    Tissue expression of S100 proteins in gallbladder mucosa of the patients with calculous cholecystitis

     Proteins of S100 group, produced by phagocytes represent endogenous activators of innate immune responses. Role of these proteins in the etiopathogenesis of cholelithiasis remains unknown. The studies aimed at the morphometric evaluation of S100A8 and S100A9 protein expression in gallbladder mucosa in patients with acute and chronic calculous cholecystitis (n = 71). The presence of proteins was detected by immunohistochemistry while quantitative analysis employed the spatial visualization technique. We found the immunopositive expression of the two studied S100 proteins in neutrophils and monocytes/macrophages of the gallbladder’s wall and a higher expression in acute cholecystitis. Quantitative study revealed higher immunoexpression of S100A9 over S100A8 in both studied groups of patients. Moreover, a reciprocal linear relationship between the expression of the studied proteins and a positive correlation between expression of either S100A8 or S100A9 and inflammatory activity (grading) in the gallbladder wall were found. The expression of S100A8 protein in the chronic cholecystitis group and in older patients correlated with leukocytosis, which suggests the role of S100A8 particularly at the chronic stage of cholecystitis. The obtained results indicated close relationship between S100A8 and S100A9 proteins in their proinflammatory functions. The increased expression of only one of them can be recognized as a useful index of local inflammatory activity in calculous cholecystitis.

    Dzięki postępom w zakresie technologii kardiologii interwencyjnej leczenie przezcewnikowe stało się w ostatnich latach realną opcją terapeutyczną w leczeniu pacjentów z ostrą zatorowością płucną z wysokim ryzykiem wczesnej śmiertelności. Obecnie stosowane techniki przezcewnikowe umożliwiają miejscową fibrynolizę lub embolektomię przy minimalnym ryzyku powikłań. Stąd mogą być rozważane u pacjentów z grupy wysokiego ryzyka jako alternatywa dla chirurgicznej embolektomii płucnej, gdy systemowa tromboliza jest przeciwwskazana lub nieskuteczna. Rozważane są również u pacjentów z grupy średniego–wysokiego ryzyka, u których pomimo leczenia przeciwzakrzepowego nie uzyskano poprawy lub nastąpiło pogorszenie stanu klinicznego. Celem artykułu było przedstawienie roli technik przezcewnikowych w leczeniu chorych z ostrą zatorowością płucną zgodnie z aktualną wiedzą i opinią ekspertów. Leczenie interwencyjne opisano w szerszym kontekście organizacji opieki nad pacjentem. Przedstawiono organizację i zadania zespołu szybkiego reagowania w zatorowości płucnej, rolę obrazowania przedzabiegowego, zasady antykoagulacji okołozabiegowej, kryteria kwalifikacji pacjentów, wyboru odpowiedniego czasu interwencji oraz rolę intensywnej terapii. Szczegółowo omówiono dostępne terapie przezcewnikowe, w tym wystandaryzowane protokoły, definicje sukcesu i niepowodzenia procedury. Niniejszy dokument został opracowany we współpracy z ekspertami z różnych polskich towarzystw naukowych, co podkreśla rolę pracy zespołowej w opiece nad chorym z ostrą zatorowością płucną

    Role of catheter-directed therapies in the treatment of acute pulmonary embolism. Expert opinion of the Polish PERT Initiative, Working Group on Pulmonary Circulation, Association of Cardiovascular Interventions, and Association of Intensive Cardiac Care

    Thanks to advances in interventional cardiology technologies the catheter-directed treatment has become recently a viable therapeutic option in the treatment of patients with acute pulmonary embolism at high risk of early mortality. Current transcatheter techniques allow local fibrinolysis or embolectomy with minimal risk of complications. Therefore they can be considered in high risk patients as an alternative to surgical pulmonary embolectomy when systemic thrombolysis is contraindicated or ineffective. They are also considered in patients with intermediate — high risk with a lack of improvement or clinical deterioration despite anticoagulation. The purpose of this article is to present the role of transcatheter techniques in the treatment of patients with acute pulmonary embolism. We describe current knowledge and experts’ opinion in this field. Interventional treatment is described in the broader context of patient care organization and therapeutic modalities. We present the organization and responsibilities of pulmonary embolism response teams, the role of pre-procedural imaging, peri-procedural anticoagulation, patients selection, timing of intervention and intensive care support. Currently available catheter directed therapies are discussed in detail including standardized protocols, definitions of procedure success and failure. This expert opinion has been developed in collaboration with experts from various Polish scientific societies signifying the role of teamwork in the care for patients with acute pulmonary embolism

