    The residual gravity acceleration effect in the Poincare dodecahedral space

    In a flat space, the global topology of comoving space can induce a weak acceleration effect similar to dark energy. Does a similar effect occur in the case of the Poincare dodecahedral space S^3/I^*? Does the effect distinguish the Poincare space from other well-proportioned spaces? The residual acceleration effect in the Poincare space is studied here using a massive particle and a nearby test particle of negligible mass, in S^3 embedded in R^4. The weak limit gravitational attraction on a test particle at distance r is set \propto [r_C \sin(r/r_C)]^{-2}, where r_C = curvature radius, in order to satisfy Stokes' theorem. A finite particle horizon large enough to include the adjacent topological images of the massive particle is assumed. The regular, flat, 3-torus T^3 is re-examined, and two other well-proportioned spaces, S^3/T^* and S^3/O^*, are also studied. The residual gravity effect occurs in all four cases. In a perfectly regular 3-torus of side length L_a, and in S^3/T^* and S^3/O^*, the highest order term in the residual acceleration is the third order term in the Taylor expansion of r/L_a (3-torus), or r/r_C, respectively. However, the Poincare dodecahedral space is unique among the four spaces. The third order cancels, leaving the fifth order term \sim \pm 300 (r/r_C)^5 as the most significant. Not only are three of the four perfectly regular well-proportioned spaces better balanced than most other multiply connected spaces in terms of the residual acceleration effect by a factor of about a million (setting r/L_a = r/r_C \sim 10^{-3}), but the fourth of these spaces is about 10^4 times better balanced than the other three. This is the Poincare dodecahedral space.     Transmission of pillar-based photonic crystal waveguides in InP technology

    Waveguides based on line defects in pillar photonic crystals have been fabricated in InP/InGaAsP/InP technology. Transmission measurements of different line defects are reported. The results can be explained by comparison with two-dimensional band diagram simulations. The losses increase substantially at mode crossings and in the slow light regime. The agreement with the band diagrams implies a good control on the dimensions of the fabricated features, which is an important step in the actual application of these devices in photonic integrated circuit

    Effects of Bioleaching on the Chemical, Mineralogical and Morphological Properties of Natural and Waste-Derived Alkaline Materials

    Bioleaching is a potential route for the valorisation of low value natural and waste alkaline materials. It may serve as a pre-treatment stage to mineral carbonation and sorbent synthesis processes by increasing the surface area and altering the mineralogy of the solid material and by generating an alkaline rich (Ca and Mg) aqueous stream. It may also aid the extraction of high value metals from these materials (e.g. Ni), transforming them into valuable ore reserves. The bioleaching potential of several bacteria (Bacillus circulans, Bacillus licheniformis, Bacillus mucilaginosus, Sporosarcina ureae) and fungi (Aspergillus niger, Humicola grisea, Penicillium chrysogenum) towards the alteration of chemical, mineralogical and morphological properties of pure alkaline materials (wollastonite and olivine) and alkaline waste residues (AOD and BOF steel slags, and MSWI boiler fly ash) at natural pH (neutral to basic) was assessed. Bioleaching was conducted using one-step and two-step methodologies. Increased solubilisation of alkaline earth metals and nickel were verified. Alteration in basicity was accompanied by alteration of mineralogy. AOD slag experienced solubilisation-precipitation mechanism, as evidenced by the decline of primary phases (such as dicalcium-silicate, bredigite and periclase) and the augmentation of secondary phases (e.g. merwinite and calcite). Nickel-bearing minerals of olivine (clinochlore, lizardite, nimite and willemseite) significantly diminished in quantity after bioleaching. Altered mineralogy resulted in morphological changes of the solid materials and, in particular, in increased specific surface areas. The bioleaching effect can be attributed to the production of organic acids (principally gluconic acid) and exopolysaccharides (EPS) by the microorganisms. The similarities between fungal and bacterial mediated bioleaching suggest that biogenic substances contribute mostly to its effects, as opposed to bioaccumulation or other direct action of living cells

