Wpływ uczestnictwa w treningu zdrowotnym na koncentrację oraz sprawność funkcjonalną starszych kobiet
Celem pracy była ocena wpływu uczestnictwa w treningu zdrowotnym na
zdolność koncentracji oraz sprawność funkcjonalną starszych kobiet. Do badania zakwalifikowano 56 mieszkanek Krakowa w wieku od 60 do 83 lat życia. Grupę badaną stanowiło
26 uczestniczek zorganizowanych zajęć gimnastyki zdrowotnej. Do grupy kontrolnej
włączono 30 seniorek, które deklarowały brak uczestnictwa w ćwiczeniach fi zycznych.
W ramach eksperymentu przeprowadzono: Test badania uwagi d2, Zmodyfi kowany test
wstań i idź na 10 metrów, 30-sekundowy test wstawania z krzesła, ocenę gibkości górnej
części ciała, oraz kwestionariusz samooceny wieku biologicznego w odniesieniu do wyglądu
zewnętrznego i stanu psychofi zycznego. Badania prowadzono od listopada 2009
do stycznia 2010 r. Osoby z wysokim poziomem koncentracji według kryterium Testu
d2 stanowiły 39% kobiet z grupy badanej i 8% z grupy kontrolnej (p<0,001). Średni czas
wykonania Testu wstań i idź na 10 metrów w grupie badanej był krótszy o 1,53 sekundy
w porównaniu do grupy kontrolnej. W grupie badanej średnia liczba powtórzeń czynności
wstawania i siadania na krześle w ciągu 30 sekund wyniosła 19,07 ± 3,98, a w grupie
kontrolnej 15,64 ± 2,89. Średni wynik oceny gibkości górnej części ciała w grupie badanej
wyniósł 1,98 cm, a w grupie kontrolnej 7,5 cm. Osoby z grupy badanej oceniły swój wiek biologiczny jako niższy średnio o 3 lata od wieku kalendarzowego w porównaniu do
osób z grupy kontrolnej. Wyniki badań wskazują, że systematyczne uczestnictwo w zajęciach
treningu zdrowotnego wpływa korzystnie na utrzymanie dobrych parametrów
zdolności koncentracji starszych kobiet.The purpose of this study was to evaluate the effects of physical exercises on
concentration and functional efficiency in elderly women. The subjects consisted of 56
women, age ranged from 60-83 years. Participants were divided into study group, which
consisted of 26 women participating in gymnastics classes and control group, which consisted
of 30 not physically active women. The tests which were carried out: the test of
attention d2, the modifi ed get up and go test for 10 meters, 30-second chair test and evaluation
of the upper body agility. The study was conducted in Krakow in the period from
November 2009 to January 2010.Women with high levels of concentration represented
39% of test group and 8% of control group (p<0,001). The time of modifi ed get up and go
test for 10 meters is on average 1.53 seconds shorter in the study group than in the control
group. The average number of cycles of standing up-sitting was 19.07 ± 3,98 in the study
group and 15,64 ± 2,89 in control group. The average result of the assessment upper body
agility in the study group was 1.98 cm and 7.5 cm in the control group. Participation in
physical training signifi cantly affects the maintenance of good parameters of concentration

No association between MRI changes in the lumbar spine and intensity of pain, quality of life, depressive and anxiety symptoms in patients with low back pain
Introduction. The association between changes in magnetic resonance imaging (MRI) and clinical symptoms in patients with low back pain (LBP) is unclear.
Aim. To evaluate correlations between combined MRI findings of the lumbar spine (LS) and pain intensity, depressive and anxiety symptoms and quality of life in patients with LBP.
Material and methods. 200 subjects (93 men and 107 women; mean age 51.42 ± 13.21 years) with LBP referred for MRI were enrolled in the study. All patients completed the Hospital Anxiety and Depression Scale (HADS), Quality of Life Scales (EQ-5D, EQ-VAS) and the Visual Analogue Scale (VAS). MRI scans were assessed according to a scoring system prepared by the authors, and the total MRI score was calculated.
Results. The mean total MRI score was 11.59 ± 6.73 points (range 0–50 points) and was higher in men than in women (p = 0.015). A correlation was observed between total MRI score and age (p < 0.001) and between total MRI score and BMI (p = 0.005). An association was found between total MRI score and EQ-5D (p = 0.012) and HADS-D results (p = 0.003). VAS and HADS-A results did not correlate with MRI score. When multivariate analysis was done, the total MRI score was only significantly related to age and BMI, and association between the total MRI score and EQ-5D or HADS-D results was not confirmed. Decreased quality of life was associated with increased intensity of pain and depressive and anxiety symptoms.
