Association of Chorioamnionitis with Aberrant Neonatal Gut Colonization and Adverse Clinical Outcomes.
ObjectiveChorioamnionitis (inflammation of the placenta and fetal membranes) and abnormal gastrointestinal colonization have been associated with an increased risk of sepsis and death in preterm infants, but whether chorioamnionitis causes abnormal pioneering gastrointestinal colonization in infants is not known. We determined the relationship between chorioamnionitis, altered infant fecal microbiome indicating abnormal gastrointestinal colonization, and adverse outcomes.Study designPreterm infants †28 weeks at birth were enrolled from 3 level III NICUs in Cincinnati, Ohio and Birmingham, Alabama. Sequencing for 16S microbial gene was performed on stool samples in the first 3 weeks of life. Chorioamnionitis was diagnosed by placental histology. Late onset sepsis and death outcomes were analyzed in relation to fecal microbiota and chorioamnionitis with or without funisitis (inflammation of the umbilical cord).ResultsOf the 106 enrolled infants, 48 infants had no chorioamnionitis, 32 infants had chorioamnionitis but no funisitis (AC), and 26 infants had chorioamnionitis with funisitis (ACF). The fecal samples from ACF infants collected by day of life 7 had higher relative abundance of family Mycoplasmataceae (phylum Tenericutes), genus Prevotella (phylum Bacteroidetes) and genus Sneathia (phylum Fusobacteria). Further, AC and ACF infants had higher incidence of late-onset sepsis/death as a combined outcome. Presence of specific clades in fecal samples, specifically, order Fusobacteria, genus Sneathia or family Mycoplasmataceae, were significantly associated with higher risk of sepsis or death.ConclusionThe results support the hypothesis that specific alterations in the pioneering infant gastrointestinal microbiota induced by chorioamnionitis predispose to neonatal sepsis or death

The fecal resistome of dairy cattle is associated with diet during nursing.
Antimicrobial resistance is a global public health concern, and livestock play a significant role in selecting for resistance and maintaining such reservoirs. Here we study the succession of dairy cattle resistome during early life using metagenomic sequencing, as well as the relationship between resistome, gut microbiota, and diet. In our dataset, the gut of dairy calves serves as a reservoir of 329 antimicrobial resistance genes (ARGs) presumably conferring resistance to 17 classes of antibiotics, and the abundance of ARGs declines gradually during nursing. ARGs appear to co-occur with antibacterial biocide or metal resistance genes. Colostrum is a potential source of ARGs observed in calves at day 2. The dynamic changes in the resistome are likely a result of gut microbiota assembly, which is closely associated with diet transition in dairy calves. Modifications in the resistome may be possible via early-life dietary interventions to reduce overall antimicrobial resistance
Rotational modes in molecular magnets with antiferromagnetic Heisenberg exchange
In an effort to understand the low temperature behavior of recently
synthesized molecular magnets we present numerical evidence for the existence
of a rotational band in systems of quantum spins interacting with
nearest-neighbor antiferromagnetic Heisenberg exchange. While this result has
previously been noted for ring arrays with an even number of spin sites, we
find that it also applies for rings with an odd number of sites as well as for
all of the polytope configurations we have investigated (tetrahedron, cube,
octahedron, icosahedron, triangular prism, and axially truncated icosahedron).
It is demonstrated how the rotational band levels can in many cases be
accurately predicted using the underlying sublattice structure of the spin
array. We illustrate how the characteristics of the rotational band can provide
valuable estimates for the low temperature magnetic susceptibility.Comment: 14 pages, 7 figures, to be published in Phys. Rev.
Model Exact Low-Lying States and Spin Dynamics in Ferric Wheels; Fe to Fe
Using an efficient numerical scheme that exploits spatial symmetries and
spin-parity, we have obtained the exact low-lying eigenstates of exchange
Hamiltonians for ferric wheels up to Fe. The largest calculation
involves the Fe ring which spans a Hilbert space dimension of about 145
million for M=0 subspace. Our calculated gaps from the singlet ground state
to the excited triplet state agrees well with the experimentally measured
values. Study of the static structure factor shows that the ground state is
spontaneously dimerized for ferric wheels. Spin states of ferric wheels can be
viewed as quantized states of a rigid rotor with the gap between the ground and
the first excited state defining the inverse of moment of inertia. We have
studied the quantum dynamics of Fe as a representative of ferric wheels.
We use the low-lying states of Fe to solve exactly the time-dependent
Schr\"odinger equation and find the magnetization of the molecule in the
presence of an alternating magnetic field at zero temperature. We observe a
nontrivial oscillation of magnetization which is dependent on the amplitude of
the {\it ac} field. We have also studied the torque response of Fe as a

