    Incorporating Domain-Specific Information Quality Constraints into Database Queries

    The range of information now available in queryable repositories opens up a host of possibilities for new and valuable forms of data analysis. Database query languages such as SQL and XQuery offer a concise and high-level means by which such analyses can be implemented, facilitating the extraction of relevant data subsets into either generic or bespoke data analysis environments. Unfortunately, the quality of data in these repositories is often highly variable. The data is still useful, but only if the consumer is aware of the data quality problems and can work around them. Standard query languages offer little support for this aspect of data management. In principle, however, it should be possible to embed constraints describing the consumer’s data quality requirements into the query directly, so that the query evaluator can take over responsibility for enforcing them during query processing. Most previous attempts to incorporate information quality constraints into database queries have been based around a small number of highly generic quality measures, which are defined and computed by the information provider. This is a useful approach in some application areas but, in practice, quality criteria are more commonly determined by the user of the information not by the provider.     Implementing telephone triage in general practice: a process evaluation of a cluster randomised controlled trial

    Background: Telephone triage represents one strategy to manage demand for face-to-face GP appointments in primary care. However, limited evidence exists of the challenges GP practices face in implementing telephone triage. We conducted a qualitative process evaluation alongside a UK-based cluster randomised trial (ESTEEM) which compared the impact of GP-led and nurse-led telephone triage with usual care on primary care workload, cost, patient experience, and safety for patients requesting a same-day GP consultation. The aim of the process study was to provide insights into the observed effects of the ESTEEM trial from the perspectives of staff and patients, and to specify the circumstances under which triage is likely to be successfully implemented. Here we report perspectives of staff. Methods: The intervention comprised implementation of either GP-led or nurse-led telephone triage for a period of 2-3 months. A qualitative evaluation was conducted using staff interviews recruited from eight general practices (4 GP triage, 4 Nurse triage) in the UK, implementing triage as part of the ESTEEM trial. Qualitative interviews were undertaken with 44 staff members in GP triage and nurse triage practices (16 GPs, 8 nurses, 7 practice managers, 13 administrative staff). Results: Staff reported diverse experiences and perceptions regarding the implementation of telephone triage, its effects on workload, and on the benefits of triage. Such diversity were explained by the different ways triage was organised, the staffing models used to support triage, how the introduction of triage was communicated across practice staff, and by how staff roles were reconfigured as a result of implementing triage. Conclusion: The findings from the process evaluation offer insight into the range of ways GP practices participating in ESTEEM implemented telephone triage, and the circumstances under which telephone triage can be successfully implemented beyond the context of a clinical trial. Staff experiences and perceptions of telephone triage are shaped by the way practices communicate with staff, prepare for and sustain the changes required to implement triage effectively, as well as by existing practice culture, and staff and patient behaviour arising in response to the changes made.     Postfledging Survival and Local Recruitment of a Riparian Songbird in Habitat Influenced By Reservoir Operations

    The impact of anthropogenic activities on breeding bird populations are typically assessed using nest success despite the importance of the postfledging period and juvenile survival for the population dynamics of many birds. Using a combination of radio telemetry data collected between 2012 and 2014, and long-term monitoring data collected between 2005 and 2016, we evaluated whether postfledging survival of Yellow Warblers (Setophaga petechia) is affected when their riparian nesting habitat becomes inundated by the Upper Arrow Lakes Reservoir in the Columbia River Valley near Revelstoke, British Columbia, Canada. Thirty-eight percent of radiotagged fledglings (n = 26) survived for at least 21 days after leaving the nest. Radio-tagged birds that fledged from nests in territories that were inundated by water tended to be have lower survival than those that fledged from nests in territories that were not inundated by water. Local recruitment was low (6.4%, n = 438). Local recruitment was nevertheless positively affected by nestling condition prior to fledging. Fledglings from territories that were not inundated by water also tended to be more likely to recruit locally than those that fledged from territories that were inundated by water. In both cases, we estimated that reservoir operations that flooded habitat reduced postfledging survival or local recruitment by approximately 50%. Our study emphasizes the importance of considering the postfledging period when developing mitigation measures or management plans aimed at minimizing the impact of anthropogenic activities on bird populations

    Diversity among Equals: Educational Opportunity and the State of Affirmative Admissions in New England

