    Finite Difference Solution of Conjugate Heat Transfer in Double Pipe with Trapezoidal Fins

    A conjugate heat transfer problem on the shell side of a finned double pipe heat exchanger is numerically studied by suing finite difference technique. Laminar flow with isothermal boundary conditions is considered in the finned annulus with fully developed flow region to investigate the influence of variations in the fin height, the number of fins and the fluid and wall thermal conductivities on the hydraulic and thermal design of the exchanger. The governing momentum and energy equations have been solved by a finite difference-based numerical algorithm. The improvement in heat transfer rates, the exchanger performance and the optimum configurations are discussed

    SAM-SoS: A stochastic software architecture modeling and verification approach for complex System-of-Systems

    A System-of-Systems (SoS) is a complex, dynamic system whose Constituent Systems (CSs) are not known precisely at design time, and the environment in which they operate is uncertain. SoS behavior is unpredictable due to underlying architectural characteristics such as autonomy and independence. Although the stochastic composition of CSs is vital to achieving SoS missions, their unknown behaviors and impact on system properties are unavoidable. Moreover, unknown conditions and volatility have significant effects on crucial Quality Attributes (QAs) such as performance, reliability and security. Hence, the structure and behavior of a SoS must be modeled and validated quantitatively to foresee any potential impact on the properties critical for achieving the missions. Current modeling approaches lack the essential syntax and semantics required to model and verify SoS behaviors at design time and cannot offer alternative design choices for better design decisions. Therefore, the majority of existing techniques fail to provide qualitative and quantitative verification of SoS architecture models. Consequently, we have proposed an approach to model and verify Non-Deterministic (ND) SoS in advance by extending the current algebraic notations for the formal models as a hybrid stochastic formalism to specify and reason architectural elements with the required semantics. A formal stochastic model is developed using a hybrid approach for architectural descriptions of SoS with behavioral constraints. Through a model-driven approach, stochastic models are then translated into PRISM using formal verification rules. The effectiveness of the approach has been tested with an end-to-end case study design of an emergency response SoS for dealing with a fire situation. Architectural analysis is conducted on the stochastic model, using various qualitative and quantitative measures for SoS missions. Experimental results reveal critical aspects of SoS architecture model that facilitate better achievement of missions and QAs with improved design, using the proposed approach

    Immune Modulation in the Treatment of Amyotrophic Lateral Sclerosis: A Review of Clinical Trials

    Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease characterized by the degeneration of motor neurons. Though many molecular and genetic causes are thought to serve as predisposing or disease propagating factors, the underlying pathogenesis of the disease is not known. Recent discoveries have demonstrated the presence of inflammation propagating substrates in the central nervous system of patients afflicted with ALS. Over the past decade, this hypothesis has incited an effort to better understand the role of the immune system in ALS and has led to the trial of several potential immune-modulating therapies. Here, we briefly review advances in the role of such therapies. The clinical trials discussed here are currently ongoing or have been concluded at the time of writing

    Yield of esophagogastroduodenoscopy and colonoscopy in cancer of unknown primary

    Objectives: Carcinoma of unknown primary origin (CUP) is heterogeneous group of cancers. Role of gastrointestinal (GI) endoscopy in this entity is under investigated. Aim of this study was to evaluate yield of Colonoscopy and Esophagogastroduodenoscopy (EGD) in localizing primary tumor in patients with CUP. METHODOLOGY: Patients with histopathologically proven CUP who underwent colonoscopy / EGD to find the primary tumor from December 2009 to December 2011 were included in the study. Abdominal symptoms and cytokeratin (CK) 7 and 20 markers were correlated with presence of primary in GI tract. Results: After giving informed consent 86 patients were included in final analysis. All patients underwent colonoscopy while 60(70%) got EGD along with colonoscopy. Mean age was 55.10 +/-11.94 years with 52(60%) male. Abdominal symptoms were present in 50%. CK7+/CK20- in 34(40%); CK7-/CK20+ in 2(2%) while CK7+/20+ in 7(8%) of metastatic tumor samples. Liver was metastatic site in 47(55%), Lymph node 12(14%) and Ascites in 8(9%). Endoscopy detected primary in 6 (7%) patients with 3 each in stomach and colon. No association of abdominal symptoms and cytokeratin markers was found with presence of GI primary site. CONCLUSION: Yield of localizing primary lesion in the GI tract by pan-endoscopy was limited. Abdominal symptoms and cytokeratin markers do not predict presence of gastrointestinal malignancies

