    Brain abscess following rituximab infusion in a patient with pemphigus vulgaris.

    BackgroundImmunocompromised patients are at increased risk for developing meningitis or, rarely, brain abscess with opportunistic organisms like Listeria monocytogenes.Case reportA 52 year-old Saudi Arabian woman who was diagnosed with pemphigus vulgaris and diabetes and had been on prednisolone and azathioprine for about 4 years. She presented with headache, low-grade fever, and left-sided weakness 2 weeks after receiving the second dose of rituximab infusion. Magnetic resonance imaging revealed an enhanced space-occupying lesion with multiple small cyst-like structures and vasogenic edema in the right temporoparietal area. Her blood culture was positive for Listeria monocytogenes, and a brain biopsy showed necrotic tissues with pus and inflammatory cells. She recovered after a 6-week course of antibiotics with ampicillin and gentamycin.ConclusionsBrain abscess due to Listeria monocytogenes is a risk that should be considered when adding rituximab to the regimen of a patient who is already Immunocompromised

    Histopathologic profile of esophageal atresia and tracheoesophageal fistula

    Purpose Few reports are available in the literature on the histology of the congenital atretic esophagus in humans. Histologic abnormalities  including congenital esophageal stenosis (CES) may contribute toward the abnormal esophageal motility after successful repair of esophageal atresia (EA) and tracheoesophageal fistula (TEF). The main aim of this study is to document the histopathologic profile in cases of EA.Methods One hundred and nineteen surgical specimens were collected from 69 consecutive EA patients who underwent surgical repair at the Aseer Central Hospital, Abha, and Armed Forces Hospital Southern Region, Saudi Arabia, from May 1999 through May 2009. This included 62 cases with EA and distal TEF, five cases of pure EA, and two cases of N-type TEF. Samples from tips of the upper pouch (UP), lower pouch (LP), and mid portion of the TEF were preserved in 10% formalin, sectioned, and stained with hematoxylin and eosin.Results The combined three elements of tracheobronchial tissue were observed in only three LP specimens. Gastrictype mucosa was seen in one UP and one LP specimen. Except for one N-type fistula, all sections showed fullthickness muscle coats. Distortion of muscles by fibrosis was most commonly seen in the UP. The muscle layer in the LP was more commonly distorted by glands with or without cartilage. Fourteen samples (10.8%) showed a histological picture consistent with CES.Conclusion Glands in the submucosa may be abnormal innumber and type, and may extend to different esophageal  coats. Muscle distortion by fibrosis, glands, or cartilage and associated CES may contribute toward esophageal dysmotility and stricture after surgery. Cutting the TEF B3–5mm distal to its origin from the trachea is adequate histologically for primary anastomosis of the atretic esophagus.     The prevalence of smartphone addiction and its correlation with depression among higher college of technology students, Oman

    Background: The aim of this study is to identify the prevalence of smartphone addiction and its correlation with depression among Higher College of Technology students in Oman.Methods: A cross-sectional study was conducted in the Higher College of Technology, Oman. A self-reported questionnaire was distributed electronically to students between January and February 2017. The questionnaire contained a validated short version of the smartphone addiction scale (SAS-SV) and a validated Arabic version of the Patient Health Questionnaire-9 (PHQ-9), along with a single question to self-assess smartphone addiction.Results: With a response rate of 86.2%, a total of 376 students were considered, of whom 34.6% were males and 65.4% were females, with an overall mean age of 20.9 years. This study showed that the prevalence of smartphone addiction in the study population using the SAS-SV was 63.8%; 88.3% of students were using their smartphones for four hours or more per day and 80% had been using smartphones for more than four years. The overall depression rate was 32.2%. As the total depression score increased by 1 point, the smartphone addiction score increased by 0.428 points (p<0.005). The prevalence of smartphone addiction was 54.9% among those who had no depression, 80.3% among those who had mild depression, 75.9% among those who had moderate depression, and 96.2% among those who have severe depression (p<0.005).Conclusions: This study shows high smartphone addiction levels based on SAS-SV scores and found a significant positive correlation between smartphone addiction score and depression scores

    Morphology Control of TiO Nanorods Using KBr Salt for Enhancing the Photocatalytic Activity of TiO and MoS/TiO Heterostructures.

