    Current Accuracy of Augmented Reality Neuronavigation Systems: Systematic Review and Meta-Analysis

    BACKGROUND Augmented reality neuronavigation (ARN) systems can overlay three-dimensional anatomy and pathology without the need for a two-dimensional external monitor. Accuracy is crucial for their clinical applicability. We performed a systematic review regarding the reported accuracy of ARN systems and compared them with the accuracy of conventional infrared neuronavigation (CIN). OBJECTIVE Explore the current navigation accuracy of ARN systems and compare them with CIN. METHODS Pubmed and Embase were searched for ARN and CIN systems. For ARN: type of system, method of patient-to-image registration, accuracy method and accuracy of the system was noted. For CIN: navigation accuracy, expressed as target registration error (TRE), was noted. A meta-analysis was performed comparing the TRE of ARN and CIN systems. RESULTS 35 studies were included, 12 for ARN and 23 for CIN. ARN systems were divided into head-mounted display and heads-up display. In ARN, four methods were encountered for patient-to-image registration, of which point-pair matching was the one most frequently used. Five methods for assessing accuracy were described. 94 TRE measurements of ARN systems were compared with 9058 TRE measurements of CIN systems. Mean TRE was 2.5 mm (CI 95% 0.7 - 4.4) for ARN systems and 2.6 mm (CI 95% 2.1 - 3.1) for CIN systems. CONCLUSIONS In ARN, there seems to be lack of agreement regarding the best method to assess accuracy. Nevertheless, ARN systems seem able to achieve an accuracy comparable with CIN systems. Future studies should be prospective and compare TREs which should be measured in a standardized fashion

    Comparing the influence of mixed reality, a 3D viewer, and MRI on the spatial understanding of brain tumours

    Background: Multiple 3D visualization techniques are available that obviates the need for the surgeon to mentally transform the 2D planes from MRI to the 3D anatomy of the patient. We assessed the spatial understanding of a brain tumour when visualized with MRI, 3D models on a monitor or 3D models in mixed reality.Methods: Medical students, neurosurgical residents and neurosurgeons were divided into three groups based on the imaging modality used for preparation: MRI, 3D viewer and mixed reality. After preparation, the participants needed to position, scale, and rotate a virtual tumour inside a virtual head of the patient in the same orientation as the original tumour would be. Primary outcome was the amount of overlap between the placed tumour and the original tumour to evaluate accuracy. Secondary outcomes were the position, volume and rotation deviation compared to the original tumour.Results: A total of 12 medical students, 12 neurosurgical residents, and 12 neurosurgeons were included. For medical students, the mean amount of overlap for the MRI, 3D viewer and mixed reality group was 0.26 (0.22), 0.38 (0.20) and 0.48 (0.20) respectively. For residents 0.45 (0.23), 0.45 (0.19) and 0.68 (0.11) and for neurosurgeons 0.39 (0.20), 0.50 (0.27) and 0.67 (0.14). The amount of overlap for mixed reality was significantly higher on all expertise levels compared to MRI and on resident and neurosurgeon level also compared to the 3D viewer. Furthermore, mixed reality showed the lowest deviations in position, volume and rotation on all expertise levels.Conclusion: Mixed reality enhances the spatial understanding of brain tumours compared to MRI and 3D models on a monitor. The preoperative use of mixed reality may therefore support the surgeon to improve spatial 3D related surgical tasks such as patient positioning and planning surgical trajectories

    Mixed Reality in Modern Surgical and Interventional Practice: Narrative Review of the Literature

    BACKGROUND Mixed reality (MR) and its potential applications have gained increasing interest within the medical community over the recent years. The ability to integrate virtual objects into a real-world environment within a single video-see-through display is a topic that sparks imagination. Given these characteristics, MR could facilitate preoperative and preinterventional planning, provide intraoperative and intrainterventional guidance, and aid in education and training, thereby improving the skills and merits of surgeons and residents alike. OBJECTIVE In this narrative review, we provide a broad overview of the different applications of MR within the entire spectrum of surgical and interventional practice and elucidate on potential future directions. METHODS A targeted literature search within the PubMed, Embase, and Cochrane databases was performed regarding the application of MR within surgical and interventional practice. Studies were included if they met the criteria for technological readiness level 5, and as such, had to be validated in a relevant environment. RESULTS A total of 57 studies were included and divided into studies regarding preoperative and interventional planning, intraoperative and interventional guidance, as well as training and education. CONCLUSIONS The overall experience with MR is positive. The main benefits of MR seem to be related to improved efficiency. Limitations primarily seem to be related to constraints associated with head-mounted display. Future directions should be aimed at improving head-mounted display technology as well as incorporation of MR within surgical microscopes, robots, and design of trials to prove superiority

