    Get PDF
Abstract The Keshe area is a part of Cenozoic magmatic belt of Central Iran. Some volcanics in this area underwent hydrothermal alteration including kaolinitization and alunitization. The altered rocks are characterized by an assemblage of alunite, kaolinite and quartz, which is typical in advanced argillic alteration. The mineralogical study suggests that the hydrothermal alteration in this area occurred in a magmatic hydrothermal environment

Abstract. Insurance companies are expected to show successful performance in achieving their goals. The extent to which awareness of this organizational performance has been successful in achieving goals and where the position of organization is situated in the modern complex and dynamic context have taken on special importance to managers and organizations. Organization survival and activity are determined by competition ability and adoption of the most suitable policy for environmental changes. One of the most effective practices used by organizations is the use of performance evaluation in order to determine weaknesses and strengths of organizations and fix them and enhance their strengths. Performance management and evaluation play a prominent role in determining and implementingstrategies, as well as contributing to organizations’ competition power. In this regard, possessing a model for evaluating organization’s strategic performance seems essential. One of the techniques is the balanced scorecard which was introduced to evaluate organizations’ performance for the first time and is still recognized as a method of strategic planning which can be applicable. The balanced scorecard is a managerial concept which helps managers at all levels controls their key activities. In this research, we aim to assess the performance of various representatives of Kosar Insurance Co. in Qazvin using a combined approach, the balanced scorecard (BSC) and analytical hierarchy process (AHP), and prioritize them and explore their strengths and weaknesses. Keywords: performance evaluation, balanced scorecard, analytical hierarchy process, Qazvin Kosar Insuranc

Background: Total white blood cell (WBC) count and mean platelet volume have previously been shown to predict outcomes in acute coronary syndrome (ACS) patients. In this prospective study, we sought to determine the prognostic value of baseline WBC count to mean platelet volume ratio (WMR) in patients with non-ST elevation acute coronary syndrome (NSTE-ACS). Methods: A total of 490 patients with NSTE-ACS were prospectively enrolled. The relationship between baseline WMR and major adverse cardiovascular events (MACE) incidence was assessed during a mean follow-up of 330.8 ± 38 days. Results: The patients’ mean age was 60.4 ± 12.9 year, 59% of them were male. The patients were categorized into two groups based on WMR values, high- and low-WMR groups (< 755 vs. ≥ 755). The incidence of MACE was significantly higher in high-WMR compared with that of low-WMR group (22.4% vs. 10.7%, p < 0.001). Total WBC counts (median 7.9 vs. 6.9 ×103/μL, p = 0.004), neutrophil count (median 4.6 vs. 4.2 ×103/μL, p = 0.021), and WMR (median 863.2 vs. 731.5, p = 0.001) were significantly higher in the MACE-positive than MACE-negative group. The high-WMR was found to be significantly associated with the MACE-free survival rate (p < 0.001). In an adjusted cox regression model, the elevated WMR was independently predicted the incidence of MACE (hazard ratio 2.419, 95% CI 1.515–3.862, p < 0.001). Conclusions: The elevated baseline WMR independently predicted the MACE incidence in patients with NSTE-ACS during long-term follow-up.

This paper presents an investigation on deaf students' interests in spending their leisure times. We design a questionnaire and distribute among all deaf students who are enrolled in high schools in two provinces of Iran. The questionnaire consists of three parts, in the first part, we ask female and male deaf students about their interests in various entertainment activities in Likert scale. In terms of gender, we find out that walking inside or outside house is number one favorite exercise for female students while male students mostly prefer to walk on the streets. Although male students prefer to go biking or running activities, female students prefer to go for picnic or similar activities. This could be due to limitations on female for running or biking inside cities. While going to picnic with members of family or friends is the third popular activity for male students, stretching exercises is third most popular activity among female students. Breathing exercise is the fourth most popular activity among both male and female students. The second part of the survey is associated with the barriers for having no exercise among deaf students. According to our survey, while lack of good attention from public and ordinary people on exercising deaf students is believed to be number one barrier among male students, female students blame lack of transportation facilities as the most important barrier. However, both female and male students believe these two items are the most important factors preventing them to exercise. Lack of awareness for exercising deaf students and lack of good recreational facilities are the third most important barriers among male and female students. The last part of the survey attempted to detect important entertainment activities. Watching TV, entertaining with mobile devices, chatting with friends and watching DVD or movies were the most important items influencing deaf students' free times.DOI: 10.5267/j.msl.2012.09.011Keywords