    In this study, the effect of KBr salt on the growth of TiO nanorods (NRs) was systematically studied. The addition of KBr with different concentrations provides a controllable growth of TiO NRs using hydrothermal method. The results revealed that the presence of KBr molecules affects the growth rate by suppressing the growth in the lateral direction and allowing for axial growth. This results in affecting the morphology by decreasing the diameter of the nanorods, and increasing the free space between them. Enhancing the free spaces between the adjacent nanorods gives rise to remarkable increase in the internal surface area, with more exposure side surface. To obtain benefit from the enlargement in the inner surface area, TiO NRs were used for the preparation of MoS/TiO heterostructures. To study the influence of the morphology on their activity, TiO NRs samples with different KBr concentrations as well as the MoS/TiO heterostructures were evaluated towards the photocatalytic degradation of Rhodamine B dyes.Open Access funding provided by Qatar National Library

    Neutralizing antibody response during acute and chronic hepatitis C virus infection

    Little is known about the role of Abs in determining the outcome of hepatitis C virus (HCV) infection. By using infectious retroviral pseudotypes bearing HCV glycoproteins, we measured neutralizing Ab (nAb) responses during acute and chronic HCV infection. In seven acutely infected health care workers, only two developed a nAb response that failed to associate with viral clearance. In contrast, the majority of chronically infected patients had nAbs. To determine the kinetics of strain-specific and crossreactive nAb emergence, we studied patient H, the source of the prototype genotype 1a H77 HCV strain. An early weak nAb response, specific for the autologous virus, was detected at seroconversion. However, neutralization of heterologous viruses was detected only between 33 and 111 weeks of infection. We also examined the development of nAbs in 10 chimpanzees infected with H77 clonal virus. No nAb responses were detected in three animals that cleared virus, whereas strain-specific nAbs were detected in six of the seven chronically infected animals after approximately 50 weeks of infection. The delayed appearance of high titer crossreactive nAbs in chronically infected patients suggests that selective mechanism(s) may operate to prevent the appearance of these Abs during acute infection. The long-term persistence of these nAbs in chronically infected patients may regulate viral replication

    Effects of hydroxyapatite and PDGF concentrations on osteoblast growth in a nanohydroxyapatite-polylactic acid composite for guided tissue regeneration

    The technique of guided tissue regeneration (GTR) has evolved over recent years in an attempt to achieve periodontal tissue regeneration by the use of a barrier membrane. However, there are significant limitations in the currently available membranes and overall outcomes may be limited. A degradable composite material was investigated as a potential GTR membrane material. Polylactic acid (PLA) and nanohydroxyapatite (nHA) composite was analysed, its bioactive potential and suitability as a carrier system for growth factors were assessed. The effect of nHA concentrations and the addition of platelet derived growth factor (PDGF) on osteoblast proliferation and differentiation was investigated. The bioactivity was dependent on the nHA concentration in the films, with more apatite deposited on films containing higher nHA content. Osteoblasts proliferated well on samples containing low nHA content and differentiated on films with higher nHA content. The composite films were able to deliver PDGF and cell proliferation increased on samples that were pre absorbed with the growth factor. nHA–PLA composite films are able to deliver active PDGF. In addition the bioactivity and cell differentiation was higher on films containing more nHA. The use of a nHA–PLA composite material containing a high concentration of nHA may be a useful material for GTR membrane as it will not only act as a barrier, but may also be able to enhance bone regeneration by delivery of biologically active molecules

    Future therapies for food allergy

    Food allergy affects 3.9% of US children and is increasing in prevalence. The current standard of care involves avoidance of the triggering food and treatment for accidental ingestions. While there is no current curative treatment, there are a number of therapeutic strategies under investigation. Allergen specific therapies include oral and sublingual immunotherapy with native food protein as well as recombinant food proteins. Allergen non-specific therapies include a Chinese herbal formula (FAHF-2) and the use of anti-IgE monoclonal antibody therapy. Although none of these treatments are ready for clinical use, these therapeutic strategies present promising options for the future of food allergy