    Functionalized Multi-Walled Carbon Nanotube-Reinforced Epoxy-Composites: Electrical And Mechanical Characterization

    Carbon nanotubes (CNTs) got great attention because of their interesting physical and mechanical properties. Due to these interesting properties observed at the nanoscale have motivated scientific community to utilize CNTs as reinforcement in composite materials. In the present study, different CNTs and epoxy nano-composites with different wt% (1, 2, 3, and 4%) of f-MWCNTs were prepared and their surface morphology and orientation has been investigated in detail. Further, the surface investigation, electrical and mechanical tests were carried out on CNTs-filled and unfilled epoxy at maximum sonication time 30 minute to identify the loading effect on the properties of the materials. Experimental results depicts well dispersion of f-MWCNTs, significant improvement that the resistivity of pure epoxy decreased from 108 .m to average value 103 .m with 1, 2, 3, and 4wt% f-MWCNTs. The 4.5wt% CNTs/epoxy was attributed to poor dispersion of f-MWCNTs in the nanocomposte. The hardness of nanocomposite loading 1, 2, 3, 4wt% of CNTs, increased 20.7%, 23.02%, 25.62%, 29.09% respectively as compared to pure epoxy. We believe that our strategy for obtaining CNT–reinforced epoxy nanocomposites is a very promising technology and will open a new doors in fields of aviation, aerospace, marine and sporting goods

    LMBiS-Net: A Lightweight Multipath Bidirectional Skip Connection based CNN for Retinal Blood Vessel Segmentation

    Blinding eye diseases are often correlated with altered retinal morphology, which can be clinically identified by segmenting retinal structures in fundus images. However, current methodologies often fall short in accurately segmenting delicate vessels. Although deep learning has shown promise in medical image segmentation, its reliance on repeated convolution and pooling operations can hinder the representation of edge information, ultimately limiting overall segmentation accuracy. In this paper, we propose a lightweight pixel-level CNN named LMBiS-Net for the segmentation of retinal vessels with an exceptionally low number of learnable parameters \textbf{(only 0.172 M)}. The network used multipath feature extraction blocks and incorporates bidirectional skip connections for the information flow between the encoder and decoder. Additionally, we have optimized the efficiency of the model by carefully selecting the number of filters to avoid filter overlap. This optimization significantly reduces training time and enhances computational efficiency. To assess the robustness and generalizability of LMBiS-Net, we performed comprehensive evaluations on various aspects of retinal images. Specifically, the model was subjected to rigorous tests to accurately segment retinal vessels, which play a vital role in ophthalmological diagnosis and treatment. By focusing on the retinal blood vessels, we were able to thoroughly analyze the performance and effectiveness of the LMBiS-Net model. The results of our tests demonstrate that LMBiS-Net is not only robust and generalizable but also capable of maintaining high levels of segmentation accuracy. These characteristics highlight the potential of LMBiS-Net as an efficient tool for high-speed and accurate segmentation of retinal images in various clinical applications

    Heavy metal content determination in locally available traditional medicines using Atomic Absorption Spectroscopy (AAS)