Conclusions. Combined MRI changes in LS do not correlate with pain intensity, depressive and anxiety syndromes or quality of life in patients with LBP
Rola wybranych składników odżywczych we wspomaganiu leczenia farmakologicznego choroby Alzheimera
Choroba Alzheimera (AD, Alzheimer’s disease) jest najczęstszą, postępującą chorobą neurodegeneracyjną, charakteryzującą się utratą pamięci i utratą zdolności do samodzielnego życia u osób w podeszłym wieku. W mechanizmie choroby podstawową rolę odgrywa niedobór acetylocholiny oraz upośledzenie przekaźnictwa cholinergicznego. Na podstawie badań przedklinicznych, w których wykazano, że niektóre składniki odżywcze, takie jak nukleotydurydyny, cholina i wielonienasycone kwasy tłuszczowe omega 3, działając synergistycznie, zwiększają stężenie fosfolipidów w błonach komórkowych synaps, prowadzą do tworzenia nowych synaps i poprawy ich funkcji, opracowano i poddano badaniom klinicznym wieloskładnikowy preparat Souvenaid® (Nutricia N.V., Zoetermer, Netherlands). W przeprowadzonych badaniach klinicznych, którym poddano 438 chorych z wczesną postacią AD, dowiedziono, że Souvenaid® jest bezpieczny, dobrze tolerowany przez pacjentów i powoduje statystycznie znamienną poprawę pamięci odroczonej i natychmiastowej. Konieczne są jednak dalsze badania kliniczne w celu potwierdzenia skuteczności tej koncepcji terapeutycznej
Żywienie kliniczne w uczelniach medycznych w Polsce. Dlaczego żywienie kliniczne nie jest nauczane na studiach medycznych?
Niedożywienie jest chorobą. Figuruje w Międzynarodowej Klasyfikacji Chorób i Przyczyn Zgonów (ICD 10, E40–E46) i jak każda choroba powinno być rozpoznawane i leczone zgodnie z obowiązującymi zasadami. Jest to tym ważniejsze, że w momencie przyjęcia do szpitala występuje ono u 35–55% chorych na choroby nienowotworowe i u 40–80% chorych na nowotwory. Związane z chorobą niedożywienie jest bezpośrednią przyczyną zwiększonej chorobowości, śmiertelności, przedłużonego pobytu w szpitalu i zwiększonych kosztów leczenia. Tymczasem mimo że w Polsce od 2012 roku ocena stanu odżywienia jest obowiązkowa, niedożywienie jest rzadko rozpoznawane i jeszcze rzadziej leczone. Główną przyczyną niedożywienia związanego z chorobą będącego najczęstszym jatrogennym powikłaniem hospitalizacji jest brak wiedzy lekarzy na temat znaczenia stanu odżywienia w profilaktyce i leczeniu chorób spowodowany nieuwzględnieniem leczenia żywieniowego w kształceniu przeddyplomowym lekarzy
Indirect calorimetry, indications, interpretation, and clinical application
The aim of this review is to inform clinicians about the new possibilities in clinical nutrition (enteral or parenteral) based on indirect calorimetry. This method allows for the provision of an appropriate number of calories to undernourished hospitalized patients for nutritional repletion or maintenance. Providing too few or too many calories may result in adverse clinical consequences and worsen the course of the disease, especially in critically ill patients. Thus, optimizing nutrition support according to the individual and specific needs of patients is an urgent task. Indirect calorimetry (IC) is the only practical, clinical method to measure resting energy expenditure (REE). However, from a clinical perspective, knowledge of only one part of total energy expenditure (TEE) is insufficient to plan feeding that covers the actual energy needs of an individual patient. Currently, there is no method better than IC for accurately determining the resting energy needs of hospitalized patients. Although many predictive equations for REE have been developed, none of them can account for all the factors associated with changes in metabolism in critically ill patients. The most widely used equation for this purpose, the Harris and Benedict equation, was issued over 100 years ago and is inaccurate in about 40% of patients. To date, indirect calorimetry remains the gold standard for assessing REE. Pneumocystis jirovecii colonization in preterm newborns with respiratory distress syndrome

Molecular and microscopic methods were used for detection of P. jirovecii in patients' specimens. Pneumocystis DNA was detected in eight nasopharyngeal aspirates (14%) of 56 newborns and in seven oral washes (21%) of 34 mothers. Pneumocystis detection immediately after birth suggests the possibility of its transplacental transmission. Comparing to non-colonized infants, more frequent occurrence of bronchopulmonary dysplasia was seen in colonized ones (P=0.02), suggesting a potential clinical importance of this pathogen in abnormal lung development.publishersversionpublishe
MicroRNAs are intensively regulated during induction of somatic embryogenesis in Arabidopsis