    C-Terminal Fragment of Vitellogenin II, a Potential Yolkin Polypeptide Complex Precursor Protein—Heterologous Expression, Purification, and Immunoregulatory Activity

    The aim of this research was to analyze the heterologous expression, purification, and immunoregulatory activity of recombinant YGP40 (rYGP40), the potential precursor of the yolkin peptide complex. The ygp40 coding sequence was codon optimized, successfully expressed in the E. coli system, and purified from inclusion bodies with a yield of about 1.1 mg/L of culture. This study showed that the protein exhibits immunomodulatory activity, expressed by the stimulation of TNF-α and IL-10 production and nitric oxide induction at a level comparable to that of the natural yolkin peptide complex obtained by other authors from hen egg yolk. At the highest dose of 100 µg/mL, rYGP40 also caused the up-regulation of iNOS expression in murine bone marrow-derived macrophages (BMDM). Moreover, no cytotoxic effects of rYGP40 on the BMDM cell line were observed

    Heterogeneity of Endocrinologic and Metabolic Parameters in Reproductive Age Polycystic Ovary Syndrome (PCOS) Women Concerning the Severity of Hyperandrogenemia—A New Insight on Syndrome Pathogenesis

    Background: Polycystic ovary syndrome (PCOS) is characterized by hyperandrogenism, anovulation, infertility, obesity, and insulin resistance, which results in increased concentrations of testosterone (T), which disturbs follicular growth and ovulation. This study aimed to assess PCOS women’s clinical, endocrinological, and metabolic parameters concerning hyperandrogenism severity. Results: 314 women (mean age 27.3 ± 4.6; mean body mass index (BMI) 25.7 ± 5.6) with PCOS, were divided into terciles according to T concentrations: <0.64 ng/mL (group 1), 0.64 to 0.84 ng/mL (Group 2) and >0.84 ng/mL (group 3). The mean concentration of T in all women was 0.59 ng/mL and correlated negatively with the number of menstrual cycles per year (MPY) (r = −0.36; p < 0.0001) and positively with Ferriman-Gallway score (FG) (r = 0.33; p < 0.0001), luteinizing hormone (LH) (r = 0.19; p < 0.0001) and dehydroepiandrosterone sulfate (DHEAS) (r = 0.52; p < 0.0001). Positive correlation between BMI and hirsutism (r = 0.16; p < 0.0001), total cholesterol (TC) (r = 0.18; p < 0.0001), low-density lipoprotein (LDL) (r = 0.29; p < 0.0001), and triglycerides (TG) (r = 0.40; p < 0.0001) was demonstrated. The division into subgroups confirmed the lowest MPY, highest LH, and hirsutism in group 3. BMI, insulin sensitivity indices, and lipid profile parameters were not different between the three T subgroups. Conclusions: We found no correlation between testosterone levels and insulin sensitivity or dyslipidemia in women with PCOS. Metabolic abnormalities may contribute more significantly than hyperandrogenemia to PCOS development

    Patrilocality and hunter-gatherer-related ancestry of populations in East-Central Europe during the Middle Bronze Age

    The demographic history of East-Central Europe after the Neolithic period remains poorly explored, despite this region being on the confluence of various ecological zones and cultural entities. Here, the descendants of societies associated with steppe pastoralists form Early Bronze Age were followed by Middle Bronze Age populations displaying unique characteristics. Particularly, the predominance of collective burials, the scale of which, was previously seen only in the Neolithic. The extent to which this re-emergence of older traditions is a result of genetic shift or social changes in the MBA is a subject of debate. Here by analysing 91 newly generated genomes from Bronze Age individuals from present Poland and Ukraine, we discovered that Middle Bronze Age populations were formed by an additional admixture event involving a population with relatively high proportions of genetic component associated with European hunter-gatherers and that their social structure was based on, primarily patrilocal, multigenerational kin-groups.
