    Pressure ulcer related pain in community populations: a prevalence survey.

    BACKGROUND: Pressure ulcers are costly to the healthcare provider and can have a major impact on patient's quality of life. One of the most distressing symptoms reported is pain. There is very little published data on the prevalence and details of pain experienced by patients with pressure ulcers, particularly in community populations. The study was conducted in two community NHS sites in the North of England. METHODS: The aim was to estimate the prevalence of pressure area related pain within a community population. We also explored the type and severity of the pain and its association with pressure ulcer classification. A cross-sectional survey was performed of community nurses caseloads to identify adult patients with pressure ulcers and associated pain. Consenting patients then had a full pain assessment and verification of pressure ulcer grade. RESULTS: A total of 287 patients were identified with pressure ulcers (0.51 per 1000 adult population). Of the 176 patients who were asked, 133 (75.6%) reported pain. 37 patients consented to a detailed pain assessment. Painful pressure ulcers of all grades and on nearly all body sites were identified. Pain intensity was not related to number or severity of pressure ulcer. Both inflammatory and neuropathic pain were reported at all body sites however the proportion of neuropathic pain was greater in pressure ulcers on lower limbs. CONCLUSIONS: This study has identified the extent and type of pain suffered by community patients with pressure ulcers and indicates the need for systematic and regular pain assessment and treatment

    Intersubunit Interactions Allowing a Carboxylate Mutant Coat Protein to Inhibit Tobamovirus Disassembly

    AbstractTobacco mosaic tobamovirus (TMV) coat protein (CP) mutant E50Q lacks a repulsive intersubunit carboxylate group and can effectively inhibit the disassembly of wild-type TMV (Culveret al.,1995,Virology206,724). To investigate the ability of this mutant CP to block disassembly, a series of second-site amino acid substitutions were added to the E50Q CP. These second-site mutations were designed to disrupt specific intersubunit stabilizing interactions involving hydrophobic or polar residues, salt bridges, and CP–RNA contacts. Results showed substitutions disrupting intersubunit interactions that face the disassembling surface of the virion dramatically reduced the ability of CP E50Q to inhibit TMV disassembly. Substitutions that disrupted the CP inner loop, RNA binding capabilities, or intersubunit interactions that faced away from the disassembling surface did not dramatically interfere with CP E50Q's ability to inhibit disassembly. Taken together, these findings suggest that intersubunit interactions made by 5β€² terminal E50Q subunits, not associated with RNA, provide the stabilizing forces that prevent virion disassembly. The role of these stabilizing interactions in TMV disassembly and their potential use for creating disassembly inhibiting CPs are discussed

    Damage identification in a concrete beam using curvature difference ratio

    Previous studies utilising changes in mode shape or curvature to locate damage rely on the fact that the greatest change occurs around the defect. However, in concrete beams this fact is undermined due to the nature of the defect as distributed multi-site cracks. In addition, differences in mode shape and curvature as ways to locate the damage is unstable because of occurrence of modal nodes and inflection points. In this paper, one interesting solution to this problem is being tested by establishing a new non-dimensional expression designated the 'Curvature Difference Ratio (CDR)'. This parameter exploits the ratio of differences in curvature of a specific mode shape for a damaged stage and another reference stage. The expression CDR is reasonably used to locate the damage and estimate the dynamic bending stiffness in a successively loaded 6m concrete beam. Results obtained by the proposed technique are tested and validated with a case study results done by Ren and De Roeck [1] also by Maeck and De Roeck [2]. Another contribution of this work is that relating changes in vibration properties to the design bending moment at beam sections as defined in Eurocode 2 specifications [3]. Linking between a beam section condition and the change in vibration data will help to give a better comprehension on the beam condition than the applied load

    SN1997cy/GRB970514 - A New Piece in the GRB Puzzle?

    We present observations of SN1997cy, a supernova discovered as part of the Mount Stromlo Abell Cluster SN Search, which does not easily fit into the traditional classification scheme for supernovae. This object's extraordinary optical properties and coincidence with GRB970514, a short duration gamma ray burst, suggest a second case, after SN1998bw/GRB980425, for a SN-GRB association. SN1997cy is among the most luminous SNe yet discovered and has a peculiar spectrum. We present evidence that SN1997cy ejected approximately 2 solar masses of 56Ni, supported by its late-time light curve, and FeII/[FeIII] lines in its spectrum, although it is possible that both these observations can be explained via circumstellar interaction. While SN1998bw and SN1997cy appear to be very different objects with respect to both their gamma ray and optical properties, SN1997cy and the optical transient associated with GRB970508 have roughly similar late-time optical behavior. This similarity may indicate that the late-time optical output of these two intrinsically bright transient events have a common physical process. Although the connection between GRB970514 and SN1997cy is suggestive, it is not conclusive. However, if this association is real, followup of short duration GRBs detected with BATSE or HETE2 should reveal objects similar to SN1997cy.Comment: 26 pages including 6 postscript figures and 3 tables.     Uniformity of V minus Near Infrared Color Evolution of Type Ia Supernovae, and Implications for Host Galaxy Extinction Determination

