Prafraksinasi minyak atsiri cara destilasi fraksi pada suhu 650-75°C (35 mm Hg) menghasilkan Fraksi sabi¬nen dan residu. Prafraksinasi ulang Fraksi sabinen pada suhu 350-45°C (10 mm Hg) menghasilkan Fraksi sabinen de¬ngan kadar sabinen 84,11%. Dari prafraksinasi residu pada suhu 62,50-70°C (10 mm Hg) dihasilkan Fraksi terpinen- 4-01, yang dengan fraksinasi kolom selanjutnya menghasilkan Fraksi terpi- nen-4-ol dengan kadar terpinen-4-01 92,23%.     A rising number of long-term survivors and potential long-term outcome related to the treatment isone of the most important issue due to the advances of management in cancer. The development ofsecondary malignancy has been reported in a number of cohorts. We present a case of breast cancerpatient developing acute myeloid leukemia (AML)- M5 within a relatively short interval of two and ahalf years from her primary treatment with adjuvant chemotherapy with 4 cycles of doxorubicin andcyclophosphamide, followed by 4 cycles of three weekly paclitaxel (AC-T) and radiotherapy. Whatcould be attributed to the occurrence of secondary leukemia in this patient will be discussed

    The Effect of Weight Gain on Recurrence During Hormonal Therapy in Breast Cancer with Positive Estrogen Receptor and Negative Her2 Subtype

    Background. Weight gain is found in patients with positive estrogen subtype breast cancer. The effect of weight gain on breast cancer recurrence is still being debated and associated with a poor prognosis. Being overweight is associated with an increase in estrogen production by adipose tissue.Methods. A retrospective and prospective cohort study with data collection was conducted from July 2018 - June 2019 at the outpatient unit of Dr. Sardjito Hospital. We reviewed medical records of patients who came from January 2013 - July 2018.  Three categories of weight gain:  weight loss, increase 0 - 5%, and increase > 5% from baseline will be evaluated.Statistical Analysis. The relationship between weight gain and some risk factors of recurrence was analyzed with cox regression and multivariate logistic regression tests. The Kaplan Meier method is used to show lifetime data.Result. Weight gain in breast cancer patients with positive Her2-negative estrogen receptor subtypes was not associated with recurrence (p = 0.264; HR 0.637; 95% CI 0.289 - 1.405). Patients with body mass index ≥ 25 kg / m2 at diagnosis had a lower risk of relapse than patients with body mass index < 25 kg / m2 (p = 0.026; HR 0.461; 95% CI, 0.234 - 0.912).Conclusions Weight gain after diagnosis in breast cancer patients with positive estrogen receptor-negative Her2 subtypes receiving hormonal therapy is not associated with recurrence

    Diffuse Large B-Cell Lymphoma Complicated by Small Bowel Obstruction after Radiotherapy: a Case Study

    ABSTRACTAlthough the spleen is frequently involved in disseminated non-Hodgkin’s lymphoma (NHL), splenic presentation as the initial or only site of disease is uncommon. Treatment modalities include surgery, chemotherapy, and radiation therapy. The priority of cancer follow up is to perform surveillance for recurrent cancer and evaluation of treatment response. Side effects of treatment are frequently missed or overlooked. A 66-year-old woman was presented to our hospital with a month history of spleen enlargement. On physical examination the spleen was palpated at Schuffner 2. Abdominal MSCT scan was suggestive of lymphoma. Surgery revealed adhesion and obstruction of the stomach. Biopsy and gastrojejunostomy shunting were done, but splenectomy was difficult. The pathology anatomy findings confirmed the diagnosis of diffuse non Hodgkin’s lymphoma large B-cell type. Immunohistochemistry showed positive CD3 and CD20. She underwent 6 cycles of rituximab, cyclophosphamide, adriamycin, vincristine, and prednisolone (RCHOP) chemotherapy. CT evaluation done 7 months later revealed that the hilus lienalis lymph nodes and spleen has decrease in size. However, a lumbosacral x-ray done due to back pain revealed metastasis on her 1st and 2nd lumbal spine. After a single fraction of radiotherapy, nausea, vomiting and abdominal distension occurred. A 3 position abdominal x-ray revealed signs of small bowel obstruction. After surgery she has received 9 cycles of zoledronic acid and remained in good condition and ambulatory. Splenic presentation as the initial or only site of non-Hodgkin’s lymphoma (NHL) is uncommon. Acute small bowel obstruction and fistula due to palliative radiation therapy for bone metastasis needs prompt and appropriate treatment

    Colorectal Cancer in Young Patient: a Distinguished Disease Entity?

