Hydrodynamical simulations of the jet in the symbiotic star MWC 560 III. Application to X-ray jets in symbiotic stars
In papers I and II in this series, we presented hydrodynamical simulations of
jet models with parameters representative of the symbiotic system MWC 560.
These were simulations of a pulsed, initially underdense jet in a high density
ambient medium. Since the pulsed emission of the jet creates internal shocks
and since the jet velocity is very high, the jet bow shock and the internal
shocks are heated to high temperatures and should therefore emit X-ray
radiation. In this paper, we investigate in detail the X-ray properties of the
jets in our models. We have focused our study on the total X-ray luminosity and
its temporal variability, the resulting spectra and the spatial distribution of
the emission. Temperature and density maps from our hydrodynamical simulations
with radiative cooling presented in the second paper are used together with
emissivities calculated with the atomic database ATOMDB. The jets in our models
show extended and variable X-ray emission which can be characterized as a sum
of hot and warm components with temperatures that are consistent with
observations of CH Cyg and R Aqr. The X-ray spectra of our model jets show
emission line features which correspond to observed features in the spectra of
CH Cyg. The innermost parts of our pulsed jets show iron line emission in the
6.4 - 6.7 keV range which may explain such emission from the central source in
R Aqr. We conclude that MWC 560 should be detectable with Chandra or
XMM-Newton, and such X-ray observations will provide crucial for understanding

Functional kernel estimators of conditional extreme quantiles
We address the estimation of "extreme" conditional quantiles i.e. when their
order converges to one as the sample size increases. Conditions on the rate of
convergence of their order to one are provided to obtain asymptotically
Gaussian distributed kernel estimators. A Weissman-type estimator and kernel
estimators of the conditional tail-index are derived, permitting to estimate

Impact of 6 month conjugated equine estrogen versus estradiol-treatment on biomarkers and enriched gene sets in healthy mammary tissue of non-human primates.
OBJECTIVE
To identify distinctly regulated gene markers and enriched gene sets in breast tissue of cynomolgus monkeys (Macaca fascicularis) treated for six months with either conjugated equine estrogens (CEE) or estradiol (E2) by analysis of corresponding mRNA levels of genes associated with breast development, carcinogenesis, apoptosis and immune regulation. Additionally, translation of three nuclear markers was analyzed.
METHODS
RNA from breast biopsies and necropsies was isolated from two independent study trials from Ethun et al. (CEE) and Foth et al. (E2) after 6 month of treatment duration. RNA was subjected to qRT-PCR and MicroArray analysis. Immunohistochemical stainings were performed for the estrogen receptor alpha subunit (ERa), the progesterone receptor (PGR) and the proliferation marker Ki67.
RESULTS
We identified a total of 36 distinctly enriched gene sets. Thirty-one were found in the CEE treatment group and five were found in the E2 treatment group, with no overlap. Furthermore, two individual genes IGFBP1 and SGK493 were upregulated in CEE treated animals. Additional targeted qRT-PCR analysis of ten specific estrogen-related genes showed upregulation of three genes (TFF1, PGR and GREB1) after CEE treatment, respectively one gene (TFF1) after E2 treatment. Immunohistochemical stains of breast biopsies showed a significant increase in expression of the PGR marker after CEE treatment.
CONCLUSIONS
In this study we identified enriched gene sets possibly induced by CEE or E2 treatment in various processes associated with cancer biology and immunology. This preliminary translational data supports the concept that different estrogen types have different effects on healthy breast tissue and may help generate hypotheses for future research
Deterministic single-photon source from a single ion
We realize a deterministic single-photon source from one and the same calcium
ion interacting with a high-finesse optical cavity. Photons are created in the
cavity with efficiency (88 +- 17)%, a tenfold improvement over previous
cavity-ion sources. Results of the second-order correlation function are
presented, demonstrating a high suppression of two-photon events limited only
by background counts. The cavity photon pulse shape is obtained, with good
agreement between experiment and simulation. Moreover, theoretical analysis of
the temporal evolution of the atomic populations provides relevant information
about the dynamics of the process and opens the way to future investigations of
a coherent atom-photon interface
Noble gases, stable isotopes, and radiocarbon as tracers of flow in the Dakota aquifer, Colorado and Kansas
A suite of chemical and isotope tracers (dissolved noble gases, stable isotopes of water, radiocarbon, and CI) have been analyzed along a flow path in the Dakota aquifer system to determine likely recharge sources, ground water residence times, and the extent of mixing between local and intermediate flow systems, presumably caused by large well screens. Three water types were distinguished with the tracers, each having a very different history. Two of the water types were found in south-eastern Colorado where the Dakota is poorly confined. The tracer data suggest that the first group recharged locally during the last few thousand years and the second group was composed of ground water that recharged earlier during a cooler climate, presumably during the last glacial period (LGP) and mixed aged water. The paleotemperature record archived in this groundwater system indicates that south-eastern Colorado was about 5°C cooler during the LGP than during the late Holocene. Similar temperature changes derived from dissolved noble gases in other aquifer systems have been reported earlier for the south-western United States. The third water type was located down gradient of the first two in the confined Dakota in western and central Kansas. Groundwater residence time of this water mass is on the order of 104-105 yrs and its recharge location is near the Colorado and Kansas border down gradient of the other water types. Remote Monitor Alarm System
A remote monitor alarm system monitors discrete alarm and analog power supply voltage conditions at remotely located communications terminal equipment. A central monitoring unit (CMU) is connected via serial data links to each of a plurality of remote terminal units (RTUS) that monitor the alarm and power supply conditions of the remote terminal equipment. Each RTU can monitor and store condition information of both discrete alarm points and analog power supply voltage points in its associated communications terminal equipment. The stored alarm information is periodically transmitted to the CMU in response to sequential polling of the RTUS. The number of monitored alarm inputs and permissible voltage ranges for the analog inputs can be remotely configured at the CMU and downloaded into programmable memory at each RTU. The CMU includes a video display, a hard disk memory, a line printer and an audio alarm for communicating and storing the alarm information received from each RTU
Oral estradiol/micronized progesterone may be associated with lower risk of venous thromboembolism compared with conjugated equine estrogens/medroxyprogesterone acetate in real-world practice.
OBJECTIVES
The Women's Health Initiative study reported an increased risk of venous thromboembolism among menopausal women treated with conjugated equine estrogens/medroxyprogesterone acetate (CEE/MPA) versus placebo. Newer hormone therapies may have a lower venous thromboembolism risk. The study compared the risk of venous thromboembolism between women treated with the combined oral product 17β-estradiol/micronized progesterone (E2/P4) and those treated with oral CEE/MPA regimens.
STUDY DESIGN
In a retrospective longitudinal study using real-world claims data from April 2019 to June 2021, women aged 40Â years or more treated with oral E2/P4 or oral CEE/MPA who did not have a venous thromboembolism diagnosis before first dispensing claim of CEE/MPA or E2/P4 identified on or after May 1st 2019 (index date) were observed for 6Â months or more after the index date. Oral E2/P4 and oral CEE/MPA had been prescribed by the treating physician in real-world practice and were observed through pharmacy dispensing records.
MAIN OUTCOME MEASURES
Venous thromboembolism risk was compared between women receiving oral E2/P4 versus oral CEE/MPA.
RESULTS
The study included 36,061 women treated with oral E2/P4 or oral CEE/MPA. In the analyses weighted by the inverse probability of treatment for control of potential confounding factors, the incidence of venous thromboembolism was significantly lower for oral E2/P4 compared with oral CEE/MPA (37/10,000 women-years for oral E2/P4 vs 53/10,000 women-years for oral CEE/MPA; incidence rate ratio 0.70, 95Â % confidence interval: 0.53-0.92).
CONCLUSIONS
Real-world evidence suggests that the risk of venous thromboembolism is significantly lower among women treated with oral E2/P4 compared with oral CEE/MPA

