Síndrome de Chiari e Hidrossiringomielia com comprometimento neurológico: um relato de caso
A Malformação de Chiari (MC) pertence a um amplo grupo de raras deformidades estruturais da junção craniocerebelomedular. O tipo I da doença caracteriza-se pela herniação tonsilar ou amigdaliana cerebelar devido à anomalia da base do crânio e da parte superior da coluna cervical, além de a porção medial do lobo inferior do cerebelo pelo canal cervical também se protuberar através do forame magno, impedindo que o líquor flua normalmente através do canal. A real prevalência da doença é desconhecida, pois muitos pacientes com herniação cerebelar são assintomáticos e o problema agrava-se na fase adulta, com queixas de cefaleia intensa e, por vezes, parestesia. O objetivo deste estudo é relatar um caso de síndrome de Chiari (SC) em uma paciente de 53 anos, ao abordar sua apresentação clínica, diagnóstico e tratamento. Paciente do sexo feminino, 53 anos, foi admitida em um hospital da rede pública de referência se queixando de cefaleia occipital intensa e cervicalgia com irradiação da dor para os membros superiores, acompanhada de parestesia nos quatro segmentos. Relatou já sentir dor há 2 anos, mas apresentou piora do quadro clínico há 8 meses. Foi, também, observada incontinência urinária devido à dissinergia detrusora-esfincteriana por provável bexiga neurogênica. Foi, então, realizado exame de imagem de ressonância magnética (RNM) do crânio e da coluna cervical, com obtenção de sequências ponderadas em T1, T2 e STIR, nos planos sagital e transverso com contraste, o qual evidenciou leve alargamento medular, além de sinais de hidrossiringomielia difusa, com hipossinal na sequência T2 intramedular na altura de D1-D2 (coluna dorsal). Foi notada discreta herniação das tonsilas cerebelares junta ao forame magno, típica da SC, sendo, por fim, confirmado o diagnóstico. A paciente, no entanto, não apresentava hidrocefalia, mesmo com a interrupção do fluxo do líquido cefalorraquidiano (LCR) para o canal vertebral. Ela encaixou- se nos parâmetros de indicação cirúrgica, sendo realizada craniotomia occipital, com acesso ao plexo coroide do quarto ventrículo do tronco encefálico com o intuito de elevar as tonsilas cerebelares baixas, herniadas no canal espinhal cervical e bloqueando o fluxo do LCR. Após a descompressão craniocervical, o curso do líquor foi restaurado e a paciente foi, por fim, encaminhada à sala de recuperação pós-operatória. A SC é uma rara doença que apresenta quadro clínico e alterações radiológicas complexas e extensas e, por vezes, o diagnóstico é retardado devido à inespecificidade dos sintomas confundidos com cervicalgias e cefaleias comuns. A hipótese diagnóstica deve ser embasada nas queixas do paciente, na anamnese minuciosa, exame clínico e nos exames de imagens, sendo a prevalência desta patologia de difícil definição e com faixas etárias distintas
Neurostimulation Combined With Cognitive Intervention in Alzheimer’s Disease (NeuroAD): Study Protocol of Double-Blind, Randomized, Factorial Clinical Trial
Despite advances in the treatment of Alzheimer’s disease (AD), there is currently no prospect of a cure, and evidence shows that multifactorial interventions can benefit patients. A promising therapeutic alternative is the use of transcranial direct current stimulation (tDCS) simultaneously with cognitive intervention. The combination of these non-pharmacological techniques is apparently a safe and accessible approach. This study protocol aims to compare the efficacy of tDCS and cognitive intervention in a double-blind, randomized and factorial clinical trial. One hundred participants diagnosed with mild-stage AD will be randomized to receive both tDCS and cognitive intervention, tDCS, cognitive intervention, or placebo. The treatment will last 8 weeks, with a 12-month follow-up. The primary outcome will be the improvement of global cognitive functions, evaluated by the AD Assessment Scale, cognitive subscale (ADAS-Cog). The secondary outcomes will include measures of functional, affective, and behavioral components, as well as a neurophysiological marker (Brain-derived neurotrophic factor, BDNF). This study will enable us to assess, both in the short and long term, whether tDCS is more effective than the placebo and to examine the effects of combined therapy (tDCS and cognitive intervention) and isolated treatments (tDCS vs. cognitive intervention) on patients with AD.Clinical Trial Registration: www.ClinicalTrials.gov, identifier NCT02772185—May 5, 2016
Estudo experimental e de simulação da dinâmica de material particulado em tambor rotatório com suspensores
Drying is a unit operation present in several industrial processes; one of the most
widely used equipment for drying are flighted rotary dryers, which are cylindrical drums
with flights in order to elevate the material and shower it over the cross section.
