    Pathways to "opportunity and excellence": collaborative curriculum innovation in South Yorkshire

    This paper reports on two aspects of a large-scale curriculum project currently taking place in four LEAs in South Yorkshire. The first of these is concerned with the positive and negative influences on effective curriculum innovation and is addressed from the perspective of the LEA project managers who are managing the delivery of the project in the region's schools. The second aspect considers what the pupils (Year 10, age 14-15) think about the new learning opportunities. The project is set in the context of regional regeneration. The paper concludes that the extremely positive responses from the sample of pupils in all three strands of the programme indicate that the greater emphasis on vocational work and work experience in schools is having a strong motivational effect on pupils who are responding with improved attendance, behaviour and achievement.</p

    Women’s experiences of over-the-counter and prescription medication during pregnancy in the UK: findings from survey free-text responses and narrative interviews

    OBJECTIVES: To explore women's experiences of over-the-counter and prescription medication advice and use during pregnancy. DESIGN: A study design consisting of an online survey and nested in-depth interviews with a subsample of participants. We analysed data from survey free-text responses and in-depth interviews using thematic analysis. Quantitative survey data is published elsewhere.The UK. PARTICIPANTS: Women were eligible if living in the UK, aged 16-45 years, were pregnant or had been pregnant in the last 5 years regardless of pregnancy outcome. A total of 7090 women completed the survey, and 34 women who collectively had experienced 68 pregnancies were subsequently interviewed. RESULTS: Medication prescribing and use during pregnancy was common. The prescribing, dispensing and taking of some advised medications were restricted through women's or prescribers' fear of fetal harm. Lack of adherence to national prescribing guidance, conflicting professional opinion and poor communication resulted in maternal anxiety, avoidable morbidity and women negotiating complex and distressing pathways to obtain recommended medications. In contrast, some women felt overmedicated and that pharmacological treatments were used without exploring other options first. CONCLUSION: Increased translation of national guidance into practice and greater personalisation of antenatal care are needed to improve the safety, efficacy and personalisation of prescribing in pregnancy

    Constraint-based, Single-point Approximate Kinetic Energy Functionals

    We present a substantial extension of our constraint-based approach for development of orbital-free (OF) kinetic-energy (KE) density functionals intended for the calculation of quantum-mechanical forces in multi-scale molecular dynamics simulations. Suitability for realistic system simulations requires that the OF-KE functional yield accurate forces on the nuclei yet be relatively simple. We therefore require that the functionals be based on DFT constraints, local, dependent upon a small number of parameters fitted to a training set of limited size, and applicable beyond the scope of the training set. Our previous "modified conjoint" generalized-gradient-type functionals were constrained to producing a positive-definite Pauli potential. Though distinctly better than several published GGA-type functionals in that they gave semi-quantitative agreement with Born-Oppenheimer forces from full Kohn-Sham results, those modified conjoint functionals suffer from unphysical singularities at the nuclei. Here we show how to remove such singularities by introducing higher-order density derivatives. We give a simple illustration of such a functional used for the dissociation energy as a function of bond length for selected molecules.Comment: 16 pages, 9 figures, 2 tables, submitted to Phys.     Electron acceleration by a transient intense-laser-plasma electrode

    Rapid strides in the technology of laser plasma-based acceleration of charged particles leading to high brightness, tunable, monochromatic energetic beams of electrons and ions has been driven by potential multidisciplinary applications in cancer therapy, isotope preparation, radiography and thermonuclear fusion. Hitherto laser plasma acceleration schemes were confined to large-scale facilities generating a few tens of terawatt to petawatt laser pulses at repetition rates of 10 Hz or less. However, the need to make viable systems using high-repetition-rate femtosecond lasers has impelled recent research into novel targetry [1,2]. Of contemporary importance is the generation of supra thermal electrons, beyond those predicted by the scaling relation, reflected in both theoretical and computational work [3,4]. In this work we present evidence of generation of relativistic electrons (temperature >200 keV, maximum energies >1 MeV) at intensities that are two orders of magnitude lower than the relativistic intensity threshold. The novel targets [6] are 15 micron sized crystals suspended as aerosols in a gas interacting with a kHz, few-mJ femtosecond laser focussed to intensities of 10 PW/cm2. A pre-pulse with 1-5% of the intensity of the main pulse, arriving 4 ns early, is critical for hot electron generation. In addition to this unprecedented energy enhancement, we also characterize the dependence of X-ray spectra on the background gas of the aerosol. Intriguingly, easier the gas is to ionise, greater is the number of hot electrons observed, while the electron temperature remains the same. 2-D Radiation hydrodynamics and Particle-in-cell (PIC) simulations explain both the experimentally observed electron emission and the role of the low-density plasma in yield enhancement. We observe a two-temperature electron spectrum with about 50 and 240 keV temperatures consistent with the measurements made in the experiments. The simulations show that the following features contribute to the high-energy electron emission. The pre-pulse generates a hemispherical plasma-profile that enhances the coupling of the laser light. Overdense plasma is generated about the hemispherical cavity on the particle due to the main pulse interaction. The gradient in the plasma density in and around the cavity serves as a reservoir of low energy electrons to be injected into the particle potential and enables the hot electron generation observed in the experiments. Higher energy electron emission is dominantly from the edges of the hemispherical cavitation. The increase in total X-ray yield observed in the experiments scales with the number of electrons generated in the low density neighborhood surrounding the particle. In a simple-man picture, the laser interacts with the particle and ejects electrons from the particle. The particle acquires a strong positive potential that can only be brought down by ion expansion that occurs over 10's of picoseconds. The particle with strong positive potential acts as an 'accelerating electrode' for the electrons ionized in the low-density gas neighborhood. These results assume importance in the context of applications such as fast fuel ignition [6] or in medical applications of laser plasmas [7] where high irradiance of energetic electrons is of consequence. 1.     Unemployment and attitudes to work: asking the ‘right’ question

