    Dental status of Portuguese HIV plus patients and related variables: a multivariate analysis

    Objective: The aim of this cross-sectional study was to evaluate the dental status of 101 Portuguese HIV+ subjects aged 22-71 years (mean = 39) and its association with clinical, socioeconomic, and behavioral variables. Materials and Methods: A calibrated dentist performed clinical examination and collected data on dental caries, periodontal status, dental plaque levels, prosthetic conditions, and need. The volunteers completed questionnaires on socioeconomic and behavioral variables as well as the Oral Health Impact Profile (OHIP-14) questionnaire. Univariate and multiple logistic regression (MLR) analyses were performed. Results: The mean number of decayed, missing or filled teeth index (DMFT index) was 16.44, standard deviation (s.d.) = 8.42. MLR demonstrated that salaried employee and those with OHIP-14 median (=17). As regards prosthetic status, 28.8% of the examined individuals used dental prosthesis. MLR demonstrated that HIV+ with DMFT > 17 or those who knew they were HIV-positive for longer than 5 years were more prone to need dental prostheses. The mean OHIP-14 index was 5.83 (s.d. = 7.79). Conclusions: The dental health status of HIV-infected Portuguese patients was unsatisfactory and related to clinical, socioeconomic, and behavioral variables.16217618

    Combined effects of carbon dioxide laser and fluoride on demineralized primary enamel: An in vitro study

    This in vitro study aimed at evaluating the efficacy of a CO2 laser (10.6 mu m) alone or combined with acidulated phosphate fluoride (APF) on the inhibition of lesion progression in primary enamel. The specimens were treated with/without CO2 laser and/or APF and submitted to pH cycling. Microhardness analysis was performed and the enamel mineral loss values were obtained. The groups treated with laser and/or APF presented lower mineral loss when compared to the control group (p < 0.05). Laser irradiation alone or combined with APF decreased lesion progression in primary enamel. However, the combined treatment did not show any significant additional effect. Copyright (c) 2007 S.     Perception of dental fluorosis and other oral health disorders by 12-year-old Brazilian children

    Aim. The aim of this study was to verify the perception of dental fluorosis as an oral health problem by 12-year-old Brazilian children and to evaluate if they were able to detect other oral disorders. Methodology. The sample consisted of 401 schoolchildren. Firstly, clinical examinations were performed using a visual method under natural light. After that, children answered a questionnaire with the purpose of assessing the self-perception of their oral health problems. Next, the volunteers were shown a photo album containing 24 photographs, and had to match each photo with a severity scale. Results. The prevalence of fluorosis was 18.2%; 81.8% of this total in fluorosis scale T-F 1. Of the 401 children, 48.9% (n = 196) answered oral health problems related to concerns, such as aesthetic appearance or pain. Among them, only two children, both with fluorosis T-F 2, correlated their problems with the presence of stained teeth. As regards the photo album analysis, the children considered photos showing fluorosis T-F 7-9 as the most severe, whereas the photo of an orally healthy patient was considered the least severe. Conclusion. Children did not show negative perception of dental fluorosis, except for dental fluorosis at severe levels (T-F 7-9), and were mainly able to detect aesthetic or pain-related problems.17320521

    Distribution and prevalence of dental caries in Bauru, Brazil, 1976-2006

    Aims: To compare caries prevalence and severity, as well as the disease distribution in the permanent dentition of 12-year-old schoolchildren in Bauru, Brazil, from 1976 to 2006. Methods: Probabilistic samples were obtained in six surveys, from 1976 to 2006, which were conducted by calibrated examiners, following the same protocol. For data analysis, the DMFT (decayed, missing and filled teeth), Significant Caries Index (SiC Index), percentage of caries-free children, Gini Coefficient and Care Index were determined. Results: The means (SD) for DMFT were 9.89(3.96), 6.98(3.80),4.30(3.11), 4.29(3.44),1.53(2.07), and 0.90(1.53) in 1976, 1984, 1990, 1995, 2001 and 2006, respectively. Except for years 1990 and 1995 and also for 2001 and 2006, there were statistically significant differences in mean DMFT among the surveys (p < 0.05). The number of caries-free children (DMFT=0), which has significantly increased over the years (p < 0.001), ranged from 0.4% in 1976 to 63.8% in 2006. The SiC Indexes were 14.34, 11.42, 7.74, 8.06, 3.89, and 2.63; the Care Index were 31.9%, 57.4%, 68.7%, 50.5%, 66.4%, and 56.3%; and the Gini Coefficients were 0.23, 0.30, 0.39, 0.43, 0.66, and 0.76 in the surveys. Conclusion: Dental caries experience and prevalence in 12-year-old schoolchildren have declined significantly during the last 30 years in Bauru.582758

    The short-term in situ model to evaluate the anticariogenic potential of ionomeric materials