    From an analysis of SNe 1972E, 1980N, 1981B, 1981D, 1983R, 1998bu, 1999cl, and 1999cp we find that the intrinsic V-K colors of Type Ia SNe with multi-color light curve shape (MLCS) parameter -0.4 < Delta < +0.2 suggest a uniform color curve. V-K colors become bluer linearly with time from roughly one week before B-band maximum until one week after maximum, after which they redden linearly until four weeks after maximum. V-H colors exhibit very similar color evolution. V-J colors exhibit slightly more complex evolution, with greater scatter. The existence of V minus near infrared color relations allows the construction of near infrared light curve templates that are an improvement on those of Elias et al. (1985). We provide optical BVRI and infrared JHK photometry of the Type Ia supernovae 1999aa, 1999cl, and 1999cp. SN 1999aa is an overluminous "slow decliner" (with Delta = -0.47 mag). SN 1999cp is a moderately bright SN unreddened in its host. SN 1999cl is extremely reddened in its host. The V minus near infrared colors of SN 1999cl yield A_V = 2.01 +/- 0.11 mag. This leads to a distance for its host galaxy (M 88) in agreement with other distance measurements for members of the Virgo cluster.Comment: 57 pages, 13 postscript figures, to appear in the August 20, 2000, issue of the Astrophysical Journal.     The experiences of caregivers of persons living with dementia in Jamaica during COVID-19

    This article provides descriptive insights of the experiences of family caregivers of persons living with dementia during the COVID-19 pandemic. Data were generated as part of a qualitative cross-national project to explore the costs and consequences of providing unpaid dementia care. Participants in Jamaica, who were recruited using community gatekeepers, information booths at health fairs, conferences, and other outreach events, were contacted by telephone to discuss their experiences of the pandemic. When face-to-face in-depth interview data collection was suspended due to the pandemic, ethical approval was received to contact all research participants who were informal unpaid family caregivers, both those whose care recipients had died and those who were active caregivers (N = 19). Participants in this study were the 10 active family caregivers (nF = 8; aged 45+; 60% from high socio-economic status). Their updates and reflections during these calls were documented in fieldnotes and analyzed for key themes. Data showed that the pandemic has illustrated the direct costs, both financial and otherwise, that informal dementia carers bear in Jamaica. It also intensified pre-existing challenges faced by family carers. We provide recommendations for sustainable support for family carers

    Protease activity as a prognostic factor for wound healing in venous leg ulcers.

    BACKGROUND: Venous leg ulcers (VLUs) are a common type of complex wound that have a negative impact on people's lives and incur high costs for health services and society. It has been suggested that prolonged high levels of protease activity in the later stages of the healing of chronic wounds may be associated with delayed healing. Protease modulating treatments have been developed which seek to modulate protease activity and thereby promote healing in chronic wounds. OBJECTIVES: To determine whether protease activity is an independent prognostic factor for the healing of venous leg ulcers. SEARCH METHODS: In February 2018, we searched the following databases: Cochrane Central Register of Controlled Trials (CENTRAL), Ovid MEDLINE, Ovid Embase and CINAHL. SELECTION CRITERIA: We included prospective and retrospective longitudinal studies with any follow-up period that recruited people with VLUs and investigated whether protease activity in wound fluid was associated with future healing of VLUs. We included randomised controlled trials (RCTs) analysed as cohort studies, provided interventions were taken into account in the analysis, and case-control studies if there were no available cohort studies. We also included prediction model studies provided they reported separately associations of individual prognostic factors (protease activity) with healing. Studies of any type of protease or combination of proteases were eligible, including proteases from bacteria, and the prognostic factor could be examined as a continuous or categorical variable; any cut-off point was permitted. The primary outcomes were time to healing (survival analysis) and the proportion of people with ulcers completely healed; the secondary outcome was change in ulcer size/rate of wound closure. We extracted unadjusted (simple) and adjusted (multivariable) associations between the prognostic factor and healing. DATA COLLECTION AND ANALYSIS: Two review authors independently assessed studies for inclusion at each stage, and undertook data extraction, assessment of risk of bias and GRADE assessment. We collected association statistics where available. No study reported adjusted analyses: instead we collected unadjusted results or calculated association measures from raw data. We calculated risk ratios when both outcome and prognostic factor were dichotomous variables. When the prognostic factor was reported as continuous data and healing outcomes were dichotomous, we either performed regression analysis or analysed the impact of healing on protease levels, analysing as the standardised mean difference. When both prognostic factor and outcome were continuous data, we reported correlation coefficients or calculated them from individual participant data.We displayed all results on forest plots to give an overall visual representation. We planned to conduct meta-analyses where this was appropriate, otherwise we summarised narratively. MAIN RESULTS: We included 19 studies comprising 21 cohorts involving 646 participants. Only 11 studies (13 cohorts, 522 participants) had data available for analysis. Of these, five were prospective cohort studies, four were RCTs and two had a type of case-control design. Follow-up time ranged from four to 36 weeks. Studies covered 10 different matrix metalloproteases (MMPs) and two serine proteases (human neutrophil elastase and urokinase-type plasminogen activators). Two studies recorded complete healing as an outcome; other studies recorded partial healing measures. There was clinical and methodological heterogeneity across studies; for example, in the definition of healing, the type of protease and its measurement, the distribution of active and bound protease species, the types of treatment and the reporting of results. Therefore, meta-analysis was not performed. No study had conducted multivariable analyses and all included evidence was of very low certainty because of the lack of adjustment for confounders, the high risk of bias for all studies except one, imprecision around the measures of association and inconsistency in the direction of association. Collectively the research indicated complete uncertainty as to the association between protease activity and VLU healing. AUTHORS' CONCLUSIONS: This review identified very low validity evidence regarding any association between protease activity and VLU healing and there is complete uncertainty regarding the relationship. The review offers information for both future research and systematic review methodology