    A combined numerical and experimental study of the 3D tumble structure and piston boundary layer development during the intake stroke of a gasoline engine

    Due to its positive effect on flame propagation in the case of a well-defined breakdown, the formation of a large-scale tumble motion is an important goal in engine development. Cycle-to-cycle variations (CCV) in the tumble position and strength however lead to a fluctuating tumble breakdown in space and time and therefore to combustion variations, indicated by CCV of the peak pressure. This work aims at a detailed investigation of the large-scale tumble motion and its interaction with the piston boundary layer during the intake stroke in a state-of-the-art gasoline engine. To allow the validation of the flow near the piston surface obtained by simulation, a new measurement technique called “Flying PIV” is applied. A detailed comparison between experimental and simulation results is carried out as well as an analysis of the obtained flow field. The large-scale tumble motion is investigated based on numerical data of multiple highly resolved intake strokes obtained using scale-resolving simulations. A method to detect the tumble center position within a 3D flow field, as an extension of previously developed 2D and 3D algorithms, is presented and applied. It is then used to investigate the phase-averaged tumble structure, its characteristics in terms of angular velocity and the CCV between the individual intake strokes. Finally, an analysis is presented of the piston boundary layer and how it is influenced by the tumble motion during the final phase of the intake stroke

    Analysis of the potential of cancer cell lines to release tissue factor-containing microvesicles: correlation with tissue factor and PAR2 expression

    BackgroundDespite the association of cancer-derived circulating tissue factor (TF)-containing microvesicles and hypercoagulable state, correlations with the incidence of thrombosis remain unclear.MethodsIn this study the upregulation of TF release upon activation of various cancer cell lines, and the correlation with TF and PAR2 expression and/or activity was examined. Microvesicle release was induced by PAR2 activation in seventeen cell lines and released microvesicle density, microvesicle-associated TF activity, and phoshpatidylserine-mediated activity were measured. The time-course for TF release was monitored over 90 min in each cell line. In addition, TF mRNA expression, cellular TF protein and cell-surface TF activities were quantified. Moreover, the relative expression of PAR2 mRNA and cellular protein were analysed. Any correlations between the above parameters were examined by determining the Pearson’s correlation coefficients.ResultsTF release as microvesicles peaked between 30–60 min post-activation in the majority of cell lines tested.     Genetic correlation between amyotrophic lateral sclerosis and schizophrenia

    We have previously shown higher-than-expected rates of schizophrenia in relatives of patients with amyotrophic lateral sclerosis (ALS), suggesting an aetiological relationship between the diseases. Here, we investigate the genetic relationship between ALS and schizophrenia using genome-wide association study data from over 100,000 unique individuals. Using linkage disequilibrium score regression, we estimate the genetic correlation between ALS and schizophrenia to be 14.3% (7.05-21.6; P=1 Ă— 10-4) with schizophrenia polygenic risk scores explaining up to 0.12% of the variance in ALS (P=8.4 Ă— 10-7). A modest increase in comorbidity of ALS and schizophrenia is expected given these findings (odds ratio 1.08-1.26) but this would require very large studies to observe epidemiologically. We identify five potential novel ALS-associated loci using conditional false discovery rate analysis. It is likely that shared neurobiological mechanisms between these two disorders will engender novel hypotheses in future preclinical and clinical studies

    NEK1 variants confer susceptibility to amyotrophic lateral sclerosis

    To identify genetic factors contributing to amyotrophic lateral sclerosis (ALS), we conducted whole-exome analyses of 1,022 index familial ALS (FALS) cases and 7,315 controls. In a new screening strategy, we performed gene-burden analyses trained with established ALS genes and identified a significant association between loss-of-function (LOF) NEK1 variants and FALS risk. Independently, autozygosity mapping for an isolated community in the Netherlands identified a NEK1 p.Arg261His variant as a candidate risk factor. Replication analyses of sporadic ALS (SALS) cases and independent control cohorts confirmed significant disease association for both p.Arg261His (10,589 samples analyzed) and NEK1 LOF variants (3,362 samples analyzed). In total, we observed NEK1 risk variants in nearly 3% of ALS cases. NEK1 has been linked to several cellular functions, including cilia formation, DNA-damage response, microtubule stability, neuronal morphology and axonal polarity. Our results provide new and important insights into ALS etiopathogenesis and genetic etiology

    The effect of an active on-ward participation of hospital pharmacists in Internal Medicine teams on preventable Adverse Drug Events in elderly inpatients: protocol of the WINGS study (Ward-oriented pharmacy in newly admitted geriatric seniors)