    Background: Some studies suggested that colorectal cancer at young age had a distinct biological characteristic: more advanced stage at time of diagnosis, poorer differentiated, and consisted of large proportion of mucin producing tumors. Aim of the study: To analyze clinical and histopathological differences between young aged colorectal cancer patients (&lt; 40 years old) and the older patients Methods: A cross-sectional retrospective study was conducted among our colorectal cancer patients in a general hospital between 1999-2004, using C.18, C.19, C20 ICD X code in medical record searching. It was requested that the patients had surgical treatment in Dr. Sardjito General Hospital. An inconclusive clinical staging and/or histological data were among the exclusion criteria. Chi-square, Fisher's exact test, T-test, and Mann Whitney U-test was performed to analyze the difference between patients &lt; 40 years old and ³ 40 years old, in respect to diagnostic staging, histological type, histological grade (differentiation), CEA level, hemoglobin, albumin, tumor location, and chief complain. With p &lt; 0.05 was considered as significant. Results: Sixteen young aged (&lt; 40 years old) and 72 older patients had been identified. No differences in gender proportion and mean of symptoms to diagnosis period between two groups. No statistical differences between young aged and the older patients in diagnostic staging, histological grade and type, CEA level, and hemoglobin. Young aged patients had higher albumin value at presentation (p = 0.014), all had left sided tumors (p = 0.035), more complain of anal pain (p &lt; 0.001), and less change of bowel habits complain (p = 0.009) Conclusion: The study results had failed to confirm the difference in respect to diagnostic staging, histological type and grade, CEA level, and hemoglobin. Most of our young aged patients had left sided tumors with chief complain of anal pain, and less complain of change of bowel habit

    Background: Diabetes Mellitus is a disease that continues to increase every year in Indonesia. Increasing prevalence of diabetes mellitus may be accompanied by an increasing prevalence of cardiovascular disease. The ratio of total cholesterol to HDL is a strong predictor of the risk of coronary heart disease. Objective: To know the difference of total cholesterol to HDL ratio in type 2 diabetes mellitus with and without hypertension in Dr. Moewardi Hospitals. Methods: An Observational analytic research with cross sectional study design. Research subjects were type 2 diabetes mellitus with hypertension and without hypertension in Dr. Moewardi Hospitals. Sampling was done by purposive sampling technique. Data retrieval was obtained from the patient's medic record. Statistical analysis of this study using Mann-Whitney test. Results: This research used 70 samples, consisting of 35 patients diabetes mellitus group with hypertension and 35 patients diabetes mellitus group without hypertension. Differences in the ratio of total cholesterol to HDL in type 2 diabetes mellitus with hypertension and without hypertension resulted in p = 0.000 (p<0.05). Conclusions: There is a difference in the ratio of total cholesterol to HDL in patients with type 2 diabetes mellitus with hypertension and without hypertension

    Background: Chronic Kidney Disease (CKD) is a disease with diverse etiology that lead progressively decrease a renal function, and sometimes can be a kidney failure. Kidney failure is a clinical condition characterized by an irreversible decrease renal function which in some levels require a medical or therapy fixed, like dialysis and kidney transplantation. The research from Persatuan Nefrologi Indonesia (PERNEFRI) also estimate that 70.000 patients with Chronic Kidney Disease can be found in Indonesia. Goals: To investigate relationship between body mass index and hypertension in patient with chronic kidney disease. Method: This study used analytic observational research with cross sectional approach. Subjects in this study amounted to 56 patients with chronic kidney disease which not undergone hemodialysis yet. The instrument used the medical record to seen body mass index and blood preassure. Results: The hypothesis was analyzed using Chi Square Test, which the statistical results is p=0,00, significance of p<0,05. It means H0 was rejected and H1 was accepted. Conclussion: There was a significant relationship between body mass index and hypertension in patient with chronic kidney disease