Development and validation of the brief esophageal dysphagia questionnaire
BackgroundEsophageal dysphagia is common in gastroenterology practice and has multiple etiologies. A complication for some patients with dysphagia is food impaction. A valid and reliable questionnaire to rapidly evaluate esophageal dysphagia and impaction symptoms can aid the gastroenterologist in gathering information to inform treatment approach and further evaluation, including endoscopy.Methods1638 patients participated over two study phases. 744 participants completed the Brief Esophageal Dysphagia Questionnaire (BEDQ) for phase 1; 869 completed the BEDQ, Visceral Sensitivity Index, Gastroesophageal Reflux Disease Questionnaire, and Hospital Anxiety and Depression Scale for phase 2. Demographic and clinical data were obtained via the electronic medical record. The BEDQ was evaluated for internal consistency, splitâhalf reliability, ceiling and floor effects, and construct validity.Key ResultsThe BEDQ demonstrated excellent internal consistency, reliability, and construct validity. The symptom frequency and severity scales scored above the standard acceptable cutoffs for reliability while the impaction subscale yielded poor internal consistency and splitâhalf reliability; thus the impaction items were deemed qualifiers only and removed from the total score. No significant ceiling or floor effects were found with the exception of 1 item, and interâitem correlations fell within accepted ranges. Construct validity was supported by moderate yet significant correlations with other measures. The predictive ability of the BEDQ was small but significant.Conclusions & InferencesThe BEDQ represents a rapid, reliable, and valid assessment tool for esophageal dysphagia with food impaction for clinical practice that differentiates between patients with major motor dysfunction and mechanical obstruction.Validated, rapid clinical assessment tools for esophageal dysphagia are lacking. The brief esophageal dysphagia questionnaire aims to gauge the severity and frequency of dysphagia with additional items to gauge food impaction. Targeting sexual health services in primary care: a systematic review of the psychosocial correlates of adverse sexual health outcomes reported in probability surveys of women of reproductive age
BACKGROUND: Women using primary care vary in need for sexually transmitted infections (STIs) testing and contraception. Psychosocial correlates of these needs may be useful for targeting services. We undertook a systematic review to identify psychosocial correlates of STI acquisition, unplanned pregnancy (UP), abortion and risky sexual behaviours in general population samples of women of reproductive age. METHODS: We searched bibliographic databases for probability surveys of women aged 16-44years in the European Union, USA, Canada, Australia, UK or New Zealand undertaken January 1994-January 2014. RESULTS: Eleven papers were included. Unplanned pregnancy was associated with smoking, depression, being single and sexual debut <16years. Abortion was associated with lack of closeness to parents, leaving home at an early age, and relationship break-up. Multiple partnerships were associated with intensity of marijuana and alcohol use, and smoking. STI diagnosis was associated with relationship break-up and younger partners. Non-use of contraception was associated with smoking, obesity, relationship status, sedentary lifestyles, fatalistic pregnancy attitudes and lower alcohol use. Condom non-use was higher (at first sex) with partners 5+years older and lower (at last sex) in less stable partnerships. CONCLUSION: Psychosocial variables, particularly relationship status and smoking, may help identify women in primary care for STI testing and contraception advice and supply
Law of Genome Evolution Direction : Coding Information Quantity Grows
The problem of the directionality of genome evolution is studied. Based on
the analysis of C-value paradox and the evolution of genome size we propose
that the function-coding information quantity of a genome always grows in the
course of evolution through sequence duplication, expansion of code, and gene
transfer from outside. The function-coding information quantity of a genome
consists of two parts, p-coding information quantity which encodes functional
protein and n-coding information quantity which encodes other functional
elements except amino acid sequence. The evidences on the evolutionary law
about the function-coding information quantity are listed. The needs of
function is the motive force for the expansion of coding information quantity
and the information quantity expansion is the way to make functional innovation
and extension for a species. So, the increase of coding information quantity of
a genome is a measure of the acquired new function and it determines the

This is a study focused on the early years of British rule in Malta (1800-1813). It explores the application to the island of the ânew modelâ of colonial government, one based on direct rule from London mediated by the continuation of existing laws and institutions. Systemic deficiencies are identified. These tended to undermine the effectiveness of direct British rule. This study also reveals, in the context of legal and constitutional continuity, unresolved tensions between modernity and tradition. The political stability of the island was damaged and the possibility of continued British possession was threatened
Proton NMR for Measuring Quantum-Level Crossing in the Magnetic Molecular Ring Fe10
The proton nuclear spin-lattice relaxation rate 1/T1 has been measured as a
function of temperature and magnetic field (up to 15 T) in the molecular
magnetic ring Fe10. Striking enhancement of 1/T1 is observed around magnetic
field values corresponding to a crossing between the ground state and the
excited states of the molecule. We propose that this is due to a
cross-relaxation effect between the nuclear Zeeman reservoir and the reservoir
of the Zeeman levels of the molecule. This effect provides a powerful tool to

Health professionalsâ responses to womenâs disclosure of domestic violence

The definitive version is available at http:dx.doi.org/10.1177/0886260514552449This study explored womenâs experiences of their responses from health professionals following disclosure of domestic violence within a health setting. The existence of health based policies guiding professionals in the provision of appropriate support following disclosure of domestic violence is only effective if health professionals understand the dynamics of violent relationships. This paper focuses on the findings from the interviews conducted with fifteen women living in the UK who disclosed their experiences of domestic violence when accessing healthcare. Following thematic analysis, themes emerged that rotated around their disclosure and the responses they received from health professionals. The first two themes revealed the repudiation of, or recognition of and failure to act upon, domestic violence. A description of how the health professionalâs behaviour became analogous with that of the perpetrator is discussed. The final theme illuminated womenâsâ receipt of appropriate and sensitive support, leading to a positive trajectory away from a violent relationship. The findings suggest that the implicit understanding of the dynamics of violent relationships and the behaviours of the perpetrator of domestic violence are essential components of health care provision to avoid inadvertent inappropriate interactions with women