Recommendations for raloxifene use in daily clinical practice in the Swiss setting
Background/aim: Raloxifene is the first selective estrogen receptor modulator that has been approved for the treatment and prevention of osteoporosis in postmenopausal women in Europe and in the US. Although raloxifene reduces the risk of invasive breast cancer in postmenopausal women with osteoporosis and in postmenopausal women at high risk for invasive breast cancer, it is approved in that indication in the US but not in the EU. The aim was to characterize the clinical profiles of postmenopausal women expected to benefit most from therapy with raloxifene based on published scientific evidence to date. Methods: Key individual patient characteristics relevant to the prescription of raloxifene in daily practice were defined by a board of Swiss experts in the fields of menopause and metabolic bone diseases and linked to published scientific evidence. Consensus was reached about translating these insights into daily practice. Results: Through estrogen agonistic effects on bone, raloxifene reduces biochemical markers of bone turnover to premenopausal levels, increases bone mineral density (BMD) at the lumbar spine, proximal femur, and total body, and reduces vertebral fracture risk in women with osteopenia or osteoporosis with and without prevalent vertebral fracture. Through estrogen antagonistic effects on breast tissue, raloxifene reduces the risk of invasive estrogen-receptor positive breast cancer in postmenopausal women with osteoporosis and in postmenopausal women at high risk for invasive breast cancer. Finally, raloxifene increases the incidence of hot flushes, the risk of venous thromboembolic events, and the risk of fatal stroke in postmenopausal women at increased risk for coronary heart disease. Postmenopausal women in whom the use of raloxifene is considered can be categorized in a 2×2 matrix reflecting their bone status (osteopenic or osteoporotic based on their BMD T-score by dual energy X-ray absorptiometry) and their breast cancer risk (low or high based on the modified Gail model). Women at high risk of breast cancer should be considered for treatment with raloxifene. Conclusion: Postmenopausal women between 50 and 70years of age without climacteric symptoms with either osteopenia or osteoporosis should be evaluated with regard to their breast cancer risk and considered for treatment with raloxifene within the framework of its contraindications and precaution