Understanding the solids discharge profiles is extremely important to describe the behavior
of particles in them. For this, usually are used rotating drums with flights without the
presence of the drying air, to facilitate the measurement of particle dynamics in these
processes. The computational fluid dynamics (CFD) is a very viable and increasingly used
method that coupled with experimental work can provide excellent results. In the use of
computational fluid dynamics, to simulate the dynamics of a particulate material in a
rotating drum with flights, was used the Euler-Euler approach. In order to compare with the
experimental results, was developed a method of image analysis that allows estimate the
holdup on flights through the area of the mass fractions on the cross sections of the drum.
Thus, the work aimed to study the influence of material type, particle diameter and rotation
speed in the behavior of particles in a rotating drum with flights and the design load. Also, it
was analyzed the influence of these variables for the dynamic friction coefficient for the
glass beads and it was observed that the particle diameter influenced the dynamic friction
coefficient and hence the particles discharge profile. The results of holdup in function of
angular position were very important to verify the effects of the dynamic operating variables
of the particles. Furthermore, in the CFD modeling, it was ranged the limits of frictional
viscosity and it was possible to obtain values very close to the experimental one. According
to the limit of frictional viscosity that was closest to the experimental results, in which it
should be applied in any region of the drum, the results allowed an interpretation on the
nature of the numerical solver (FLUENT ®), as well as the interpolations generated between

equipamentos mais usados para a secagem são os secadores rotatórios, que são tambores
cilíndricos dotados de suspensores, que ao girarem promovem o cascateamento do material
particulado. A compreensão dos perfis de descarga de sólidos dos suspensores é importante
para seu dimensionamento. Para isto, geralmente são utilizados tambores rotatórios com
suspensores, sem a presença do ar de secagem, que facilitam as medidas da dinâmica das
partículas nestes processos. Um método bastante viável e cada vez mais utilizado é a
fluidodinâmica computacional, que aliada aos trabalhos experimentais pode fornecer
resultados que ajudam na melhor compreensão dos fenômenos envolvidos. Utilizou-se para
os estudos em fluidodinâmica computacional o software comercial FLUENT®, com a
abordagem Euler-Euler. Com a finalidade de comparar com os resultados experimentais,
desenvolveu-se um método de análise de imagens que permite através da área das frações
mássicas, nas seções transversais do tambor, quantificar o holdup nos suspensores, visto que
o software não o fornece de forma direta. Desta maneira, o trabalho teve como objetivo
estudar a influência do tipo de material, diâmetro de partícula e velocidade de rotação no
perfil de descarga das partículas em um tambor rotatório com suspensores e no
carregamento ideal. Foi também analisada a influência destas variáveis no coeficiente de
atrito dinâmico para as esferas de vidro e observou-se que o diâmetro da partícula influencia
no coeficiente de atrito dinâmico e consequentemente no perfil de descarga das partículas.
Os resultados do perfil de variação de carga nos suspensores foram bastante conclusivos
para se verificar os efeitos das variáveis operacionais na dinâmica das partículas. Além
disso, na modelagem em CFD, variou-se os limites de viscosidade friccional e foi possível
obter valores de holdupcondizentes com os experimentais. De acordo com o limite de
viscosidade friccionalem que o perfil de descarga mais se aproximou dos resultados
experimentais,esta deveria ser aplicada em qualquer região do leito, os resultados
permitiram uma interpretação sobre a natureza numérica do resolvedor FLUENT®, assim
como as interpolações geradas entre uma célula e outra da malha computacional
Particles dynamics study in flighted rotating drum: experimental and numerical analysis using lagrangian and eulerian approaches
Rotary dryers are the most commonly used equipment in the granular or particulate
material drying, because of their high processing capacity and versatility of operation
when compared to other types of dryers. However, the design of these equipment is quite
complex, since it involves, in addition to heat and mass transfer, complex fluid-dynamic
processes. Therefore, rotary drums with flights, but without the presence of drying air,
are generally used for the study of the dynamics of particulate material in these dryers.