Introduction: The purpose of this study was to assess the within-day and between-day reliability of abdominal and lumbar multifidus muscle size in patients with unilateral lumbar disc herniation (LDH) using ultrasonography (US). Materials and methods: In this study, 15 patients with unilateral LDH (20-60 years old) were recruited. To assess within-day and between-day reliability, three images were taken with one hour and one week intervals respectively. The images were taken at rest and during contraction. Results: The within-day and between-day reliability of abdominal muscle thickness measurements using US in patients with unilateral lumbar disc herniation (LDH) in both rest and contraction state was found to be high with ICC=0.87 for within and ICC=0.75 for between-day rates of transverse abdominis (TrA) muscles at rest and ICC=0.78 and 0.75, respectively, in contraction state. For internal oblique muscles, ICC=0.70 at rest and 0.79 in contraction state and ICC=0.73 at rest and 0.77 in contraction state were found for within-day and between-day, respectively. Within-day and between-day reliability at rest with ICC=0.76 and 0.76 and in contraction state with ICC=0.75 and 0.74 were demonstrated for the external oblique muscle. Within-day and between-day reliability rates at rest with ICC=0.88 and 0.84, respectively, and in contraction state with ICC=0.84 and 0.80, respectively were demonstrated for the lumbar multifidus muscle. Conclusion: The results of the present study suggest that US is a reliable method for evaluating the thickness of the abdominal and lumbar multifidus muscles. However, further research is recommended to support the findings of the present study. Keywords: Ultrasonography, disc herniation, abdominal muscles, lumbar multifidus muscle, reliabilit

Adenoids and tonsils are active lymphoid organs and play an important role against invading antigens of upper aerodigestive tract in children. The present study analyzes the changes in cellular and humoral immunity of children six months after adenotonsillectomy. The study population consisted of 30 children whit chronic adenotonsillar hypertrophy and 30 age-matched healthy children. In all children serum level of IgM and IgG, percentage of T lymphocytes (CD3), T helper cells (CD4), T cytotoxic cells (CD8) and B lymphocytes (CD20) were measured before surgery. These parameters were remeasured in patients 6 months after adenotonsillectomy. Before the operation, a reduction in percentage of T lymphocytes (CD3),TCD4,TC8 and B CD20 was seen compared to control group. This reduction was only significant in T lymphocytes (CD3).The serum IgM and IgG levels were not different in two groups. Six months after operation, the percentage of lymphocytes T CD3, T CD8 and BCD20 was increased and reached the control group. The IgM level was also significantly decreased in patients after operation. Our results indicate that cellular and humoral immunity decreases in children with chronic adenotonsiller hypertrophy preoperatively and increases to healthy children level, six months postoperatively. It means that chronic adenotosillar hypertrophy affect some parameters of cellular and humoral immunity and adenotonsillectomy by removing chronic stimulations and reverses these changes without any negative effect on immune function of patients. © 2007 Tehran University of Medical Sciences. Aggressive Adenoid Cystic Carcinoma of Maxillary Sinus in a 43-Year-Old Male: Rare Case Report and Review of Literature

    Get PDF
Adenoid cystic carcinoma (ACC) is a rare malignant tumor, mostly involving the minor salivary glands. Herein, we present a case of ACC in a 43-year-old man with symptoms of dental abscess as the initial presentation of the tumor. In spiral computed tomography (CT) scan, soft tissue mass with the erosion of maxillary sinus wall on the right side of the alveolar ridge was evident. Histopathological examination of the excised tumor with immunohistochemical studies (C-kit, Vimentin, pan-cytokeratin, p53, p63, and ki67 positive reaction) confirmed grade 2 ACC in the maxillary sinus. The patient underwent hemimaxillectomy and right-neck dissection. Due to the extension of tumor cells excessively into the surrounding tissues and involvement of orbital bone, complete and total resection of the tumor with safe margins could not be done. After surgery, adjuvant radiotherapy was considered for the patient. At the end of treatment, the patient lost his eye vision. Seventeen months from initial diagnosis, he was still alive without lung or distant metastasis
