    This is the case of 63-year-old mother and her 35-year-old daughter who drank herbal juice (Rhizoma Dioscoreae Nipponicae and Ficus formosana Maxim) and then developed diarrhea, cold sweating, and myoclonus. On physical examination, the mother had a normal consciousness level, a normal respiratory rate, and bilateral miosis. After the administration of 2Β g of pralidoxime (PAM) and 0.5Β mg of atropine, the mother felt better. The daughter only had mild symptoms of sweating and vomiting before arrival at the emergency department (ED)

    A review of Lassa fever outbreaks in Nigeria From 1969 to 2017: Epidemiologic profile, determinants and public health response

    Introduction: Lassa fever outbreaks have occurred in Nigeria since the 1969 till date. This is in spite of the fact that the reservoir and modes of transmission have been known for all these years. This review aimed at describing the epidemiology and determinants of the Lassa fever outbreaks in Nigeria from 1969 to 2017 and the public health response to these outbreaks.Method: The guidelines for the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) were used to conduct the review between May 2017 and January, 2018. We searched PubMed, Science direct, WHO library databases and Google Scholar for articles published from 1970 till January 2018. Other relevant websites such as those of the World Health Organization, Nigeria Centers for Disease Control and Prevention were searched for Lassa fever outbreak reports.Results: Twenty-six articles and reports were included in the final review. These described twenty-one outbreaks involving 5442 suspect cases, 768 confirmed cases and 631 deaths from suspected or confirmed Lassa fever. Thirty-two states and the Federal Capital Territory have ever recorded outbreaks of Lassa fever. Lassa fever cases now occur in various states in Nigeria all year round with dry season peaks. Nosocomial transmission has remained a consistent determinant. Public health responses have changed over time starting from predominantly case management in initial outbreaks to a centrally coordinated response supporting states and institutions over the years.Conclusion; Lassa fever outbreaks have increased in frequency and geographic spread with case fatality ratio remaining unacceptably high. The same determinants have persisted with nosocomial transmission a consistent factor. Public health response has consistently improved with the last two years showing the most coordinated response. We recommend that the definition of Lassa fever in the Nigerian Integrated Disease Surveillance and Notification System (IDSR) be revised to reflect the current reality to ensure better Lassa fever control.Key words: Lassa fever, Outbreak, Viral haemorrhagic fevers, Nigeria

Available on open access from PeerJ via the DOI in this recordData Availability: The following information was supplied regarding data availability: The simulation codes, simulation animations, and results are available in the Supplemental Files.Background During counter movement jumps, adding weight in the eccentric phase and then suddenly releasing this weight during the concentric phase, known as accentuated eccentric loading (AEL), has been suggested to immediately improve jumping performance. The level of evidence for the positive effects of AEL remains weak, with conflicting evidence over the effectiveness in enhancing performance. Therefore, we proposed to theoretically explore the influence of implementing AEL during constrained vertical jumping using computer modelling and simulation and examined whether the proposed mechanism of enhanced power, increased elastic energy storage and return, could enhance work and power. Methods We used a simplified model, consisting of a ball-shaped body (head, arm, and trunk), two lower limb segments (thigh and shank), and four muscles, to simulate the mechanisms of AEL. We adjusted the key activation parameters of the muscles to influence the performance outcome of the model. Numerical optimization was applied to search the optimal solution for the model. We implemented AEL and non-AEL conditions in the model to compare the simulated data between conditions. Results Our model predicted that the optimal jumping performance was achieved when the model utilized the whole joint range. However, there was no difference in jumping performance in AEL and non-AEL conditions because the model began its push-off at the similar state (posture, fiber length, fiber velocity, fiber force, tendon length, and the same activation level). Therefore, the optimal solution predicted by the model was primarily driven by intrinsic muscle dynamics (force-length-velocity relationship), and this coupled with the similar model state at the start of the push-off, resulting in similar push-off performance across all conditions. There was also no evidence of additional tendon-loading effect in AEL conditions compared to non-AEL condition. Discussion Our simplified simulations did not show improved jump performance with AEL, contrasting with experimental studies. The reduced model demonstrates that increased energy storage from the additional mass alone is not sufficient to induce increased performance and that other factors like differences in activation strategies or movement paths are more likely to contribute to enhanced performance.UQ Graduate School ScholarshipAustralian Research Council (ARC

    Primary cardiac osteosarcoma in a 42-year-old woman

    We describe here a 42-year-old woman who was admitted to hospital with a pedunculated mass in her left atrium. She was diagnosed with a primary cardiac osteosarcoma with special immunohistochemical characteristics. Echocardiography and computed tomography can be used to differentiate cardiac osteosarcomas from routine intracardiac tumors. The patient was treated by surgical removal of the mass. Two years later, she has shown no evidence of disease recurrence. We discuss primary osteosarcomas in the cardiac cavity and their management

    The Effect of Transposable Element Insertions on Gene Expression Evolution in Rodents