When the drum is in the ideal loading configuration, there is the greater interaction
between the particulate material and the flowing air, which is interesting in a case of
drying. Based on this, in this work a rotating drum with variable number of flights was
built, aiming at the study of the rotary drum in the ideal loading condition. In this way,
applications of geometric models for the prediction of this condition were studied and the
ideal rotation speed of the drum was determined for different operating conditions of the
equipment. This study determined a model for predicting the ideal Froude number as a
function of the drum filling fraction, number of flights and material properties, such as
density, static angle of repose and dynamic friction coefficient of the material. Besides
these, studies were also carried out regarding the angle of last discharge, evaluating the
effects of the operational conditions on the discharge of solids in the drum. As a
complementary tool to the experimental studies, numerical simulations help the
understanding of the flow of granular materials. Regarding the numerical approach, two
different approaches for predicting the granular flow inside the drum with flights were
used: Eulerian and Lagrangean. In the development of an Eulerian model, using
Computational Fluid Dynamics (CFD), it was possible to determine the boundary
conditions and to find the best model to reproduce the experimental results, in which the
application of the model of turbulence k-ɛ was essential in the description of particle
dynamics. In the simulations using the Lagrangian approach (through the Discrete
Element Method (DEM)), parameters related to the properties of the particles, such as
Poisson Ratio and Shear Modulus, were investigated, as well as particle-particle and
particle-wall interactions through coefficients of restitution and static and rolling friction.
In this work, a set of parameters was found through which the simulation represented the
experimental data well. In addition, it was observed that the use of a smaller shear
modulus than the actual one to decrease the computational cost was a valid assumption,
mainly for lower rotation speeds. Comparing the two methodologies, it was possible to
find the advantages and disadvantages of each one of them. The lagrangean approach
allowed to observe each particle individually, being possible the determination of the
length of fall of the particles. However, with the change in operating conditions, such as

equipamentos mais utilizados, devido a sua alta capacidade de processamento e
versatilidade de operação quando comparados com outros tipos de secadores. No entanto,
o projeto desses equipamentos é bastante complexo, uma vez que a secagem envolve,
além de transferências de calor e massa, complexos processos fluidodinâmicos. Por isso,
geralmente utilizam-se tambores rotatórios com suspensores, mas sem a alimentação do
ar de secagem, para o estudo da dinâmica de material particulado nesses secadores.
Quando o tambor está com a configuração de carregamento ideal, há a maior interação
entre o material particulado e o ar em escoamento, o que é interessante em um caso de
secagem. Com base nisso, neste trabalho foi construído um tambor rotatório com número
de suspensores variáveis, apresentando como objetivo o estudo do tambor rotatório na
condição de carregamento ideal. Desta maneira, foram estudadas aplicações de modelos
geométricos para a predição desta condição e determinou-se, experimentalmente, a
velocidade de rotação ideal do tambor para diferentes condições operacionais do
equipamento. Através deste estudo foi determinado um modelo para a predição do
Número de Froude ideal em função da fração de enchimento do tambor, número de
suspensores e propriedades do material, como densidade, ângulo de repouso estático e
coeficiente dinâmico de fricção do material. Além destes, foram realizados estudos
referentes ao ângulo de última descarga, avaliando os efeitos das condições operacionais
na descarga de sólidos no tambor. Como uma ferramenta complementar aos estudos
experimentais, as simulações numéricas surgem no auxílio à compreensão do escoamento
dos materiais granulares. No que diz respeito à abordagem numérica, duas diferentes
aproximações para a previsão do escoamento granular no interior do tambor com
suspensores foram utilizadas: Euleriana e Lagrangeana. No desenvolvimento de uma
modelagem Euleriana que representa o escoamento de interesse, utilizando a
Fluidodinâmica Computacional (CFD), foi possível então, determinar as condições de
contorno e encontrar o melhor modelo para que a abordagem Euleriana reproduzisse os
resultados experimentais, em que a aplicação do modelo de turbulência k-ɛ se mostrou
essencial na descrição da dinâmica das partículas. Já nas simulações que utilizam a
abordagem Lagrangeana, através do Método dos Elementos Discretos (DEM), foram
investigados parâmetros relativos às propriedades das partículas, como Razão de Poisson
e Módulo de Cisalhamento, além das interações partícula-partícula e partícula-parede
através dos coeficientes de restituição e de atritos estáticos e de rolamento. Neste trabalho,
encontrou-se um conjunto de parâmetros para que a simulação conseguisse representar
bem os dados experimentais. Além disso, observou-se que o uso de um módulo de
cisalhamento menor do que o real para diminuição do custo computacional foi uma
suposição válida, principalmente para velocidades de rotação mais baixas. Comparando
as duas metodologias, foi possível encontrar as vantagens e desvantagens de cada uma
delas. A abordagem lagrangeana permitiu observar individualmente cada partícula, sendo
possível a determinação do comprimento de queda das partículas do suspensor. No
entanto, com a mudança de condições operacionais, como o número de suspensores do
tambor, os resultados apresentados pela abordagem Euleriana foram melhores
