    Which diagnostic tests are most useful in a chest pain unit protocol?

    Background The chest pain unit (CPU) provides rapid diagnostic assessment for patients with acute, undifferentiated chest pain, using a combination of electrocardiographic (ECG) recording, biochemical markers and provocative cardiac testing. We aimed to identify which elements of a CPU protocol were most diagnostically and prognostically useful. Methods The Northern General Hospital CPU uses 2–6 hours of serial ECG / ST segment monitoring, CK-MB(mass) on arrival and at least two hours later, troponin T at least six hours after worst pain and exercise treadmill testing. Data were prospectively collected over an eighteen-month period from patients managed on the CPU. Patients discharged after CPU assessment were invited to attend a follow-up appointment 72 hours later for ECG and troponin T measurement. Hospital records of all patients were reviewed to identify adverse cardiac events over the subsequent six months. Diagnostic accuracy of each test was estimated by calculating sensitivity and specificity for: 1) acute coronary syndrome (ACS) with clinical myocardial infarction and 2) ACS with myocyte necrosis. Prognostic value was estimated by calculating the relative risk of an adverse cardiac event following a positive result. Results Of the 706 patients, 30 (4.2%) were diagnosed as ACS with myocardial infarction, 30 (4.2%) as ACS with myocyte necrosis, and 32 (4.5%) suffered an adverse cardiac event. Sensitivities for ACS with myocardial infarction and myocyte necrosis respectively were: serial ECG / ST segment monitoring 33% and 23%; CK-MB(mass) 96% and 63%; troponin T (using 0.03 ng/ml threshold) 96% and 90%. The only test that added useful prognostic information was exercise treadmill testing (relative risk 6 for cardiac death, non-fatal myocardial infarction or arrhythmia over six months). Conclusion Serial ECG / ST monitoring, as used in our protocol, adds little diagnostic or prognostic value in patients with a normal or non-diagnostic initial ECG. CK-MB(mass) can rule out ACS with clinical myocardial infarction but not myocyte necrosis(defined as a troponin elevation without myocardial infarction). Using a low threshold for positivity for troponin T improves sensitivity of this test for myocardial infarction and myocardial necrosis.     Acetylene terminated matrix resins

    The synthesis of resins with terminal acetylene groups has provided a promising technology to yield high performance structural materials. Because these resins cure through an addition reaction, no volatile by-products are produced during the processing. The cured products have high thermal stability and good properties retention after exposure to humidity. Resins with a wide variety of different chemical structures between the terminal acetylene groups are synthesized and their mechanical properties studied. The ability of the acetylene cured polymers to give good mechanical properties is demonstrated by the resins with quinoxaline structures. Processibility of these resins can be manipulated by varying the chain length between the acetylene groups or by blending in different amounts of reactive deluents. Processing conditions similar to the state-of-the-art epoxy can be attained by using backbone structures like ether-sulfone or bis-phenol-A. The wide range of mechanical properties and processing conditions attainable by this class of resins should allow them to be used in a wide variety of applications

    Light Colored Scalar as Messenger of Up-Quark Flavor Dynamics in Grand Unified Theories

    The measured forward-backward asymmetry in the t tbar production at the Tevatron might be explained by the additional exchange of a colored weak singlet scalar. Such state appears in some of the grand unified theories and its interactions with the up-quarks are purely antisymmetric in flavor space. We systematically investigate the resulting impact on charm and top quark physics. The constraints on the relevant Yukawa couplings come from the experimentally measured observables related to D0--D0bar oscillations, as well as di-jet and single top production measurements at the Tevatron. After fully constraining the relevant Yukawa couplings, we predict possible signatures of this model in rare top quark decays. In a class of grand unified models we demonstrate how the obtained information enables to constrain the Yukawa couplings of the up-quarks at very high energy scale.Comment: 13 pages, 11 figures, version as published in PR

    Magnetic reversals in a simple model of MHD

    We study a simple magnetohydrodynamical approach in which hydrodynamics and MHD turbulence are coupled in a shell model, with given dynamo constrains in the large scales. We consider the case of a low Prandtl number fluid for which the inertial range of the velocity field is much wider than that of the magnetic field. Random reversals of the magnetic field are observed and it shown that the magnetic field has a non trivial evolution linked to the nature of the hydrodynamics turbulence.Comment: 4 pages, submitted to PR

    Bose-Einstein Condensation Temperature of Homogenous Weakly Interacting Bose Gas in Variational Perturbation Theory Through Six Loops

    We compute the shift of the transition temperature for a homogenous weakly interacting Bose gas in leading order in the scattering length a for given particle density n. Using variational perturbation theory through six loops in a classical three-dimensional scalar field theory, we obtain Delta T_c/T_c = 1.25+/-0.13 a n^(1/3), in agreement with recent Monte-Carlo results.Comment: 4 pages; omega' corrected: final result changes slightly to 1.25+/-0.13; references added; several minor change

    Generalized Boltzmann equations for on-shell particle production in a hot plasma

    A novel refinement of the conventional treatment of Kadanoff--Baym equations is suggested. Besides the Boltzmann equation another differential equation is used for calculating the evolution of the non-equilibrium two-point function. Although it was usually interpreted as a constraint on the solution of the Boltzmann equation, we argue that its dynamics is relevant to the determination and resummation of the particle production cut contributions. The differential equation for this new contribution is illustrated in the example of the cubic scalar model. The analogue of the relaxation time approximation is suggested. It results in the shift of the threshold location and in smearing out of the non-analytic threshold behaviour of the spectral function. Possible consequences for the dilepton production are discussed.Comment: 22 pages, latex, 2 ps figure

    The Ratio of W + N jets To Z/gamma + N jets As a Precision Test of the Standard Model

    We suggest replacing measurements of the individual cross-sections for the production of W + N jets and Z/gamma + N jets in searches for new high-energy phenomena at hadron colliders by the precision measurement of the ratios (W+0 jet)/(Z+0 jet), (W+1 jet)/(Z+1 jet), (W+2 jets)/(Z+2 jets),... (W+N jets)/(Z+N jets), with N as large as 6 (the number of jets in ttbarH). These ratios can also be formed for the case where one or more of the jets is tagged as a b or c quark. Existing measurements of the individual cross sections for Wenu + N jets at the Tevatron have systematic uncertainties that grow rapidly with N, being dominated by uncertainties in the identification of jets and the jet energy scale. These systematics, and also those associated with the luminosity, parton distribution functions (PDF's), detector acceptance and efficiencies, and systematics of jet finding and b-tagging, are expected to substantially cancel in calculating the ratio of W to Z production in each N-jet channel, allowing a greater sensitivity to new contributions in these channels in Run II at the Tevatron and at the LHC.Comment: 10 pages, 8 figures, added reference