Several genes encoding transcription factors (TFs) were indicated to have a key role in the induction of somatic embryogenesis (SE), which is triggered in the somatic cells of plants. In order to further explore the genetic regulatory network that is involved in the embryogenic transition induced in plant somatic cells, micro-RNA (miRNAs) molecules, the products of MIRNA (MIR) genes and the common regulators of TF transcripts, were analyzed in an embryogenic culture of Arabidopsis thaliana. In total, the expression of 190 genes of the 114 MIRNA families was monitored during SE induction and the levels of the primary (pri-miRNAs) transcripts vs. the mature miRNAs were investigated. The results revealed that the majority (98%) of the MIR genes were active and that most of them (64%) were differentially expressed during SE. A distinct attribute of the MIR expression in SE was the strong repression of MIR transcripts at the early stage of SE followed by their significant up-regulation in the advanced stage of SE. Comparison of the mature miRNAs vs. pri-miRNAs suggested that the extensive post-transcriptional regulation of miRNA is associated with SE induction. Candidate miRNA molecules of the assumed function in the embryogenic response were identified among the mature miRNAs that had a differential expression in SE, including miR156, miR157, miR159, miR160, miR164, miR166, miR169, miR319, miR390, miR393, miR396, and miR398. Consistent with the central role of phytohormones and stress factors in SE induction, the functions of the candidate miRNAs were annotated to phytohormone and stress responses. To confirm the functions of the candidate miRNAs in SE, the expression patterns of the mature miRNAs and their presumed targets were compared and regulatory relation during SE was indicated for most of the analyzed miRNA-target pairs. The results of the study contribute to the refinement of the miRNA-controlled regulatory pathways that operate during embryogenic induction in plants and provide a valuable platform for the identification of the genes that are targeted by the candidate miRNAs in SE induction
Amyloidoza łańcuchów lekkich immunoglobulin z punktu widzenia kardiologa
Light-chain amyloidosis (amyloidosis AL) is diagnosed in approx. 70% of patients with cardiac amyloidosis. This type of amyloidosis has the worst prognosis, especially if the diagnosis is made in advanced stages. The majority of patients are referred to a cardiologist, but unfortunately only every fifth of them has the proper diagnosis. Therefore, strategies promoting early diagnosis are important. One of them is the measurement of serum free light chains concentration in every patient with heart failure with preserved ejection fraction. The acknowledgement of free light chains (FLCs) cardiotoxicity rendered the picture of AL amyloidosis from infiltrative cardiomyopathy into a toxic one. Best improvement in regard to heart failure is achieved upon hematological treatment resulting in decrease of FLCs concentration. Therefore, cardiological treatment is rather a supportive therapy. The role of cardiologist is the rapid diagnosis of the disease and referral of the patient to the hematologist. The standard heart failure treatment encompassing use of beta-blockers and angiotensin converting enzyme inhibitors aggravates orthostatic hypotension and congestion. Instead, up-to-date hematological treatment improves the prognosis of AL amyloidosis markedly, as long as early diagnosis is made.Amyloidozę łańcuchów lekkich (amyloidozę AL) rozpoznaje się u około 70% pacjentów z amyloidozą serca. Ta postać choroby wiąże się z najgorszym rokowaniem, szczególnie jeśli wykrywa się ją na zaawansowanym etapie. Kardiolog jest najczęściej odwiedzanym specjalistą przez pacjentów z amyloidozą AL. Niestety tylko u co piątego pacjenta jest stawiana właściwa diagnoza. Dlatego ważne jest, aby promować działania umożliwiające wczesne stwierdzenie choroby. Należy do nich oznaczanie wolnych łańcuchów lekkich (FLC) w surowicy u pacjentów z niewydolnością sercaz zachowaną frakcją wyrzutową. Wykazanie kardiotoksycznej roli FLC zmieniło postrzeganie amyloidozy AL jako choroby polegającej wyłącznie na pozakomórkowym gromadzeniu się nieprawidłowych złogów białkowych. Największą poprawę funkcji serca uzyskuje się, obniżając stężenie FLC w surowicy poprzez leczenie cytoredukcyjne. Leczenie kardiologiczne ma znaczenie uzupełniające. Rola kardiologa sprowadza się do jak najszybszego rozpoznania choroby i przekazania pacjenta do hematologa. Standardowa farmakoterapia niewydolności serca, obejmująca beta-adrenolityki i inhibitory konwertazy angiotensyny, u pacjentów z amyloidozą wywołuje nasilenie hipotensji ortostatycznej i objawów zastoinowych. Natomiast dzięki nowoczesnemu leczeniu cytoredukcyjnemu i antyamyloidowemu istotnie poprawiły się wyniki leczenia, pod warunkiem wczesnego rozpoznania amyloidozy AL