    Modeling viral and drug kinetics: Hepatitis C virus treatment with pegylated interferon alfa-2b

    Administration of peginterferon alfa-2b plus ribavirin results in an early hepatitis C virus (HCV) RNA decay followed by an increase as the drug concentration declines between doses. Upon administration of the next dose 1 week later, the same pattern is observed. We have incorporated pharmacokinetic/pharmacodynamic analysis into a model of viral dynamics to describe the effect that changes in drug concentration and effectiveness can have on viral levels. To illustrate the relationship between pharmacokinetics and viral dynamics, we fit the model to data from four HCV/human immunodeficiency virus co-infected patients, and obtained good agreement with the measured serum HCV RNA levels. We were able to account for the observed increases in HCV RNA, and estimate virion and drug half-lives that are in agreement with previous reports. Models incorporating pharmacokinetics are needed to correctly interpret viral load changes and estimate drug effectiveness in treatment protocols using peginterferon alfa-2b

    Association Between Race/Ethnicity and Survival of Melanoma Patients in the United States Over 3 Decades

    Melanoma is a treatable and preventable skin cancer. It is responsible for 75% of deaths among all skin cancers. Previous studies have found that race/ethnicity may play a role in survival among melanoma patients. However, there are no studies that cover 30 years and take race into account for the U.S. population. This study is a secondary analysis of the National Cancer Institute\u27s Surveillance, Epidemiology, and End Result (SEER) Program. Adults with primary cutaneous melanoma from 1982 to 2011 were included; the final sample size was 185,219. The outcome was survival; both cause-specific and all-cause mortality were examined. The main exposure was race/ethnicity. Kaplan?Meier survival analysis was used to estimate overall survival. Cox proportional hazards regression was used to estimate unadjusted and adjusted hazard ratios (HRs). A P-value less than 0.05 was considered statistically significant. More than 50% of patients in all races/ethnicities were diagnosed at the in situ or localized stage. Non-Hispanic White patients were more frequently diagnosed at the in situ stage. Overall, more men were diagnosed than women. The majority of cases among all races were men. Non-Hispanic Black females represented the smallest percentage of melanoma cases among all races. The smallest number of diagnoses across all races/ethnicities was made from 1982 to 1991. Median follow-up was 81 months and no collinearity was observed in the adjusted models. When examining cause-specific mortality and controlling for site and stage at diagnosis, gender, age and decade of diagnosis, the HR for non-Hispanic Black patients was lower than that for non-Hispanic White patients (HR 0.7; 95% confidence interval (CI): 0.6?0.8). However, when examining all-cause mortality, this difference disappeared (HR 1.1; 95% CI: 1.0?1.2). Stage at diagnosis impacted HR; patients diagnosed with distant metastases had significantly worse survival. When taking cause-specific mortality into consideration and after controlling for stage and site at diagnosis, gender, and age and decade of diagnosis, non-Hispanic Black patients had a lower HR compared to non-Hispanic White patients. However, this difference disappeared when examining all-cause mortality. Further research is needed to explore this finding and to determine what factors may be associated with late-stage melanoma diagnosis

    Eosinophilic Pneumonia in a Patient with Bronchial Myiasis : Case report and literature review

    Pulmonary myiasis is an unusual form of myiasis in humans and has been recently identified as a cause of eosinophilic pneumonia. We report the case of a 13-year-old Omani boy who presented to the Royal Hospital, Muscat, Oman, in October 2014 with respiratory distress. Bronchial aspirates revealed features of eosinophilic pneumonia. Possible larvae identified in the cytology report, a high immunoglobulin E level and the patient history all indicated bronchial myiasis. The patient was treated with steroids and ventilation and has since been diseasefree with no long-term side-effects. To the best of the authors’ knowledge, this is the first case of bronchial myiasis in Oman
