    Culture‑proven disseminated Mycobacterium Bovis infection (BCG‑Osis) following intravesical BCG immunotherapy in a patient with bladder carcinoma‑in‑situ: ‘Case report’.

    We are reporting a case of culture-proven disseminated Mycobacterium Bovis infection (BCG-Osis), involving lung, bone-marrow and urinary tract, after intravesical Bacillus Calmette–Guérin (BCG) immunotherapy for bladder carcinoma-in-situ. A 71-year-gentleman presented with fever shortly after intravesical BCG instillation. He was initially treated for presumed Urinary Tract Infection, but negative urine culture and persistent fever prompted us to consider alternative diagnoses. Empirical treatment for BCG-Osis was commenced after initial negative workup including blood culture, echocardiography, computed tomography (CT) Chest, and Kidney-Ureter-Bladder (KUB). However, he remained febrile and leukopenic, so bone-marrow examination was performed along with repeating CT Chest, which revealed non-caseating granulomas and small nonspecific pulmonary nodules, respectively, supporting our provisional diagnosis. Interestingly, Mycobacterium Bovis was finally isolated from one of his urine specimens, confirming our diagnosis

    Prognostically Significant Fusion Oncogenes and Gene Mutations in Pakistani AML Patients

    Background: The onset and progression of leukemia is associated with many genetic abnormalities including gene mutations and production of fusion oncogenes. Molecular studies on fusion oncogenes and mutations in different populations have been done. However, not much research on correlation of the fusion oncogenes with acute myeloid leukemia have been done in Pakistan.Methods: Genetic analysis of 105 AML patients was done to investigate AML1-ETOand CBFB-MYH11 fusion oncogenes and mutations in NPM1 and NRAS genes. The genomic DNA and cDNA were subjected to amplification, electrophoresis, and Sanger sequencing.Results: The frequency of AML1-ETO was 26% in AML patients and 34.2% in AML-M2 patients. CBFB-MYH11 was present in 11.4% AML patients. A total of six mutations in 4 regions of NPM1 gene and 2 regions of NRAS gene were detected. 3’UTR of NPMI gene had three variants; g.1128C>T (57.1%), g.1185-/T insertion (80.95%), and g.1163A>T (57.14%) while c.867_871subGTGGA >CAAGTTTGC (2.86%) was present in exon 12. NRAS gene had two mutations c.12C>T (51.4%) and c.33A>T (11.43%) in exon 2. c.867_871subGTGGA >CAAGTTTGC , and g.1163A>T in NPM1 gene and c.33A>T in NRAS gene were the novel findings in this ethnic population.Conclusion: This genetic analysis may help to modulate the treatment strategies and improve survival of patients.Keywords: Acute Myeloid Leukemia; Fusion Oncogenes; AML; AML1-ETO; CBFB-MYH11; AML-M2; Nucleophosmin, NPM1; NRAS

    Interactive Influence of Nutrient Density and Feeding Pattern on Production Performance of Broiler Breeders

    Background: In addition to energy and protein, the minerals also play a vital role in the production performance of birds. The present study was conducted to examine the effect of high energy feed and separate calcium feeding on feed cleanup time and production parameters in broiler breeder pullets. Methods: Nine hundred female birds of the same age (26 week) were divided in nine groups having one hundred birds in each group.  Three different dietary treatments were offered. The group A, B and C were given feeds having 2750, 2900, 2950 Kcal ME / kg and 14.5%, 15%, 15.5% CP respectively along with 165, 155 and 150 grams peak feed allowance at 60 % production.  The second and third groups were given 10 g separate calcium source in the evening.  The experiment continued for a period of twelve weeks.Results: The feed intake was significantly lower in group C as compared to A and B. A significantly lower feed cleanup time and higher hatchability percentage was observed in group B compared to groups A and C. The FCR eggs was significantly lower for groups B and C compared to group A.  The FCR chicks was significantly lower in group B and C compared to group A.  The cost of feed to produce chicks was significantly lower for group B compared to group A and C. Feed consumed to produce one chick was higher for group A compared to groups B and C.  The feed cost to produce one chick was highest for group A and lowest for group B.Conclusion: These results suggested that feed B has lowest feed consumption, cost to produce one chick and feed cleanup time while highest hatchability hence it can be used in the broiler breeder industry to decrease the cost of production and increase profitability