    Objectives. Aiming to contribute to the study of mechanisms involved in the anticariogenic properties of dental materials, this study assessed the suitability of a short-term in situ model to evaluate the anticariogenic potential of ionomeric materials. Methods. The study used a 3-phase crossover, double blind design, and in each phase eight volunteers wore palatal appliances containing four enamel blocks restored with one of the following materials: composite resin (CR-Z250) (negative control), a conventional glass ionomer cement (GIC-Ketac-Fil) or a resin-modified GIC (RM-GIC-Vitremer). The restored blocks were covered with a 'test plaque' of S. mutans, placed in palatal. appliances and a cariogenic challenge was made during 1 min with 20% sucrose solution. After 45 min, test plaque was collected for fluoride (F) analysis. Enamel surface microhardness was previously determined at one side of the restoration and the percentage of surface microhardness change (%SMC) in retation to baseline (other side) was calculated. F concentration in enamel was also evaluated. Results. Split-plot ANOVA showed a statistically tower %SMC on enamel around the ionomeric materials than around the CR (p<0.05). This result was supported by a statistically higher F concentration in test plaque (P<0.001) and in enamel (P<0.001) restored with the ionomeric materials when compared to the CR. Conclusions. The results suggest that the short-term in situ model tested is useful for studying the anticariogenic potential of dental materials that release fluoride. (C) 2005 Elsevier Ltd.     Risk indicators and risk predictors of dental caries in schoolchildren

    The purpose of this study was to identify risk indicators of high caries level at baseline (HCLB) based on cross-sectional data and predictors of high caries increment (HCI) based on a 7-year-follow-up examination in 6-8-year-old schoolchildren. Two hundred and six schoolchildren were examined in 1997 and in 2004 by the same two calibrated dentists, in Piracicaba, Brazil. At baseline, dental caries, presence of sealants, fluorosis, and oral hygiene status were recorded. The children's parents completed a questionnaire concerning socioeconomic level, fluoride use, dental service utilization, dietary and oral hygiene habits. HCLB and HCI were defined considering the upper quartile of the total caries experience distribution (dmfs+DMFS) and caries increment distribution, respectively. Logistic regression models were adjusted estimating the Odds Ratio (OR), 95% confidence intervals and p-values. Having white spot lesions (OR=5.25) was found to be a risk indicator of HCLB. Schoolchildren with dental fluorosis (OR=0.17) or those who brushed the teeth more than two times a day (OR=0.37) presented less probability of HCLB. The predictors of HCI were: dmfs>0 (OR=2.68) and mothers' educational level up to 8 years of schooling (OR=2.87).     Cárie dentária em população ribeirinha do Estado de Rondônia, Região Amazônica, Brasil, 2005/2006 Dental caries in a riverine community in Rondônia State, Amazon Region, Brazil, 2005-2006

    O objetivo foi analisar experiência de cárie dentária na população ribeirinha residente às margens dos rios Machado e Preto (Rondônia, Brasil), em 2005 e 2006. Foram examinados 469 indivíduos com formulário preconizado pela Organização Mundial da Saúde, sob luz natural e utilização de espátulas de madeira e sonda CPI. Na faixa etária de 4-5 anos de idade, ceod = 4,30 e 19,64% livres de cárie; 6-10 anos, CPOD = 1,04, ceod = 3,52, 17,05% livres de cárie; aos 12 anos, CPOD = 2,65 e 30,76% livres de cárie; aos 18 anos, CPOD = 5,41 e 19,51% livres de cárie; 35-44 anos, CPOD = 17,74 e 2,98% livres de cárie; 65-74 anos, CPOD = 21,56 e 4,34% livres de cárie. Na análise por componentes, constatou-se que o componente cariado tem maior prevalência nas idades de 0-3, 4-5, 6-10, 12 e 18 anos. Em adultos e idosos, o componente que mais contribui é o perdido. Conclui-se que a população apresenta índices de cárie dentária elevados, sendo necessária a atuação em âmbito educativo, preventivo e curativo.<br>This study aimed to analyze dental caries patterns among riverine people from Rondônia State, Brazil (Machado and Preto rivers) in 2005 and 2006. A total of 469 subjects were examined, using the World Health Organization form, under natural light, using a wooden tongue depressor and CPI probe in cases of doubts about the presence of dental caries. The results were: 4-5-year age bracket, dmtf = 4.30 and 19.64% caries-free; 6-10 years, DMTF = 1.04, dmtf = 3.52 and 17.05% caries-free; 12 years, DMTF = 2.65 and 30.76% caries-free; 18 years, DMTF = 5.41 and 19.51% caries-free; 35-44 years, DMTF = 17.74 and 2.98% caries-free; 65-74 years, DMTF = 21.56 and 4.34% caries-free. When each component was analyzed separately in the dmtf and DMTF indices, decay was most prevalent in the 0-3, 4-5, 6-10, 12, and 18-year brackets. However, in young and older adults, the most prevalent component was missing teeth. In conclusion, the study population showed a high dental caries index, thus highlighting the need for educational, preventive, and curative measures