    Traditional medicine (TM) is a system of health care that has ancient roots. It has been defined as practices designed to promote mental, physical and spiritual well-being of mankind. Usually (TM) are made from various types of medicinal plants. Sometimes they contain other ingredients such as animal parts and minerals however, the primary source of remedies is botanical. The use of traditional medicine has started beyond recorded history, but has increased tremendously in recent years. It is considered as alternative treatments which are widely used in developing countries. Recently became more popular in most developed countries as well. According to the world health Organization (WHO) reports that more than 70% of the population all over the world relies on non-conventional medicine. This high percentage indicates the positive attitudes towards herbal medicine in various places in the world, probably due to the people’s believe that such products are more likely to be safe ‘natural’ rather than ‘synthetic’ origin. In Malaysia, folk medicines are widely used .The huge demand of TM has been driven the total Malaysian market for TM to US $ 1.29 billion in 2005. The global popularity of(TM) has created great concern about the safety and efficacy of traditional medicines. Although WHO has developed guidelines for the quality control of herbal drugs, there is still a gap between available knowledge and implementation. Processors of herbal drugs are still not much aware of WHO’s guidelines yet they continue their work without proper quality control which results in products with lots of contaminants such as heavy metals, pesticides residues and microbes, therefore inappropriate consumption of herbal preparation can cause serious health complication even toxicity.. In our study we are focusing on identifying the presence of toxic heavy metals in locally available traditional medicines in the east coast region of Malaysia. Traditional medicine samples were collected from different sources such as shops, open market, night market and weekend market. Most of them are not registered with the Malaysian drug authority. All samples were digested using freshly prepared aqua regia (mixture of concentrated nitric acid and hydrochloric acid 1:3 ratio). 0.5 g of each sample was weighed and placed in 100 ml PTFE beaker, after which 9 ml of freshly prepared aqua regia was added. The mixture was then heated over water bath for 4-5 hours or until the sample had completely dissolved. The mixture was then allowed to cool and filtered; the residue was then washed with deionised water, and the extract was then made up to 50 ml with deionised water. The samples were then analysed by using atomic absorption spectroscopy (AAS). Out of twenty over traditional medicine samples most of them has shown the presence of lead (Pb), chromium (Cr), ferrum (Fe) and zinc (Zn). The results for nickel (Ni) and cadmium (Cd) were found below the detection limit using flame ionization technique and expected to be detected using graphite furnace AAS (GFAAS) method which will be the second part of this study. From the results we conclude that the traditional medicines available in local markets contain heavy metals which are considered toxic for human if consumed more than the permissible amount.     Quality of life after stroke in Pakistan

    Background: There is very little information about the quality of life (QOL) of stroke survivors in LMIC countries with underdeveloped non communicable health infrastructures, who bear two thirds of the global stroke burden. Methodology: We used a sequential mix methods approach. First, a quantitative analytical cross-sectional study was conducted on 700 participants, who constituted 350 stroke survivor and their caregiver dyads. QOL of stroke survivor was assessed via Stroke Specific Quality of Life Scale (SSQOLS) whereas QOL of caregivers was assessed through RAND-36. In addition; we assessed complications, psychosocial and functional disability of stroke survivors. Following this quantitative survey, caregivers were qualitatively interviewed to uncover contextually relevant themes that would evade quantitative surveys. Multiple linear regression technique was applied to report adjusted β-coefficients with 95% C.I. Results: The QOL study was conducted from January 2014 till June 2014, in two large private and public centers. At each center, 175 dyads were interviewed to ensure representativeness. Median age of stroke survivors was 59(17) years, 68% were male, 60% reported depression and 70% suffered post-stroke complications. The mean SSQOLS score was 164.18 ± 32.30. In the final model severe functional disability [adjβ -33.77(-52.44, -15.22)], depression [adjβ- 23.74(-30.61,-16.82)], hospital admissions [adjβ-5.51(-9.23,-1.92)] and severe neurologic pain [adjβ -12.41(-20.10,-4.77)] negatively impacted QOL of stroke survivors (P \u3c 0.01). For caregivers, mean age was 39.18 ± 13.44 years, 51% were female and 34% reported high stress levels. Complementary qualitative study revealed that primary caregivers were depressed, frustrated, isolated and also disappointed by health services. Conclusion: The QOL of Stroke survivors as reported by SSQOLS score was better than compared to those reported from other LMIC settings. However, Qualitative triangulation revealed that younger caregivers felt isolated, depressed, overwhelmed and were providing care at great personal cost. There is a need to develop cost effective holistic home support interventions to improve lives of the survivor dyad as a unit
