    10. Effect of insecticide-treated bed nets on the dynamics of multiple Plasmodium falciparum infections

    The rates of acquisition and loss of individual genotypes belonging to the FC27 family of the Plasmodium falciparum merozoite surface protein 2 (msp2) gene were studied in 120 children aged 5 months to 2·5 years, in a randomized controlled trial of insecticide-treated bed nets (ITNs) in Kiberege village, Tanzania. Analysis of longitudinal changes in positivity for individual alleles in samples collected at intervals of one month indicated that the average duration of infections, allowing for undetected parasite genotypes, was 73 d in those aged <18 months and 160 d in children aged ≥18 months, consistent with a shift from acute to chronic infection with age. Overall, 51% of genotypes infecting the host were estimated to be detected by polymerase chain reaction-restriction fragment length polymorphism analysis in any one sample of 0·5 μL of packed peripheral blood cells. In children less than 18 months old this sensitivity was 61% (SE = 6%) compared with 41% (SE = 6%) in older children. Conversely, the rate of appearance of new parasite genotypes was higher in children <18 months of age than in older children, but this partly reflected the difference in sensitivity. The overall incidence of new infections was estimated to be reduced by 17% in ITN users. There was no statistically significant difference between users and non-users in observed infection multiplicity, sensitivity, recovery rate, or estimated infection rates for individual alleles.     Evaluation of C-reactive protein and haptoglobin as malaria episode markers in an area of high transmission in Africa

    Field studies of malaria in endemic areas frequently use the presence or levels of parasitaemia, together with the measurement of fever, as the primary criteria with which to identify cases. However, since malaria cases do not always present with measurable fever, and since asymptomatic parasitaemia occurs, additional episode markers might be useful epidemiological tools. We have measured the C-reactive protein and haptoglobin levels in paediatric patients presenting to a village health post in the Kilombero District in Tanzania and in convalescent sera from the same patients, in order to evaluate these acute-phase reactants as alternative markers of Plasmodium falciparum episodes. Among afebrile patients, C-reactive protein levels were highly correlated with parasite density. High C-reactive protein levels are therefore probably indicative of recent clinical malaria episodes in currently afebrile individuals with high parasite densities. An appropriate case definition for malaria in epidemiological studies in endemic areas might therefore be hyperparasitaemia accompanied by either, or both, measurable fever and raised C-reactive protein levels. This would give less biased estimates of the overall burden of malaria morbidity than does a definition which requires measurable fever. Levels of haptoglobin were highly negatively correlated with parasitaemia, but did not appear to be useful episode markers because this correlation was probably not related to acute morbidity.     Exploring Counselor Educator Dispositions Related to Teaching

    This study explores students’ preferences for counselor educator (CE) teaching dispositions. Forty-eight counselor education students completed a Q sort and answered post-sort qualitative questions. The study found four types of student preferences: a focus on experiential teaching, a focus on content and affect orientation, a focus on the educator-student relationship, and a focus on developing clinical skills. Also among the findings are a set of items that were unanimously unimportant to the participants: CE engagement in research and gatekeeping. These preferences are situated within the scholarship of teaching and learning and evidence-based practices in counselor education. Practical and research implications are shared

    A Hardy inequality in twisted waveguides

    We show that twisting of an infinite straight three-dimensional tube with non-circular cross-section gives rise to a Hardy-type inequality for the associated Dirichlet Laplacian. As an application we prove certain stability of the spectrum of the Dirichlet Laplacian in locally and mildly bent tubes. Namely, it is known that any local bending, no matter how small, generates eigenvalues below the essential spectrum of the Laplacian in the tubes with arbitrary cross-sections rotated along a reference curve in an appropriate way. In the present paper we show that for any other rotation some critical strength of the bending is needed in order to induce a non-empty discrete spectrum.Comment: LaTeX, 20 page

    Destruction of Superconductivity by Impurities in the Attractive Hubbard Model

    We study the effect of U=0 impurities on the superconducting and thermodynamic properties of the attractive Hubbard model on a square lattice. Removal of the interaction on a critical fraction of fcrit0.30f_{\rm crit} \approx 0.30 of the sites results in the destruction of off-diagonal long range order in the ground state. This critical fraction is roughly independent of filling in the range 0.75<ρ<1.000.75 < \rho < 1.00, although our data suggest that fcritf_{\rm crit} might be somewhat larger below half-filling than at ρ=1\rho=1. We also find that the two peak structure in the specific heat is present at ff both below and above the value which destroys long range pairing order. It is expected that the high TT peak associated with local pair formation should be robust, but apparently local pairing fluctuations are sufficient to generate a low temperature peak

Researchers recommend clinicians receive regular clinical supervision; however, clinical supervision strategies and models related to working with ASPD are lacking. We identify supervisors’ primary task as exploring and improving clinicians’ attitudes toward clients having ASPD and examine this task within the discrimination model of clinical supervision. A case study is offered as an illustration for how to approach working with ASPD in supervision

    The influenza surveillance system in Victoria is comprised of several components, including a general practitioner sentinel surveillance system, surveillance for influenza-like illness (ILI) in consultations made by the Melbourne Medical Deputising Service, laboratory confirmed influenza notified to the Victorian Department of Health and strain typing performed by the World Health Organization Collaborating Centre for Reference and Research on Influenza. As measured by ILI from both the MMDS and GPSS, the 2011 influenza season in Victoria was mild compared to previous seasons and was not dominated by any type or subtype of influenza. There were 13 laboratory confirmed influenza outbreaks in 2011, nearly all of which were in aged care facilities. GPs continue to swab more patients, a trend started in 2009, with a significantly lower percent of these testing positive for influenza than previous years. The proportion of ILI and swabbed patients who were vaccinated was also significantly lower in 2011 than previously. Strain analysis undertaken by the WHO Collaborating Centre indicated a good antigenic match between the 2011 vaccine and circulating strains. The Victorian influenza surveillance system continues to provide a reliable, consistent system for monitoring the epidemiology of ILI and laboratory confirmed influenza in Victoria.VIDRL receives support for its influenza surveillance program from the Victorian Government Department of Health.     Enzymatic hydrolysate from velvet antler suppresses adipogenesis in 3T3-L1 cells and attenuates obesity in high-fat diet-fed mice

    The purpose of the current study was to investigate the potential anti-obesity activity of an enzymatic hydrolysate of velvet antler in inhibiting adipogenesis in 3T3-L1 cells and in high-fat diet (HFD)-fed obese mice. The enzy- matic hydrolysate was prepared using the commercial food grade protease, Protamex. The velvet antler Protamex hydrolysate (VAPH) indicated profound inhibitory effects on adipogenesis dose-dependently by decreasing the accumulation of triglycerides and down-regulating expression levels of adipogenesis-related proteins C/EBPα, SREBP-1, and PPARγ. In a mouse model of HFD-induced obesity, oral administration of VAPH (100 and 300 mg/kg for 13 weeks) significantly reduced the body weight gain that had resulted from the HFD. VAPH treat- ment also lowered the serum glucose and triglyceride levels, while increasing the HDL-C level. Furthermore, the treatment greatly reduced hepatic lipid droplet accumulation as well as the size of adipocytes. Current findings H has profound anti-obesity effects and could be an effective candidate for preventing obesity and obesity-related chronic diseases
