    Social Comparison and Perceived Breach of Psychological Contract: Their Effects on Burnout in a Multigroup Analysis

    This study focuses on the mediator role of social comparison in the relationship between perceived breach of psychological contract and burnout. A previous model showing the hypothesized effects of perceived breach on burnout, both direct and mediated, is proposed. The final model reached an optimal fit to the data and was confirmed through multigroup analysis using a sample of Spanish teachers (N = 401) belonging to preprimary, primary, and secondary schools.     Perceived sexual harassment at work: meta-analysis and structural model of antecedents and consequences

    Although sexual harassment has been extensively studied, empirical research has not led to firm conclusions about its antecedents and consequences, both at the personal and organizational level. An extensive literature search yielded 42 empirical studies with 60 samples. The matrix correlation obtained through meta-analytic techniques was used to test a structural equation model. Results supported the hypotheses regarding organizational environmental factors as main predictors of harassment.Pese a que el acoso sexual ha sido extensamente estudiado, la investigación empírica no ha llegado a conclusiones firmes relativas a sus antecedentes y consecuencias personales y organizacionales.     Older Workers and Affective Job Satisfaction: Gender Invariance in Spain

    Older employees’ affective job satisfaction is an aspect that arouses growing interest among researchers. Among the affective measures of job satisfaction, the Brief Index of Affective Job Satisfaction (BIAJS) is one of the most used in the last decade. This study is intended to the test the gender invariance of the BIAJS in two samples of workers over age 40 in Spain. The first sample, of 300 participants and the second sample, of 399 participants, have been used to test gender invariance of the BIAJS. In comparison with the original English version, the Spanish version of the BIAJS has adequate psychometric properties. The findings allow us to consider it a valid and reliable tool to assess older people’s affective expressions about their work.     The use of health services by immigrant populations has been considered one of the most important indicators of the integration of these communities (Dias et al., 2009). However, knowledge about the quality and efficacy of this access to care, especially for immigrant women, is still scarce in Portugal (Fonseca et al., 2009). Although domestic studies have, in recent decades, sought to trace the different social profiles of immigrant women in Portugal, especially with regard to family or work relationships (Wall et al., 2005), research on health is still sparse. Gender-centered or intersectorial analysis has not yet been done. This paper presents a reflection on the determinants that influence immigrant women's (in)ability to access health care, emphasizing the factors that may be acting to block integration in this sector

    Emerging Issues in Occupational Disease: Mental Health in the Aging Working Population and Cognitive Impairment - A Narrative Review

    Cognitive impairment has often been reported in scientific literature as a concern derived from chronic exposure to work-related stress. Organizational factors can contribute to the onset of this concern especially in a susceptible population such as elderly workers. The aim of our study was to review the last five years of scientific literature, focusing on experimental and epidemiological studies, possible mechanisms implicated in the onset of cognitive decline due to work-related stress, and the recent organizational strategies to prevent detrimental effects of stress on cognitive processes. A literature search was performed in scientific platforms Medline and Web of Science, by means of specific string search terms, restricting the search to the years of publication 2014–2019. Thirty-three articles were identified and qualitatively evaluated, reporting narratively the main point of interest. At this stage, six articles were excluded because they did not meet the inclusion criteria. Only a few articles considered the population of the elderly workers, often with a short follow-up period. Strategies to manage stress with organizational procedures are scarce. Mechanisms implicated in the development of cognitive impairment due to stress are not fully explained and seem to include a chronical decrease in the inhibitory process of neurological pathways. Further research that focused on strategies to manage stress in elderly workers, with the aim of preventing cognitive impairment processes, is warranted

    Investment case for two-year post university speciality training in family medicine in Tajikistan: how much is needed for continuing and scaling up the improved education of family doctors?

    Background: A new two-year Post University Specialty Training (PUST) programme in family medicine was introduced to improve the quality of postgraduate speciality medical education in Tajikistan. Postgraduate education of family doctors (FDs) needs to be urgently scaled up, as 38% of FD positions in Tajikistan remained unfilled in 2018. Moreover, the international financial support for the PUST programme is ending. This investment case assesses the minimum funding needed for the continuation and scale-up of PUST and establishes the rationale for the investment in the light of a recent evaluation. Methods: The costs of the programme were calculated for 2018 and a scale-up forecast made for the period 2019-2023. The impact of the scale-up on the shortage of FDs was assessed. An evaluation using a Multiple Choice Questionnaire and Objective Structured Clinical Examination (OSCE) assessed and compared theoretical knowledge, clinical skills and competencies of PUST trained and conventionally trained FDs. Results: The annual costs of the programme were US228,000in2018.ThetotalinvestmentneededforscalingupPUSTfrom31newFDsin2018to100FDgraduateseachyearby2023wasUS 228,000 in 2018. The total investment needed for scaling up PUST from 31 new FDs in 2018 to 100 FD graduates each year by 2023 was US 802,000.However, when the retirement of FDs and population growth are considered, the scale-up will result only in maintaining the current level of FDs working and not solve the country's FD shortage. The PUST FDs demonstrated significantly better clinical skills than the conventionally trained interns, scoring 60 and 45% of OSCE points, respectively. Theoretical knowledge showed a similar trend; PUST FDs answered 44% and interns 38% of the questions correctly. Conclusions: The two-year PUST programme has clearly demonstrated it produces better skilled family doctors than the conventional one-year internship, albeit some enduring quality concerns do still prevail. The discontinuation of international support for PUST would be a major setback and risks potentially losing the benefits of the programme for family medicine and also other specialities. To guarantee the supply of adequately trained FDs and address the FD shortage, the PUST should be continued and scaled up. Therefore, it is essential that international support is extended and a gradual transition to sustainable national financing gets underway