    This report reviews the practice of Affirmative Admissions as a strategy for achieving diversity within New England colleges and universities. It shows how educational leaders perceive Affirmative Admissions, the nature of regional Affirmative Admissions policies, and the numbers of student affected by current enrollment strategies. This report is part of a larger series on educational access and opportunity in New England. Research was organized into five components: (1) analysis of pertinent legal issues related to postsecondary access and equity; (2) interviews with postsecondary campus and state leaders (n=104); (3) interviews with K-12 leaders and educators at state, district, and school levels (n=45); (4) a survey of 221 postsecondary education institutions in New England; and (5) econometric analyses of student data. The focus was on groups of institutions, 18 groups clustered by admissions policies and restrictions. The most compelling conclusion is that there is no significant evidence that colleges have reduced standards to admit greater numbers of minority students. By increasing educational access to a broader segment of the population, the regions higher education institutions have taken crucial steps toward assuring the vitality and vibrancy of New Englands future economy and civic life. The study also indicates that the pool of qualified minority students is much too small, highlighting the need to improve the preparation of minority students. Three appendixes contain details about survey methodology, participating institutions, and regression coefficients.     Prostate cancer survivorship essentials for men with prostate cancer on androgen deprivation therapy: Protocol for a randomised controlled trial of a tele-based nurse-led survivorship care intervention (PCEssentials hormone therapy study)

    Introduction Androgen deprivation therapy (ADT) is commonly used to treat men with locally advanced or metastatic prostate cancer. Men receiving ADT experience numerous side effects and frequently report unmet supportive care needs. An essential part of quality cancer care is survivorship care. To date, an optimal effective approach to survivorship care for men with prostate cancer on ADT has not been described. This protocol describes a randomised trial of tele-based nurse-led survivorship that addresses this knowledge gap: (1) determine the effectiveness of a nurse-led survivorship care intervention (PCEssentials), relative to usual care, for improving health-related quality of life (HR-QoL) in men with prostate cancer undergoing ADT and (2) evaluate PCEssentials implementation strategies and outcomes, including cost-effectiveness, compared with usual care. Methods and analysis This is an effectiveness-implementation hybrid (type 1) trial with participants randomised to one of two arms: (1) minimally enhanced usual care and (2) nurse-led prostate cancer survivorship essentials (PCEssentials) delivered over four tele-based sessions, with a booster session 5 months after session 1. Eligible participants are Australian men with prostate cancer commencing ADT and expected to be on ADT for a minimum of 12 months. Participants are followed up at 3, 6 and 12 months postrecruitment. Primary outcomes are HR-QoL and self-efficacy. Secondary outcomes are psychological distress, insomnia, fatigue and physical activity. A concurrent process evaluation with participants and study stakeholders will be undertaken to determine effectiveness of delivery of PCEssentials. Ethics and dissemination Ethics approval was obtained from the Metro South Health HREC (HREC/2021/QMS/79429). All participants are required to provide written informed consent. Outcomes of this trial will be published in peer-reviewed journals. The findings will be presented at conferences and meetings, local hospital departments, participating organisations/clinical services, and university seminars, and communicated at community and consumer-led forums.     Effects of Conceptual Training and Procedural Training for Teaching Aviation Instrument Holding Patterns

    ABSTRACT Twenty private pilots without instrument ratings trained how to fly instrument holding patterns. Holding patterns are advanced aviation maneuvers learned during instrument training where pilots maintain a particular "race-track" figure that keeps the airplane essentially stationary. Participants were randomly assigned to a procedural training group or a conceptual training group. The step-by-step sequence of actions to fly instrument holds was emphasized in the procedural training group. The reasons for flying instruments holds and the interrelationship of elements in a dynamic environment were emphasized in the conceptual training group. Training stimuli included reading text and watching videos. Participants who were conceptually trained showed no difference in situation awareness when flying a typical instrument hold in a flight simulator compared to when flying a more difficult, atypical instrument hold in a flight simulator. However, the procedurally trained participants showed significantly less situation awareness when flying the atypical instrument hold compared to when flying the typical instrument hold. It was found that participants who required more attempts to answer questions correctly during training showed better situation awareness when flying atypical holding patterns. Finally it was found that participants required more attempts to answer questions correctly during the training delivered via video than they did during the training delivered via text

    Light painting photography makes particulate matter air pollution visible

    The World Health Organization estimates that air pollution causes approximately seven million premature deaths worldwide each year. Solutions to air pollution are well known, yet this rarely equates to easily actionable. Here we demonstrate how art science collaboration can successfully highlight the issue of air pollution and create wider civic discourse around its amelioration. We document a light painting photographic technique that uses data from calibrated low-cost particulate matter sensors to measure and depict air pollution. We also use a postcard technique to grasp individuals’ sentiments regarding air pollution. The photographs from three countries, Ethiopia, India and United Kingdom, visually highlight the importance of location and occupation upon human exposure. The photographs are used as a proxy to communicate and create dialogues, spaces and places about air pollution. The sentiment analysis show how this approach can foster awareness and create agency for stakeholders to take actions to tackle air pollution