    Background : Acute myocardial infarction (AMI) is a manifestation of acute myocardial ischemia and is commonly due to rupture of atherosclerotic plaque or thrombus in coronary arteries and results in a lack of blood supply to the myocardium. The infiltration of inflammatory cells and process are characteristic of acute myocardial infarction. The increase in the number of leukocytes is based on the exact reaction of normal bone marrow to external stimuli, one of which is the inflammation procces. Objective : The aim of this study is to determine the differences between leukocyte level in patient with ST elevation myocardial infarction (STEMI) and Non-ST elevation myocardial infartion (NSTEMI) Methods : An observational study with cross sectional approach. Sample was taken by purposive sampling technique. Total sample are 72 people, 36 people with STEMI and 36 people with NTSTEMI. Data collected by using medical records of patient in RSUD Dr. Moewardi Surakarta and it is analyzed by unpaired T Test. Results: Leukocyte level of 13,144 ± 4,7726 thousand/mm3 in patient with STEMI and 10,192 ± 3,2456 thousand/mm3 in patient with NSTEMI. By the significancy value 0,003 (p < 0.05). Conclusion : There are differences between leukocyte level in patient with ST elevation myocardial infarction (STEMI) and Non-ST elevation myocardial infarction (NSTEMI). Leukocyte level in patient with STEMI is higher than NSTEMI

    Background: Hypertension is the leading cause of death that was ranked third after stroke and tuberculosis, which reached 6.7% of deaths in all age groups in Indonesia. Had a lot of people have hypertension without knowing for years. Therefore hypertension is referred to as the silent killer. The prevalence of risk factors for hypertension is higher among patients with chronic renal failure and will increase with the severity of chronic kidney disease. Objective: This study aims to determine the relationship of cholesterol levels and hypertension in patients with chronic kidney disease at the Hospital Dr Moewardi Surakarta. This study uses an observational analytic design with cross sectional approach using secondary data obtained from medical records. The study design was used to study the relationship between cholesterol levels and hypertension in chronic kidney disease. Methods: From the analysis using Chi-square test there is a significant correlation between cholesterol levels and hypertension in patients with chronic kidney disease. With a p-value of 0.001 (p <0.005). Based on the results of this study concluded that there is a relationship with hypertension cholesterol levels in patients with chronic kidney disease at the Hospital Dr. Moewardi Surakarta.     ABSTRACT The Relation of Low Dense Lipoprotein with Glomerular Filtration Rate in Patient with Chronic Kidney Disease at Dr. Moewardi Hospital of Surakarta Baiq Selsilya Prapita Nilda, Suryo Aribowo Taroeno BACKGROUND: Chronic kidney disease is a serious health problem because the prevalence of CKD keep increased with age troughout the world, including in Indonesia. Patient with lower renal function display an atherogenic lipid profile with high LDL cholesterol and total cholesterol. LDL is considered contributing the progression of chronic kidney disease. Circulating lipoproteins play a direct role in the pathogenesis of glomerulosclerosis and tubulointerstitial changes. OBJECTIVE: This study aimed to investigate the relationship between LDL with GFR in patients with chronic kidney disease at Dr. Moewardi Hospital of Surakarta. METHODS: This study used observational analytic design with cross sectional approach using secondary data obtained from medical records. The study design was used to study the relationship between LDL with GFR in chronic kidney disease. RESULTS: From the analysis using Spearman's test there is a negative relationship between LDL with GFR in patients with chronic kidney disease, with a p value of 0.044 (p<0,05) and r= -0,213. Based on the results of this study concluded that there is correlation between LDL with GFR in patients with chronic kidney disease at Dr. Moewardi Hospital of Surakarta. CONCLUSION: There is a relationship between LDL with GFR in patients with chronic kidney disease at Dr. Moewardi Hospital of Surakarta. Keywords: LDL, GFR, Chronic Kidney Diseas
