Spatial distribution of nitrate health risk associated with groundwater use as drinking water in Merida, Mexico
Water containing nitrate levels above 45 mg/l is not recommended for human consumption and its prolonged intake is associated with various health conditions. In Merida city, Mexico, the only source for water supply is a karstic aquifer, but the absence of sewerage and drainage networks makes it highly vulnerable to anthropogenic contamination. In this work, the concentration and spatial distribution of nitrate in the Merida’s karstic aquifer were assessed by statistical and geostatistical techniques. The sources of nitrate contamination were tracked by making statistical correlations between nitrate concentrations and key ions; the potential risk to human health was also estimated by using the Hazard Index (HI). A total of 177 groundwater samples were collected from the four water supply systems serving Merida, during 2012 and 2013. Nitrate concentrations from collected samples varied between 15.51 and 70.61 mg/l, with maximum and minimum concentrations per sampling point ranging from 47.47 to 70.61 mg/l and from 15.51 to17.32 mg/l, respectively. Significant positive correlations (P < 0.05) between nitrates and chlorides, sulphates and potassium were found, which may indicate potential contamination from domestic wastewater and agricultural activities. The spatial distribution of nitrate concentrations in the aquifer revealed an increase in nitrates following a trajectory South – North West, towards central and northwestern zones within Merida Metropolitan Area. From the health risk analysis, it was found that infants exposed at current nitrate levels are at a higher risk (HIMAX = 1.40) than adults (HR<1.0) and therefore, there is a clear need for implementing effective strategies to protect groundwater quality and to better manage and control nitrate pollution sources

C-SURFER : grazoprevir plus elbasvir chez des patients infectés par le VHC de génotype 1 et une insuffisance rénale chronique naïfs de traitement et pré-traités
L’infection par le VHC chez les patients présentant une insuffisance rénale chronique de stades 4 et 5 (IRC 4/5, définie par une ClCr<30mL/min avec ou sans dialyse) est associée à un risque accru de décès et d’échec de la transplantation rénale et les options thérapeutiques restent limitées. Dans une étude de phase II, le grazoprévir (GZR ; inhibiteur de la protéase NS3/4A) et l’elbasvir (EBR ; inhibiteur de NS5A), tous deux éliminés par voie hépatique, ont été très efficaces dans le traitement de l’infection par le VHC de génotype 1 (G1). Nous les avons évalués dans une étude de phase III chez des patients G1 ayant une IRC 4/5.
Deux cent vingt-quatre patients G1 IRC 4/5 ont été randomisés pour recevoir un traitement immédiat par GZR 100mg/EBR 50mg une fois par jour pendant 12 semaines ou différé (placebo puis traitement actif). Le critère d’évaluation principal était la réponse virologique soutenue à la semaine 12 de suivi (RVS12 ; système COBAS TaqMan v2.0 [LIQ 15 UI/mL]).
Deux cent trente-cinq patients ont reçu au moins une dose de médicament expérimental (bras traitement immédiat, n=111 ; bras PK, n=11 ; bras traitement différé, n=113) : 52 % infectés par le G1a, 80 % naïfs de traitement anti-VHC, 6 % avaient une cirrhose, 34 % étaient diabétiques, 19 % présentaient une IRC 4, 81 % une IRC 5 et 76 % étaient sous hémodialyse. Une RVS12 était obtenue chez 115 des 116 patients (99 %, IC à 95 % : 95,3–100,0) recevant le GZR/EBR (1 rechute à S12 chez un G1b sans cirrhose IRC 5). Des EI graves sont survenus chez respectivement 16 (13 %) et 18 (16 %) patients des bras GZR/EBR et placebo et les arrêts de traitement en raison d’un EI étaient de 0 % et 4 %, respectivement.
Excellente efficacité et bonne tolérance de la combinaison GZR 100mg/EBR 50mg chez les insuffisants rénaux.
L’association GZR/EBR une fois par jour pendant 12semaines est très efficace et généralement bien tolérée chez les patients présentant une infection par le VHC de génotype 1 et une IRC de stade avancé
