    Intragastric Migration of Gastric Band Diagnosed During Surgery: A Case Report and Literature

    Intragastric band migration (IGBM) is one of the major complications of gastric banding. In this report, we aimed to present a case of IGBM, which was diagnosed intraoperatively, and to review the relevant literature. A 59-year-old male patient was admitted to our outpatient clinic due to epigastric pain persisting for the past three months. The patient had a history of gastric banding surgery owing to obesity with open surgery nine years ago. Postoperative follow-up was not done properly and the patient had started to gain weight in the third postoperative year. Incisional hernia was found in physical examination and operation for gastric band removal and hernia repair was planned. During surgery, the band could not be found around the stomach, therefore, gastroscopy was performed and it was found that the majority of the band was placed in the stomach. The patient was intraoperatively diagnosed with IGBM and the band was removed through gastrotomy, and hernia repair was performed. The patient was discharged at postoperative 6th day without any complication. Although IGBM is rarely seen, it should be considered as a long-term complication in cases with dysfunctional gastric band and in patients who started to gain weight after operation.     Changes in morphology and physiology of Convolvulus oleifolius var. Deserti in different habitats

    Morphological features, photosynthetic pigment contents, proline and total protein amounts of Convolvulus oleifolius Desr. var. deserti Pamp./Surmeli yayilgan in different vegetation zones of Apostol Island are studied. Samples were collected from littoral-epilittoral and interior vegetation zones of Apostol Island, Bodrum-Turkbuku-MuglaTurkey. There are obvious differences in terms of stem length, organ size, hair density and inflorescence. Photosynthetic pigments in littoral-epilittoral vegetation zone are less than in interior vegetation zone. Prolin amount is significantly higher, but total protein amount is significantly lower in littoral-epilittoral vegetation zone than in interior vegetation zone. It may be thought that the morphological and physiological features of the C. oleifolius var. deserti exhibited considerable differences which appeared be the product of adaptive mechanisms to different habitat. Gövde uzunluğu, organ büyüklüğü, tüy yoğunluğu ve çiçeklenme durumunda belirgin farklılıklar vardır. Fotosentetik pigmentler littoral-epilittoral vejetasyon zonda, interior vejetasyon zondakinden daha azdır. Littoralepilittoral vejetasyon zonda prolin miktarı interior vejetasyon zondakinden önemli derecede daha yüksekken, toplam protein miktarı önemli derecede daha düşüktür. C. oleifolius var. deserti'nin morfolojik ve fizyolojik özelliklerinin çeşitli habitatlara uyum mekanizması sonucu ortaya çıkan önemli farklılıkları sergilediği düşünülebilir.     Background: Mastalgia is classified as cyclic and non-cyclic. Cyclical one constitutes approximately 60-70% of mastalgia. It is seen between 2 to 3 decades of age, and it usually responds to treatment. Objectives: To compare efficacy of vitex agnus castus (VAC) preparation with meloxicam and placebo in treatment of mastalgia. Methods: The study was designed as prospective and placebo-controlled clinical trial. Three groups were constituted. Before grouping, prolactin level was tested for every patient. The group1 enrolled the patients with cyclical mastalgia and high level prolactin. VAC was administered to patients from study group1. The second group enrolled the patients with cyclical mastalgia along with normal prolactin level, and to which meloxicam was given. Third group was placebo group, and placebo was applied. Before treatment, VAS (Visual analogue scale) scoring was applied to every patient. Prolactin level was tested for patients from group 1. After 3 months of study, VAS scoring was applied to every patient, but only prolactin levels were re- evaluated in patients from group1. Results: 95 patients were analyzed; enrolling women aged 19 to 54 years. Intensity of mastalgia diminished in VAC group 1, 2 more than in placebo group (p<0.0001). In addition to VAS scoring, prolactin level in group 1 after treatment was significantly reduced, comparing with before treatment (p<0.0001). In conclusion, we found that VAC preparation was effective in the treatment of mastalgia and hyperprolactinemia, with good tolerability. © 2011 Düzce Medical Journal

    The Value of Cervical Smears Collected with The ThinPrep Technique in The Detection of Endometrial Cancer

    OBJECTIVE: As liquid-based cervical cytology show promise for detecting endometrial adenocarcinoma, the aim of this study was to evaluate the cytological findings in liquid-based cytology of women in with endometrial cancer. Pap smear findings were correlated with histological grade and stage of the tumor to determine whether the presence of atypical cells have any prognostic value. STUDY DESIGN: This prospective study comprises 51 women with endometrial cancer from which a pre-operative cervical Thinprep Pap smear was available. In all cases endometrial thickness was measured transvaginally. The Bethesda nomenclature system for cervico-vaginal cytology was used to define atypical glandular cells (AGCs), atypical squamous cells (ASCs) and endometrial adenocarcinoma cells (EACs). Statistical analysis was done with the independent T test and χ2 test. RESULTS: Cytological atypia was present in a total 26 (51%) cases. AGCs or EACs were present in Pap smears of 19 (37%) women. The rate of FIGO grade 2-3 tumor in patients with and without AGCs/EACs was 36.8% (7/19) and 12.5% (4/32), respectively (p=0.09). Figures for endometrial thickness were 14.8 ± 5.6mm and 16.8 ± 6.6mm, respectively (p=0.3). Thirty-two percent (6/19) of patients with AGCs/EACs were FIGO stage 1C or more, whereas the rate for patients with normal cytology was 20% (5/25) (p=0.59). CONCLUSION: The sensitivity of the ThinPrep Pap smear in detecting endometrial cancer is low. AGCs/EACs on Pap smear do not correlate with tumor grade or stage and there is no association between endometrial thickness and incidence of AGCs/EACs on Pap smear

    A giant retroperitoneal hemangiopericytoma: Case report

    Hemangioperisitoma, kapiller perisitlerden köken alan, genellikle agresif seyreden, nüks-metastaz yapmaya meyilli ve nadir görülen bir yumuşak doku tümörüdür. Tümörün 5cm’den büyük olması, artmış mitotik aktivite, yüksek sellülarite, immatür veya pleomorfik tümör hücreleri, kanama ve nekroz odaklarının gözlenmesi malignitenin göstergeleridir. Temel tedavisi radikal cerrahidir. Bu olguda dev retroperitoneal hemanjioperisitoma nedeniyle opere edilen 48 yaşındaki erkek hastayı sunmayı amaçladık.Hemangiopericytoma is a rare soft tissue sarcom that originates capillary pericyte and usually shows agressive prognosis and tendency to metastasis or recurrence. It is reported that a malignant course is asssociated with a large tumor (greater than 5 cm), an increased mitotic rate, high cellularity, immature and pleomorphic tumor cells and foci of hemorrhage and necrosis.