    <p>Abstract</p> <p>Background</p> <p>The potential of clinical interventions, aiming at reduction of preventable Adverse Drug Events (preventable ADEs) during hospital stay, have been studied extensively. Clinical Pharmacy is a well-established and effective service, usually consisting of full-time on-ward participation of clinical pharmacists in medical teams. Within the current Hospital Pharmacy organisation in the Netherlands, such on-ward service is less feasible and therefore not yet established. However, given the substantial incidence of preventable ADEs in Dutch hospitals found in recent studies, appears warranted. Therefore, "Ward-Oriented Pharmacy", an on-ward service tailored to the Dutch hospital setting, will be developed. This service will consist of multifaceted interventions implemented in the Internal Medicine wards by hospital pharmacists. The effect of this service on preventable ADEs in elderly inpatients will be measured. Elderly patients are at high risk for ADEs due to multi-morbidity, concomitant disabilities and polypharmacy. Most studies on the incidence and preventability of ADEs in elderly patients have been conducted in the outpatient setting or on admission to a hospital, and fewer in the inpatient setting. Moreover, recognition of ADEs by the treating physicians is challenging in elderly patients because their disease presentation is often atypical and complex. Detailed information about the performance of the treating physicians in ADE recognition is scarce.</p> <p>Methods/Design</p> <p>The design is a multi-centre, interrupted time series study. Patients of 65 years or older, consecutively admitted to Internal Medicine wards will be included. After a pre-measurement, a Ward-Oriented Pharmacy service will be introduced and the effect of this service will be assessed during a post-measurement. The primary outcome measures are the ADE prevalence on admission and ADE incidence during hospital stay. These outcomes will be assessed using structured retrospective chart review by an independent expert panel. This assessment will include determination of causality, severity and preventability of ADEs. In addition, the extent to which ADEs are recognised and managed by the treating physicians will be considered.</p> <p>Discussion</p> <p>The primary goal of the WINGS study is to assess whether a significant reduction in preventable ADEs in elderly inpatients can be achieved by a Ward-Oriented Pharmacy service offered.     Prognostic value of early, conventional proton magnetic resonance spectroscopy in cooled asphyxiated infants

    BACKGROUND: Neonatal hypoxic-ischemic encephalopathy (HIE) commonly leads to neurodevelopmental impairment, raising the need for prognostic tools which may guide future therapies in time. Prognostic value of proton MR spectroscopy (H-MRS) between 1 and 46 days of age has been extensively studied; however, the reproducibility and generalizability of these methods are controversial in a general clinical setting. Therefore, we investigated the prognostic performance of conventional H-MRS during first 96 postnatal hours in hypothermia-treated asphyxiated neonates. METHODS: Fifty-one consecutive hypothermia-treated HIE neonates were examined by H-MRS at three echo-times (TE = 35, 144, 288 ms) between 6 and 96 h of age, depending on clinical stability. Patients were divided into favorable (n = 35) and unfavorable (n = 16) outcome groups based on psychomotor and mental developmental index (PDI and MDI, Bayley Scales of Infant Development II) scores (>/= 70 versus < 70 or death, respectively), assessed at 18-26 months of age. Associations between 36 routinely measured metabolite ratios and outcome were studied. Age-dependency of metabolite ratios in whole patient population was assessed. Prognostic performance of metabolite ratios was evaluated by Receiver Operating Characteristics (ROC) analysis. RESULTS: Three metabolite ratios showed significant difference between outcome groups after correction for multiple testing (p < 0.0014): myo-inositol (mIns)/N-acetyl-aspartate (NAA) height, mIns/creatine (Cr) height, both at TE = 35 ms, and NAA/Cr height at TE = 144 ms. Assessment of age-dependency showed that all 3 metabolite ratios (mIns/NAA, NAA/Cr and mIns/Cr) stayed constant during first 96 postnatal hours, rendering them optimal for prediction. ROC analysis revealed that mIns/NAA gives better prediction for outcome than NAA/Cr and mIns/Cr with cut-off values 0.6798 0.6274 and 0.7798, respectively, (AUC 0.9084, 0.8396 and 0.8462, respectively, p < 0.00001); mIns/NAA had the highest specificity (95.24%) and sensitivity (84.62%) for predicting outcome of neonates with HIE any time during the first 96 postnatal hours. CONCLUSIONS: Our findings suggest that during first 96 h of age even conventional H-MRS could be a useful prognostic tool in predicting the outcome of asphyxiated neonates; mIns/NAA was found to be the best and age-independent predictor