    Background:Many genomes contain a substantial number of transposable elements (TEs), a few of which are known to be involved in regulating gene expression. However, recent observations suggest that TEs may have played a very important role in the evolution of gene expression because many conserved non-genic sequences, some of which are know to be involved in gene regulation, resemble TEs. Results:Here we investigate whether new TE insertions affect gene expression profiles by testing whether gene expression divergence between mouse and rat is correlated to the numbers of new transposable elements inserted near genes. We show that expression divergence is significantly correlated to the number of new LTR and SINE elements, but not to the numbers of LINEs. We also show that expression divergence is not significantly correlated to the numbers of ancestral TEs in most cases, which suggests that the correlations between expression divergence and the numbers of new TEs are causal in nature. We quantify the effect and estimate that TE insertion has accounted for ~20% (95% confidence interval: 12% to 26%) of all expression profile divergence in rodents. Conclusions:We conclude that TE insertions may have had a major impact on the evolution of gene expression levels in rodents

    The effect of ultrasound pretreatment on some selected physicochemical properties of black cumin (Nigella Sativa)

    Background In the present study, the effects of ultrasound pretreatment parameters including irradiation time and power on the quantity of the extracted phenolic compounds quantity as well as on some selected physicochemical properties of the extracted oils including oil extraction efficiency, acidity and peroxide values, color, and refractive index of the extracted oil of black cumin seeds with the use of cold press have been studied. Methods For each parameter, three different levels (30, 60, and 90 W) for the ultrasound power and (30, 45, and 60 min) and for the ultrasound irradiation time were studied. Each experiment was performed in three replications. Results The achieved results revealed that, with enhancements in the applied ultrasound power, the oil extraction efficiency, acidity value, total phenolic content, peroxide value, and color parameters increased significantly (P 0.05). Conclusions In summary, it could be mentioned that the application of ultrasound pretreatment in the oil extraction might improve the oil extraction efficiency, the extracted oil’s quality, and the extracted phenolic compounds content.info:eu-repo/semantics/publishedVersio

    Interaction of Chandipura Virus N and P Proteins: Identification of Two Mutually Exclusive Domains of N Involved in Interaction with P

    The nucleocapsid protein (N) and the phosphoprotein (P) of nonsegmented negative-strand (NNS) RNA viruses interact with each other to accomplish two crucial events necessary for the viral replication cycle. First, the P protein binds to the aggregation prone nascent N molecules maintaining them in a soluble monomeric (N0) form (N0-P complex). It is this form that is competent for specific encapsidation of the viral genome. Second, the P protein binds to oligomeric N in the nucleoprotein complex (N-RNA-P complex), and thereby facilitates the recruitment of the viral polymerase (L) onto its template. All previous attempts to study these complexes relied on co-expression of the two proteins in diverse systems. In this study, we have characterised these different modes of N-P interaction in detail and for the first time have been able to reconstitute these complexes individually in vitro in the chandipura virus (CHPV), a human pathogenic NNS RNA virus. Using a battery of truncated mutants of the N protein, we have been able to identify two mutually exclusive domains of N involved in differential interaction with the P protein. An unique N-terminal binding site, comprising of amino acids (aa) 1–180 form the N0-P interacting region, whereas, C-terminal residues spanning aa 320–390 is instrumental in N-RNA-P interactions. Significantly, the ex-vivo data also supports these observations. Based on these results, we suggest that the P protein acts as N-specific chaperone and thereby partially masking the N-N self-association region, which leads to the specific recognition of viral genome RNA by N0

    Normal levels of p27Xic1 are necessary for somite segmentation and determining pronephric organ size

    The Xenopus laevis cyclin dependent kinase inhibitor p27Xic1 has been shown to be involved in exit from the cell cycle and differentiation of cells into a quiescent state in the nervous system, muscle tissue, heart and retina. We show that p27Xic1 is expressed in the developing kidney in the nephrostomal regions. Using over-expression and morpholino oligonucleotide (MO) knock-down approaches we show normal levels of p27Xic1 regulate pronephros organ size by regulating cell cycle exit. Knock-down of p27Xic1 expression using a MO prevented myogenesis, as previously reported; an effect that subsequently inhibits pronephrogenesis. Furthermore, we show that normal levels of p27Xic1 are required for somite segmentation also through its cell cycle control function. Finally, we provide evidence to suggest correct paraxial mesoderm segmentation is not necessary for pronephric induction in the intermediate mesoderm. These results indicate novel developmental roles for p27Xic1, and reveal its differentiation function is not universally utilised in all developing tissues

    Ultrafast laser-scanning optical resolution photoacoustic microscopy at up to 2 million A-lines per second

    The imaging speed of optical resolution photoacoustic microscopy (OR-PAM) using pulsed excitation is fundamentally limited by the range ambiguity condition, which defines the maximum laser pulse repetition frequency (PRF). To operate at this theoretical upper limit and maximize acquisition speed, a custom-built fiber laser capable of operating at a PRF of up to 2 MHz was combined with a fast laser scanning optical OR-PAM system based on a stationary fiber-optic ultrasound sensor. A large area (10 mm Γ— 10 mm) of the mouse ear was imaged within 8 s, when acquiring 16 million A-lines and operating the laser at a PRF of 2 MHz. This corresponds to a factor of four improvement in imaging speed compared to the fastest OR-PAM system previously reported. The ability to operate at high-imaging frame rates also allows the capture of hemodynamic events such as blood flow. It is considered that this system offers opportunities for high throughput imaging and visualizing dynamic physiological events using OR-PAM