    Mechanical Performance of Polymeric ARGF-Based Fly Ash-Concrete Composites: A Study for Eco-Friendly Circular Economy Application

    At present, low tensile mechanical properties and a high carbon footprint are considered the chief drawbacks of plain cement concrete (PCC). At the same time, the combination of supplementary cementitious material (SCM) and reinforcement of fiber filaments is an innovative and eco-friendly approach to overcome the tensile and environmental drawbacks of plain cement concrete (PCC). The combined and individual effect of fly ash (FA) and Alkali resistance glass fiber (ARGF) with several contents on the mechanical characteristics of M20 grade plain cement concrete was investigated in this study. A total of 20 concrete mix proportions were prepared with numerous contents of FA (i.e., 0, 10, 20, 30 and 40%) and ARGF (i.e., 0, 0.5, 1 and 1.5%). The curing of these concrete specimens was carried out for 7 and 28 days. For the analysis of concrete mechanical characteristics, the following flexural, split tensile, and compressive strength tests were applied to these casted specimens. The outcomes reveal that the mechanical properties increase with the addition of fibers and decrease at 30 and 40% replacement of cement with fly ash. Replacement of cement at higher percentages (i.e., 30 and 40) negatively affects the mechanical properties of concrete. On the other hand, the addition of fibers positively enhanced the flexural and tensile strength of concrete mixes with and without FA in contrast to compressive strength. In the end, it was concluded that the combined addition of these two materials enhances the strength and toughness of plain cement concrete, supportive of the application of an eco-friendly circular economy. The relationship among the mechanical properties of fiber-reinforced concrete was successfully generated at each percentage of fly ash. The R-square for general relationships varied from (0.48–0.90) to (0.68–0.96) for each percentage of FA fiber reinforced concrete.     Prognostic model to predict postoperative acute kidney injury in patients undergoing major gastrointestinal surgery based on a national prospective observational cohort study.

    Background: Acute illness, existing co-morbidities and surgical stress response can all contribute to postoperative acute kidney injury (AKI) in patients undergoing major gastrointestinal surgery. The aim of this study was prospectively to develop a pragmatic prognostic model to stratify patients according to risk of developing AKI after major gastrointestinal surgery. Methods: This prospective multicentre cohort study included consecutive adults undergoing elective or emergency gastrointestinal resection, liver resection or stoma reversal in 2-week blocks over a continuous 3-month period. The primary outcome was the rate of AKI within 7 days of surgery. Bootstrap stability was used to select clinically plausible risk factors into the model. Internal model validation was carried out by bootstrap validation. Results: A total of 4544 patients were included across 173 centres in the UK and Ireland. The overall rate of AKI was 14·2 per cent (646 of 4544) and the 30-day mortality rate was 1·8 per cent (84 of 4544). Stage 1 AKI was significantly associated with 30-day mortality (unadjusted odds ratio 7·61, 95 per cent c.i. 4·49 to 12·90; P < 0·001), with increasing odds of death with each AKI stage. Six variables were selected for inclusion in the prognostic model: age, sex, ASA grade, preoperative estimated glomerular filtration rate, planned open surgery and preoperative use of either an angiotensin-converting enzyme inhibitor or an angiotensin receptor blocker. Internal validation demonstrated good model discrimination (c-statistic 0·65). Discussion: Following major gastrointestinal surgery, AKI occurred in one in seven patients. This preoperative prognostic model identified patients at high risk of postoperative AKI. Validation in an independent data set is required to ensure generalizability