    Attitudes research has repeatedly demonstrated that the vast majority of unemployed people want a job, and that their work commitment is generally at least as strong as employed people’s. But until now it has not asked if they are more likely than employed people to prefer unemployment to an unattractive job. While this oversight reflects a noted widespread reluctance to respond directly to right-wing authors’ assertions, this article argues that it is partly attributable to existing studies using survey questions inappropriate for researching unemployment. Responses to the British Cohort Study / National Child Development Study agree / disagree statement ‘having almost any job is better than being unemployed’ were analysed. Being ‘unemployed and seeking work’ associated strongly with disagreeing with the statement across all recent datasets in both studies, even when a number of relevant variables were controlled for

    The interlayer cohesive energy of graphite from thermal desorption of polyaromatic hydrocarbons

    We have studied the interaction of polyaromatic hydrocarbons (PAHs) with the basal plane of graphite using thermal desorption spectroscopy. Desorption kinetics of benzene, naphthalene, coronene and ovalene at sub-monolayer coverages yield activation energies of 0.50 eV, 0.85 eV, 1.40 eV and 2.1 eV, respectively. Benzene and naphthalene follow simple first order desorption kinetics while coronene and ovalene exhibit fractional order kinetics owing to the stability of 2-D adsorbate islands up to the desorption temperature. Pre-exponential frequency factors are found to be in the range 101410^{14}-1021s−110^{21} s^{-1} as obtained from both Falconer--Madix (isothermal desorption) analysis and Antoine's fit to vapour pressure data. The resulting binding energy per carbon atom of the PAH is 52±52\pm5 meV and can be identified with the interlayer cohesive energy of graphite. The resulting cleavage energy of graphite is 61±561\pm5~meV/atom which is considerably larger than previously reported experimental values.Comment: 8 pages, 4 figures, 2 table

    Effects and cost of different strategies to eliminate hepatitis C virus transmission in Pakistan: a modelling analysis

    Background The WHO elimination strategy for hepatitis C virus advocates scaling up screening and treatment to reduce global hepatitis C incidence by 80% by 2030, but little is known about how this reduction could be achieved and the costs of doing so. We aimed to evaluate the effects and cost of different strategies to scale up screening and treatment of hepatitis C in Pakistan and determine what is required to meet WHO elimination targets for incidence. Methods We adapted a previous model of hepatitis C virus transmission, treatment, and disease progression for Pakistan, calibrating using available data to incorporate a detailed cascade of care for hepatitis C with cost data on diagnostics and hepatitis C treatment. We modelled the effect on various outcomes and costs of alternative scenarios for scaling up screening and hepatitis C treatment in 2018–30. We calibrated the model to country-level demographic data for 1960–2015 (including population growth) and to hepatitis C seroprevalence data from a national survey in 2007–08, surveys among people who inject drugs (PWID), and hepatitis C seroprevalence trends among blood donors. The cascade of care in our model begins with diagnosis of hepatitis C infection through antibody screening and RNA confirmation. Diagnosed individuals are then referred to care and started on treatment, which can result in a sustained virological response (effective cure). We report the median and 95% uncertainty interval (UI) from 1151 modelled runs. Findings One-time screening of 90% of the 2018 population by 2030, with 80% referral to treatment, was projected to lead to 13·8 million (95% UI 13·4–14·1) individuals being screened and 350 000 (315 000–385 000) treatments started annually, decreasing hepatitis C incidence by 26·5% (22·5–30·7) over 2018–30. Prioritised screening of high prevalence groups (PWID and adults aged ≥30 years) and rescreening (annually for PWID, otherwise every 10 years) are likely to increase the number screened and treated by 46·8% and decrease incidence by 50·8% (95% UI 46·1–55·0). Decreasing hepatitis C incidence by 80% is estimated to require a doubling of the primary screening rate, increasing referral to 90%, rescreening the general population every 5 years, and re-engaging those lost to follow-up every 5 years. This approach could cost US8⋅1billion,reducingto8·1 billion, reducing to 3·9 billion with lowest costs for diagnostic tests and drugs, including health-care savings, and implementing a simplified treatment algorithm. Interpretation Pakistan will need to invest about 9·0% of its yearly health expenditure to enable sufficient scale up in screening and treatment to achieve the WHO hepatitis C elimination target of an 80% reduction in incidence by 2030.