    Rationale: Holt-Oram syndrome (HOS) is an autosomal dominant heart-hand syndrome caused by mutations in the TBX5 gene. Overexpression of Tbx5 in the chick proepicardial organ (PEO) impaired coronary blood vessel formation. However, the potential activity of Tbx5 in the epicardium itself, and Tbx5\u27s role in mammalian coronary vasculogenesis, remains largely unknown. Objective: To evaluate the consequences of altered Tbx5 gene dosage during PEO and epicardial development in the embryonic chick and mouse. Methods and Results: Retroviral-mediated knockdown or upregulation of Tbx5 expression in the embryonic chick PEO as well as proepicardial-specific deletion of Tbx5 in the embryonic mouse (Tbx5(epi-/-)) impaired normal PEO cell development, inhibited epicardial and coronary blood vessel formation and altered developmental gene expression. The generation of epicardial-derived cells (EPDCs) and their migration into the myocardium was impaired between embryonic day (E) 13.5-15.5 in mutant hearts due to delayed epicardial attachment to the myocardium and subepicardial accumulation of EPDCs. This caused defective coronary vasculogenesis associated with impaired vascular smooth muscle cell recruitment, and reduced invasion of cardiac fibroblasts and endothelial cells into myocardium. In contrast to wildtype hearts that exhibited an elaborate ventricular vascular network, Tbx5(epi-/-) hearts displayed a marked decrease in vascular density that was associated with myocardial hypoxia as exemplified by HIF1α upregulation and increased binding of Hypoxyprobe-1. Tbx5(epi-/-) mice with such myocardial hypoxia exhibited reduced exercise capacity compared to wildtype mice. Conclusions: Our findings support a conserved Tbx5 dose-dependent requirement for both proepicardial and epicardial progenitor cell development in chick and mouse coronary vascular formation

    Telemedicine Improves Performance of a Two-Incision Lower Leg Fasciotomy by Combat Medics:A Randomized Controlled Trial

    Introduction:The primary aim of this randomized controlled trial was to assess if a head-mounted display (HMD) providing telemedicine support improves performance of a two-incision lower leg fasciotomy by a NATO special operations combat medic (combat medic).Materials and Methods:Thirty-six combat medics were randomized into two groups: One group performed a two-incision lower leg fasciotomy with the assistance of an HMD, while the control group completed the procedure without guidance. A Mann-Whitney U test was used to determine the possible differences in release of compartments and performance scores, as assessed by a supervising medical specialist. A Fisher's exact test was used to compare the proportions of collateral damage between groups. An independent-samples t-test was used to interpret total procedure times. The usability and technical factors involving HMD utilization were also assessed.Results:Combat medics in the HMD group released the anterior compartment (P <= .001) and deep posterior compartment (P = .008) significantly better. There was significantly more iatrogenic muscle (P <= .001) and venous damage (P <= .001) in the control group. The overall performance of combat medics in the HMD group was significantly better than that of the control group (P < .001). Combat medics in the control group were significantly faster (P = .012). The combat medics were very satisfied with the HMD. The HMD showed no major technical errors.Conclusions:This randomized controlled trial shows that a HMD providing telemedicine support leads to significantly better performance of a two-incision lower leg fasciotomy by a combat medic with less iatrogenic muscle and venous damage

    Re-Irradiation in Patients with Recurrent Rectal Cancer is Safe and Feasible

    BACKGROUND: There is no consensus yet for the best treatment regimen in patients with recurrent rectal cancer (RRC). This study aims to evaluate toxicity and oncological outcomes after re-irradiation in patients with RRC in our center. Clinical (cCR) and pathological complete response (pCR) rates and radicality were also studied. METHODS: Between January 2010 and December 2018, 61 locally advanced RRC patients were treated and analyzed retrospectively. Patients received radiotherapy at a dose of 30.0-30.6 Gy (reCRT) or 50.0-50.4 Gy chemoradiotherapy (CRT) in cases of no prior irradiation because of low-risk primary rectal cancer. In both groups, patients received capecitabine concomitantly. RESULTS: In total, 60 patients received the prescribed neoadjuvant (chemo)radiotherapy followed by surgery, 35 patients (58.3%) in the reRCT group and 25 patients (41.7%) in the long-course CRT group. There were no significant differences in overall survival (p = 0.82), disease-free survival (p = 0.63), and local recurrence-free survival (p = 0.17) between the groups. Patients in the long-course CRT group reported more skin toxicity after radiotherapy (p = 0.040). No differences were observed in late toxicity. In the long-course CRT group, a significantly higher cCR rate was observed (p = 0.029); however, there was no difference in the pCR rate (p = 0.66). CONCLUSIONS: The treatment of RRC patients with re-irradiation is comparable to treatment with long-course CRT regarding toxicity and oncological outcomes. In the reCRT group, less cCR was observed, although there was no difference in pCR. The findings in this study suggest that it is safe and feasible to re-irradiate RRC patients
